Enhanced ERA Import Posting Report - 16712

The ERA Import Posting report now shows interest totals as well as provider level adjustments. Although we collected this data in the past, it was never shown on the report until this release.

New Claim Edits - 16432-16435, 16437, 16440

The Claim Edits library has the following new claim edits:

Activate this edit to ensure that any CPT4 code specified in the first parameter requires all of the associated CPT4 codes specified in the second parameter on the same encounter.

To configure this edit, you must enter two parameters. The first parameter is a CPT4 code. The second parameter is an additional CPT4 code(s) that is required on the encounter when a CPT4 code from the first parameter is found on a claim.

200 CPT4(s) requires a particular number of units

Activate this edit ensure that the specified number of units exist for each listed CPT4 code.

To configure the edit, you must enter two parameters. The first parameter is the CPT4 code ranges you want the application to check. A range can include one or more CPT4 codes. The second parameter is the number of units that must exist for each CPT4 code range.

The required number of units that you specify for each CPT4 code range is a specific number, not a minimum. This means that the application checks to ensure that the actual number of units for each listed CPT4 code range matches exactly the required number of units specified in the edit.

201 Referring provider not allowed for SIM Department(s) <Unknown>

Activate this edit to ensure that no referring provider exists for the charge if the charge is for one of the selected SIM departments.

To configure the edit, list the SIM departments that you do not want referring providers associated with charges.

202 Referring provider must equal PCP for SIM Department(s) <Unknown>

Activate this edit to ensure that if a referring provider exists for the charge, it matches the PCP for the selected SIM departments.

The purpose of this edit is not to check that a referring provider exists, but to ensure that if a referring provider is present on the charge, it matches the PCP for the SIM department specified in the edit.

203 CPT(s) with particular number of units requires diagnoses

Activate this edit to ensure that the following condition exists: A CPT4 code with a specified number of units has a specific diagnosis (ICD9) applied.

Each CPT4 code you specify in the edit must have the specified ICD9 code applied, but only when the number of units for the CPT4 code matches the number of units specified in the edit.

To configure this edit, you must enter CPT4 codes, the number of units for each, and the ICD9 code that must be present when the specified number of units exists for each listed CPT4 code.

204 Modifier(s) require(s) encounter modifier(s)

Activate this edit to set up specific modifier requirements. When a modifier or set of modifiers that you specify in the first parameter are found on a claim, the corresponding modifier or set of modifiers that you specify in the second parameter must be present on the encounter.

To configure this edit, you must enter two parameters. The first parameter is a modifier or set of modifiers. The second parameter is a modifier or set of modifiers that is required on the encounter by the first parameter.

Enhanced Fax Manager Error Handling - 16425

NextGen EMR Fax Manager and Fax Monitor Utilities now have new error handling and debugging capabilities. This enhancement enables you to better handle situations where the same file is processed over and over or communication with the fax printer driver suddenly goes bad. The changes in functionality include:

File extensions for all bad files are renamed to .fmb

NextGen EMR Fax Monitor Utility automatically attempts to restart the Fax Manager when it looses communication with the fax printer driver.

DUR Preferences - 16554The Drug Utilization Review (DUR) preferences enable you to create a pre-defined list of the Override Reasons required for Acknowledging medication's interactions as well as to set up the DUR Interaction Display Level, which enables you to display a specific level of medication interactions. Both options are available from the DUR window and the Medications tab of the NextGen EMR Preferences window.

Note: Changing the Interaction Display Level requires pertinent user rights and

permissions. Contact your system administrator in regard to your user rights and permissions.

The following Levels of Interactions can be set up for display:

Do Not Display Drug Interactions: Does not display any DUR Interactions (NextGen EMR default).

Level 1 - Contraindicated Interactions: Displays only DUR Interactions that require your action to reduce adverse effects of a prescribed medication.

Level 2 - Severe Interactions: Displays DUR Interactions that require your action to reduce adverse effects of a prescribed medication, as well as DUR Interactions that may cause risk to a patient and may require your actions.

Level 3 - Moderate Interactions: Displays DUR Interactions that require your action to reduce adverse effects of a prescribed medication; or those that may cause risk to a patient and may require your actions or those Interactions that entail conservative measures and/or recommendations until more medical data is available.

Display All Drug Interactions: Displays all DUR Interactions, including alternate therapy interactions.

Note: Your system administrator may set a specific Minimum Level. In such a

case, you are NOT able to set your own Interaction Display Level lower than the Minimum Level set for the entire system.

New Edit and Review Patient Modes - 16835

The Edit and Review Patient Modes are part of an optional feature that NextGen EMR offers you to comply with the specific guidelines provided by the Certification Commission for Healthcare Information Technology (CCHIT). These two modes enable you to handle patient charts in a more efficient way, therefore, creating a more secure and easily sustainable environment for patients' medical data.

Important: This feature is optional and must be manually enabled by NextGen

Healthcare. We recommend that initially you enable this feature ONLY in your test environment, as there is a significant change to the daily workflow that is associated with this functionality. If you wish to have Edit and Review Patient Modes enabled, contact the NextGen Healthcare Customer Support Department.

The Edit and Review Patient modes provide the following functionality:

Review (read-only) Mode: multiple users can select a patient and access his or her chart. All patient data is displayed in read-only mode. You cannot make any changes to the selected patient's information in the Review Mode.

Edit Mode: only one user at a time can enter new patient information or change existing patient information. All other users are able to access patient's information in the Review Mode. If you attempt to add or edit patient information while another user has the same patient selected in the Edit Mode, a message displays to warn you that a specific user is editing information for that patient.

After the Edit and Review Patient Modes are enabled in your environment, the Edit Mode

menu item is added under the View menu of the Main menu and an additional Edit button is added to the toolbar below the menu bar:

When you are in the Edit Mode, any patient you select is automatically opened, enabling you to add or modify patient information. The Edit Mode displays on the title bar for the selected patient.

All other users are able to review information for the selected patient in the Review Mode. The Review Mode displays on the title bar for the selected patient.

After you finish adding or modifying patient information, click the Edit button to cancel the selection and allow other users access to the selected patient's information.

When working in the Review Mode, the changes to the patient information that are made by another user working in the Edit Mode are not displayed unless you select Refresh from the drop-down list on the History toolbar.

Options Added for Invalid Logins - 17089

The Password Requirements dialog box contains two new settings under Invalid Login Options that restrict unsuccessful user login attempts:

Number of invalid login attempts – Defines the maximum number of consecutive unsuccessful login attempts users can make.

Deactivate user after failed login attempts – When selected, this check box deactivates a user's ID when the user has reached the defined maximum number of invalid login attempts.

New Operations Security Rights for ICS - 16612, 16722

The System Administrator application has two new Operations security rights:

Rename Documents - This right controls whether or not the user can rename documents in the ICS application.

Audit - This right controls whether or not the user can run the Lost Pages audit within the ICS application.

Settings for Interaction Severity Levels - 16554

The System Administrator application has new rights and Universal Preferences for setting the severity level at which drug interaction warnings display to users.

Rights

Under Operations on the Rights tab, the new Interaction Display Level right enables EMR users to set the levels of DUR Interactions that they want displayed. If this permission is set to No, the Interaction Level Display Default from the Universal Preferences is used.

Universal Preferences

In the Universal Preferences, the General Options Preferences now contain two new settings:

Interaction Level Display Default – A value between 0 and 4 sets the default level of DUR Interactions that displays to users. If no value is set, all medication interactions display to users.

Listed below are the levels and their meanings:

Level 1 – Contraindicated Drug Combination: This drug combination is clearly contraindicated in all cases and should not be dispensed or administered to the same patient.

Level 2 – Severe Interaction: Action is required to reduce the risk of severe adverse interaction.

Level 3 – Moderate Interaction: Assess the risk to the patient and take action as needed

Level 4 – Undetermined Severity: Alternative Therapy Interaction: Assess the risk to the patient and take action as needed.

Interaction Level Displayed Minimum – If users have the right to set their own levels, then a value between 0 and 4 determines which levels users can set for themselves. Users cannot set the levels to less than this level. If no value is set, users who have the right to set their own levels can turn off all warnings.

0 – Off; Do not display any warnings

1 – Display only level 1 warnings

2 – Display only level 1 and level 2 warnings

3 – Display only level 1, level 2 warnings

4 – Display only level 1, level 2, and level 3 warnings

Option to Show Batch Details - 16366NextGen ICS has added the Show Batch Details check box to the Image Batches dialog box. This check box controls whether or not the ICS application displays the following information about a batch:

Total number of pages in a batch

Number of un-filed documents in a batch

Application Launcher > ICS > Tasks menu > Batches...

The Show Batch Details check box is un-checked by default. To view the page count and the number of un-filed documents for the batches, place a check mark in the option.

Note: Displaying total pages and the un-filed document count for batches

requires intensive database processing. If you have many large batches, you can improve ICS performance by leaving the Show Batch Details option un-selected.

Lost Page Audit - 16612The ICS application now includes the Lost Pages audit. This feature enables

authorized users to locate and restore pages to batches that have lost their indexing values. Lost pages do not display in the list of available documents and images for a patient in the Search Results dialog box. Running the Lost Pages audit restores the ability of the application to display all pages for each batch in the search results. NextGen Healthcare recommends that you run the Lost Page audit in ICS at regular intervals.

Note: You must have the Audit operations right assigned to you in the System Administrator application to be able to run the Lost Pages audit.

Contrast/Brightness Controls Added to ICS Viewer - 16766

NextGen ICS now has a Contrast/Brightness option on the shortcut menu that displays when you right-click on an image in the Viewer. Selecting the option displays the Gamma Correction dialog box with the following adjustable controls:

Brightness

Contrast

Gamma

The settings you make with the controls, when saved, apply to all images the Viewer displays.

Enhanced Quick File Feature - 16850The ICS Quick File feature is now more automated. When you select a Quick File button, the ICS application now files the page and moves to the next page automatically. Previously, it was necessary to click the File button manually to file each page. When filing a scanned or imported document to separate documents, you are no longer required to click the New Doc button. Several actions you can take during the Quick File process prompt the application to file the current page to a new document automatically. These actions are outlined in the table below.

In addition, you can now:

Select a patient before you select a document type for filing.

Perform a new patient lookup at any time when Quick Filing a document.

Actions that prompt ICS to file to a new document:

If you ... Then ...Click a different Quick File button for the second (or later) page of a document...

ICS files to a new document, beginning with the current page. Previously, you had to click the New Doc button to file the scanned or imported document as multiple documents.

Select a different patient in the middle of the filing process ...

New Option to Delete Inventory Line Records - 16614, 16616, 16617

NextGen Optik now has a Remove Row option to delete inventory line records from the following areas of the application:

Site-to-Site Inventory Transfer/Add New

Inventory Return/Add New

PO/Add New

Inventory Write Off / Add New

You can access the Remove Row option by right-clicking on the inventory line record you want to remove.

Previously, the application required that you double-click on an inventory line record to delete it.

Tax Options Added to Eyeglass Order Form - 17216

NextGen Optik now has two new check boxes on the Eyeglass Order form:

Add Tax - Select this option to add tax to an inventory item that is not configured as taxable in the Optik Inventory Master.

Remove Tax - Select this option to remove the tax from the order total for all taxable inventory items.

The Add Tax and Remove Tax options replace the Suppress Tax option on the Optik Eyeglass Order form.

Inventory Items Locked During Reconciliation - 16658, 16929

The application now prevents inventory sales, purchase orders, receipts, transfers, returns and write offs from being processed for the location that has an in-progress status reconciliation. The application does not process inventory activity for the location until the reconciliation status has been updated to Done or Deleted.

Other ID Column Added to Patient Lookup - 16820

The Other ID field, located in the Patient Information dialog box's External Tab, now displays as a column in the Patient Lookup dialog box's results for NextGen EPM, NextGen EMR, NextGen Optik, and NextMD.
