At National Institute for Locomotor Disabilities(Divyangjan), Kolkata a short course on Orthotic and therapeutic management of Stroke patients organized by International Society of Prosthetics and Orthotics(ISPO)-Indian National Member Society from 2nd -4th November 2017.

The programme was started with National Anthem. It was inaugurated by Dr. Abhishek Biswas, Director, National Institute for Locomotor Disabilities (Divyangjan), Kolkata, Department of Empowerment of persons with disabilities, Ministry of Social Justice & Empowerment, Govt. of India, on 2nd November, 2017 by lightening the lamp as customary of Indian culture. In his inaugural address he said that stroke is the leading cause of Disability and death in India, by this short course programme it will be helpful for treating post stroke patients. He also extended his thanks to ISPO-India for choosing NILD, Kolkata as venue for this course.Dr. Ameed Equebal, Asstt. Director (Training) expressed in his address that this programme will help to extend the knowledge about proper management regarding How & When to do and not to do in rehabilitation of patients and Persons with Disabilities (Divyangjan). Dr. P K Lenka, HOD, Department of P & O also spoke about importance of Stroke Rehabilitation.

Dr. Elizabeth Condie, Task Force of ISPO & Retd. Reader at the National Centre for Prosthetics and Orthotics, University of Strathclyde, Scotland UK spoke regarding importance of Stroke rehabilitation. She has emphasized on stroke rehabilitation on emerging stroke patients globally.

Mr. Aratatran Patra, Organizing Secretary, welcomed to all dignitaries, participants, Patients, Sponsors, and Exhibitors for their participation in the 3 days programme and he has highlighted the full cooperation and support of NILD, Kolkata specially the active role Dr. A. Biswas, Director, NILD, Kolkata to make this Short Course happen in this eastern part of the Country . He has briefed about the activities of International Society of Prosthetics and Orthotics-Indian National Member Society.

Day-I session was started after the inauguration program. Ms. Elizabeth Condie explained about course, its significance and importance in term of patient point of view. She also explained about various epidemiology, causes and current medical care in stroke followed by Short Tea break. Post tea break

session started with the well-known Neurologist Prof.(Dr.)Biman Kanti Roy from Bangur Institute of Neurology, Kolkata. He spoke about Epidemiology and medical management of Stroke in India. He focused on importance of rehabilitation team effort in recovery of stroke.

Next topic was covered by Ms. Elizabeth Condie, who spoke about international consensus on orthotic and therapeutic management of stroke. She highlighted various outcome factors from earlier consensus conference followed by

Post Lunch session started with presentation of Mr. Roy Bowers, where he talked about biomechanical aspect of walking in stroke. He discussed about alteration in human gait in stroke with comparison of normal gait. In next presentation, Mr. Mark Smith highlighted about core principles of physical therapy during dealing with stroke patients followed by tea break.

In post tea break session, Dr. Judy L. Ranka presented a paper on Occupational therapy, functional assessment and treatment for Stroke patients. This also includes various types of assessment techniques and importance of treatment. Next presentation was on principles of lower limb orthotic management by Dr. Roy Browers, where main highlights were various types of Orthoses available for stroke and their prescription guideline.

Day-II Program was started with presentation by Dr. Abhishek Biswas on Organisation and structure of stroke services in India. He spoke about government policies and various guidelines available for stroke patients. Mr. Mark Smith delivered his talk on continuing physiotherapy and gait training. He spoke about impact of gait training program in recovery phase and highlighted the role of physiotherapy in treatment protocol in stroke patients.

Post tea session was started with Dr. Judy L. Ranka and she delivered her talk on Upper limb and hand training. She spoke about involvement of upper extremity in stroke. She focused over physiological changes occurred due to stroke. Next paper presentation was by Mr. Mark Smith on shoulder assessment and significance of physical therapy in involvement of shoulder in stroke. Pre Lunch Session was ended with the lecture of Dr. Judy L Ranka on Upper limb orthotics including shoulder slings. Dr. Judy Ranka talked about role of shoulder slings and their impact in terms of biomechanics. She also discussed about various designs of hand splints its indications, contraindications and how it can help a post stroke patient.

Post Lunch session was started by Dr. Prasanna K. Lenka on various types of orthotic management and their application over patients. He also highlighted the impact of properly fitted ankle foot orthosis and their biomechanical significance. Mr. Pravin Kumar talked about current practice of physiotherapy in stroke in India. He mentioned few case studies where therapeutic management was discussed. Occupational Therapy management of stroke patients in India was covered by Mr. Pankaj Bajpai. He explained occupational therapy management of Stroke Patients in Indian scenario and he mainly focused on early intervention in post stroke cases.

The Pre Lunch session of Day-II was concluded with the discussion on various ongoing treatment protocols on international level and its effectiveness. This discussion was done by Mr. Mark Smith and Dr. Judy L Ranka. They also presented videos where pre and post treatment was showed on patient with stroke. This session continued after Tea Break where Ms. Elizabeth Condie continued the remaining portion and highlighted the outcome measures in stroke treatment. She also emphasis and pointed about national guideline development. Dr. Judy L. Ranka continued the session where along with Mr. Mark Smith, she talked about other impacting factors in stroke which may leads alterations in treatment protocol

The concluding session of Day-II was Round table discussion: “The future: possible developments for stroke services in India”. The round table discussion was with all faculties and participants.

Day-III session was started with prepared of interested professional to participate in three different three Workshops in P&O, PT and OT into three different groups. Total 6 patients were included during this session among total 15 patients with stroke after pre assessment done on 1st nov 2017 before the course started. Group1 was headed by Mr. Roy Bowers , Group 2 was headed by Dr. Judy L. Ranka and Group 3 was headed by Mr. Mark Smith. In Group 1, Mr. Roy Bowers explained about assessment, measurement, casting and fabrication techniques of ankle foot orthosis. Dr. Judy L Ranka explained about patient to audience of group 2 and shows assessment techniques. She answered the questions asked by the participants. She also shown various prefabricated hand splints and discussed about their prescription and fitment position. She fabricated one hand splint for the patients and fitted him. In Group 3, Mr. Mark Smith addressed the participants with introduction of patients. He explained physical assessment and few test over patient to identify the condition. He showed techniques of physiotherapy about position, holding and application of pressure on patients. He also shown the pre and post altered gait pattern of patient after therapy. Participants of Group 2 and Group 3 were interchanged with Dr. Judy L Ranka and Mr. Mark Smith with new 2 patients. Tea break was followed by these workshop sessions.

Participants in Practical Session 

In post tea session fitment and checkout clinic started where Mr. Roy Bowers and Mr. Mark Smith explained the gait pattern to all audiences. Discussion about gait deviation and possible solutions was also take place. Many questions were also addressed by Ms. Elizabeth Condie, Mr. Mark Smith and Mr. Roy Bowers. This checkout clinic was again continued in post lunch session with another patient. After this checkout session, open questions were called from participants in clinical problem solving session. Ms. Elizabeth Condie, Dr. Judy L Ranka, Mr. Mark Smith, Mr. Roy Bowers, Dr. Abhishek Biswas, Mr. Pankaj Bajpai, Dr. Prasanna K Lenka and Mr. Praveen Kumar was available on the dias and answered all questions.

Valedictory function was started with experience sharing by Dr. Abhishek Biswas, Dr. Prasanna Kumar Lenka, Ms. Elizabeth Condie. Ms. Elizabeth Condie spoke about take home message for free and full communication between professional groups is essential for the effective and highest quality of care for stroke patients, and indeed all patients requiring rehabilitation. This must be established as soon as the patient has a stroke, and better links need to be established between the acute hospitals and rehabilitation centres such as NILD. Orthotists must be included at an early stage as full members of the stroke team. At present, it appears that they are involved rather late in the rehabilitation phase, and this is not in the interests of the patients. Interdisciplinary education and training of all staff ( therapists, nurses, doctors , orthotists, outreach workers and others) involved in the care of stroke patients should be continued and developed as a matter of urgency.
