    Primary Health Care is the termfor the health care serviceswhich play a role in the localcommunity. It refers to the workofhealth careprofessionals who act as a firstpoint of consultation forall patients within the healthcare system.Such aprofessional would usually bea primary care physician, suchas ageneralpractitionerorfamily physician,or a non-physician primary careprovider, such as a physician

    assistant ornurse practitioner.Depending on the nature of thehealth condition, patients maythen be referred for secondaryor tertiary care.

    Primary care involves the widestscope of health care, including allages of patients, patients ofall socioeconomic and geographicorigins, patients seeking to maintainoptimal health, and patients with allmanner of acute and chronicphysical, mental and social healthissues, including multiple chronicdiseases.

    Secondary Health Care is anintermediate level of healthcare, that is concerned with theprovision of specific technical,therapeutic or diagnosticservices. Specialist

    consultation procedures andhospital admissions fall into thiscategory of care. Theseservices are episodic andusually focused on a particularhealth problem.Continuity of care is lesscritical. Secondary Health Careis provided to a larger group ofpeople from a largergeographic area than those

    served by Primary Health Care.

    Patients in this health care end uphere when they have a medicalcondition that cant be handled byprimary care.

    Tertiary care requires highlyspecialized equipment andexpertise such as coronaryartery bypass surgery, renal orhemodialysis, some plasticsurgeries or neurosurgeries,severe burn treatments or anyother very complex treatmentsor procedures.

    Tertiary care is specializedconsultative health care, usuallyforinpatients and on referral from aprimary or secondary healthprofessional, in a facility that haspersonnel and facilities foradvanced medical investigation andtreatment
