Foreword

Studies of the effects of nitric oxide in people have been accelerating since its discovery in the late 1980’s. Its broad range of biologic effects is hard to envision for a molecule with so short a half-life. As a gas, it’s here and then it’s gone. Yet Dr. Abbas Qutab has gracefully woven a tale showing where nitric oxide is a participant in many different organ systems and medical conditions. How he was able to gleam this information from many thousands of medical research publications is, quite frankly, amazing. This synopsis, remarkably, doesn’t talk down to the reader by using sophisticated, sometimes unintelligible medical terms. Rather it takes the reader from commonly understood conditions such as pain and coronary arteries, which virtually everyone understands, to a simple explanation of how the presence or absence of nitric oxide can make a difference in people’s health. You have to be impressed that he was able to capture the important, positive messages that sometimes are buried in the medical literature. I hope that those who read this book come away as impressed as I was in this description of why Noni, via nitric oxide, may be so helpful for so many different conditions.

INTRODUCTION

It must have been three or four years ago now, that one of my favorite patients, whom I will call “Sonny” showed up in my office for his regular checkup. “I’ve stopped all my meds,” Dr. Abbas,” he said, a little sheepishly, but with a determination in his eye. This big bear of a man, charming and jovial, had fought a long-standing war with high blood pressure. We were mostly winning this battle, with the normal medications, and the contribution of his rigid discipline (most of the time) when it came to diet and exercise. Sonny always has a joke and a smile for us when he comes in to the clinic, and often brings gifts of fresh fruit or vegetables -he’s a great guy. As you can imagine, I was a bit appalled at this news that he no longer took the drugs that I counted on to protect him, but he eagerly told me what he was taking instead. “Nitric oxide, doc. Extracts of the Noni tree - it’s something to do with creating nitric oxide in the body. I heard about it from a friend of mine who takes it for her arthritis- she hasn’t felt this well in years. So I gave it a try. Go on, take my blood pressure.” He grinned and pulled up his sleeve in anticipation. To my surprise, his blood pressure was normal. He hadn’t seen 120 over 80 for at least five years without medication. Sonny explained that he had now been taking this Noni extract product for three months, and had been off

his medications for one month. I insisted he come back monthly for a while, so I could monitor this situation, and each time he came into my office, his blood pressure remained normal. He was taking no drugs at all, just this proprietary blend of extracts of Noni fruit. He was thrilled and proud of his discovery. And I was completely intrigued. This launched my investigation into the humble Noni tree, with its strange little ugly fruit. I read everything I could get my hands on, and discussed it with many colleagues, many of whom brushed it off as another scam or possible placebo effect. Now, while it is true that placebo effects are powerful, they just don’t normalize blood pressure problems as severe as Sonny’s. And they don’t reverse illnesses as crippling as the fibromyalgia that

had been plaguing Nancy, another of my patients. The change in her wellbeing since she started to take a proprietary blend of Noni extracts has been almost miraculous. In my study, I learned this proprietary nutraceutical creates nitric oxide in the body. Nitric oxide, known as NO, has been extensively proven in thousands of studies to

one atom of oxygen and one of nitrogen. It was best known as a toxic gas and an ingredient in air pollution until 1987, when it was first shown to be produced by various cells throughout the human body and to help cells communicate with each other. I spoke at great length with scientists who were researching Noni, its nitric oxide effect, and I found myself wanting to help spread the word. I now travel extensively, lecturing to both doctors and laypeople on the benefits of this proprietary blend of Noni extracts that have such a high nitric oxide effect. I must emphasize that I make no money whatsoever from the sale of any Noni extract or nitric oxide products. It is simply that I have never before found a product that I could so wholeheartedly endorse, and I wish to extend the benefits I see so regularly in my practice to people all over the world.

EXTRACTS OF NONICREATE NITRIC OXIDE

By now there certainly aren’t many people interested in health and healing who haven’t heard of Noni. Typing “Noni” into Google brings up six or seven million websites where this topical fruit tree is referenced. Since the middle of the 1990’s, Noni, or Morinda citrifolia, has catapulted onto the worldwide stage and grown exponentially, with sales now estimated at about one billion dollars annually. This shrubby evergreen tree, with its small, foul-smelling fruit has been called the “Ancient Cure from Paradise,” the “Painkiller Tree,” and the “Aspirin of the Ancients.” In Australia it has been called “Cheesefruit” from the overwhelming smell. It grows wild throughout the Southeast Asian countries, but its original home was Polynesia, Micronesia and the Hawaiian Islands. Polynesians have successfully used this health-promoting plant and its fruit for thousands of years, yet they readily admit to not knowing how or why it works. Southeast Asians have always used all parts of the Noni tree medicinally-the bark, root, leaf, fruit and flower. Researchers have explored the Noni fruit, leaves, and roots to try to ascertain the exact ingredient or ingredients that promote health. Noni is high in Vitamin C and other vitamins, amino acids (the building blocks of protein), minerals, and contains several other compounds including the antioxidant carotenoids, and bioflavonoids. It is also has potassium, which is a consideration for anyone suffering from kidney disease or insufficiency. Although it has less potassium content than orange juice or tomato juice, you still may want to be aware of this. This list of constituents alone do not adequately explain the benefits patients all over the world are experiencing from taking Noni, considering they naturally occur in many nutritious plant foods.

Determined to solve this puzzle, scientists began to notice a correlation between the success stories of patients using Noni and the benefits of having enough nitric oxide in the body. Due to its importance in the human body, a vast amount of research was being conducted on the properties of nitric oxide. It seemed possible that nitric oxide (NO) was the mechanism at work- perhaps Noni caused a nitric oxide effect in the people who took it. Research was undertaken in 1999 and 2000, by Dr. Thomas Burke and his researchers at Integrated Systems Physiology, an independent lab in Denver, Colorado. They conducted in-vitro experiments which tested diluted Noni juice on endothelial

cells, the cells that line the inner surface of blood vessels. That was the answer for which everyone had been searching- the Noni juice did indeed create nitric oxide! This suddenly helped to explain the enormous improvements patients were having drinking this humble tropical juice. The shrubby little tree that had the weight of the collective voice of generations was finally being confirmed in the scientific arena. Having isolated at least one of the main

mechanisms by which Noni works in the body, this advanced Noni product is now available on the market. This product has been available internationally, now, for three years. And the reports of health improvements are pouring in from around the world. There is now even a test that can show you how well the proprietary blend of Noni extracts is generating NO in your own

This quick and easy saliva test measures within 30 seconds, and in early trials, showed increases in nitric oxide by-products of up to 400% in people who had taken two droppers-full three times a day for only four days. So what is nitric oxide anyway? What does it do for us, and where does it come from?

NITRIC OXIDE:THE MOLECULE OF LIFE

Nitric oxide (NO) helps maintain, defend, and repair every cell of the body. That’s quite a statement. But the fact that it is one of the most important molecules to human life is, without a doubt, one of the major discoveries of this century. Scientists knew that NO was produced in bacteria, but no one realized its importance in higher animals such as mammals. This discovery caused quite a sensation in the late 1980’s. By 1992 it was named “The Molecule of the Year” by Science magazine. In 1998 it brought the Nobel Prize for Physiology or Medicine to three scientists, Robert Furdhgott, Luis Ignarro and Ferid Murad, for their discovery of the signaling properties of NO. The scientific literature on NO is growing exponentially, and to date, in 2006, there are more than 70,000 scientific papers published regarding some aspect of nitric oxide and its beneficial role in patient care. But the most important fact in all this is that it brings better health to millions around the world. Most people have never even heard of nitric oxide, known as NO. NO is not nitrous oxide, the laughing gas that some dentists use to sedate their patients. It is not nitric acid, a highly corrosive acid which is used in the making of explosives and fertilizers. And it is not nitrogen dioxide, a poisonous air pollutant, a component of acid rain. Nitric oxide may be a simple molecule, made up of one part nitrogen, and one part oxygen, but scientists and researchers around the globe have been astonished by the seemingly limitless list of functions of nitric oxide in the body. “In my 25 years of research, I have never seen a molecule [NO] that so pervasively influences normal and abnormal body functions,” said Dr. Solomon Snyder, the director of neuroscience at Johns Hopkins Medical School. 1

WHAT DOES NO DO IN THE BODY? Although it is a highly reactive gas, NO is however the most important “signaling” or “messenger” molecule in our body. This means that although it has only a short life of a few seconds, in that time it instigates processes involving an extensive array of physiological functions that control blood circulation, inflammation, learning and memory, digestion, and penile erection. One of NO’s broadest functions is in controlling blood pressure by dilating (relaxing and widening) blood vessels, which improves blood flow and helps prevents the clotting of the blood. This holds profound implications for the treatment of cardiovascular disease. It may have been what led Sonny to my office three years ago. It is also a powerful tool to help patients with impotence problems, as it can initiate and maintain erection by dilating blood vessels. This explains the mechanism of Viagra. Plentiful studies in the laboratory have shown that nitric oxide is also vitally important to the immune system. It fights and kills bacteria, viruses, fungi, parasites, and cancer cells, or stops their ability to multiply. It also acts like a neurotransmitter, relaying messages between nerve cells. More of its roles are continually being isolated and identified, and research into the potential clinical applications of nitric oxide is still in its infancy. The possibilities seem endless. Across the board it is truly an important and busy molecule!

WHERE DOES NO COME FROM? Let me tell you a little about where NO comes from. The primary way that nitric oxide is created is from L-arginine, one of

the essential amino acids, which are building blocks of protein. The essential amino acids are called that because the body cannot create them from other compounds, but must get them from food, such as meat and fish. Then various enzymes are needed, plus oxygen, to release nitric oxide from L-arginine. The biochemistry depends on the action of one of a family of enzymes, called nitric oxide synthases or NOS, which converts L-arginine to another amino acid, L-citrulline, forming nitric oxide in the process. But nitric oxide is also created from substances known as nitrates and nitrites found in plants such as green leafy vegetables, and Noni, as we now know, has proven to be high in these substances. Proprietary extracts of the Noni tree contain large amounts of nitrates and nitrites, which are then converted into nitric oxide by normal stomach acid. 2 3 This NO protects us by killing almost all the bacteria that have been swallowed in our food. But in addition, the nitric oxide diffuses through the stomach wall and enters the red blood cells, or binds to proteins and is carried to all parts of the body. For thousands of years, humans have used nitrates to preserve meat from bacterial spoilage. Now, you may think of nitrates and nitrites in a negative way, having heard that they seem to be harmful chemicals added to foods such as bacon, sausage, and smoked meats. But strangely enough, it appears that nitrates may have been blamed incorrectly. Recent research shows that there is a healing potential instead; nitrates are an aid to cardiovascular health through their conversion into nitric oxide. A study published in Harvard’s “Heart Letter” in 2004 found that nitrates and nitrites

(through nitric oxide’s action) keep arteries relaxed and more flexible, which in turn keeps blood pressure low, and prevents strokes and heart attacks. 4 Other studies carried out by the National Institutes of Health, the University of Alabama and Wake Forest University 5 have confirmed these findings, that nitrate, a common salt, can dilate blood vessels and improve blood flow. In an article published in the November 2003 issue of Nature Medicine, researchers reported that nitrite is easily converted into nitric oxide, suggesting that nitrates and nitrites may in fact be a source of nitric oxide for the body’s needs. 6

Since nitric oxide, from nitrates and nitrates, dilates blood vessels, this may help to explain why some people feel a bit faint, or get headaches after eating smoked meats with nitrates. But I don’t suggest you start eating lots of hot dogs- those high fat little guys made from questionable meats will never be a health food! There is a much better source of nitric oxide, and it is from a proprietary blend of Noni extracts. So, since nitric oxide is released from arginine, why not just eat lots of protein that is high in L-arginine, or take arginine supplements? Internet ads and gyms across America are abuzz with the arginine - nitric oxide link. And the truth is L-arginine is indeed a good source of nitric oxide, and most Americans probably get enough in the protein that we regularly eat to make our own nitric oxide. But there are a few good reasons why taking a substance which releases nitric oxide directly, from nitrates and nitrates, is preferable. First, there are several co-factors that are necessary in order to form nitric oxide in your cells. Secondly, the environment around the cells making nitric oxide must not be too acidic or too alkaline. If one or more co-factors is missing or not in adequate supply, and/or the cell environment is too acidic or too alkaline, then

nitric oxide will not be created, or not at optimum levels. Exercise also plays a part in creating the optimum amount of nitric oxide from L-arginine. In people who are sedentary, or those whose chronic illness or overweight issues limit their ability to exercise, nitric oxide is not produced in optimal quantities. Another factor to consider when ensuring a good supply of NO is the fact that L-arginine has other roles to play in the body besides its ability to generate nitric oxide, primarily in the metabolism of protein, for muscle growth. Its other functions include detoxifying ammonia in the body (a by-product of energy production) and promoting the secretion of several hormones, including insulin, and growth hormone. Since creating NO is not L-arginine’s only job, it seems preferable to take a plant substance that directly generates NO. And finally, there is the herpes link. People who suffer from either herpes Type 1, the oral form that causes cold sores, or herpes Type 2, the genital form, need to be aware that L-arginine can actually “feed” the herpes virus, which needs arginine to replicate. Those patients are regularly urged to avoid the L-arginine-rich foods, and to eat L-lysine foods or take lysine supplements, which may reduce herpes outbreaks, or limit them. This is due to the fact that L-lysine competes with L-arginine in the body, so the supplementation of L-lysine can tip the balance in its own favor. This ameliorates herpes outbreaks, especially the oral form. L-arginine has not been

conclusively proven to worsen herpes Type 2 symptoms, but its general connection to herpes has been shown in many trials. And further, with regard to the herpes issue, recent detailed studies have shown that nitric oxide actually inhibits the replication of the herpes virus7, which is good news for those who take the proprietary blend of Noni extracts.. This herpes link may seem small to those of you who have never had cold sores, or aren’t familiar with this nasty virus. But unfortunately herpes is especially prevalent in humans. In fact, over 100 million people here in the U.S. alone are afflicted with the virus, in conditions such as cold sores, genital herpes, and shingles, its painful cousin. So, eating a big steak or downing L-arginine capsules isn’t

always the answer. However, the proprietary blend of Noni has a large amount of nitrates and nitrites which can be converted into nitric oxide when they reach the hydrochloric acid in the stomach. This nitric oxide can then pass through the stomach wall, rapidly diffusing through the body. This method of getting nitric oxide is faster and relies on no other conditions for its creation, giving you

nitric oxide quickly when and if it is needed.

DO NITRATES/NITRITES LEAD TO PEROXYNITRITES, THE DAMAGING FREE RADICALS? In my lectures I am often asked if taking a product high in nitrates and nitrites might lead to damaging levels of peroxynitrites,

which are a type of free radical. In a nutshell, no. This is because the amount of nitrates and nitrites in the proprietary blend of Noni extracts, while high, is not immediately converted to nitric oxide. Contents of the stomach break down fairly slowly. Also, the NO diffuses through the stomach lining and enters the blood vessels where it attaches to amino acids and proteins, and while attached to these proteins the nitric oxide cannot react with oxygen to form peroxynitrite. Peroxynitrite is only created when nitric oxide is released as a gas from the proteins and then interacts with oxygen. Finally, the body, through an additional mechanism of NO, has a safety valve to protect it from peroxynitrites. There is a powerful antioxidant molecule, called glutathione, also generated by NO, which can mop up peroxynitrites. There is absolutely no way that ingestion of plant substances such as the proprietary blend of Noni extracts would lead to the formation of peroxynitrites.

Heart attacks happen when blood can’t circulate through the coronary arteries in the heart resulting in the death of a part of the heart muscle- it simply dies and scars over from a lack of oxygen. This happens partially because of narrowing of the arteries caused by cholesterol build-up. But heart attacks can also be due to blood clots forming in these narrowed arteries caused by inflammation, leading the artery to clog up just like plumbing in the sink. Dr. Robert Vogel, professor of medicine at the University of Maryland Medical Center described this condition in a very picturesque way; that the clogged “endothelial lining had the texture of Velcro”, promoting this condition called atherosclerosis. Atherosclerosis consists of plaques containing cholesterol and lipids on the walls of the arteries. To some degree it naturally occurs with aging, but there are many other risk factors; high blood pressure, smoking, diabetes, high cholesterol, and/or a family history of heart disease. As well as a heart attack, atherosclerosis can lead to stroke, and eye and kidney problems. With this build up of plaque lining the artery walls, the endothelial cells lose their capacity to produce nitric oxide. Inactivity and poor diet also weaken the endothelial cells, as can high blood sugar from diabetes. When these fragile endothelial

cells are damaged, they can not create as much nitric oxide.8 By taking a substance high in nitrates and nitrates such as the proprietary blend of Noni extracts, nitric oxide helps to prevent heart disease in several ways. It dilates the arteries, making them wider, so more blood, more oxygen and nutrients can get through to the heart muscle. Furthering this irony, in Nobel’s later years he suffered a heart condition which caused him to have severe angina pain, and his doctor recommended he take small doses of nitroglycerine! He refused, unwilling or unable to believe that the explosive substance he knew so well could help, not harm, the human body. John P. Cooke, M.D., director of vascular medicine at Stanford University’s medical school, states, “NO is the body’s own form of nitroglycerin. Large efforts are currently being made in the pharmaceutical industry, to find more powerful and selective heart drugs based on this new awareness of nitric oxide and its role in blood vessel maintenance and health. However, it seems that a natural substance, such as the proprietary blend of Noni extracts is the superior answer, without the side effects so common to pharmaceutical drugs.

NITRIC OXIDE AND PAIN RELIEF An enormous number of people regularly suffer from pain, dramatically reducing their quality of life. A 2005 USA Today/ABC News/Stanford University Medical Center poll showed that 19% of American adults, or almost 1 in 5, suffer from chronic pain and 44% have acute or short-term pain. These were attributed to medical injury or to joint pain, heart disease or cancer.10

When you are in pain, it is hard to think about anything else. Nothing else seems to matter much- all you can think about is being pain-free. Or finding a way to live with it somehow. So how does pain work and what happens in the body? Pain may be anything from unpleasant to agonizing, but it does have a function; it is actually a necessary survival instinct. It is the reflex that makes you whip your hand away from that hot stove in a reaction that seems faster than thought. It is the instinct that forces you to avoid using a muscle that needs to rest after an injury. In this regard, pain has a positive outcome. But when chronic pain from inflammation or an injury continues unabated, it seems to have no rhyme or reason. We just want it to stop. All pain, indeed all sensation, results from the functioning of the nervous system, that incredibly complicated web of nerves and cells and messengers that communicates sensory information. From the brain to every cell in the body, the nervous system handles the messages for such sensations such as cold, touch, and heat, and of course pain, from bruises, sprains, and from other more chronic sources such the inflammation of arthritis. Many drugs are prescribed for pain relief. Scientific research shows that the pain relieving effects of the medications like morphine, aspirin, and oxycontin are due to the release of Nitric Oxide.11 Take tennis elbow, for example. NITRIC OXIDE AND SEXUAL FUNCTION One of the hottest topics surrounding nitric oxide and its uses in the body is with regard to sexual health. Research is beginning to show that smokers, or those with atherosclerosis, high blood pressure, or diabetes may have less nitric oxide in their bodies, which can lead to impotence. Since the discovery that it is nitric oxide which dilates blood vessels in the penis to initiate and maintain erections, pharmaceutical companies have jumped to create drugs to make use of this fact. tissue damage- a very vicious cycle. From there it is an easy step to the many other inflammatory diseases that haunt our society today; hypertension, obesity, high cholesterol, diabetes, arthritis, lupus, and many other syndromes. For these millions of people who suffer from inflammatory disorders, a heightened nitric oxide effect from a substance such as the proprietary blend of Noni extracts could be extremely beneficial. This is called peristalsis, and is what makes food move smoothly along the intestinal tract being absorbed as nutrients, and finally being expelled as waste. It also helps to control the relaxation of muscle along the intestinal walls, and the function of the valves, called sphincters at the bottom of the esophagus, at the bottom of the stomach where it joins the duodenum, and the ileocecal valve where the small intestine meets the large. So people who suffer from spasms of these valves, or those with IBS, Irritable Bowel Syndrome, may benefit from a heightened level of NO in the body. Once again, many of the people taking the proprietary blend of Noni extracts find that they have improved digestion reduced constipation. It seems to be one of the first things to improve, along with a general sense of wellbeing and improved energy.

NO AND THE IMMUNE SYSTEM Another main function of nitric oxide in the body is its contribution to the immune response. NO acts as a powerful weapon against infections. As we saw earlier in the section on NO and digestion, various pathogens (bacteria and viruses) can enter our bodies through the digestive tract with our food intake. By taking the proprietary blend of Noni extracts, nitric oxide is released in the stomach which can kill the bacteria and other pathogens that may have been swallowed in our food.

NO AND THE PREVENTION OF CANCER Key players of your immune system, your white blood cells, use nitric oxide to defend against cancerous tumors. Several scientific studies show that nitric oxide can inhibit cancer cells, and there is much research being conducted with an eye to creating drugs that will release nitric oxide releasing agents as a tumor defense mechanism.14 15 16

An exciting study was conducted by researchers from the University of Hawaii led by Annie Hirazumi. Mice with the deadly Lewis Lung Carcinoma were treated with Noni extract, and subsequently their life span was significantly increased between 105%-123%. The experiment was repeated on a different lot of mice with similar results. The researchers concluded that the Noni juice “...seems to act indirectly by enhancing host immune system involving macrophages and/or lymphocytes.”17 18

Another cancer trial published in the cancer journal, Angiogenesis, looked at the effects of Noni juice on tumors; it was found that Noni was highly effective in reducing both the growth rate and the proliferation of malignant tumors.19

Cancer is one of the leading causes of death in America today, and one in four Americans will have some form of cancer in his or her lifetime. NO AND INSOMNIA Insomnia is an increasingly common and debilitating problem for many people. They may have difficulty falling asleep, may wake up often during the night, wake too early, or feel that their sleep is just not refreshing. Unfortunately, according to the National Sleep Foundations nationwide polls, 58% of adults experience symptoms of insomnia a few nights or more per week, 29% experience it every night or almost every night, and 68% report that sleepiness interferes with their concentration and makes handling stress on the job more difficult. Frighteningly, 19% report that they have dozed off while driving during the past year. So insomnia is becoming a chronic problem that has a major impact on how people function during the day. It can cause irritability, poor judgment, and as you can see, can increase the risk of accidents. It can also worsen existing medical problems and lead to new ones, since so much physical and mental rejuvenation takes place when we are sleeping. Once again, adequate levels of nitric oxide may help- it has been shown that nitric oxide plays an important part in the initiation and continuation of good sleep. Various studies have concluded that when nitric oxide formation is suppressed or interfered with, normal sleep mechanisms are also suppressed.20 21

NO AND DIABETES According to the American Diabetes Association, there are approximately 20 million Americans who have diabetes, which is around 7% of the population. And the percentage has been growing, unfortunately, in parallel with the increase in obesity which plagues our society. Research has consistently shown that people suffering diabetes have a reduced amount of nitric oxide in their bodies.

In both Type 1 and Type 2 diabetes blood and urine tests show reduced levels of nitrates and nitrites, which indicate a lessened ability to generate NO. This leads to poor circulation, including heart disease, kidney problems, damage to the retinas of the eyes, loss of feeling in fingers and toes (peripheral neuropathy), and slower or otherwise impaired wound healing. The reason why NO is under produced in diabetics is not completely understood by scientists and researchers at this point. One possibility may be that diabetes leads to an accumulation of acid molecules, which is called acidosis, in the body. This is due to the fact that the main dysfunction in diabetes is a reduced ability to process sugar into energy. The result is too much acid in the body. Acid cells tend to suppress the production of nitric oxide, or use up available stores of nitric oxide.22

Another possibility for NO underproduction is that diabetics may have less NO in their systems due to a destructive process caused by the diabetes itself, which causes oxygen radicals to destroy or inactivate NO at an accelerated rate.23 Or it may be the fact that those with diabetes seem to be less able to generate nitric oxide from L-arginine. It may even be that low L-arginine intake in the diet or lack of exercise leading to low oxygen in the body may contribute to the diabetic’s reduced NO state. Or it may be all of the above factors combined. In any event, many people with diabetes struggle with many health issues. One is relating to the nervous system. Diabetics tend to experience a loss of sensation, a phenomena that is caused directly by an abnormality in nerve function. The damage to nerves, leading to a loss of nerve function, is in great part due to poor circulation caused by decreased production of, or loss of, NO. One example of this is in the diabetic patient’s digestive track. NO’s role as an important signaling molecule makes it vital

to the messaging system which controls smooth muscle cells. This regulates the movement of food through the digestive track, called intestinal motility, or peristalsis. It is the reduced levels of NO in these patients that may be the cause of nerve dysfunction in the stomach, called gastroparesis, that delays or stops stomach emptying. The unpleasant and uncomfortable result for the diabetic is nausea, vomiting, bloating, and weight loss. They need increased levels of available NO to bring greater blood flow, with essential oxygen and nutrients, to those vulnerable nerve cells.

Since diabetics have a lessened ability to generate NO from L-arginine, simply adding more arginine-rich foods to the diet does not help. It is powerful new knowledge that the proprietary extracts of Noni can, without depending on L-arginine, directly supply high levels of nitric oxide to the diabetic patient. Finally, if that wasn’t enough for the diabetic patient, nitric oxide also mimics insulin action in moving glucose out of the blood and into cells.24 One could speculate that perhaps nitric oxide intake via the proprietary blend of Noni extracts might lead to a lessened need for as much drug usage for the control of blood sugars. I look forward to hearing from more diabetic patients in the future.

revealing some of the most exciting advances of knowledge in biology. NO is distributed everywhere throughout the brain, and may have an involvement in almost all of its normal physiological functions. It is unlike any known neurotransmitter; unstable, and seems to be produced when needed by neurons. As we have seen earlier, acting as a messenger between cells is one of its primary functions throughout the body, but this is especially true in the brain, where there is more of the enzyme that creates NO, nitric oxide synthase, than in any other organ of the body.25

Many studies are being conducted with regard to NO’s brain and nervous system connection, and researchers are suspecting that NO may turn out to be the molecule that is at the basis of learning and memory. Simply put, a cell in the brain is responsible for releasing a chemical messenger called glutamate, which stimulates another cell, a receptor cell, with this chemical to release nitric oxide. If it is strongly stimulated, the receptor sends back an NO molecule to tell the sender that the message was received, and asks it to send an even stronger message next time. This cycle is known as “long term potentization”, a fascinating and normal path that creates memory. NO is also certainly involved in behavior, as it is the neurotransmitter found in a number of nerve pathways within brain regions that regulate the emotions. In a very interesting study conducted at Johns Hopkins University, mice were specially bred without the gene that enables them to make nitric oxide in the brain cells. These mice turned out to exhibit dramatically more sexual and aggressive behavior than normal mice. Because this behavior was heightened, it appears that removing the gene causes a loss of social inhibition. One of the co-researchers of this study, Dr. Solomon Snyder, director of the Department of Neuroscience at Johns Hopkins, said, “… nitric oxide may be the neurotransmitter that puts a brake on our social behavior.

The highly aggressive behavior in mice whose brains cannot make nitric oxide appears to be the most pronounced change in aggression ever associated with a neurotransmitter.”26 27

In this same study, it was noted that there were emotional changes to these altered mice, with regard to fear. When confronted with a situation that would cause a normal mouse to be hesitant, the study mice acted fearlessly. The findings could, of course, have significant implications for studying human aggressive and violent behavior, but much more research is needed. As well as looking at NO’s involvement in normal brain function, scientists are also investigating its involvement in pathology or disease. Problems in the brain that seem to be related in some way to nitric acid production are schizophrenia, multiple sclerosis, and strokes. Since nitric oxide can prolong cell life, it may be an effective way to treat certain diseases where cells die prematurely, such as Parkinson’s disease and Alzheimer’s disease. I regularly recommend that my patients take the proprietary blend of Noni extracts, and it is wonderful for me to see them feeling better. I often see improvements in brain function such as learning and memory. How exciting it is that this source of beneficial nitric oxide has been discovered and made available to all of us.

NO AND WOUND AND FRACTURE HEALING For wounds or fractures to heal properly, there must be an adequate supply of NO, which regulates cell division and proliferation necessary for tissue repair, as well as ensures that cells mature, and collagen is synthesized.28

Let me explain just a little of how this works. When NO stimulates cell division, one cell becomes two, two cells become

four, etc. There are 10 to 20 growth factors that are involved in this process, and several of these simply can not complete their function with out nitric oxide. Besides NO’s importance to cell proliferation, it is also involved in making sure that each new cell is identical to its parent.

As cells proliferate, they must become mature cells capable of responding to external signals. If there is not an adequate supply of NO for these processes, normal maturation does not occur, so a cell cannot recognize, process and transmit information from outside of the cell to the cell interior. The bottom line is that wound healing cannot occur.

Nitric oxide also protects the wound while it is healing, by effectively reducing the number of dangerous cells including bacteria and fungi which may infect the wound. In this way nitric oxide may promote the health of certain cells and tissue. An interesting study with wounds of diabetic patients found not only that nitric oxide production is reduced in those with non-healing wounds, but that healing would only occur when that deficiency of NO was corrected. There proved to a critical level of nitric oxide production that is essential for healing.29

NO is also involved in the healing of bones. Researchers at the Orthopedic Research Institute, at the University of New South Wales in Australia found that after a fracture, NO activity was found in the broken area, rising to a peak on the 15th day after the break. They double checked this finding, wishing to ascertain the dependence of healing on the nitric oxide, by

using an inhibitor of NO. A significant decrease in healing was observed in the rats that had been given the inhibitor. These results show for the first time that NO is generated in the body when a fracture takes place and that supplementation of NO can reverse the slowness or inhibition of healing produced by NO inhibitors.30

Studies testing substances that increase the amount of NO have shown an increase in bone mass in experimental animals, and early evidence suggests that the same applies in humans. Work with NO is creating promising methods of treating bone fractures, and also bone diseases such as osteoporosis.31 32

NO AND TRAUMA As you’ve seen throughout this book, nitric oxide has powerful benefits for many chronic disease conditions common to man. But it also offers benefits for acute problems such as trauma treatment, as well. NO is used in the intensive care hospital setting to give significant improvement to patients in respiratory distress, and it even saves lives. In another example, treating Down’s syndrome babies with nitric oxide before and after cardiac surgery gives much better results than using simple oxygen.

FINAL THOUGHTS Research into all the potential clinical applications of nitric oxide is still in early days, but is extremely promising, and has come a long way in a relatively short time. With NO’s involvement in so many aspects of physiological function, everything from blood pressure to sexual health, this unstable gas, this tiny simple molecule, is one of the bright stars on the horizon for drug companies around the world. They are in a race to create drugs that will treat a whole gamut of human disease. Some of these will hopefully be useful and indeed beneficial in extreme circumstances. But unfortunately, in too many instances pharmaceutical companies look for a synthetic drug that they can patent, which usually comes with a host of side effects, some dangerous, as I am sure you are only too aware. But those of us who know about Noni, and its proprietary blend of extracts, are in a fortunate position. We can ingest a natural, gentle plant substance which generates healthful levels of nitric oxide in our bodies, to benefit from the numerous protective and healing qualities of NO. He is one of the very few physicians in the United Sates who has been awarded an honorary Sc.D. from the World Health Organization Charted University for his work in promoting alternative medicine. Dr Qutab travels extensively and has taught thousands of physicians in the United States and Europe on how to integrate alternative medicine into their practices.

Dr. Qutab is board-certified in naturopathic medicine and has received several leadership awards for promoting complementary medicine. He is on the advisory board of the Congressional Health Care Reforms Committee in Washington, D.C. and in 2003 received the Physician of the Year award. Dr. Qutab has also extensively researched nitric oxide and all of its functions in the body. He has co-authored several books on Ayurvedic medicine.
