Advanced Spine and Posture will provide reasonable accessibility and other accommodations to individuals with disabilities and/or communication limitations, as required by law. Facility Access All Advanced Spine and Posture facilities are wheelchair accessible and designed with open floor plans to facilitate full-time supervision. Team members are trained and available to provide additional support, such as escorting patients to and from their car, holding doors open, sitting knee-to-knee and helping patients to complete in-office forms, and making phone calls to coordinate outside care or arrangements. Language Accommodations Spanish-language forms and Spanish-speaking employees are available at every Advanced Spine and Posture location. Other translation needs or requests will be addressed on a case-by- case basis, utilizing telephonic translation services if necessary. A Language Identification Guide is available to assist individuals with limited English proficiency. Disability Accommodations Hearing Impairment: All healthcare findings, treatment plans, financial information, and recommendations are provided in writing. Written communication and/or non-verbal communication is utilized on the treatment floor. If necessary, in-person or video ASL interpretation can be arranged upon request. Email communication is available at [email protected]. Visual Impairment: Large-print copies of documents are available, and/or documents are read aloud. Patients are provided any necessary personal assistance in navigating the clinic and utilizing equipment. Other accommodation needs or requests will be addressed on a case-by-case basis.
