HousingWelcome to Module 3 of the Nursing Home Transition (NHT) training series. This training series is intended for NHT Coordinators and Supervisors.

The purpose of this module is to provide an overview of the housing component of the Nursing Home Transition (NHT) Program. In addition, this course identifies current resources for participants who wish to further access detailed NHT housing information.

ObjectivesAfter completing this module, you will be able to:

Describe the barriers to accessing housing. Identify the housing resources that are available for Nursing Home Transition. Describe the role of Regional Housing Coordinators (RHCs). Explain Tenant-based Rental Assistance (TBRA) and the general role of the

Pennsylvania Housing Finance Agency (PHFA) in TBRA. Explain 811 Project Rental Assistance (PRA) program. List types of home adaptations. Explain what funding sources may be available for home adaptations. Explain how to utilize Home Maintenance Deductions.

Housing BarriersLet’s begin this module by first exploring some of the barriers that prevent individuals from accessing housing.

Barriers to HousingWhen working with people on their transition out of a nursing home, you may run into many barriers in trying to locate or secure housing for them. The following lists important information regarding barriers when assisting individuals to transition out of a nursing home.

General Barriers to TransitionFinding affordable and accessible housing may be a barrier to transition. Another barrier may be locating appropriate housing in a specific area the NHT participant wants to reside in because he or she wants to be closer to conveniences, family and informal supports, medical care, transportation, and/or other services.

Many times, there is a waiting list involved when working with the Housing Authority. In some areas of the state, it is very difficult to locate housing due to issues that are specific to the community, such as natural gas drilling in some parts of PA.

Individual Backgrounds That Present BarriersOnce housing is located, you could encounter individuals who have backgrounds that present barriers to securing a home that meets their needs and preferences. For instance:

Some people have encountered extraordinary medical expenses that wiped out their savings and left them in debt or with a negative credit history.

Others may have been unable to keep up with the expenses of their previous home while staying in the nursing home.

Some may have a criminal record. Some may have lost or misplaced personal identification or other important

documents.

Additional BarriersAlthough in direct violation of established housing standards, including those of the Fair Housing Act, occasionally people face barriers due to their:

Disability. Age. Race. National origin. Family composition. Sexual orientation. Religion.

Need for Home AdaptationsAnother barrier to finding housing is the need for home adaptations. Some existing homes require adaptations, but the NHT participant may not have the funds to make these necessary alterations. There are options to access funding for home adaptations. These options will be explored later in the module.

Housing ResourcesIn the previous section, we focused on barriers to housing. In this next section, we will explore resources for locating housing for NHT participants, including PA Housing Search.

Locating Housing ResourcesFinding affordable rental housing in the private market takes a bit of detective work, but as you gain experience, you will learn the best sources in your community. There are many resources available to assist you in locating affordable, private housing for NHT participants, including:

Landlord associations. Rental agents. Regional Housing Coordinators (RHCs). Public Housing Authority Housing Choice Voucher List. Newspaper listings, circulars, internet. For rent signs. Social Service agencies. Word of mouth. PAHousingSearch.com.

PA Housing SearchNHT participants who are in need of a new residence are frequently referred to PAHousingSearch.com. This resource provides access to information about affordable rental housing opportunities throughout Pennsylvania. Although this information is frequently updated, NHT Coordination Agencies (NHTCAs) should still confirm availability of rental units with the landlord or property manager.

PA Housing Search includes both market-rate rental housing and subsidized rental units.

How are searches organized?PA Housing Search allows you to search by city, county, or zip code for rental housing.

RHC OverviewThe Regional Housing Coordinator (RHC) Program was developed by the Self-Determination Housing Project of Pennsylvania (SDHP) and the Pennsylvania Housing Finance Agency (PHFA). The RHC Program serves as a single point of contact for NHTCAs for technical assistance related to housing and services for individuals with disabilities.

The RHC Program was created to help break down silos and promote better communication between housing and social service systems. This is done by improving information flow between the affordable housing community and service providers.

Role of the RHCRegional Housing Coordinators (RHCs) work in conjunction with state agencies, organizations, housing professionals, and others. A role of the RHC is to facilitate access to housing options and opportunities for adults over 60 and people with disabilities.

Take a moment to learn about the RHC roles and responsibilities.

Role of RHCThe RHCs work with state agencies, organizations, housing professionals, and others to facilitate access to housing options and opportunities for adults over 60 and people with disabilities of all ages. They provide outreach by developing collaborative relationships with groups and agencies in their region that have contact with individuals with disabilities.

RHCs help NHTCAs understand how to locate affordable accessible housing and overcome common barriers preventing access to housing. They also provide technical assistance to solve complicated housing issues.

EducationRHCs educate program administrators, NHT staff, and service providers to assist them with identifying housing resources in their region for older adults and individuals with disabilities.

TrainingRHCs facilitate training sessions and provide technical assistance to groups and agencies about available resources and the housing needs of individuals with disabilities. Related training topics include Home and Community Based Services waivers, home adaptations, landlord communication, and the Prepared Renters Program (PREP).

PREP is a collaborative effort between the Pennsylvania Housing Finance Agency (PHFA) and the Self Determination Housing Project (SDHP). The program was designed to provide individuals with the tools they need to become successful renters.

PREP TrainingPREP starts with the basics of renting by helping individuals determine how much they can afford to spend for their housing, and what to look for in an apartment.

Then, it walks them through the process of applying for housing, including gathering the information needed to submit a rental application.

Next, it provides information about the lease, the rights and responsibilities of the tenant and landlord, and strategies to address such barriers as poor credit or a criminal record.

PREP Training TopicsPREP training focuses on topics that renters need to think about before they start looking for housing. It helps them anticipate the challenges they might face based on their personal circumstances.

PREP training topics include:

Assessing housing needs. Understanding the language of a lease. Communicating with a landlord. Creating and working within a budget. Providing financial education. Helping to resolve credit issues. Understanding a difficult housing market. Choosing affordable housing in a desirable location. Overcoming a criminal history. Identifying housing discrimination. Communicating with neighbors. Understanding how to be a good neighbor. Avoiding eviction.

The training also includes tips on being a successful tenant once housing is obtained.

Participation in PREPRegional Housing Coordinators offer technical assistance for PREP at no cost to agency professionals. Participating in PREP training will help NHTCAs acquire the knowledge and skills needed to provide guidance to individuals transitioning to the community.

Tenant Based Rental Assistance (TBRA) and 811 Project Rental Assistance (PRA)In this next section, we will explore the Tenant Based Rental Assistance (TBRA) and 811 Project Rental Assistance (PRA) and the role that the Pennsylvania Housing Finance Agency (PHFA) plays in both. In addition, we will more closely examine the TBRA and 811 PRA application processes and eligibility requirements.

The Role of PHFA in TBRAIn order to make the Commonwealth a better place to live while fostering community and economic development, the Pennsylvania Housing Finance Agency (PHFA) provides the capital for decent, safe, and affordable homes for older adults, persons of modest means, and those with special housing needs.

PHFA has partnered with the Office of Long Term Living to create a temporary rent subsidy for income-eligible people transitioning out of nursing homes.

TBRA OverviewTBRA is a statewide program designed to assist with securing housing where affordable housing is difficult to obtain due to waiting lists up to 24 months in length. TBRA is a subsidy provided directly to a household to be used to rent the housing of their choice so long as it is compliant with the acceptable housing quality standards.

Under TBRA, the tenant typically pays a portion of their income toward rent and utilities and the balance of the rent is paid by PHFA, using the appropriate source of funds. This rental assistance is temporary and will be available for approximately two years. It will serve as a bridge only until a permanent housing subsidy becomes available.

A participant must be on one or more waiting lists for a permanent housing subsidy, and when they receive an offer of subsidized housing, their TBRA eligibility ends.

TBRA EligibilityLet’s now turn our attention to TBRA eligibility. TBRA requirements generally relate to the income of the household as well as the condition of the rental unit, its fair market rent, and its location. Take a moment to review the NHT TBRA eligibility requirements.

To be eligible for NHT TBRA, participants must be:

At an income level that makes them eligible for a permanent subsidy. Transitioning out of a nursing facility. A resident of Pennsylvania. Eligible AND on a waiting list for a local Housing Choice Voucher (HCV) or other

subsidized housing.

TBRA Application ProcessIf an individual meets all the TBRA criteria, the next step is to apply for TBRA assistance.

Let’s review the TBRA application process.

Identify Potential ApplicantThe NHT Coordinator identifies a potential applicant and assists them with applying for placement on a subsidized housing waiting list or Housing Choice Voucher (HCV). The NHT Coordinator should contact the Housing Authority or building manager to obtain an estimated wait time for available housing in the applicable county.

TBRA is only a resource for individuals who are able to be placed on a subsidized housing waiting list.

Obtain PHFA TBRA CertificationThe NHT Coordinator obtains the PHFA TBRA Certification Questionnaire from either PHFA or the County Housing Authority. The NHT Coordinator works with the participant to collect the necessary information to complete the form.

Since procedures vary by county, contact PHFA for additional assistance when completing your first TBRA application.

Complete PHFA TBRA CertificationThe NHT Coordinator completes the PHFA TBRA Certification Questionnaire and submits it to PHFA along with other required information. PHFA reviews the form to determine income and asset eligibility and other housing requirements.

Submit Request for Unit Approval FormIf eligible, PHFA will send a Rental Assistance Packet. The participant and the NHT Coordinator then have 60 days to find housing and submit a Request for Unit Approval Form to PHFA.

Complete Housing Quality Standards Inspection

PHFA will conduct a Housing Quality Standards inspection, review the lease, and determine if the rent is reasonable.

Calculate Rent and TBRA SubsidyPHFA will calculate the applicant’s rent and the TBRA subsidy.

Enter Lease AgreementThe NHT Coordinator helps the applicant and the landlord enter into a lease agreement that includes a PHFA lease addendum and rental assistance contract.

Schedule Annual Inspection and Re-CertificationAt 11 months, if a permanent subsidy has not been found, PHFA will schedule an annual inspection and re-certification.

Begin Permanent Subsidized HousingAt or before 24 months, the TBRA subsidy ends and the participant moves into permanent subsidized housing.

NHT-TBRA One-Time Payment RequestsNHT-TBRA One-Time Payment Requests are for people in a nursing home, ready to transition out of the nursing home and may lose their apartment, etc., if they do not pay their rent.

All counties work directly with PHFA (not the Housing Authority) on the One-Time Payment Requests. The NHT Coordinator fills out the NHT-TBRA Payment Request Form and W-9 form and sends them to PHFA. They are then sent to DHS/OLTL to be approved. Once approved by DHS/OLTL, they are then returned to PHFA with approval for payment.

One example for requesting a One-Time Payment is as follows: Mrs. McCluskey signed her lease on November 1. Due to the extended enrollment process and home adaptations that need to be completed, Mrs. McCluskey will not be able to move in until the end of December. The nursing home is receiving most of her check. She will not receive her full Social Security check in enough time to pay the December’s rent. The One-Time Payment Request could assist Mrs. McCluskey with her situation.

811 PRA OverviewThe Section 811 Project Rental Assistance (PRA) program is a collaboration between the Pennsylvania Housing Finance Agency (PHFA) and the Pennsylvania Department of Human Services (DHS) to provide extremely low-income persons with disabilities access to affordable, integrated, and accessible housing.

811 PRA will provide a project-based rent assistance subsidy. The rental assistance will pay the difference between the approved fair market rent for that apartment and the amount the tenant can affordably pay.

811 PRA EligibilityThe DHS Local Lead Agencies (LLAs) and their stakeholders will conduct outreach, identify potential applicants, screen for eligibility, and refer applicants to vacant apartments.

Take a moment to review the 811 PRA eligibility requirements.

To be eligible for 811 PRA:

At least one person in the household must be non-elderly (age 18 to 61), disabled, and receiving or be eligible to receive Medicaid (long-term services and supports).

The gross household income must be at or below 30% Area Median Income (extremely low income).

811 PRA PrioritiesThe 811 PRA program was developed to primarily assist adults with Autism, intellectual and physical disabilities, and/or serious mental illnesses and transitional age youth with disabilities. These groups are then prioritized based on the setting in which the applicant currently lives.

Let’s review these priorities.

Priority 1Priority 1 individuals are institutionalized but able to live in the community in permanent supportive housing. This includes, but is not limited to:

Private and public psychiatric hospitals. Nursing facilities. Facilities for those with intellectual disabilities.

Priority 2Priority 2 individuals are at-risk of institutionalization with no permanent supportive housing. This includes, but is not limited to:

People who are living with caregivers in unstable situations. The homeless. People aging out of the Early and Periodic Screening, Diagnosis, and Treatment

Program with no family supports. Individuals aging out of foster care.

Priority 3Priority 3 individuals are in congregate care settings and desire to live in the community. This includes, but is not limited to persons in:

Community residential rehabilitation facilities. Long-term structured residences. Personal care homes. Domiciliary care.

811 PRA Eligible PropertiesAn eligible multifamily property can be any new or existing property funded with federal HOME and/or Low Income Housing Tax Credit program with at least five housing units. Properties must be fully integrated. Units must meet the program criteria for integration, accessibility, and access to transportation and services.

Home AdaptationsIn this next section, we will explore types of home adaptations that may need to be made and various sources of funding that can be used to cover the cost of these adaptations.

Types of Home AdaptationsIn some cases, NHT participants may have housing that does not meet their current needs. For instance, if an individual has become wheelchair dependent, their former home may not have wide enough doorways or an accessible entrance.

There are many adaptations that can be done to an existing home or apartment to make it safe and functional, such as installing a ramp or stair glide or widening doorways.

Home adaptations can range from involved projects that require outside funding, like adjusting the height of counters and sinks in the bathroom or kitchen, to simple adaptations to existing features, such as changing knobs to handles that are easier to grip.

Funding SourcesThere are options to access funding for home adaptations if an individual does not have the funds necessary to make adaptations. If the individual is eligible, funds from a Home and Community Based Services (HCBS) waiver could cover home adaptation costs at an existing residence.

If the individual is not waiver eligible, he or she may apply for a low interest loan through the Pennsylvania Housing Finance Agency (PHFA), the PA Assistive Technology Foundation (PATF), or the United States Department of Agriculture (USDA). Assistance may also be available through the Veterans Administration, or other programs such as Family Caregiver Support Program (FCSP), Options, or Special Nursing Home Transition Funds (SNHTF).

HCBS Waiver FundingThe Medicaid Home and Community Based Services (HCBS) waiver program provides funding for services that assist eligible individuals to live in the community.

Individuals who qualify for an HCBS waiver receive services in their home or in other community-based settings in order to help them live independently in the community. Waiver services often include covering the cost of home adaptations.

Family Caregiver Support Program (FCSP) FundingThe Family Caregiver Support Program (FCSP) provides assistance to family and friends caring for a loved one, 60 years of age and older, who help with daily activities such as meal preparation, light housekeeping, grooming and personal care.

OverviewIndividuals who are admitted to a nursing facility can apply for the home maintenance deduction to allow them to maintain their existing community housing, secure new housing, or pay for transition expenses. Applying for the Home Maintenance DeductionSo how does someone apply for the Home Maintenance Deduction?

Individuals who will be realistically discharged from a nursing facility within 6 months, unless SSI eligible, can apply for the Home Maintenance Deduction. The Home Maintenance Deduction can be granted more than once as it renews with new nursing home admissions from the community.

Home Maintenance Deductions are approved by County Assistance Offices (CAOs). If a CAO approves an individual for the Home Maintenance Deduction, that individual can keep a portion of his or her income to maintain or establish housing or pay for other bills

or services that may be helpful toward the individual’s goal of discharge into the community.

A new MA-51 must be completed as part of the process if the person is noted as long term. The physician must fill out the form stating that the individual is nursing facility clinically eligible, short-term, and will be discharged within 180 days. A nursing facility social worker or admission coordinator can help the NHT participant with the Home Maintenance Deduction application process.

Suggested UsesOnce approved for a Home Maintenance Deduction, an NHT participant may use his or her Home Maintenance Deduction for many purposes, including the following:

Saving up for the first month’s rent in an apartment. Saving up to pay cash up front to turn utilities back on. Paying fees to secure needed forms of identification that may have been lost. Paying off back utility bills or debts, such as outstanding medical expenses that

may prevent a successful transition. Purchasing any items necessary to maintain or establish a home and set up a

household.
