APPLICATION FORM Full Terms and Conditions OAKWELL CENTENARY SOCIETY 1. These Terms and Conditions are the Lottery rules. By entering the lottery, entrants agree to be bound by these rules. 2. This lottery is being promoted by and on behalf of Barnsley Football Club to assist in the creation of amenities and facilities for sport and in particular the support of youth development. 3. For the purpose of the Gambling Act 2005, this lottery is defined as a ‘Society Lottery.’ 4. This lottery is licensed to Barnsley Football Club 5. All tickets will be priced at £2 per week 6. An entry in the competition is only valid if the subscription has been paid for the appropriate weeks draw. The entry must be correctly marked paid for the appropriate weeks draw in the society’s computer records at the clubs offices at the time of the draw 7. It is the responsibility of members to ensure that the correct name and address has been recorded against their membership and reference numbers and where appropriate their agent has recorded their entry as paid for the appropriate weeks draw. 8. An entry not correctly marked, or left unmarked, will count as unsold and cannot win a prize. 9. All members must be registered by name and address, Non-de-plumes will not be accepted. 10. All numbers are issues by the promoter, on behalf of the society any alteration of numbers shown on membership cards will be considered void. 11. All prizes will be paid direct to winners unless alternative have been made. No claims are required – winners will be paid automatically. A result sheet showing winning entries will be published each week. No responsibility can be accepted for any error, misprint or omission. 12. The draw will be made each week at a time and place arranged by the society. This will be the date of the lottery (as define in the 1976 lotteries and Amusement act/Gambling Commission Act 2005). Details of winners will be announced on a weekly basis during the week following the draw, all members may, if they wish, attend the draw and may be invited to participate in the draw. 13. Prizes will be determined by the society in line with the gambling Commission Act 2005. 14. Additional prizes and consolation prizes may be paid from time to time at the discretion of the society. 15. Agents are considered to be acting on behalf of members. No responsibility can be accepted for any loss caused by the default, neglect or error of any agent. 16. The competition is open to any person of 16 years of age and over. 17. The decision of the promoter will be final and binding in respect of any dispute or difference arising out of the activities of the lottery. 18. New members will be sent a welcome letter advising them of their unique personal membership number. 19. Members can pay by cash, cheque or standing order 20. All lottery entry sales are final and no refunds shall be made at any time. All entrants acknowledge that their payment of £2 per ticket to enter the lottery does not guarantee that they will win any prize. 21. If a cash prize is awarded, this will be made by cheque in the name of the entrant only. 22. Full payment for each ticket must be received either in the form of cash or cleared funds before the ticket can be entered into the draw. Only tickets for which full payment has been received either in the form of cash or cleared funds are eligible to win the prize. 23. All entrants are solely responsible for providing Barnsley Football Club lottery department with their accurate and up-to-date contact details and Barnsley Football Club will be in no way liable for the failure or inability to contact any entrant due to any errors, omissions or inaccuracies in the contact details that the entrant has provided. In the event that an entrant changed their contact details, they will be solely responsible for advising the lottery department of the change. 24. Each game is unique. 25. Lottery membership can be cancelled at any time by notifying The Lottery dept. in writing, by phone or via e-mail. 26. Any complaints relating to the lottery should be sent in writing to Barnsley Football Club lotteries dept giving details of the complaint and supporting documentation. 27. Barnsley Football Club is committed to protecting the member’s privacy. Data that is collected from the member is used lawfully in accordance with the Data Protection act 1998 and is used solely for the purpose of processing lottery chances, subsequent entry into the lottery draw, and informing winners that they have won a prize. 28. Barnsley Football Club will not sell, rent or grant access of any personal data to any third party without express prior permission. 29. In the event of any dispute regarding the rules, the decision of Barnsley Football Club shall be final and no correspondence or discussion shall be entered into. 30. Barnsley Football Club reserves the right to amend these rules at any time. If Barnsley Football club does this, it will publish the amended rules. 31. Barnsley Football Club reserves the right to disqualify any entrant if it has reasonable grounds to believe the entrant has breached any of these rules. 32.
