Greetings from the 20th CTO Club

Recent developments of devices and technical approaches such as the retrograde approach and antegrade dissection re-entry (ADR) have led to a greatly improved success rate for recanalization of chronic total occlusion (CTO). Also, a new algorithm for crossing CTO has been proposed from the Asia Pacific CTO club. The algorithm can help to choose the appropriate strategic approach to CTO PCI and lead to success for recanalization of CTO even in non-expert CTO operators. However, when a difficult case (J-CTO score≧2) is suggested for non-expert CTO operators, it is best to either perform the case with a proctor or refer the case to an expert CTO operator because more depth insight and theoretical procedure are required for success in CTO PCI.This year marks the 20th anniversary of the CTO Club. The CTO Club has provided the best knowledge and procedure for complex CTO lesions via live demonstrations by Japanese CTO expert operators for 20 years. In recent years, CTO-PCI procedures have been proactively performed worldwide. Joint sessions have been also held with CTO clubs from other countries. The Chinese language live course was extremely popular last year, hence, this live course will be held this year too.The procedure and methodology of CTO PCI are becoming a mature stage, and the success rate for recanalization of CTO is now beyond 90% according to data from the Japanese Expert Registry. However, there are still some issues that need to technically overcome complex CTO lesions, such as heavily calcified lesions, tortuous lesions and long lesions. The CTO Club will continue to confront and overcome these issues with experts from all over the world for further development. We are looking forward to seeing you at the 20th CTO Club in 2019.

Fireside Session: Proctorship LiveIn this Proctorship Live demonstration, the less experienced physician will act as the first operator, while the expert physician will act as the second operator. The less experienced, younger physician, will of course have a strong desire to master CTO procedures and therefore will engage directly in the case. Meanwhile, the expert physician will remain close by to advise throughout the procedure. By having the younger physician engage directly in the procedure, he or she will be able to acquire the tips, tricks and techniques to complete the case successfully, and be educated on the common pitfalls unique to young operators. Our goal is to share CTO knowledge, and teach techniques for successfully completing complex CTO cases with high proficiency and safety.

New Device Session “Global intelligence for advanced ADR”Japanese CTO-PCI strategy has been reaching the highest state of development with the own evolution of CTO techniques. However, recanalization of some CTO lesion subsets remains still challenging and requires higher skill sets. Special live session: Quality of CTO-PCI: TIME TO GLOBALIZE

The physicians in Japan, mainly expert operators, have now made it possible to exceed the initial success rate of CTO-PCI 90%. However, this fact has simultaneously proved that approximately 10% of cases are difficult to treat. The approach to CTOs has changed with an increased number of devices that we can select. Since the Corsair, which was specializing in passing interventional collaterals, was developed approximately 10 years ago, the Retrograde approach has gradually become popular. As a result, it has increased treatment options for long CTOs and others without entry that were difficult with the Antegrade approach. At the same time, many CTO registries have increased both in Japan and overseas, in particular in Japan, the standardization of the CTO procedures, including the Retrograde approach, has been developed. Besides, in recent years, the importance of the Antegrade approach has been reviewed, with the support of the development of Gaia and Gaia Next, which emphasize wire direction control within the CTOs. This is caused by a failure factor of the Retrograde approach that has reached its limit of approach to CTOs after passing through collateral channels. We, once again, recognize that it is necessary to improve technology of the wiring itself to CTOs, and we may be entering an era of improving the quality of CTO-PCI such as lowering complications by maximizing the benefits of PCI. In this session, we will provide a place for participants to be able to learn the CTO experts’ techniques in Japan, featuring on the cases that are even now predicted to be difficult to treat. As a technical aspect, we will also provide lectures on the redefinition of R-CART from Japan and the latest abroad activities.

Greetings from the 20th CTO Club

Recent developments of devices and technical approaches such as the retrograde approach and antegrade dissection re-entry (ADR) have led to a greatly improved success rate for recanalization of chronic total occlusion (CTO). Also, a new algorithm for crossing CTO has been proposed from the Asia Pacific CTO club. The algorithm can help to choose the appropriate strategic approach to CTO PCI and lead to success for recanalization of CTO even in non-expert CTO operators. However, when a difficult case (J-CTO score≧2) is suggested for non-expert CTO operators, it is best to either perform the case with a proctor or refer the case to an expert CTO operator because more depth insight and theoretical procedure are required for success in CTO PCI.This year marks the 20th anniversary of the CTO Club. The CTO Club has provided the best knowledge and procedure for complex CTO lesions via live demonstrations by Japanese CTO expert operators for 20 years. In recent years, CTO-PCI procedures have been proactively performed worldwide. Joint sessions have been also held with CTO clubs from other countries. The Chinese language live course was extremely popular last year, hence, this live course will be held this year too.The procedure and methodology of CTO PCI are becoming a mature stage, and the success rate for recanalization of CTO is now beyond 90% according to data from the Japanese Expert Registry. However, there are still some issues that need to technically overcome complex CTO lesions, such as heavily calcified lesions, tortuous lesions and long lesions. The CTO Club will continue to confront and overcome these issues with experts from all over the world for further development. We are looking forward to seeing you at the 20th CTO Club in 2019.
