Section 1 – Introduction

This directory provides a list of ODS Advantage PPO’s network providers. To get detailed information about your health care coverage, please see your Evidence of Coverage.

The “network providers” listed in this directory have agreed to provide you with your health care coverage. You may go to any of our network providers listed in this directory. If you have been going to one network provider, you are not required to continue going to that same provider. In some cases, you may get covered services from non-network providers.

Other physicians/providers are available in our network. If you would like a list of providers available in other counties that are not included in this directory you can visit www.odscompanies.com/odsadvantage or call our Member Services Department at 877-299-9062 from 7a.m. to 8 p.m. Pacific time, seven days a week. TTY users should call 711.

How to get care from out-of-network providers As a member of our plan, you can choose to receive care from out-of-network providers. Our plan will cover services from either in-network or out-of-network Medicare providers, as long as the services are covered benefits and are medically necessary. However, with the exception of emergencies or urgent care, if you use an out-of-network provider, your share of the costs for your covered services may be higher. Here are other important things to know about using out-of-network Medicare providers:

At times you may get a bill from the provider asking for payment that you think you do not owe. Send us this bill, along with documentation of any payments you have already made. It’s a good idea to make a copy of your bill and receipts for your records.

o If the provider is owed anything, we will pay the provider directly. o If you have already paid more than your share of the cost of the service, we will

determine how much you owed and pay you back for our share of the cost.

• Please note: While you can get your care from an out-of-network provider, the provider must participate in Medicare. We cannot pay a provider who has decided not to participate in Medicare. You will be responsible for the full cost of the services you receive. Check with your provider before receiving services to confirm that they have not opted out of Medicare.

What services will your provider need to get prior authorization from the plan? Prior authorization is approval in advance to get services. Some in-network medical services are covered only if your provider gets prior authorization from our plan. You do not need prior authorization to obtain out of network services or emergency services. Covered services that need prior authorization are listed in the Evidence of Coverage.

How to get covered services when you have an emergency

What is a “medical emergency” and what should you do if you have one?When you have a “medical emergency,” you believe that your health is in serious danger. A medical emergency can include severe pain, a bad injury, a sudden illness, or a medical condition that is quickly getting much worse.

If you have a medical emergency:

• Get help as quickly as possible. Call 911 for help or go to the nearest emergency room, hospital, or urgent care center. Call for an ambulance if you need it. You do not need to get approval or a referral first from your PCP.

What is covered if you have a medical emergency?You may get covered emergency medical care whenever you need it, anywhere in the United States. Our plan covers emergency services Worldwide. Our plan covers ambulance services in situations where getting to the emergency room in any other way could endanger your health. For more information, see the Evidence of Coverage.

If you have an emergency, we will talk with the doctors who are giving you emergency care to help manage and follow up on your care. The doctors who are giving you emergency care will decide when your condition is stable and the medical emergency is over.

After the emergency is over you are entitled to follow-up care to be sure your condition continues to be stable. Your follow-up care will be covered by our plan.
