国籍を記入してください。

※台湾，香港，マカオ国籍の場合

→台湾，中国（香港），中国（マカオ）と記入してくだ

さい。Write your country.

*Taiwan, Hong Kong, or Macau Nationality Holders

→ Write"Taiwan" ,"China (Hong Kong)" or "China (Macau)".

写真の裏面に氏名を記入すること。Write your name on the back side of your photo.

氏名をパスポートの通りに英語（大文字)で記

入してください。Write your name in capital letter as written in your passport.

母国の住所を記入してください。Write the address in your home town in alphabet.

年収欄は換金レートも記入してください。Write the exchange rate in Annual income column.

経費支弁者が年収がない場合は，職業欄に「預金残

高」と記入してください。年収欄には預金残高を記入し

てください。When your supporter does not have annual income, write "Deposit balance" in Occupation. And write his/her deposit balance in Annual income.

本人負担を選択した場合は，ご自身の情報も記入して

ください。When you select "self" in No.26 (1), write your information too.

《For students who are in Meiji University》Please calculate your lesson hours. (100 min per class.)

《Example》If you take 12 classes per week, you

can write "20 hours" ( 12 classes × 100

min÷60min).
