多剤服用高齢者における,薬物療法の適正化による服用薬剤数の
減少効果に関する系統的文献レビュー

Polypharmacy, the use of multiple medications, is commonly prescribed in the elderly but leads to reduced compli-ance with drug treatment regimens and increased risk of adverse drug reactions. This study was performed to systemati-cally review the results of previous studies to assess the eŠects of interventions to improve prescription quality on reduc-tion of the number of medications in elderly patients with polypharmacy, and to determine the most eŠective types of in-tervention in such cases. Relevant articles in the English language literature were retrieved by keyword searches on MED-LINE, the Cochrane Library, and cited references. The criteria for inclusion in this review were as follows: 1) studies inelderly subjects taking multiple medications or frail elderly subjects assumed to be taking multiple medications; 2) studyinterventions were intended to improve quality of drug use; 3) changes in the number of medications prescribed duringthe intervention period were reported; 4) the study designs were controlled clinical studies. Twenty-seven articles, in-cluding 28 controlled studies, matched all the inclusion criteria. The interventions in the studies included in the reviewwere categorized into two groups: a medication review by medical professionals (26 studies); and a request to prescrib-ing physicians for re-evaluation of the drug use for their patients (2 studies). Medication reviews by medical profes-sionals, mainly pharmacists, resulted in a signiˆcant reduction of prescribed drugs (median, 0.45 drugs; 95%CI, 0.110.76). The diŠerences in eŠects among intervention methods could not be investigated because of a lack of diversity inthe methods used.

背 景
高齢者は複数の疾患をかかえていたり,複数の診
療科や医療機関を受診したりするため,複数の薬剤
を服用している割合が高く,10 種類以上を服薬し
ている場合もある.1)しかし服用薬剤の種類が増え
るに従って,服薬コンプライアンスは低下し,薬剤
の相互作用や有害事象,さらには医療事故が発生し
易くなり,コストが増加する.2)したがって,医師
は患者に対する投薬について,定期的に処方を見直
し,漫然とした処方は避けなければならない.しか
しながらそういった定期的な処方の見直しはかなら
ずしも十分に行われていない.3)
欧米では,1980 年代後半から多剤服用している
高齢者に対して,薬剤師が処方をレビューし,不適
切な投薬の中止など,薬物療法の適正化を図ろうと
する介入の効果に関する報告がなされている.近年
それら報告の系統的レビュー4)がなされ,薬剤師に
よる処方レビューは高齢者の服用薬剤数をわずかで
はあるが減少させていた.一方で,医師による処方
レビューや,処方を行う医師への介入など,薬剤師
による処方レビュー以外の効果を検証した報告も散
見されるが,どのような介入であれば効果があるの
か,知られてはいない.また薬剤師による介入と,
他の介入との効果の比較は行われていない.
そこで,多剤服用している高齢者の,処方を改善
しようとする介入の効果を検証した研究を系統的に
レビューし,どのような介入を用いた研究が行われ

ているか,またそれらの介入により処方薬剤数はど
の程度減少し,どの介入がより効果が大きいかを明
らかにすることを試みた.
方 法
1. 文献検索 検索には,MEDLINE(出版日
Dec. 2006 まで)と Cochrane Library (2006, Issue4)
を用いた.検索用語として polypharmacy, aged, el-
derly, drug utilization, medication review を用い,
Humans, Clinical Trial, English で限定した.適格と
判断された論文や,総説のリファレンス検索も行っ
た.
2. 文献選択 適格規準 inclusion criteria とし
て,以下の規準を用いた.
研究対象が高齢者であること,もしくは研究対
象に高齢者が含まれており,高齢者に対する結
果が報告されていること
多剤服用患者であること(各原著論文の定義に
よる),もしくは多剤服用が想定される虚弱高
齢者 frail elderly であること
多剤服用している状態に対して,処方を改善し
ようとする介入,もしくはその介入を包含する
介入を行っていること
介入による,対象者の服用薬剤数の変化を報告
していること
研究デザインが前向きの比較研究であること
除外規準 exclusion criteria として,以下の規準を
用いた.
特定の疾患や,特定の系統の薬剤に限定した研
究
高齢者が含まれていない研究,もしくは高齢者
における結果を報告していない研究
3. データ抽出 適格と判断された論文から,
以下のデータを抽出した.

間後の服用薬剤数の平均値
両群間における服用薬剤変化数の差,その有意

4. 分析 本研究のアウトカムとして,「(介入
期間前後での介入群の服用薬剤数の減少数)-(介
入期間前後での対照群の服用薬剤数の減少数)」を
求め,“介入の効果”と定義した.文献検索の結果,
介入の効果を定量的に統合するメタアナリシスを行
うのに必要な情報(平均変化値の標準偏差など)が
記載されていない研究が多いと判断したため,メタ
アナリシスは行わず,記述統計のみを行った.
また,研究における属性(後述)により介入の効
果に差がないか,検定を行った.介入の効果は正規
分布に従っていなかったため,Kruskal-Wallis 検定
(正確有意確率)を用いた.介入の効果との関連要
因の ad hoc な探索分析として,介入前の処方薬剤
数との相関を検討した.
統計ソフトは,SPSS 12.0J (SPSS Inc. Chicago,
USA)を用いた.
結 果
1. 文献検索の流れ データベース検索により,
3,727 件(MEDLINE 1,341 件,CCTR ユニーク
2,386 件:MEDLINE との重複除く)が見い出され
た(Fig. 1).タイトルとアブストラクトより,
3,641 件を除外した.次に全文入手をして適格であ
るか否かの判断をリファレンス検索の結果を含め
152 件行い,以下のような論文を除外した.
研究デザインが,横断研究,前後比較研究,時
系列研究の論文
処方薬剤の減少数の平均値を報告していない
(算出することができない)論文
重複論文やサブグループ分析を報告している論
文
適格規準を満すと判断された論文は 27 編531)で
あり,その中に介入群と対照群との 28 の比較研究
があった.
2. 比較研究の特徴 研究が行われた年代は,
1970 年代が 1 研究,1980 年代が 4 研究,1990 年代
が 10 研究,2000 年代が 13 研究であった.研究が
行われた地域は,米国 19 研究,英国・北アイルラ
ンド 4 研究,カナダ 3 研究,フィンランド 1 研究,
欧州諸国 1 研究であった.外来患者もしくは地域の
薬局利用者に対する介入研究が多い(16 研究)が,
入院患者(4 研究)や老人福祉施設入所者(6 研究),

在宅ケア利用者(2 研究)に対する介入研究も行わ
れており,幅広いセッティングで効果検証が試みら
れていた.研究対象の平均年齢の範囲は 6185 歳で
あった.研究対象数の範囲は 48 名2,454 名で,100
名程度の比較的規模の小さい研究が多かった(Ta-
ble 1).
研究デザインはRandomized controlled trial(RCT)
が 19 研究,Controlled clinical trial (CCT)が 9 研
究であった.しかし RCT のランダム化は,block-
randomization (cluster RCT)や,ランダム化の方
法に関する記述のないものがあった.アウトカムで
ある服用薬剤数の測定に関して,マスキング(盲検
化)を担保した研究はなかった.介入前の平均服用
薬剤数は 4.1-11.5 種類であった.平均追跡期間 fol-
low-up period は 4 週2.5 年で,数ヵ月程度の比較
的短い研究が多かった(Table 1).
処方の改善を目的とした介入は,大部分の研究
(26 研究)で個々の症例(患者,入所者など)の処
方内容をレビュー(review of drug regimen)し特異
的な処方の改善点を示すものであった.それ以外で
は,特異的な処方の改善点を明示せず,処方を行っ
ている主治医への処方の見直し依頼が 2 研究14,24)あ
り,その 1 つは多剤服用状態であるので服薬内容の
見直しを検討するように主治医へ依頼する手紙を送
る介入,もう 1 つは現在服薬している薬剤 1 週間分
すべてを服用時間毎に記した一覧表を主治医に提示
して,多剤服用の問題に取り組むよう間接的に促し
ている介入であった(Table 1).
個々の症例の処方内容をレビューし,特異的な処
方の改善点を示した主体は,薬剤師が 14 研究,医
師が 2 研究,医師及び薬剤師が 4 研究,医師と薬剤
師を含む多職種のチームによる介入 6 研究であり,
大部分の研究で薬剤師が介入に関与していた.薬剤
師による介入の方法は,薬剤師が多剤服用高齢者の
薬歴やカルテをレビューしたり,患者に面接を行っ
て副作用に関する情報を収集したりして,現在の薬
物療法が適切であるかを検討し,必要があれば,そ
の薬物治療の中止又は変更などの勧告を行うという
ものであった.薬剤師同様,医師が多剤服用高齢者
の現在の薬物療法が適切であるかの判断を行うもの
もあった.多職種チームによる介入には,医師,薬
剤師,看護師の 3 職種が合同で患者ケアプランを立
てているものと,いわゆる高齢者総合機能評価
comprehensive geriatric assessment (CGA)32)の一部
として薬物治療の評価を行っているものがあった
(Table 1).
研究を以下の属性により,サブグループに分けた.
地域:北米,欧州
セッティング:外来患者若しくは地域の薬局利
用者,入院患者,在宅ケア患者,老人福祉施設

3. 介入の効果 介入期間前後で介入群の服用
薬剤数は,大部分の研究(20 研究)で減少し(中
央値-0.50 種類,信頼区間-0.76-0.11),3 種類
以上減少した研究が 2 研究14,19)あった.対照群でも
服用薬剤数が減少した研究が多かった(10 研究)
(Table 2).
介入群と対照群の両群間における服用薬剤数の減
少数の差,すなわち介入の効果は介入群の方が減少
が大きい(又は増加が小さい)研究がほとんど(24
研究)であった(中央値 0.45 種類,信頼区間 0.11
0.76).両群間の差に対する検定結果が記載されて
いない研究が多かった.また両群間の差の検定を報
告している 14 研究のうち,8 研究で有意な差であ
った(Table 2).
介入群及び対照群それぞれの,介入期間前後にお
ける服用薬剤の減少数は,介入前の服用薬剤数と有
意な(p<0.001)正の相関(介入群 r=0.75,対照
群 r=0.71)を示した.また,介入の効果は,介入
期間前の服用薬剤数と正の相関(r=0.32)を示し,
統計学的に有意な傾向(p=0.11)であった(Fig.
2).
地域,セッティング,研究デザイン,処方レビ
ューの有無,処方レビューワー,処方レビューワー
による処方変更の有無によるサブグループ間で,介
入効果の違いが有意であったのは,地域と研究デザ
インで,北米の研究が欧州のそれよりも,CCT が
RCT よりも大きな減少を示した(Table 3).
考 察
今回の系統的レビューにより多剤服用している高
齢者にターゲットを絞って介入を行うことにより,
服用薬剤数が約 0.5 種類減少するという結果が示さ
れた.前向きの比較研究という質の高い研究デザイ
ンで行われた多数の研究で結果の方向は揃ってお
り,この結果は信頼性が高いと思われる.
今回見い出された,多剤服用高齢者の薬物療法の
適正化を意図した介入の効果を検証した研究の大部
分は,薬剤師が処方をより集約的にレビューすると
いう介入の効果検証であった.処方内容の質を高め
るには,薬剤師が持つ薬物治療に関する専門的な知
識を活かすことの意義が再確認され,多剤服用高齢
者が適切な薬物治療を受けるために,薬剤師の果た
す役割は非常に重要である.

し,薬剤師は,処方せんの中に疑わしい点がある場
合に疑義照会を行うなどして処方の改善に努める.
したがって今回の系統的レビューにおける薬剤師に
よる介入に近いが,カルテや検査データへのアクセ
スなどは制限されており,処方される薬剤数を減少
させる効果は系統的レビューで示された効果よりは
下回るであろう.したがって,薬剤師が患者への十
分な問診に加えて,カルテや検査データを参照する
ようになれば,不適切な処方の回避によって服用薬
剤数を現状よりもさらに減少させることが期待でき
ると思われる.
薬剤師ではなく医師が処方をレビューする効果を
みた研究14,25)が少数あり,いずれも効果があるとす
る結果であった.しかし実際の医療現場において,
処方のレビューワーとして医師を配置するのは難し
いと思われるため,今回の系統的レビューにもあっ
たような,まず薬剤師がレビューを行い,それを医
師がスーバーバイズする方法を検討すべきであろ
う.また別の介入方法として CGA による研
究7,11,15)もあったが,服用薬剤数の減少は CGA の
目的の 1 つであって,服用薬剤数を減少させる目的
のみで CGA を行うことは想定し難い.
介入前の服用薬剤数が大きい研究は,介入の効果
が大きい傾向を示したが,これは妥当な結果である
ため,服用薬剤数が特に多い個人若しくは集団に対
して優先的に介入を行うことが望ましいと考えられ
る.
サブグループ分析で,CCT が RCT よりも効果が
大きいとする結果であったが,これは介入の効果が
大きいと予測される対象者を介入群に割り付けよう
とするバイアスの存在が疑われ,真の介入効果は
RCT の結果により近いことが推定される.また北
米での研究で示された介入の効果は欧州での研究よ
りも有意に大きいとする結果であったが,介入前の
服用薬剤数が北米での研究の方が有意ではないが多
い傾向にあり(データ示さず),これが関係したの
かもしれない.
処方のレビューワーによる介入効果の違いは,サ

ブグループ分析で有意な差がなく,また医師のみの
サブグループ以外はすべて薬剤師が関与していたた
め,どのような介入でその効果が高いのかを明らか
にすることはできなかった.
処方のレビューワーによる処方変更の有無でも介
入の効果に差は示されなかったが,これは処方権を
持たない第三者によるレビューが有効に機能し得る
ことを示している.ただし,適切な改善点が提案さ
れ,処方を行っている主治医もその処方変更の提案
に同意し易いような良質な処方レビューがなされる
ことが条件となろう.
適格論文の 1 つである Meyer らの論文14)では,
処方内容に踏み込まず,主治医の判断による処方見
直しの依頼と,医師が処方をレビューして処方変更
の提案という 2 つの介入群を対照群と比較してお
り,後者のみ介入の効果があった.単に処方を見直
すよう処方を行っている主治医に依頼をするだけで
は効果が十分ではなく,現在の処方に対する第三者
としての視点がより重要なことを示唆しているが,
さらなる研究が必要であろう.
服用薬剤数減少によりヘルスアウトカムが改善す
ることも期待されるが,これを論じることは今回の
系統的レビューの目的を超える.しかし文献検索の
過程で見い出された複数の論文16,17,22,30,31,37,3951)
は,多剤服用高齢者の有害事象や入院・死亡といっ
たヘルスアウトカムを測定し,その多くで介入によ
る効果がないとする結果であった.今後,不適切な
処方を減少させ,有害事象を減少させ,患者のヘル
スアウトカムを改善させ,患者の生命予後を改善さ
せるかを検討するため,介入の方法を洗練させ,サ
ンプルサイズを大きくした研究が望まれる.
今回適格とされた論文における介入は,いずれも
人的資源や時間をある程度要するものであり,費用
対効果は別に検討する必要がある.この文献検索の
過程において,主に薬剤師による介入の費用効果性
を検証しようと試み,13,27,30,3336,38,39,4143,45,46)薬剤費
を含む医療費が削減されたとする報告13,27,30,33,35,38,41)
もあった.
適格研究の中には研究者自身が介入を行っている
研究5,10,14,20)があったが,それらでは過剰な介入が
行われ,効果を過大評価する方向にバイアスが生じ
得る.また患者と,処方を行う薬剤師や医師はその
割り付けがマスクされておらず,研究の意図にそっ
た行動を取るようなバイアスが働く可能性がある.
逆に,薬剤師と医師を含む多職種チームによるいわ
ゆる CGA を行った研究では,多剤服用高齢者の服
用薬剤数が,主要アウトカムでなく,副次的アウト
カムであった研究もあり,服用薬剤数の減少効果は
介入に対して過少評価の可能性がある.
結 論
多剤服用している高齢者に対して,処方内容が医
学的に適切であるか判断する処方レビューといった
介入などに関する研究が行われていた.それらの介
入を行うことにより,服用薬剤数を約 0.5 種類減少
させ,特に服用薬剤数が多い高齢者ではその減少数
が大きい傾向にあった.しかし,どのような介入方
法が薬剤数をより減少させるのかは十分明らかでな
かった.
