Oklahoma Youth Soccer Futsal State Cup * Player Age Verification-Team Affidavit Oklahoma Soccer Assoc. 2018 FUTSAL STATE CUP Code of Conduct Certification The team and rostered players are registered and in good standing with OSA and of their respective club. Players are a current resident in Oklahoma. Coaches & Managers have submitted and passed their annual Adult Background Check; they have completed the appropriate concussion certification. The team will be participating in the Oklahoma Futsal State Cup competition have meet or have provided: a) Rosters of participating players have been checked and birth dates for each player has been verified. b) All players have medical releases, and are properly registered. c) No player is restricted from playing due to a concussion received in League or State play. Participating teams are representatives of their club and State. Teams are expected to conduct themselves in an appropriate manner. The following are standards of conduct for team players and team officials. Violation of these standards can jeopardize the team’s participation in the Oklahoma Futsal State Cup and may result in a forfeit for the team. 1. Players shall conduct themselves in a mature manner when in public areas at and away from the venue. No alcoholic beverages, tobacco products, or other intoxicants shall be in the possession of players. 2. Team officials and players are expected to cooperate with and be courteous to the OSA officials, and dignitaries on all dealings. This includes Referees, and event volunteers. 3. Only 3 team officials will be allowed on the team bench; team officials must have their adult passes. Team officials are expected to cooperate with game officials, and to follow all establish protocols and/or procedures. The coach may convey tactical instructions to players during a match; no coaching is allowed from other than the technical area. Degrading remarks directed towards players, opponents, or game officials in any language is NOT allowed. 4. Award Ceremonies will be held at a designated area at the end of each Championship Game. First Place Team and Second Place Team are expected to attend the ceremony. Players and teams will be awarded. 5. Sportsmanship: Coaches will be held responsible for the good sportsmanship of themselves, their players, players’ parents, and team spectators. Technical instruction from any part of the spectator area from any individual may result in the removal from the venue. Any spectator asked to leave must immediately depart the venue. 6. Referee Abuse: Abuse of Officials in any capacity will not be tolerated; there will be a zero-tolerance policy in effect within regards to Dissent. The Referee has the right to eject a player, parent, coach, or spectator from any game for continual disobedience or as a result of an incident that warrants ejection. It is understood that the club will be held responsible for any false information*. Code of Conduct Certification

The team and rostered players are registered and in good standing with OSA and of their respective club. Players are acurrent resident in Oklahoma. Coaches & Managers have submitted and passed their annual Adult BackgroundCheck; they have completed the appropriate concussion certification.
