Definitions The following terminology is used throughout this document: Collection Sites: Community pharmacies who are registered in the program

to collect unused and expired health products from the public.

Consumer Health Products: Health Canada defines consumer health products as products sold directly to consumers. Unlike prescription drugs, selection and use of consumer health products does not require the oversight of a health care practitioner, and consumers can safely select and use these products to maintain and improve their health.

Health Products: Prescription drugs, non-prescription drugs, and natural health products.

Member: Brand-owner/steward/producer who is a member of Health Products Stewardship Association (HPSA).

Natural Health Products: Vitamins and minerals, herbal remedies, homeopathic medicines, traditional medicines such as traditional Chinese medicines, probiotics, and other products like amino acids and essential fatty acids.

OMRP: Ontario Medications Return Program

OSCP: Ontario Sharps Collection Program

Pharmaceuticals: Subject to subsection (2), a drug within the meaning of section 2 of the Food and Drugs Act (Canada) that is sold to consumers in Ontario, whether it is sold by the producer of the pharmaceutical or by another person; and includes a natural health product within the meaning of the Natural Health Products Regulations made under that Act.

Regulation: Environmental Protection Act 298/12

Producer: Means a person who manufactures or processes a pharmaceutical or sharp for the purpose of sale.


1 Executive Summary

Since 2013, the Health Products Stewardship Association (HPSA) has administered the Ontario Medications Return Program (OMRP) for prescriptions drugs, non-prescription drugs and natural health products as well as the Ontario Sharps Collection Program (OSCP) for medical sharps that are sold for use in the province of Ontario but limited to the “consumer/residential” waste stream.

These programs are operated under the principles defined by Ontario Regulation 298/12 “Collection of Pharmaceuticals and Sharps – Responsibilities of Producers” and which is Ontario’s first Extended Producer Responsibility regulation created under the Environmental Protection Act (EPA).

These programs were developed from past successes of HPSA’s disposal systems in British Columbia, Manitoba and Prince Edward Island on behalf of its producer members selling prescription drugs, non-prescription drugs, natural health products and medical sharps.

The OMRP and OSCP were designed to ensure that collection sites are available to all regions of the province, and that health products returned by the public are packaged, handled, transported, and disposed of in a safe, compliant and environmentally responsible manner.

Figure 1 shows the participation rate of community pharmacies licensed by the Ontario College of Pharmacists as well as the number of stewards (161) supporting these programs in 2017. Participation of pharmacies has remained above 90% as required under the EPA Reg 298/12.

Figure 2 is a summary of the OMRP performance over the past five years. Since 2016 the net weight of health product waste sent for destruction has been reported, rather than the gross weight collected through pharmacies.

2 Program Overview

2.1 Collection Sites HPSA acted collectively on behalf of its producer members, allowing industry to meet the regulatory requirement of more than 90% of accredited retail pharmacies being registered in OMRP and OSCP by January 1, 2017. The eligible number of accredited pharmacies in Ontario was 4,284 as posted by the Director on the Ministry of the Environment and Climate Change (MOECC) website as of October 1, 2017. HPSA’s collection network was comprised of 3,929 pharmacy collection sites on December 31, 2017, or approximately 91.7% of eligible pharmacies in Ontario. HPSA’s collection site network increased over the course of 2017. HPSA continues to develop new guidelines and to reach out to newly registered pharmacies. 3 Annual Report Requirements and Program Results Summary

Regulation 298/12, Section 9 “Annual Report”

Section 9 (2) A report required under subsection (1) may be prepared on behalf of more than one producer of pharmaceuticals or sharps and, if this is done, the report shall set out, at a minimum, the following information with respect to those producers:

Section 9 (2) 1 – Requirement:

The name of each producer on whose behalf the report is prepared.

Section 9 (2) 1 – Result:

• The list of producer members (human and animal health) for pharmaceuticals (health products) is attached in Appendix 1 a).

• The list of producer members (human and animal health) for sharp devices is attached in Appendix 1 b). Section 9 (2) 2 Item #1 – Requirement:

• The number of collection locations and the location of each of the collection locations at which collection of the designated material of the producers was provided for by all of the producers…

• The regulation requires producers to have 90 per cent of retail locations in Ontario, as of October 1. Section 9 (2) 2 Item #1 - Result

• As of December 31, 2017, 91.7% of eligible pharmacy collection locations in Ontario were registered with HPSA or 3,929.

Section 9 (2) 2 Item #2 – Requirement:

... a description of any changes that occurred with respect to the number and location of collection locations from the previous calendar year.

Section 9 (2) 2 Item #2 – Result:

• HPSA’s collection network for the OMRP and OSCP as reported in 2016 was 3,752. A net increase of 177 for a total of 3,929, the end of 2017 sites was a result of outreach to newly accredited pharmacies across Ontario.


The information mentioned in paragraphs 2 to 7 of subsection (1), reported with respect to all of the producers as a group.


The total weight of all of the following that was collected at the collection locations during the previous calendar year:

i. Pharmaceuticals. ii. Containers described in paragraph 3 of section 2, excluding any containers that were recycled. Section 9 (1) 2 – Result:

• In 2017, a total of 291,273.39 kilograms of pharmaceutical waste was destroyed by incineration. • This number includes the weight of containers in which consumers may have brought the

pharmaceuticals to the collection locations. • Section 4.2 of this report provides a summary by district of the total weight of pharmaceuticals returned

and disposed of.



The total weight of all of the following that was collected at the collection locations during the previous calendar year:

i. Sharps. ii. Containers described in paragraph 3 of subsection 3 (1).


• In 2017, a total of 292,153.96 kilograms of sharps waste was collected. • The total collected includes the weight of sharps containers in which consumers brought the medical

sharp devices to the collection locations. • Section 4.3 of this report provides a summary by district of the total weight of sharp devices collected.


A description of how the designated material of the producer collected at collection locations was handled and how it was disposed of during the previous calendar year.


• Pharmaceuticals collected in the OMRP are packaged in lined, reusable, 23L/60L pharmaceutical waste pails/tubs at the collection locations. When ready for shipment, collection locations tie off the pail liner, secure the lid to the base and arrange for shipment. Stericycle, ULC or ECS Cares transports the waste to the HPSA approved Stericycle disposal site where the pharmaceuticals and any associated packaging are incinerated. See Section 5.2 for more detail.

• Biomedical sharps collected in the OSCP are received from consumers in sharps containers labelled with a Biohazard symbol. At the collection location these sharps containers are packaged in lined, corrugated cardboard waste boxes. When ready for shipment, collection locations tie off the box liner, seal the box and arrange for shipment. Stericycle, ULC and ECS Cares transports the boxes of biomedical sharps to the HPSA approved disposal site where the boxes of biomedical sharps/sharps containers are autoclaved. See Section 5.3 for more detail.

• Cytotoxic sharps collected in the OSCP are received from consumers in sharps containers labelled with a cytotoxic symbol. At the collection location these sharps containers are packaged in lined, corrugated cardboard, boxes with cytotoxic labelling on the outside. When ready for shipment, collection locations tie off the box liner, seal the box and arrange for shipment. Stericycle, ULC. and ECS Cares transports the boxes of cytotoxic sharps to the HPSA approved disposal site where the boxes of cytotoxic sharps and sharps containers are incinerated. See section 5.3 for more detail.



A description of actions taken by the producer during the previous calendar year, their effectiveness and outcomes achieved as part of complying with sections 2, 3, 4 and 7.


Section 2: 2. “A Producer of a pharmaceutical shall provide for…..” Action Required Effectiveness Outcome

2.1 “…the collection of the pharmaceutical at collection locations from consumers who bring the pharmaceutical to the collection locations.”

• A producer funded collection network has been established and is administered by the ISO, HPSA.

• All registered collection locations have signed a Public Waste Protocol with HPSA certified waste hauler.

• 3,929 collection sites were registered and actively participating in the OMRP as of December 31, 2017.

2.2 “…the disposal of the pharmaceutical.”

• Through a public request for proposal (RFP) process, HPSA selected Stericycle, ULC.

• Stericycle, ULC has the required Environmental Compliance Approvals (ECA’s) in place to perform this service.

• 291,293 kilograms of pharmaceuticals were disposed of in 2017.

2.3 “…the collection, and disposal of containers in which consumers bring the pharmaceuticals to collection location and that have come into direct contact with the pharmaceutical.

• As part of the HPSA service agreement, Stericycle, ULC and ECS Cares provides the collection and Stericycle (Brampton facility) provides for the disposal of all pharmaceuticals and containers collected.

• All material collected is incinerated in Ontario.

Section 3: 3. “The producer of a sharp shall provide for….” 3.1 “… the collection of the sharps at the collection locations from consumers who bring the sharp to the collection locations.”

• A producer funded collection network has been established and is administered by the ISO, HPSA.

• All registered collection locations have signed a Public Waste Protocol document acknowledging they understand the procedures for safe collection, handling and storage of medical sharps received from the public.

• 3,929 collection sites were registered and actively participating in the OSCP as of December 31, 2017.

3.2 “…. the disposal of the sharp.”

• Through a public (RFP) process, HPSA selected Daniels Healthcare for the disposal of sharps waste.

• 268,382 kilograms of biomedical sharps were autoclaved in Ontario in 2017.


3.3 “… the collection of containers designed for the safe handling of sharps in which consumers bring the sharp to the collection locations.”

• Through a public RFP process, HPSA selected Stericycle, ULC and ECS Cares for the collection of sharps waste and sharps containers.

• 23,772 kilograms of cytotoxic sharps were disposed of through incineration.

Section 4: 4(1) “Subject to subsection (2), a producer of a pharmaceutical or sharp shall ensure that the number of collection locations at which collection of the designated material of the producer is provided in a calendar year is, at a minimum, the lesser of the following;”

4. (1) 1 “The number equal to 90% of all retail locations in Ontario, as of October 1 in the preceding calendar year, at which a pharmaceutical or sharp was sold.” 4. (1) 2.” The number equal to 90 per cent of pharmacies in Ontario, as of October 1 in the preceding calendar year, in respect of which a certificate of accreditation is in effect under section 139 of the Drug and Pharmacies Regulation Act, as set out by the Director on the internet through the Ministry’s website.”

• The Director, on the Internet through the Ministry’s website, posted 4,284 as the number of registered pharmacies that were eligible to participate in the OSCP and OMRP programs.

• The participation rate of total eligible collection locations in the combined programs in Ontario is 91.7%.

• 3,929 collection locations were registered by December 31, 2017 exceeded the 90% registration requirement.

4. (2) “A producer of a pharmaceutical or sharp shall ensure that there is at least one collection location at which collection of the designated material of the producer is provided for in each local municipality in Ontario in which there is a retail location at which the pharmaceutical or sharps is sold.”

• HPSA has reviewed the list of participating collection locations and determined there is at least one collection location in every municipality in Ontario.

• The industry run collection network has a high level of consumer accessibility.

4.3 “A producer of a pharmaceutical or sharp shall ensure that at each collection location at which the producer provides for

Section 7 A description of any actions taken by the producer during the previous calendar year that exceeded the actions required for the purposes of section 7, with respect to educational and public awareness activities in order to promote to consumers the collection locations and the availability of collection.

7. (1) “A producer of a pharmaceutical or sharps shall ensure that the following information is made available publicly and free of charge…”

7. (2) “The information described in subsection (1) shall, at a minimum, be made available,

a) on the internet through the producer’s website; and b) in print at each collection location at which collection of the designated material of the producer is provided for.”

• HPSA has notified producer members of this requirement. The HPSA list of producers with the web-link can be found on HPSA’s website at http://www.healthsteward.ca/about-us/partners/brand-owners.

• A copy of collection locations was mailed to all participating pharmacies in the second quarter of 2017.

• Information is available online and at collection locations.


A description of any actions taken by the producer during the previous calendar year that exceeded the actions required for the purposes of section 4, in order to provide for the collection of the designated material of the producer.


• HPSA contacted all non-registered, eligible retail pharmacies to introduce the OMRP and the OSCP and to provide direction/information about the programs.


A description of any actions taken by the producer during the previous calendar year that exceeded the actions required for the purposes of section 7, with respect to educational and public awareness activities in order to promote to consumers the collection locations and the availability of collection.


• HPSA exceeded the actions required with respect to educational and public awareness in several ways. A detailed description of the actions can be found in the promotion and education strategy section below (Section 6).

Section 9 (3) – Requirement:

The report shall include an opinion from a licensed public accountant confirming the accuracy of, (a) if the report is prepared in respect of one producer, the information referred to in paragraphs 1, 2, 3, 4 and 7 of subsection (1); or (b) in the case of a report prepared on behalf of more than one producer, the information referred to in paragraphs 2 and 3 of subsection (2), other than the information referred to in paragraphs 5 and 6 of subsection (1).

Section 9 (3) – Result:

• A full report from a licensed public accountant is included in Appendix 2 confirming the accuracy as per (b) above.

Section 9 (4) – Requirement:

The report shall be made available publicly and free of charge on the internet through the producer’s website for a minimum of three years.

4 Collection System 4.1 Program Performance

In 2017, 291,273 kilograms of medications and 292,154 kilograms of sharps, including cytotoxic sharps, were collected through HPSA’s extensive collection network. This translates into a 12.1% increase for the OMRP and a 5.2% for the OSCP over 2016 performance. Working closely with pharmacies and service providers, HPSA initiated major changes in containers and collection services in the latter part of 2017. We deployed HPSA brander 60 L green tubs as well as 4.2 L sharps containers throughout the province.

4.2 Ontario Medications Return Program The collection sites for the OMRP are community pharmacies are listed in Appendix 4. Registered pharmacy locations are easily identified through a web-based search on the HPSA website. An independent public accountant provides an opinion to verify that the mass balance of materials is accurate as they flow in Ontario through the “chain of custody” as follows: • A community pharmacy registered in the HPSA program is typically scheduled on a service

frequency based upon their historical waste generation. The pharmacy may also initiate a request for pickup directly to the service provider or to HPSA, and a service date is provided to the pharmacy.

• At the pharmacy the driver affixes a barcode label to each HPSA collection container, which identifies the collection site and specific container. The driver uses a handheld PDT to scan a barcode on the HPSA container to record that the pail has been secured for transport.

• The driver leaves the desired amount of replacement container(s) with a receipt from the driver’s PDT documenting the number of waste containers collected and the supply of new collection containers.


• The medications pail is delivered to the transfer and processing facility, where each container’s unique barcode is scanned to validate receipt and ensure the number of containers collected at each pharmacy is equal to the number of containers received.

• HPSA medications pails collected from the sites are weighed, manifested and shipped to the incineration facility in Brampton, Ontario.

• Upon receipt in Brampton, the medications are weighed to verify the received weight, incinerated and a Certificate of Destruction is provided to HPSA.

4.3 Ontario Sharps Collection Program

The collection sites for the OMRP are community pharmacies are listed in Appendix 4. Registered pharmacy locations are easily identified through a web-based search on the HPSA website.

Biomedical sharps are sharps products used by consumers that are designed to puncture the skin for medical purposes and must be properly placed in an approved sharps container, colour coded yellow with a universal Biohazard symbol on the outside, with a lid that can be permanently closed in order to be accepted at collection locations. At the pharmacy the sharps containers must then be placed into approved secondary containment packaging in the form of a corrugated cardboard box with the universal Biohazard symbol on the outside and lined with a 2 mm plastic bag. Once the lined box is full it is sealed for transportation and disposal. When a prescription is filled for a cytotoxic medication that has a sharp, the recommended practice is to provide a new sharps container with a cytotoxic label to the customer at the same time. These containers must then be placed into approved secondary containment packaging with


the universal biohazard symbol on the outside and a cytotoxic label prominently displayed on the outside of the box. Once the lined box is full it is sealed for transportation and disposal. • All employees working at the collection location must be knowledgeable about the program

offered to consumers. • The pharmacy shall only receive sharps in a hard-shell container (approved sharps container)

which is labelled with the universal biohazard symbol. • Upon receiving sharps containers, pharmacists should ensure that the tops of the containers

are securely closed and placed in the provided over packaging.

5 Promotion and Education Strategy The Promotion and Education (P & E) Strategy for Ontario, deployed by HPSA on behalf of member producers of pharmaceuticals and sharps, has specific components geared toward each stakeholder participating in the OMRP and the OSCP. The HPSA P & E strategy is intended to effectively communicate program standards to participating pharmacies who voluntarily collect sharps and pharmaceutical waste and - most importantly - consumers, whose participation is critical to the success of the OMRP and the OSCP.

5.1 Regulatory Requirements for Promotion and Education From a producer compliance perspective, the basic components of the P&E strategy are those required by Regulation 298/12. These requirements include:

• HPSA sent over 32,900 brochures and bookmarks to pharmacies, municipalities, doctor’s offices/hospitals and other partners for public distribution.

• Promotional and educational materials intended for the public can be ordered, free of charge, using the promo order form found on HPSA’s website. A sampling of the materials available for distribution can be viewed at www.healthsteward.ca.

In addition, some chain pharmacies actively promote awareness about the need for proper management and safe disposal of medications and sharps.

5.2 Retail Pharmacy Regulation 298/12 requires participating pharmacies to have an agreement in place and conduct annual training for pharmacists and pharmacy technicians with respect to the safe collection, handling and storage requirements for sharps and pharmaceuticals. The Agreement and the regulation are both important educational tools and help ensure program consistency at the retail pharmacy level.

5.2.1 Educational Material

HPSA provides pharmacy employees with posters specifically designed for them about how the programs operate, which materials are eligible for collection, and what should go into the disposal containers provided. Pharmacists can also order bilingual educational materials about HPSA programs geared toward consumers by completing the HPSA promo order form supplied to pharmacies at the time of registration, as well as with the distribution of two semi-annual newsletters. Pharmacists may also view and order materials online by downloading the promotional order form from the HPSA website (http://www.healthsteward.ca/collection/ontario).

Every year, participating retail pharmacies receive a package that contains the following consumer related P & E Strategy materials. 5.2.2 Newsletters

In 2017, HPSA sent newsletters to pharmacies which included information and updates on the OMRP and the OSCP. The Spring newsletter was distributed to participating pharmacies through a direct mail-out and via email in May 2017. The Fall newsletter was distributed by email and fax, as well as through contacts at several pharmacy chains. Newsletters allow HPSA to reach pharmacists with relevant news and updates concerning the OMRP and OSCP directly. HPSA’s Spring and Fall pharmacy newsletters for Ontario are attached in Appendix 3.

5.2.3 Direct Mail Out to Pharmacists Every month, newly registered pharmacies receive a “welcome” package that includes a letter reviewing the role of the pharmacy, along with information intended for the pharmacist and pharmacy staff. This package includes OMRP and OSCP posters, a full copy of the collection location site list, promotional order form, and a sample selection of promotional materials. HPSA reaches out to approximately 40 pharmacies monthly. Newly accredited pharmacies are sent a fax with a welcome letter providing supporting program information in addition to a registration form. HPSA has also enlisted the assistance of the major chain pharmacies to communicate this message through distribution of HPSA newsletters and promotion of the program through internal pharmacy emails.

5.3 HPSA Website HPSA’s bilingual website has a participating retail pharmacy locator tool for consumers, which allows visitors to enter their postal code to access a drop-down menu that provides an address and contact information for the collection site. The website also contains videos for consumers that were developed through HPSA’s partnerships - “I Don’t Flush” and “Teen Takers” were created to help encourage consumers to properly dispose of their expired and unused medications.

In 2017, HPSA continued to advertise through Google AdWords, which enabled direct reach to Ontarians searching for information about medications and sharps. Advertisements were designed to direct traffic directly to HPSA’s website. From January 1, 2017 to December 31, 2017 the AdWords campaigns generated a total of 45,317 impressions.

5.4 HPSA Producer Members’ Website

Under Regulation 298/12 producers are required to have a link on their website that takes consumers to the HPSA website where information on the OSCP and OMRP can be found. A list of HPSA producer member website links can be found in Appendix 5.

5.5 Pharmacy Participation

5.5.1.1 Rexall / Pharma Plus Rexall/Pharma Plus promotes medicine cabinet clean outs by providing customers with a disposal bag for unused and expired medications (see below), as well as a checklist designed to help consumers keep only useful medications in their home. The Rexall website (www.rexall.ca/medicinecabinet) contains information for consumers on how to ensure they only have safe and effective medications in the medicine cabinet. Rexall and Pharma Plus stores are registered as collection locations in the HPSA programs.

5.5.2 Shoppers Drug Mart In the provincial services section of the Shoppers Drug Mart website, Ontarians are reminded that they can safely dispose of their medications, syringes, pens, needles and lancets.

In 2017, SDM and LLC partnered with the Drug Free Kids Canada and HPSA on a national campaign to secure medications in the home and to clean-out Canada’s households of unused and expired prescription drugs. 5.6 Municipalities HPSA continued to work with some municipalities in 2017. Municipalities interested in receiving supplies (sharps containers) could signed a MOU and distribute promotional materials, including public brochures and flyers, in exchange of free sharps containers.

In 2017, HPSA advertised in 737,550 municipal collection calendars sharps and meds collection programs. The breakdown by region can be found in the Table 4 below.

6. Conclusion In 2017, on behalf of its member producers, HPSA continued to increase the effectiveness and efficiency of its health products and sharps collection network. The OMRP and OSCP adhere to the four core objectives of HPSA’s programs. These are safety, diversion, environment and compliance. HPSA is engaged in preventing the illicit diversion and abuse of medications in households and to reduce drug and needle related accidents by providing consumers with easy access to retail pharmacy locations where materials can be returned for proper disposal. HPSA also worked toward the ongoing goal of keeping unused/expired medications and used needles out of sewage treatment systems and household garbage to reduce the impact of health products on the environment. While maintaining a primary focus on safety, health and environment related objectives, HPSA also helped member producers collectively achieve their obligations required by Ontario’s Environmental Protection Act. In the fifth full calendar year of the OMRP and the OSCP, 291,273 kilograms of health products and 302,154 kilograms of used sharps were returned by consumers to participating retail pharmacies. The 90% threshold of collection locations in Ontario being registered and voluntarily participating in these programs has also been exceeded. Collecting more than 594 tonnes of post-consumer sharps and pharmaceuticals waste from households in Ontario in 2017 and properly managing the end of life of these materials has been a significant achievement by HPSA and its member producers. We look forward to building on these gains in 2018.
