Online Livestock Affidavits

Designed to collect same information as paper ID forms. Benefits: Information is stored in a sortable database, reduces number of times

paper forms changed hands and exhibitors get immediate confirmation of their entries (via email).

2015 Guidelines

Completion of the online livestock affidavit form is required for all animals being identified for the Nebraska State Fair & Ak-Sar-Ben.

Process

Animal ID information will be entered into an online form via this web link: http://livestockaffidavit.unl.edu

Up to 4 exhibitors can be listed on 1 form. If more than 4 exhibitors are in the same family a second form will need to be filled out.

Once your form(s) are submitted you will receive a confirmation email. This is your official proof of your entries. Be sure to save this email!

Deleting Entries:

If “Save Animal Information” is clicked without completing the form, required

information will be outlined in red.

If you’ve added one too many animals, deleting an entry is as simple as clicking the red “x”.

Due Dates for Online Forms:

Market Beef – April 15, 2015 (midnight)

All other Species – June 15, 2015 (midnight) **Entries made into the online form are date/time stamped. Any entries made after the due date/time will be ineligible. What if I need to make a correction after submitting?

Changes should be submitted in writing to your local Extension Office (email is acceptable). They will then contact the State 4-H Office to make the update.
