How it worksProgram staff work collaboratively with
youth and families in the wraparound model to build on youth and family strengths, address identified needs, and connect families to interventions and resources.
Families are considered equal partners in the Connections program. Each family involved in the program is assigned to a family assistance specialist whose primary role is to support and advocate for the parents of probationary youth.

participationof team members drawn from familymembers’ networks of interpersonal andcommunity relationships. The wraparoundplan reflects activities and interventionsthat draw on sources of natural support.
Collaboration4. Collaboration. Team members workcooperatively and share responsibility fordeveloping, implementing, monitoring,and evaluating a single wraparound plan.The plan reflects a blending of team
members’perspectives, mandates, and resources.The plan guides and coordinates eachteam member’s work towards meeting theteam’s goals.
Community Based5. Community-based. The wraparoundteam implements service and support
strategiesthat take place in the most inclusive,most responsive, most accessible,and least restrictive settings possible; andthat safely promote child and family
integration into home and community life.
Culturally Competent6. Culturally competent. The
wraparoundprocess demonstrates respect for and builds
onthe values, preferences, beliefs, culture, andidentity of the child/youth and family, and
theircommunity.
Individualized7. Individualized. To achieve the goals laid out in the wraparound plan, the teamdevelops and implements a customized setof strategies, supports, and services.
Strengths Based8. Strengths based. The wraparoundprocess and the wraparound plan identify,build on, and enhance the capabilities,knowledge, skills, and assets of the childand family, their community, and otherteam members.
Persistence9. Persistence. Despite challenges, theteam persists in working toward the goalsincluded in the wraparound plan until theteam reaches agreement that a formalwraparound process is no longer required.
Outcome Based10. Outcome based. The team ties thegoals and strategies of the wraparoundplan to observable or measurable indicatorsof success, monitors progress in termsof these indicators, and revises the planaccordingly.
