Dear Colleagues,

In recent employee surveys, you told us there was more to do to improve our benefits for you and your dependents. And we listened. We’ve taken action to make our benefits choices in 2022 more affordable, flexible, and inclusive, and to help you manage the stress we all face in these unprecedented times.

Next year, FMCNA will take on a significantly larger share of the total cost of your benefits to help you manage the rising costs of healthcare and to keep premium rates the same. FMCNA will spend over $500 million on benefits in 2022.

We’re introducing new and expanded benefits that provide you more flexibility to make your life simpler, additional tools to help with your mental health, broader support to address your diverse needs, and easier access to information about what we have to offer.

Please spend some time carefully reviewing our benefits inside this guide and on FMC4ME so you can make the best choices for you and your family.

This year, Open Enrollment is mandatory — your benefits will NOT roll over. You MUST take action between November 6 and November 19 for medical, dental, vision, supplemental life, disability, and voluntary coverage, as well as make elections for Flexible Spending and Health Savings Accounts for 2022.

2022 Highlights You wanted benefits costs to be more affordable and flexible. We

are taking on a larger share of the costs to keep your rates the same.

There are NO PREMIUM INCREASES for our UnitedHealthcare medical, dental, vision, life insurance, or disability plans for 2022.

Consumer with HRA Plan premiums are being lowered to $10 per paycheck for Employee, $20 for Employee + 1, and $30 for Family coverage. Families may have an opportunity to save up to $5,800 in payroll contributions just by switching from the Standard Plan!

– Employees who make $50,000/year (or $24.04/hour), or less, as of October 1, 2021, can access FMCNA funded HRA dollars right away.

Overview of Medical PlansWe offer you four national medical plan choices through UnitedHealthcare (UHC). You may be eligible for other health plans, like HMO coverage, depending on your home ZIP code.

• Health Saver with Health Savings Account (HSA) Plan

• Standard Plan

• Premium Plan

• Consumer with Health Reimbursement Account (HRA) Plan

• Enhanced for 2022, with a $10 paycheck cost for Employee, $20 for Employee + 1, and $30 for Family coverage (a reduction from 2021 rates)

• View Regional and HMO plans

In order to determine which plan(s) you’re eligible for, visit the Employee Service Center, or the ESC. When you log in with your Employee ID, the site will populate your personalized plan options.

When you stay in-network, all four UHC plan choices:

Give you access to UHC’s nationwide network of doctors, specialists, and hospitals.

Have an out-of-pocket maximum, which “caps” the amount you could pay for services in a year.

Offer free virtual visits from the comfort of your home through Amwell, Doctor on Demand, Teladoc, and Optum Virtual Care. Only $49 through the Health Saver with HSA Plan.

Earn up to $250 annually for participating in Health Actions through Rally Health, our digital wellness program.

Expanded UHC coverage and support:

• Expert second opinion through 2nd.MD connects you with board-certified doctors via video or phone, to make sure you have the right treatment plan for surgeries, chronic conditions, or any new diagnosis. Note that consultations are required for members prior to certain procedures, like musculoskeletal surgeries, knee replacements, or spinal surgeries. Coordinate your surgery with UHC Advocate4Me.

• Free virtual physical therapy through SWORD to get support for reducing your back, knee, shoulder, and other musculoskeletal pains. You will be connected with a physical therapist who will assign exercises and monitor completion of your personalized physical therapy program by using special digital tools.

• Expanded coverage for gender affirming care and cancer support, including gender reversal, egg preservation, and increased allowances for items like wigs.

• Enhanced fertility coverage for all couples. Receive personalized guidance to empower decisions, find quality care, and navigate family building coverage. Note that you must enroll in the UHC fertility program to receive support for fertility services.

** Aggregate: If you cover other family members, you’ll need to collectively meet the entire family deductible before you and the plan begin to share in costs. You will also need to collectively meet the entire family out-of-pocket maximum before the plan will pay 100% of costs for covered services for the rest of the year.

*** Embedded: If you cover other family members, each person will be subject to a deductible and out-of-pocket maximum equal to the plan’s individual deductible and out-of-pocket maximum. For example, a family of four with the Consumer with HRA Plan pays $4,000 in deductibles for two or more family members expenses (not 4x the $2,000 individual deductible).

Earn Money with Rally Health

It’s easy to earn rewards for your healthcare needs with Rally Health. There are two ways to participate:

UHC-enrolled employees can earn up to $250 dollars, along with Rally Coins, through a variety of activities for taking healthy actions. Family members may also participate and can earn Rally Coins. Visit Rally Health on the ESC for details.

Non UHC-enrolled, benefits-eligible employees can earn Rally Coins by participating in one of five activities.

New! Dispense as Written: Generic drugs are available at lower costs and with the same benefits as many brands you may see advertised. If you specify that a drug must be dispensed with the brand name in place of an available generic, you will pay the difference in cost between the brand name drug and the generic equivalent.

Smart90 Program: When you get a 90-day supply of a prescription medication through Smart90 at CVS, Walgreens, or by mail-order, it saves you time and money (since you can save up to the cost of a copay).

Diabetes Assistance: We’re committed to bringing down the cost of insulin products. Through the Patient Assurance Program, eligible members will pay no more than $25 per 30-day supply.

Health Reimbursement Account (HRA)Enroll in the Consumer with HRA Plan to save with the FMCNA Funded Health Reimbursement Account (HRA). HRA dollars will roll over if not used, up to a maximum of $2,000.

Health Savings Account (HSA)Enroll in the Health Saver with HSA Plan to contribute to a Health Savings Account. A Health Savings Account is a tax-advantaged saving and spending account, so you can use the money to pay for eligible medical, dental, and vision expenses, as well as other eligible expenses, like long-term care coverage. If you are ill or injured and cannot work for an extended period, disability coverage can help with your expenses.

Short-Term Disability (STD) and Long-Term Disability (LTD) coverage can provide you with a portion of your pay if you become disabled and are unable to work. Due to pre-existing condition limitations, employees who enroll in disability coverage when they are already disabled, will not qualify for the benefit.

Any amounts over that will require EOI.

3. Log in and enroll between November 6 and November 19:

P Visit PeopleSoft or log in to the ESC from FMC4ME using your Employee ID and network password.

P To receive benefits information year-round, be sure to enter an email when starting your benefits election process online.

P Click on the Open Enrollment banner to get started and make your elections.

P In PeopleSoft, select, Open Enrollment: Choose Your Benefits.

P To proceed with your enrollment selections, you will need to enter your Employee ID and network password again.

P Review and make any edits to your benefit elections. If you change your medical elections for 2022, you will automatically be re-enrolled in the $50 surcharge and will have to complete the affidavit to opt out or waive coverage. Reminder! It resets each year, so don’t skip out on this step!

P Print your FMCNA Benefits Confirmation Statement and keep a copy for your records.

P Remember: The benefits you choose will be effective beginning January 1, 2022, through the end of the year. You cannot change your coverage during the year unless you experience a qualifying life event, such as marriage, birth, adoption or placement of a child, or employment status change for you or your spouse/domestic partner. Verify Newly Added Dependents: Employees are required to verify newly added dependents. You must upload verification documents by Friday, November 19, 2021, to enroll them. Surcharge Information

Spousal Surcharge: Employees who elect to cover a working spouse/domestic partner who is eligible for group medical coverage through their employer, but opts to be covered under a FMCNA plan, will be assessed a $50 per-pay-period surcharge in addition to the bi-weekly rates.

Tobacco User Surcharge: We support your health and wellness by offering programs like Quit For Life® for all employees. If you use tobacco products, you can still qualify to waive the $20 bi-weekly tobacco surcharge by completing the Quit For Life® program. The program offers online support through Quit For Life® mobile app, a Live Tobacco Free course, and personalized support. Learn more at Quit For Life.
