January 15, 2016 The information contained in this report summarizes new, interim, and final actions taken by the Oregon Medical Board between December 16, 2015, and January 15, 2016. Scanned copies of Interim Stipulated Orders, Orders of Emergency Suspension, Stipulated Orders, Final Orders, Termination Orders, Modification Orders and Voluntary Limitations are included at the end of this report in the order that they appear in the report. These orders are marked with an * asterisk. Scanned copies of Consent Agreements are not posted, as they are not disciplinary action and impose no practice limitations. Complaint and Notices of Proposed Disciplinary Action are not listed in this report, as they are not final actions by the Board. Both Orders, however, are public and are available upon request. Printed copies of the Board Orders not provided with this report are available to the public. This Order terminates Licensee's February 5, 2015, Consent Agreement. *Arakaki, Charles Mori, MD; Applicant; Portland, OR On January 8, 2016, Applicant entered into a Stipulated Order with the Board for willfully violating any provision of the Medical Practice Act, and representing in any manner that he is authorized to practice medicine in this state. This Order reprimands Applicant and fines Applicant $5,000. *Ashaye, Olurotimi Ayodele, MD; MD24024; Meridian, ID On January 7, 2016, Licensee entered into a Stipulated Order with the Board for unprofessional conduct; disciplinary action by another state of a license to practice medicine; and failure to report to the Board any adverse action taken against the licensee by another licensing jurisdiction. This Order prohibits Licensee from treating chronic pain with any DEA scheduled medications with the exception of prescribing Suboxone/Subutex for the treatment of addiction and prescribing DEA scheduled medications for inpatient treatment upon discharge; and requires Licensee to disclose Order to present and future employers.

This Order reprimands Licensee; fines Licensee $2,500; places Licensee on probation; prohibits Licensee from treating chronic pain with any DEA scheduled medications; restricts Licensee's treatment of acute or intermittent pain; prohibits Licensee from prescribing Methadone for acute pain or methadone maintenance therapy; prohibits Licensee from prescribing benzodiazepines or muscle relaxants in combination with any opioid medications with specific exceptions; restricts Licensee's treatment of substance use disorder; and requires Licensee to complete 150 hours of pre-approved continuing medical education. *Osborne, Dori Florence, PA; PA160844; Portland, OR On January 7, 2016, the Board issued an Order Terminating Consent Agreement. This Order terminates Licensee's November 7, 2013, Consent Agreement.

This Order terminates Licensee's July 11, 2013, Corrective Action Agreement. *Rysenga, Juliet Catherine, MD; MD18740; Ontario, OR On January 7, 2016, Licensee entered into a Stipulated Order with the Board for unprofessional or dishonorable conduct, and gross or repeated negligence in the practice of medicine. This Order reprimands Licensee; requires Licensee to have surgical cases monitored by a pre-approved, board-certified ophthalmologist; requires Licensee to complete a pre-approved charting course and a pre-approved boundary course; prohibits Licensee from prescribing for her family or herself; requires Licensee to maintain a relationship with a pre-approved healthcare provider who will provide reports to the Board; and prohibits Licensee from prescribing antipsychotic medications. Sinnott, Robert Casserly, MD; MD13902; Medford, OR On January 8, 2016, Licensee entered into a Consent Agreement with the Board. In this Agreement, Licensee agreed to practice under the supervision of a Board approved mentor for six months, and complete 50 hours of pain management CME. *Stringham, Charles Howard, MD; MD09749; Salem, OR On January 7, 2016, Licensee entered into a Stipulated Order with the Board for unprofessional or dishonorable conduct; gross or repeated negligence in the practice of medicine; and prescribing a controlled substance without a legitimate medical purpose, or prescribing controlled substances without following accepted procedures for examination of patients, or prescribing controlled substances without following accepted procedures for record keeping. This Order retires Licensee's medical license while under investigation. *Watkins, Robert Camp, MD; MD12030; Clackamas, OR On January 7, 2016, the Board issued an Order Terminating Stipulated Order. This Order terminates Licensee's August 10, 1998, Stipulated Order and Licensee's April 18, 2002, Order Modifying Terms of Stipulated Order. *Wright, John Charles, MD; MD28914; Gold Beach, OR On January 7, 2016, Licensee entered into a Stipulated Order with the Board for unprofessional conduct, and repeated negligence in the practice of medicine. This Order retires Licensee's medical license while under investigation.

This Order terminates Licensee's March 27, 2015 Interim Stipulated Order. *Yoon, Justin Kyungho, MD; MD162038; Pendleton; OR On January 15, 2016, Licensee entered into an Interim Stipulated Order to voluntarily withdraw from practice and place his license in Inactive status pending the completion of the Board's investigation into his ability to safely and competently practice medicine.
