They can range from very simple, cheap, pre fabricated kits which can be modified. Intervention with these entry level solutions is limited and effects can be short lived. If this is the most appropriate solution however we will always advise accordingly. For long term and more prescriptive devices we work with our orthotic lab to provide high quality, durable and lightweight orthosis. These are available in a range of materials including thermoplastics and carbon fibre which all come with a wide range of top covers. Lab made orthotics are completely bespoke. These are made using impression casts of the patients feet which enables a fit like no other, often likened to standing in your own footprint. The podiatrist's prescription is then added to the underside (or forms part of the plate) of the orthotic device. This is the crucial difference between prescription orthotics and off the shelf insoles. Prescription orthoses will impact on how the structures of the limb and foot function in relation to one other and to ground reaction forces. Ultimately this results in changes in loads, pressures and function and improvement in symptoms caused by functional anatomical anomalies. If you have just been fitted with new orthoses a breaking in period is required. Wear your orthotics for 1-2 hours on day one, 2 -3 on day two and so on. After about 10 days you should be wearing your new orthoses all day. You should wear these most of the time. You can expect bespoke orthoses to last for many years. Top covers can become worn, but these are easily replaced. Remember that orthoses are heat sensitive, if they become wet allow to dry naturally. Remove from shoes overnight to minimise odour and prolong the life of the orthotic. Orthoses
