With proper nutrition, children learn more easily, adults work more productively, and seniors are healthier and more apt to maintain

their independence. Federally funded through the U.S. Department

of Agriculture, SNAP helps people with low incomes buy food, and buy plants and

seeds to grow food to eat.

Child Support Enforcement help families receive child support, an important component in helping many families become self-sufficient and reduce or eliminate their need for public assistance.

Minnesota’s child protection system responds to allegations of maltreatment and abuse, and helps support families to safely care for their children.

In CY 2019, Otter Tail County received 1,074 reports alleging child maltreatment or child welfare reports. 423 Reports were opened for assessment. Reports are screened based on the MN Child Maltreatment Intake, Screening and Response Path Guidelines published by the MN Department of Human Services.

Adoption or transfer of permanent legal and physical custody (TPLPC) are options for children in foster care who cannot be safely or timely reunified with their parents. While adoption is the preferred option, for some children, neither adoption nor reunification are appropriate permanency options, but TPLPC to a relative is. Adoption and TPLPC is a process that establishes a legal relationship between a child and their prospective adoptive parents or their adult relative, giving the adoptive parent(s) or relatives permanent decision-making authority for the child.

CHILDREN’S MENTAL HEALTH SERVICESChildren’s Mental Health (CMH) programs are voluntary services that support children and adolescents experiencing severe emotional disturbance (SED), along with their family. The overall objective is to improve functioning within home and educational settings, and to support families in managing symptoms & behavior in the most inclusive environment possible.

Otter Tail County Human Services provides CMH Case Management Services and contract with Lakeland Mental Health Center to do so as well. In CY 2019 100 children received CMH Case Management Services.

ADULT PROTECTION/VULNERABLE ADULTSAdult Mental Health (AMH) programs support the overall objective

of helping people with mental health issues live as independently as possible, be active, contribute to their community and have quality of life. The goal is to provide access to the right level of treatment at the right time, and thereby reduce the need for hospitalization.

Otter Tail County Human Services provides AMH Case Management Services and contract with Lakeland Mental Health Center to do so as well. In CY 2018 203 people received AMH Case Management Services.

Medical Assistance (MA) home care and Minnesota waiver programs pay for a range of medical and health care-related support services. These services enable low-income Minnesotans, who are elderly or who have disabilities, to live as independently as possible in their communities. Waivers refer to home and community-based services available under a federal Medicaid waiver as an alternative to institutional care.

MnCHOICES is an assessment and planning tool. It uses a person-centered planning approach to help people make decisions about long-term services and supports. This assessment is an in-person visit, usually where the person lives and there is no cost for the assessment.

May 6, 2020 Otter Tail County Board of Commissioners (via email) 515 West Fir Avenue Fergus Falls, MN 56537 Dear Commissioners: We would like to inform you that Pheasants Forever will soon be acquiring in fee title a 6.83-acre tract of land with funding from the Minnesota Outdoor Heritage Fund. This property is located in Section 5, Tumuli Township (131 North, Range 42 West). We have included an aerial photo that shows the location of this property. Pheasants Forever intends to donate the property to the U.S. Fish and Wildlife Service (Service) for management as a Federal Waterfowl Production Areas (WPA). The tract will be open to the public for hunting, fishing, wildlife observation and photography, along with environmental education and interpretation. The property will remain in perennial cover. The uplands may be re-seeded to a more diverse mixture of native grasses and wildflowers and any feasible wetland restoration will be completed fall of 2020. This property will benefit numerous grassland dependent wildlife species, including waterfowl, pheasants, and declining pollinators. Water storage, flood reduction, and water quality improvements are other values that will be accrued through this restoration work. We will work very closely with our neighbors to ensure there are no negative impacts to their property. In order to maintain the grassland ecosystem, common management activities you may see on this tract in the future include prescribed burning, conservation grazing or haying, and invasive plant control. The Service will be making annual Refuge Revenue Sharing Act payments in lieu of property taxes on this tract. The Service will also dispense a onetime Trust Fund payment to the county to help make up for tax revenue shortfalls. The county will be notified by letter when this payment is released. 2020/2021 All-Hazard Mitigation Plan Purpose The Federal Disaster Mitigation Act of 2000 (DMA 2000) established programs and funding: “to reduce the loss of life and property, human suffering, economic disruption, and disaster assistance costs resulting from natural disasters” A local government plan is required to maintain eligibility for FEMA hazard mitigation grant programs. In January 2019, the County Board agreed to participate in this planning process, through a grant (now received) by state emergency management. The agreement also provides coordination of this project by a consultant, also selected by the state (University of Minnesota, Duluth and sub-contracted planner.) Local participants (county, city, public, etc.) are required to provide a match/in-kind contribution equal to $11,250.00. We estimate this will easily be met through time spent on the project. Timeline Overview 22-Month total timeline (March 2020 – December 2021) Most plans take 14-18 months. Many tasks occur concurrently, others must be done in succession Due to the COVID-19 Pandemic, timing for completing the update may be affected.

If necessary, state emergency management (HSEM) will work to extend our project contracts with FEMA to accommodate an extended timeline.

FARMLAND LEASE

PHELPS MILL COUNTY PARK

THIS LEASE AGREEMENT (hereafter "Lease") is entered into this o/ fl, day of April 2020, by and between Otter Tail County, (hereafter "Lessor") and, Dary] Tysdal (hereafter "Lessee"). In consideration of the covenants and promises made herein, the parties agree as follows:

1. Leased Premises. Lessor hereby leases and Lessee hereby accepts and rentsapproximately 46.5 acres of hay field farmland located in Phelps Mill County Park at 29283 Co Hwy 45, Underwood, Minnesota (hereafter the "Leased Premises), said Leased Premises is situated in the Southeast Quarter (SE¼) of the Southwest Quarter (SW¼) and Government Lot 3 of Section 26, Township 134 North, Range 41 West of the 5th Principal Meridian in the Maine Township, County of Otter Tail, State of Minnesota. See Exhibit "A".

2. Termination. This lease is for a term of one (1) haying season beginning on April 1, 2020,and ending on November 30, 2020, unless sooner terminated under the provisions of this Lease.

3. Rent. Lessee agrees to pay to the Lessor as rent for the Leased Premises a lump sum of Two thousand, two hundred, one and 99/100 Dollars ($2,201.99) for the hay field farmland per season. The rent shall be paid in two equal installments, the first being due and payable on or before June 1, 2020 and the second being due and payable on or before November 1, 2020.

4. Use of Premises. Lessee agrees during the term of this Lease to use and occupy thepremises only for the purpose of cutting, baling and temporarily storing hay. No part of the Leased Premises shall be used for a purpose which is illegal, offensive, or which may cause cancellation of any insurance policy covering the premises. The Lessee is responsible for the control on any noxious weeds in the hay field areas. Use of any herbicide will be coordinated with and approved by the Lessor. All hay shall be removed from the Leased Premises by November 30, 2020.

5. Facilities. Lessee agrees not to remove any dirt from the Leased Premises.

6. Condition of Premises upon Termination. Lessee has examined and knows the conditionof the Leased Premises and agrees that it is suitable for Lessee's needs. On the last day of the term of this Lease or on the sooner termination thereof, Lessee shall peaceably surrender the Leased Premises in as good condition as when received, except for reasonable wear and tear and except for damage which Lessor is obligated to repair.

7. Condition of Premises During Lease Term. The Lessee, during the term of this Lease,shall, at its expense, keep said leased premises in good condition.

8. Alterations and Modifications. Lessee shall not make any alterations or modifications tothe Leased Premises.

9. Subletting and Assignment. Lessee shall not have the right to assign this Lease or subletthe premises or any portion thereof without the written consent of Lessor.

10. Insurance. Lessee shall provide insurance coverage for the Leased Property. TheLessee shall maintain insurance to cover liability from operations under the Lease Agreement, whether such operations are by the Lessee or by anyone directly or indirectly employed under the Lease Agreement. The Lessee shall provide the following minimum limits of liability:

(a) $200,000-Per occurrence,(b) $200,000-Annual aggregate,( c) $200,000 -Annual aggregate applying to Products and Completed Operations,(d) $50,000-Fire damage, and(e) $5,000- Medical expense (any one person per occurrence).

The liability limits specified above are the minimum limits required, and any and all additional limits provided to the Lessee will be available on an excess, umbrella or other basis to the Additional Insured for any and all covered claims. An excess or umbrella liability policy may be used in conjunction with primary coverage limits to meet the minimum limit requirements.

Lessee shall, at its own cost and expense, maintain general liability insurance, insuring against claims for bodily injury, death or property damage occurring in or about the Leased Premises.

11. Lessee's Access to Property. As a part of the agreement Lessor agrees to allow Lesseeunlimited access to the hay field farmland. Access to the hay fields will be through the field approach north of the building site as shown on Exhibit "A".

12. Lessor's Access to Leased Premises. Lessee shall allow Lessor and/or Lessor's agentsor employee's unlimited and unrestricted access to the Leased Premises.

13. Event Parking on Leased Premises. Lessee shall allow the Lessor's and Phelps MillCoW1try1s agents or staff unlimited and unrestricted access to the Leased Premises for the following two events to be held at Phelps Mill County Park: 2020 Governor's Fishing Opener Picnic and the annual Phelps Mill Festival. The Lessor will be holding the 2020 Governor's Fishing Opener Picnic on Friday, May 8, 2020 and Phelps Mill Country will be holding the annual Phelps Mill Festival on Saturday and SW1day, July 11 and 12, 2020 with festival set up on Friday July 10. The east hay field on the Leased Premises, approximately 14.3 acres, will be used as the Parking Area for both events. The hay in the area designated as East Hay Field (Festival Parking Area) on Exhibit "A" of this Lease will be cut by Phelps Mill CoW1try's agents approximately 4 to 8 days prior to the Phelps Mill Festival. With these two events it is anticipated that no haying would occur prior to the Phelps Mill Festival, however, the Lessee shall have the option to cut and bale the hay in this area as part of this Lease Agreement prior to Phelps Mill Country's agents cutting the hay for the Phelps Mill Festival. A request to cut and bale the hay shall be made in writing at least two weeks before the hay is scheduled to be cut by Phelps Mill Country's agents. Any bales produced shall be removed from the site or moved to the edge of the hay field prior to the Phelps Mill Festival. The Lessee shall fully cooperate with Lessor and Phelps Mill Country in preparing for and conducting these events.

14. Indemnification. Lessee agrees to indemnify and hold Lessor harmless from any andall liability arising from Lessee's use of the Leased Premises, including but not limited to payment

of Lessor's reasonable attorneys' fees and expenses. Lessee so agrees to indemnify and hold Lessor harmless as a result of any claim, loss of damage or liability or expense arising out of or resulting from any affirmative act, negligence, or omission of the Lessee or their agents, contractors, subcontractors, or employees. Lessee agrees to furnish Lessor with a certificate of general liability coverage, which names the Lessor as an Additional Insured.

15. Notice. All communications, demands, notices, or objections permitted or required tobe given or served under this Lease shall be in writing and shall be deemed to have been duly given or served if delivered in person to the other party or mailed and addressed to the other party to the address set forth next to that party's signature at the end of this Lease. Any party may change its address by giving notice in writing, stating its new address, to any other party as provided in the foregoing manner. Commencing on the tenth day after the giving of notice, the newly designated address shall be that party's address for the purposes of all communications; demands, notices, or objections permitted or required to be given or served under this agreement.

16. No Partnership, Joint Venture, or Fiduciary Relationship Created Hereby. Nothingcontained in this Lease shall be interpreted as creating a partnership, joint venture, or relationship of principal and agent between the Lessor and the Lessee, it being understood that the sole relationship created hereby is one of landlord and tenant.

17. Cumulative Rights. No right or remedy herein conferred on or reserved to the Lesseeor the Lessor is intended to be exclusive of any other right or remedy provided by law, but each shall be cumulative in and in addition to every other right or remedy given herein or elsewhere or hereafter existing at law or in equity or by statute.

18. Attorneys' Fees. If any action at law or in equity shall be brought in court to recoverany rent under this Lease or for or on account of any breach of or to enforce or interpret any of the covenants, terms, or conditions of this Lease or for the recovery of the possession of the premises, the prevailing party shall be entitled to recover from the other party as part of the prevailing party's costs its reasonable attorneys' fees, the amount of which shall be fixed by the court and shall be made a part of any judgment or decree rendered.

19. Successors and Assigns. Except as otherwise specifically provided for herein, thisLease shall be binding on and shall inure to the benefit of the parties hereto and their respective assigns and successors.

20. Amendment, Modification, or Waiver. No amendment, modification, or waiver of anycondition, provision, or term of this Lease shall be valid or of any effect unless made in writing, signed by the party or parties to be bound, or its duly authorized representative, and specifying

with particularity the extent and nature of such amendment modification, or waiver. Any waiver by any party of any default of another party shall not affect or impair any right arising from any subsequent default.

21. Severable Provisions. Each provision, section, sentence, clause, phrase, and word ofthis Lease is intended to be severable. If any provision, section, sentence, clause, phrase, and word hereof is illegal or invalid for any reason whatsoever, such illegality or invalidity shall not affect the validity of the remainder of this Lease.

22. Entire Agreement. This Lease, including all exhibits, contains the entire understandingof the parties hereto with respect to the transactions contemplated hereby and supersedes all prior agreements and understandings between the parties with respect to the subject matter. No representations, Warranties, undertakings, or promises, whether oral, implied, written, or otherwise, have been made by either party hereto to the other unless expressly stated in this Lease or unless mutually agreed to in writing between the parties hereto after the date hereof, and neither party has relied on any verbal representations, agreements, or understandings not expressly set forth herein.

23. Captions, Headlines, or Titles. All captions, headings, or titles in the paragraphs orsections of this Lease are inserted for convenience of reference only and shall not constitute a part of this Lease as a limitation of the 'scope of the particular paragraphs or sections to which they apply.

24. Minnesota Law. This Lease shall be construed and enforced in accordance with thelaws of the State of Minnesota.

OTTER TAIL COUNTY PREPAREDNESS/REOPENING PLAN

Otter Tail County is committed to providing a safe and healthy workplace for all our employees and customers. To ensure that, we have developed the following COVID-19 Preparedness/Reopening Plan in response to the pandemic. Managers and employees are all responsible for implementing this plan. Our goal is to mitigate the potential transmission of COVID-19 in our workplaces and communities, and that requires full cooperation among our staff, management, and the public. Only through this cooperative effort can we establish and maintain the safety and health of our workplace.

Otter Tail County will consider the health of all employees and the public as we move forward with reopening offices for public on-site access while applying best practices for social distancing in the workplace. This document will incorporate preparedness and reopening the buildings to the public as well as bringing employees back to their worksites. Employees are responsible for implementing and complying with all aspects of this COVID-19 Preparedness/Reopening Plan.

EMPLOYEES: SCREENING AND POLICIES

Employees have been informed of and encouraged to self-monitor for signs and symptoms of COVID-19. The following policies and procedures are being implemented to assess an employee’s health status prior to entering the workplace and for employees to report when they are sick or experiencing symptoms.

Otter Tail County has implemented leave policies that promote employees remaining at home when sick, when household members are sick, or when required by a health care provider to isolate or quarantine themselves or a member of their household. Please find policies specifically related to this public health emergency on the employee dashboard.

Reduce Transmission Among Employees

Employee Exhibits Signs and Symptoms of COVID-19:

a. Employees will be trained and required to follow the attached COVID-19 Guidelines and Employee Screening Tool, attached as Appendix A, including taking their temperature before reporting to work.

b. Employees that have symptoms (i.e., fever, cough, shortness of breath, chills, muscle pain, sore throat and new loss of taste or smell) should notify their supervisor and stay home. Sick employees should follow CDC and MDH recommendations which generally provide to stay at home for at least 10 days and for 3 days without a fever, whichever is longer, and call their healthcare provider about testing. Consult CDC and MDH for most recent information. Additionally, if an employee is being advised by a medical provider, that recommendation should also be followed. Based on CDC guidelines, employees are not required to provide a return to

work statement from a doctor to allow medical providers to concentrate on treating those with COVID-19 and it may be difficult for an employee to obtain one.

c. If an employee is at work and exhibiting COVID-19 symptoms as referenced above, the employee should be promptly sent home and asked to call their healthcare provider about testing. The employee should be asked to identify all individuals he/she was in close contact (within six feet) for at least one hour during the prior 48-hour period (for additional information, review the CDC’s definition of close contact). It may be determined appropriate for employees in close contact may be sent home. Additionally, if an employee tests positive for COVID-19, MDH will conduct a contact investigation and employees may be notified if they had possible exposure to COVID-19 in the workplace. To protect privacy as required by state and federal law, Otter Tail County will not identify, by name, an individual who has contracted the virus.

Employee Having Exposure to COVID-19:

If an employee may have had exposure to a person with suspected or confirmed COVID-19, that employee should remotely work for at least 14-days and monitor for symptoms as recommended by CDC and MDH. If an employee cannot remote work because he/she is part of the critical infrastructure and may have had exposure to a person with suspected or confirmed COVID-19, the CDC has set forth guidelines to ensure continuity of operations while allowing the employee to work. The guidelines include pre-screening, regular monitoring, wearing a mask, social distancing, and disinfecting and cleaning workspaces. While social distancing, employees should maintain six feet and practice social distancing as work duties permit in the workplace. Also, maintain physical distance when taking breaks including avoid congregating in the break room.

Best Practices to Mitigate Spread:

a. Temperature: Employees will take their temperature before reporting to work. If the employee’s temperature is over 100.4 degrees, has a fever or feels feverish, employee will notify their supervisor and is not to report to work. If able, the employee can remotely work.

b. Handwashing: Basic infection prevention measures are being implemented in our workplaces at all times. Employees are instructed to wash their hands for at least 20 seconds with soap and water frequently throughout the day, but especially at the beginning and end of their shift, prior to mealtimes, and after using the restroom. Hand sanitizers are also available throughout the building but should not take the place of regular handwashing if possible.

c. Respiratory Etiquette: Employees are instructed to cover their mouth and nose with their sleeve or tissue when coughing or sneezing and to avoid touching their face, in particular their mouth, nose and eyes, with their hands. Tissues used should be disposed of in the trash with handwashing or sanitizing immediately afterward.

d. Social Distancing: Social distancing is being implemented in the workplace through the

following engineering and administrative controls being considered: Remote working, staggered shifts to reduce the number of employees in the workplace at one time; maintaining six feet apart between employees and co-workers, and employees and the public using signage or

instructions; regulating number of employees in county vehicles; and support communications plans to address employee concerns. Cloth masks have been made available to employees. Partitions will be used at front desks and for employees having face-to-face interviews or meetings with clients. Employees can have customer interactions through drop-off, pickup or delivery if it can be done safely. During this pandemic, employees, visitors, and the public are prohibited from gathering in groups. Additionally, refrain from gathering in confined areas such as breakrooms, hallways, and small meeting rooms. Where able, do not share phones, computer equipment, desks, cubicles, workstations, offices or other personal work tools and equipment. If sharing equipment, ensure proper cleaning and disinfecting between employees.

e. Sanitizing and Cleaning: Regular housekeeping practices are being implemented, including

routine cleaning and disinfecting of work surfaces, equipment, and areas within the work environment. Employees should regularly clean high-touch areas in their workstations such as phones, keyboards, touch screens, and controls. Facilities employees will continue regular cleaning of common areas. If an employee develops COVID-19 symptoms while at work, the work area will be immediately cleaned and may be unavailable for access by others to mitigate exposure to other employees.

f. Facemasks: Employees will wear cloth face coverings in work settings where other physical

distancing measures will be difficult to maintain. Situations where social distancing is difficult includes small common areas such as coffee counters within departments, hallways, and small meeting rooms. Employees will also wear cloth face coverings when engaging with the public. Wearing a cloth face covering is not a substitute for maintaining social distancing, which is one of the best ways to prevent disease transmission. Please see Face Mask FAQs for further information regarding face masks.

g. County Vehicles: One person per vehicle should be practiced as much as possible to minimize

the spread of COVID-19. If you must travel with another person, such as a client or to transport children, follow CDC guidance for rideshare such as wearing a cloth face covering, passengers sitting in the back of the vehicle, and do not transporting those with symptoms if possible.

County vehicles should be cleaned before and after employee use, particularly if the vehicle is shared with other employees. At a minimum, clean and disinfect commonly touched surfaces in the vehicle. Other guidance is found on the CDC website.

h. Communication and Training: The Preparedness/Reopening Plan will be available to all

employees through the Employee Dashboard. Training will be provided to employees and supervisors will monitor how effectively these policies are being implemented. This plan will be updated as determined necessary.

PHASE I: REOPENING

Otter Tail County will be reopened in phases and the buildings will remain locked during Phase I, which will begin on *****. This guide is a general overview of expectations during reopening buildings and each department will develop its own Preparedness/Reopening Plan, specifically identifying services that will

require public access to the buildings for face-to-face engagements with employees. The main objective of the phased plan is to keep our citizens and employees safe and healthy while gradually reopening County services. Related strategies include the following:

All buildings remain closed to general walk-in traffic during Reopening Phase I. The public is encouraged to access services virtually, online or by phone. Service by appointment only and building escort will be offered for certain services that cannot be completed via other methods or are difficult to complete online due to lack of electronic solution, complexity of transaction, or customer barrier to accessing the service line. Electronic processing of documents and applications, and delivery of services via virtual methods are continued and increased where possible. Methods for effective virtual service delivery will continue. It is recognized that some individuals are limited in their ability to access services via technology and departments will accommodate their needs to ensure services are received. Physical spaces are appropriately prepared (room arrangements to support social distancing, protective barriers installed, enhanced cleaning protocols continued, etc.) Employees continue to work remotely where effective service delivery can be provided through remote work arrangements.

Appointment Scheduling

Public access to visit County facilities will be by appointment-only services. Screening will be given to individuals accessing the building and they will also be asked to wear a cloth face covering/mask (recognizing that face masks are not recommended for everyone such as children under age 2 or those who have trouble breathing). Also, encourage that the building is accessed by only those individuals that are necessary. Those needing to enter the building will be screened. When appointments are made, the following script should be followed to ensure the individual is aware of the screening tool, that the appointment is limited to only necessary individuals attending, and that a face mask should be worn to the appointment. If they do not bring a mask, we will offer a disposable one.

In the interest of the health and safety of the public and staff during this pandemic, do not visit a County facility for an appointment if you have had any of the following in the last 10 days: a fever of 100.4 degrees Fahrenheit or higher, shortness of breath, new muscle aches, new sore throat, or a new or increased cough OR if you have had close contact with someone that was diagnosed or suspected to have COVID-19 within the last 14-days. Due to the COVID-19 pandemic, we need everyone (who is able) seeking appointment services to visit our facilities wearing a cloth covering or mask. Who needs to attend the appointment with you and are they free of symptoms? Are you, and anyone who will be with you, able to wear a cloth face mask or covering to your appointment? (If the visitor is unable to wear a mask or refuses to wear a mask, discuss alternatives as well as whether it is necessary to request supervisor assistance to determine how best to proceed.)

Each department is responsible to ask health screening questions as set forth above. Additionally, employees are to use the screening tool (attached as Appendix B) for the public at the time of their appointment. Screening questions will also be posted on the entrance doors. At the time of the appointment, a staff member from the Department will meet the public member or client at the front door of the building and review the screening tool with them prior to allowing access to the building. If the visitors are experiencing any symptoms, the appointment should be rescheduled. Individuals entering the buildings should wash their hands or sanitize upon entrance to the building. Employees should offer disposable masks if not being worn by the individual. If the person is unable to wear a mask or refuses to wear a mask, you may need to consider alternatives. During the appointment, staff should wear a face mask and practice social distancing. When the appointment has ended, the visitors will be escorted out of the building by staff. After the appointment employees may be responsible for cleaning the area. Facilities will clean as well but may not be available during regular work hours to sanitize an area used for an appointment.

Otter Tail County is limiting public access to all county buildings to appointment in their Phase I Reopening.

Employees scheduling a face to face appointment with the public are required to follow the guidelines and

use the screening tool. If required by your department, discuss with your supervisor and document the client

screening tool.

The following screening must be used when proceeding with the face-to-face contact:

I. Complete screening prior to making an appointment, via phone call and timely (e.g. the day of the

visit).

a. Make it known that these questions are being asked to all people that are served to protect

the health of client and you to minimize the risk of spreading illness.

b. Tell the person that if you are able to meet with them you will need to ensure 6 feet social

distancing from them during the appointment and will not be able to shake their hand, hold

their children, or come in close personal contact. They will also be required to use a face

covering during the appointment. Tell the person it is not personal, that you are just

following the guidelines.

Screening

Have you had any of the following symptoms that you cannot attribute to another health condition?

1) Please answer “Yes” or “No” to each question. Do you have:

o Fever (100.4 F or higher), or feeling feverish?

o Chills?

o A new cough?

o Shortness of breath?

o A new sore throat?

o New muscle aches?

o New headache?

o New loss of smell or taste?

2) Have you come into close contact (within 6 feet) with someone who has a laboratory confirmed

COVID-19 diagnosis or that has self-isolated due to fever or symptoms of respiratory illness in the

Based on the screening, use the guidelines below:

I. If the person answers yes to any of the questions, tell them you are unable to meet face-to-face with

them currently. Offer to meet with them over the phone, utilize secure email for correspondence,

etc.

II. If the person reports no symptoms, no close contact (within 6 feet) with someone who has a

laboratory confirmed COVID-19 diagnosis or that has self-isolated due to fever or symptoms of

respiratory illness in the past 14 days, you may schedule an appointment with the following

guidelines:

a. Put on a face covering or mask. Use other PPE as indicated.

b. Only use what you need to do your visit.

c. Ensure social distancing of at least 6 feet.

d. Perform hand hygiene (wash hands for 20 seconds or use an alcohol-based sanitizer that

contains 60 to 90% alcohol, before and after your appointment.

e. Discard PPE and clean equipment used during the appointment (electronic pens, pens, other

equipment)
