to oversee the development of the Primary Care Directory
members comprising representatives from the government, the Hospital Authority, academia as well as different healthcare bodies
Objectives of the Directory
provide the public and healthcare service providers an easily accessible electronic database containing practice-based information of primary care professionals of various disciplines in the community
foster partnership between individuals and primary care practitioners as health co-ordinators
facilitate the co-ordination among different primary care providers functioning as multi-disciplinary teams

Professionals to be included
western medical practitioners (doctors)
dentists
Chinese medicine practitioners
other members of the primary care team (e g nurses and other members of the primary care team (e.g. nurses and allied health professionals)
Sub-directories
sub-directories with modifications will be developed for different professionals
the development will be in phases
starting with the Doctor and Dentist sub-directories

Maintenance requirement
Continuing Medical Education (CME)
Continuing Professional Development (CPD)
Delisting from the Directory
voluntarily withdrawn from the Directory
ceased to be a registered doctor or dentist
failed to meet the requirement for remaining in the Directory

How to enrol Registered doctors and dentists can enrol by assessing
the Directory on-line at https://www.pcdirectory.gov.hk

Provide personal and practice-based information to completethe enrolment process online
Action by Department of Health
Online Enrollment in the Primary Care Directory
Submit HKID card copyby fax / post within one week*
application and the need to submit HKID card copy within one week
Relevant information of application to be sent to MCHK / DCHK for verification of registration status
Vetting of relevant information provided by the applicant
Successful applicant will receive an emailwith account activation link
within 4 weeks upon submitting HKID card copy
Click into the link, select own usernameand password to activate the account
Information will be made available to the public upon official launch of the Directory by
end of March 2011
