Areas of Focus:

Business Development - supports and focuses on tasks and processes aiming at preparing and supporting the implementation of growth opportunities throughout the ambulatory continuum

Physician Relations- seeks to be a trusted partner and assist providers and office staff in connecting patients to LH’s clinically integrated network

Community & Business Relations – engages with communities and businesses in SWFL in providing H&W education, navigation, and follow-up

Continuum Relations- assists with transitioning patients from acute to post acute care, getting the patient the right care at the right place

Healthy Life Centers: offers a variety of H&W education, lectures, and services that support healthy lifestyles, early detection of disease, and CDM

Most internal referrals can be placed in Epic.

Referrals will either go to Centralized Scheduling or directly to the service line.

Centralized Scheduling will direct your patient to the most convenient location for them.

Authorization for the order is obtained for most imaging, rehabilitation, pain management, and sleep medicine referrals.
