1) J& Desert Greens Custom Lawn Care agrees to the proposed rates the City of Tempe have
offered in their best and final offer to RFP #15-064. if .J.k Desert Greens Custom Lawn Care is
awarded to the short list, pricing for all jobs (future and existing) will reflect this fee schedule.
2) J.A. Desert Greens Custom Lawn Care acknowledges the monthly turf service changes of
February to once per month.
3) The owner of J.A. Desert Greens Custom Lawn Care and any of its employees agree to a
background check, drug screening, and E-Verify. J.A. Desert Greens Custom Lawn Care uses
Arcpoint labs for its background checks and drug screening. The process used to screen for
drugs is a multi point urine analysis. The background check is reviewed for any misdemeanors
and felonies. The applicant is automatically dismissed if anything that falls into certain
categories is found such as but not limited to: theft, vandalism, assault, substance abuse. To
verify eligibility to work in the US. J.A. Desert Greens Custom Lawn Care is enrolled with E-
Verify with www,uscis.gov/
4) The catch up provision will only go into effect if:
A: It is the first time J.A. Desert Greens Custom Lawn Care is servicing the property
AND
B: The property requires SO% more work hours than normally scheduled for properties
less than 15,000 sq ft, and 25% more work hours for properties that are over 15,000 sq ft.
5) J.A. Desert Greens Custom Lawn Care would remove any small branches, sticks,
miscellaneous debris for free. As well J.A. Desert Greens Custom Lawn Care would review
customer properties and will replace low dollar items free of charge (defined as under $1.99 of
materials and around 1 minute to replace; example 1/4" drippers that have become corroded
with calcium build up).
When originally submitted RFP #15-064 I was under the impression that I had not gotten a
employer identification number. I was incorrect, the EIN for J.A. Desert Greens Custom Lawn
Care is 46-2498849.

Animal waste pick-up/removal. Eventually sprinkler repair, tree trimming and hard scape landscaping.

Desert landscape is typically maintained with first removing all large
debris (sticks, pine-cones, etc.) by band. Then small debris(leaves, dust,
etc.) will be raked/blown into a collection pile and vacuumed up for
disposal. Turf landscape is the same except after a initial sweep of for
large debris, we will edge and weed, then mow the lawn.
28. Describe how your company is staffed to support this contract and what measures can be taken to increase personnel and equipment capacity, if needed.
Employees are screened to meet certain requirements (want to
work outdoors, have a high customer service competence, pass
both background and drug screening). If expansion is required equipmeM
will be procured and employees will be hired.
29. Describe how the company will perform quality control to ensure services are provided as specified and that help prevent deficiencies in the quality of services delivered,
The owner will preform random quality control visits to properties, as well keep an open line of communication to clients

The following changes are the result of a Pre-Proposal Conference held on November 5, 2014:
1. The Proposal Due Date remains unchanged at November 20, 2014 at 3:00 P.M. (MST).
2. The deadline for inquiries is hereby changed from November 7, 2014 to November 12, 2014 at 5:00 P.M.
3. Scope of Work section, Page, 18, General Overview, Paragraph 1, Purpose: Remove the reference to "owner-occupied" that appears in the second line of this paragraph. The City will allow both owner-occupied residences and renter-occupied residences to participate in this program.
4. Proposal Questionnaire, Page 25, Question #14: for those companies that have an extensive list of service vehicles, it will not be necessary to identify each and every vehicle. A general summary of the types of vehicles and the number of vehicles that will be made available to this contract shall be identified.
5. Pricing Section, Page 30 and Page 31: This Pricing Section is deleted in its entirety and is replaced with the attached Revised Pricing Section.
6. Questionnaire — The City has included a Word version of the Questionnaire that can be used to input responses to the various questions listed.
There are no other changes at this time. Please be sure to sign and return this addendum with proposal per the revised due date noted above.
The balance of the specifications and bid solicitation instructions to remain the same. Bidders/Proposal Offerors we to acknowledge receipt and acceptance of this addendum by returning of signed addendum with bid/proposal response. Failure to sign and return an addendum prior to bid/proposal opening time and date may make the bid/proposal response non-responsive to that portion of the solicitation as materially affected by the respective addendum.

Question #35 Describe the company's customer service philosophy
Landscape maintenance is a service industry. Typically once a threshold has been met,
one cannot do a better job than anyone else. Yes quite frequently J.A. Desert Greens
finds themselves picking up(literally) after sub-par landscapers. I would argue to say
that we do pick up leaves equally well as anyone else, if not better than most. We at
J.A. Desert Greens are different in the fact that we put the customers needs above
everything, if there is one word to describe J.A. Desert Greens we want it to be
'Service'. Service is the one thing I see missing from this industry and is my major
goal in building this business.

list.
J.A. Desert Greens Custom Lawn Care is a small business who is involved in the
community via Tempe Chamber of Commerce. If awarded the short list J.A. Desert
Greens will grow as a business and further its involvement in the community. With
the growth will come more jobs, more jobs in Tempe will keep more money in the
city of Tempe. Our highest goal is customer satisfaction. J.A. Desert Greens strongly
believes in an open line of communication with customers. Every employee is fluent
in English and can effectively communicate if needed. Customers will see a huge
intangible value in easily being able to express their wants and needs with our
company.
