Parks and Reservations
Login Screen
This login screen is used to login into the park and recreation system to make a
reservation for an event at a park, community center, picnic pavilion or a marina
slip and summer camp. A valid email address is needed to create a profile for use
with the system. If it is the first time you have utilized this system, use the link on
the page Click Here to create a new profile. This will take you to the Create Profile
screen to set up a profile to be used for any reservation event offered by the parks
and recreation system. If you have a profile but have forgotten your password,
please contact park administration and do not create a second profile. See Create
Profile screen below.

Create Profile Screen:
All fields marked with an asterisk are required fields and must be provided.
First Name – customer’s first name
Middle Name – customer’s middle name – this is optional
Last Name - customer’s last name
Address - address of the customer
City - This is pre-filled for Jacksonville but can be changed
State - State for the address
Zip - Zip code of the address
Home Phone - a valid home phone number
Fax - Fax number – optional
Alternate - an alternate number in which the customer can be
reached – optional
Pager - Pager number – optional

Save changes - This is used to save all data

I want to reserve a picnic pavilion
After reviewing all of the specifications for reserving a picnic pavilion, choose the
link at the bottom of the square labeled:
Click here to reserve a picnic pavilion!
This will take you to the list of available parks that contain pavilions for rent.

Choose A Park
Choose the park from the list on the left in which to reserve the picnic pavilion.
After selecting the park, you will be then pick a date for the reservation. At this
time you must give an estimate as to how many people will be in attendance. See
screen below.

Park Availability
Please be as accurate as possible as JSO may terminate your event if your actual
attendance is greater than your estimate.
You will then be forwarded to the availability screen. This will inform you as to
whether the park you have chosen is available for the date you have chosen. If
there are no picnic pavilions available, Reserved will be shown as the status for the
pavilion you have chosen and you will need to choose the BACK arrow to return to
the selection process. If the pavilion is available the following screen will be
displayed:

Availabilities
Choose the NEXT arrow to continue the reservation process. A questionnaire will
be displayed and all questions must be answered. This questionnaire will determine
what permits or licenses must be presented before the reservation can be
approved. See screen below:

Screen Example:
Choose the NEXT arrow once questionnaire has been completed. Once completed a
summary and complete total for the reservation will be presented. This
questionnaire will change the specifications for the reservation

Summary Screen:
Please review the reservation to verify all entries are correct. If items were
selected on the questionnaire then the reservation becomes a special event. This
cost is different than the regular rental of a picnic pavilion. If no selections were
made on the questionnaire as in the screen below then the summary and total cost
for the reservation will reflect that in the cost presented for the reservation. See
below:

Nothing was selected on the questionnaire above. Only a description of the event
was entered. After the NEXT arrow is chosen, the summary and total cost is
presented.

Choose the NEXT arrow to continue the reservation process. You must accept the
reservation agreement in order to continue. Please review this agreement so that
you will not be in any violations and your reservation to be revoked or canceled.
If you select the I DO NOT ACCEPT THIS AGREEMENT button the reservation
process will be canceled. To proceed with the reservation select the I ACCEPT THIS
AGREEMENT button. You will then be taken to the Checkout-Credit Card screen.
Unless you go into the office of parks and recreation to make your reservation the
only payment option you will have is credit card. See screen below:

Checkout Screen:
The amount will be reflected from what was answered on the questionnaire form.
Please review the payment information. The balance amount due should be exactly
as it was on the summary page. Please select the type of credit card you are using.
Presently the parks and recreation department only accepts Visa and MasterCard.
Once the information has been entered choose the NEXT arrow to continue. You
will be emailed a receipt and confirmation for your reservation. You are also able to
print the receipt that will be on the screen by using the link labeled Printer Friendly
Version

Confirmation Screen:
The confirmation screen is your permit. Please print this screen and bring with you
to the park event as validation of your reservation. Choose the NEXT arrow to
complete the reservation process. This will return you to the home screen to either
make another reservation or sign out.

I want to hold an event at a park
After reviewing all of the specifications for holding an event in a park, choose the
link at the bottom of the square labeled:

This will take you to the list of available parks available for holding an event
Where do you want to hold your event?
Select a district and you will be given a list of parks to choose from. You can also
click on an area on the map and also get a list of parks to select from. Choosing a
district Yields a list of parks for that particular district. See screen below:

This list can also be displayed by selecting the 3 on the map for planning district 3.

Choose a park from the list and you will be taken to the Pick a Date screen as
above to begin the reservation process.
Pick a Date
Choose the NEXT arrow. This will check the availability of the chosen park.

Availability
Choose the NEXT arrow. This will take you to the Questionnaire. Unlike the
pavilion reservation, the cost for holding an event in the park is the same whether
you select items on the questionnaire or leave them blank. You must answer the
questionnaire in order to proceed.

Choose the NEXT arrow. You will then be taken to the summary page for your
reservation.

Choose the NEXT arrow. You will then need to accept or not accept the agreement
as above.

After you accept the agreement you then be taken to the Checkout-Credit Card
screen for payment. All payments done outside of the parks and recreation
department will have to be processed by credit card. At this time parks and
recreation only takes Visa and MasterCard.
Check-Out Screen
Choose the NEXT arrow to continue. You will be emailed to the account on the
profile a copy of the reciept. You also have the ability to print a reciept from the
screen by selecting the link Printer Friendly Version

Reservation Confirmation
The Reservation Confirmation screen is your permit or authorization document.
Print and bring this with you to the event. Select the NEXT arrow. This will take
you back to the home screen to make another reservation or sign out.

I want to go to my summary page to view my existing reservations
or view / edit my profile
This link will display all reservations you have created for your profile in the parks
and recreation system. Click on the reservation number and you will be able to
review information entered for the particular reservation.

Fields
Customer Name - Customer who made the reservation
Status - Status of the reservation:
Approved
Cancelled
Hold
Reservation Date - Date of the event
Reservation Time - Time of the event
Park Name - Name of the park where the event will be held
Facility Type - What time of facility where the facility will be held
Requested on - When the reservation was made
Hold Expire Date - When reservation will expire if all required documents
are not provided
Est. Attendance - Number of guess present at the event
Special Event - Is this event classified as a special event which will
require permits
Special Use - Will the event have special use items such as alcohol,
food, etc.
Permit Serial # - Permit number assigned to the reservation.
Permit Issued - As a permit been issued for this reservation

Reservation Amount - Total cost of the reservation
Paid - Indicates that the reservation been paid for
Cancel This Reservation - This is used to cancel the reservation

My Profile at the top of the page is used to modify your profile record for the parks and recreations
system. Select the link labeled View or edit your profile hear

I want to make a Summer Camp Reservation
After reviewing all of the specifications for a summer camp reservation, choose the link at the bottom of
the square labeled: Click here to go to Summer Camps !
This will take you to the screen and menu for registering your kids for the summer camp program at the
City of Jacksonville.

Welcome to Summer Camps
You must first go the My Account tab on the left hand of the screen to enter an account to be used. If you
have initially established an account with the parks and recreation system for any of the other events
available, you will be able to use the same account. Under the My Account tab is how you will set up and
alter your kids for the summer camp programs.

My Account
The first tab under My Account is Edit my profile. Select this option if you need to change any
information about yourself such as address, email address, password. The same fields will be visible as
with the edit my profile screen above. Refer to this section for any questions about the information
needed on this screen. If all of your information is the same, just select another option to continue.

The next option labeled Add Member to my Account is used to add the kids you will be responsible for
the payment for summer camp.
Form must be completed for all members. All fields that are marked with asterisks are required. All
other fields are optional. Please enter as much information as you can as this will assist the park personal
with the care of your children
Member Profiles
Fields
Status: This field is prefilled which sets the status to
ACTIVE to allow eligibility for summer camp
First Name: First name of child
Middle Name: Middle name of child
Last Name: Last name of child
Gender: Gender of child
Birthday: Birthdate of the child
Phone: If child has a phone, enter their phone number
Can child swim unassisted: Is the child able to swim?
Can child walk home: Is child allowed to walk home alone?
Last for of SSN: Last four digits of the child’s social security number
Student Number: Number assigned to the child via the school system
Mother’s Phone #1: telephone number the mother can be reached
Mother’s Phone #2: telephone number the mother can be reached

Father’s Phone #1: telephone number the father can be reached
Father’s Phone #2: telephone number the father can be reached

Voucher Eligibility: Is the child eligible for receiving a voucher, this will
be determined by park personnel. Parents are
unable to modify this field.
Medical Information:
Describe any medical conditions, allergies, medications, or any other conditions of the participant of
which we should be aware. Please enter any information that can be useful in the caring for your child
Doctor’s Name: Name of the child’s Doctor
Doctor’s Phone Number: Number to reach the child’s doctor
Emergency Information: This information will be used in case any emergency arise
with your kid and the mother or father cannot be
reached
Name: Name of the individual to contact
Relationship: What relation this person is to the kid
Address: The address of the emergency person
Home Phone: Home Phone number of the emergency person
Work Phone: Work Phone number of the emergency person. If
there is no work phone please enter a cell number
or the home phone number
Cell Phone: Cell phone number of the emergency person
Save: Use this button to save the information to the
jaxParks summer camp system
Cancel: To cancel this entry

Please verify that the message Member added successfully is display at the bottom of the screen. This
will assure that the member was successfully added to the JaxParks summer camp reservation system and
is connected to your customer profile.

Add Member to my Account
Select your member in order to change any the information if entered incorrectly. Only the fields marked
with an asterisk can be altered.’

Camp Registrations
Register Member for Camp
Choose your member that you would like to register for camp. If you have more than on member register
all members for camp Then pay for them at the end.

Choose an available camp from the list.
Choose the session you are registering for. The drop down list will contain all of the available sessions
for the camp you have chosen.

Fields
Member’s First Name: Should be the child’s first name
Member’s Last Name: Should be the child’s last name
Member’s Age: This is the age of the child
Camp: Name of the camp
Session: Session of the camp you are registering for
Online Slots Available: Available slots to be used to register online
Extended Care: Will your child need extended care beyond the camp time.
There is an additional fee for this service.
Can your child use the rest room without assistance?
Does your child need assistance with using the rest room.
Does your child require one-to-one supervision?
Can your child operate alone?
Activity Amount: This will cover the extra activities provided by the camp
Session Amount: This is the cost of the camp and can be covered by a
voucher if you qualify. You must apply with the parks
and recreation system.
Extended Care Amount: Amount for the extended care described above.
Teen Power: If your child will be participating in Teen Power
Total Amount Due: This will be the total amount due for the session of camp
you are registering for. Any vouchers will be deducted
once you paying for the session.
General Release of Liability Terms:
See below for the General Release of Liability for summer
camp. If you agree, please select the check box else
depress the cancel button to cancel the entire transaction
Add to Shopping Cart: Depress this button to either go to the payment screen or
select another child to register for camp. All children
must be part of your customer profile
Cancel: This will delete the transaction and return you to the
home page in which you began.
General release of Liability

I hereby release and agree to hold harmless the City of Jacksonville, Department of
Recreation and Community Services, its employees and sponsors, from all liabilities
and claims that may accrue from injury to my child or property. I further hereby give
my release for field trips and the publishing of any and all photographs or likeness. I
further agree to sign my child in every day upon arrival and departure from the camp
location. I also give my permission to take necessary arrangements to provide
emergency care and treatment for my child if an accident or serious illness occurs. I
also understand that falsification of information detailed in the application about my
child’s age will result in immediate termination of my child from camp.

point. If this is chosen, you would need to begin the process

Pay Now: Use this button only if you are ready to pay for the registration.
Please make sure you have registered all members you need
summer camp sessions for before using the Pay Now button.

Use this button to continue registering member for summer camp.
This button will take you back to the registration process by
listing your members for selection. If the Pay Now button is chosen, you will be
taken to the payment method screen. At this time unless you come into the
parks and recreation department the only payment option you will have is credit
card. Cash, Money Order, and checks can be used if you place registration in
the office.

Credit Payment
Your shopping cart will be viewable at the top of the screen. The following fields will need to be
entered in order to process your credit card for payment.

Total Amount Due: This will be the complete amount needed for registration.
Voucher Amount: If you have qualified for a voucher the amount for the
camp session will show here. Remember – Only the
camp session is valid for voucher payment and you will
be responsible for all other costs.

session starts there are no refunds. There is a 20.00 Administrative Fee per child. Please read and reply to this
statement before hitting the next button. If you forgot to add something to the registration, choose the back button to
return to the registration screen to add or delete any of the options.

Total Amount Due - Amount due for payment and are prefilled
Voucher Amount - This will be included if the member has been assigned a voucher
Cash Amount - Amount that will be due in Cash for cash payment
Money Order Number - Number for money order if used for payment
Money Order - Amount for payment on the money order
Check Number - Number for check if used for payment
Check Amount - Amount for payment on the check
Amount Paid - This should be total of all items being used for payment. The
amount cannot be more or less than the Total amount due.

Once the payment has been processed you will be presented with a receipt for the payment. There will be
a button at the bottom of the screen that will let you enter a new registration.
Payment confirmation
You will receive a payment confirmation at the end of the transaction. This will be your receipt for the
payment. You can print a version of this page and keep for your purposes by using the link on the page
labeled Printer Friendly Version

From the Citizen’s home page you select the link for making a marina’s reservation. Go to the link
labeled I want to go to my summary page to view my existing reservation or view / edit my profile to edit
your profile and to see all of the current reservations that you have.

I want to make a marina reservation at Metropolitan Park
Choose this link from the citizen home screen to begin the creation of a reservation for a marina slip at
Metropolitan Park. At this time this is the only park providing marina slips.
After reviewing all of the specifications for a marina reservation, choose the link at the bottom of the
square labeled: Click here to reserve a Marina Slip!
This will take you to the list of available events being held at the Metropolitan Park Marina. You can only
reserve a marina slip for current events.

Select an Event Date
Select the event you would like to reserve a boat slip and hit the NEXT arrow. You will then be taken to
the Vessel Information screen. You will need to enter all of the information pertaining to the boat or raft
you are reserving the slip for.

Fields
First Name - First name of the owner of the vessel
Middle Initial - Middle initial of the middle name for the owner of the vessel – this is optional
Last Name - Last name of the owner of the vessel
Address - Owners address City - City for the address
State - State for the address Phone - Phone number of the owner Fax - Fax number of the owner
Email - email address of the owner Vessel Name - Name of the vessel
Vessel Type - Type of the vessel Registration - Registration number for the vessel Length Overall - Length of the vessel
Beam - Measurement of the vessels width Draft - How deep the vessel will sit in the water
Captain/master - Owner or captain of the vessel Boat Phone - Phone number on the vessel Insurance Co - Insurance Company used to insure the vessel
Policy # - Insurance Policy number for the vessel Policy End Date - Expiration date of the policy for the vessel
Comments - Any information to be used for the vessel
Back - Return to the previous screen
Next - Continue with the reservation process

Fields
Type - The slip needed for your vessel Max LOA - Overall length of your vessel
Max Beam - Maximum width size of your vessel Slots Available - Number of slips available for the vessels that fit the
specifications
Price - Cost for reserving the particular slip Back - Return to the previous screen
Next - proceed with the reservation process

Summary
A summary of the total cost for the reservation will be created and displayed.
Please review your reservation. If all information is correct select the NEXT arrow. If any part of the
reservation needs to be changed use the BACK arrow.
The NEXT arrow will take you to the agreement page to either agree or disagree with the rules and
regulations for the marina slip reservation. Please review this page before continuing on. Use the scroll
bars to read the complete agreement.

Please review the agreement and select the button that pertains to your choice at the bottom of the
screen

After selecting the I Accept This Agreement button, you will then be taken to the Checkout-Credit Card
page to process the payment. Payments made online will be made by using a credit card. At this time

At this point the reservation is complete. Use the NEXT button at the bottom of the screen to return to
the home page.
