    Dear Parents/Caregivers: April 16, 2010

    Transportation applications are available at http://www.pembrokepublicschools.org/departments.cfm?subpage=383279 .

    If you have more than one student at any of the schools you need to list all students on the same application.

    1. All students in grades 7-12 wishing to utilize school transportation MUST purchase a bus pass.2. Over 2 mile waivers are issued at the elementary level grades K-6 only.3. All students in grades K-6 less than 2 miles wishing to use transportation MUST purchase a bus pass.4. K-6 students residing over 2 miles do not pay. However, we ask that you complete an application so

    we can update our records.

    5. All K-6 students will receive their bus pass on the first day of school from their teacher.6. The bus fees this year are $180.00 per student, with a family cap of$280.00.7. A $30.00 discount will be offered to all application purchases on or before Friday, May 14, 2010.

    (Envelopes must be postmarked no later than Friday, May 14, 2010). The discounted fees are $150.00for the first student with a family cap of$250.00.

    8. If you believe your student(s) will be eligible for a free/reduced bus pass, you must complete afree/reduced application (available on line) and submit it along with your transportation application to

    the Business Office at 72 Pilgrim Road.

    9. In order to accommodate your K-6 students request to receive transportation to one of the licenseddaycare centers in Pembroke please complete the alternative daycare request form and submit it with

    your transportation application.

    10.NO student in grades 7-12 will be allowed to board the bus on the first day of school without a valid2010-2011 bus pass. In order to guarantee the receipt of your students bus pass in time for the first

    day of school, all applications MUST be received in the Business Office at 72 Pilgrim Road postmarked no later than Tuesday, June 1, 2010.

    11.The Business Office will not issue bus passes on a walk in basis two weeks prior to the start of school.All applications received/postmarked after June 1, 2010 will be wait-listed until the applications can

    be processed and the passes mailed home. Bus passes will be issued after the deadline on a space

    availability basis.
