Agreement with Partner Agency

Provision of AT Loans and Demonstrations (Scope of Work)

1. Purpose. Partner Agency operates a loan and demonstration program of assistive technology equipment at its xyz location.

2. Loans and Demonstrations. Loans and Demonstrations must be available to anyone. A charge may be levied to cover the expense of loan or demonstration. Evaluative and outcome data is to be collected on loan and demo activities using ATinNH data collection forms and submitted via the ATinNH online data collection system. Completed data collection sheets are to be retained for possible data verification purposes. Loans and Demonstrations are defined under separate cover. 3. Inventory. Partner Agency must maintain and keep up-to-date an inventory listing of its equipment available for loans and demonstrations in a format specified by ATinNH on the online ATinNH data collection system.

4. Information and Awareness. Partner Agency may from time to time be contacted for information on assistive technology devices or strategies; contractor is encouraged to collect data on such requests on ATinNH provided data collection sheets and submit these on an annual basis. 5. Quarterly Meetings. The Contractor is required to participate in quarterly partner meetings; while in person participation is preferred, meetings are also accessible via telephone.

6. Payment. ATinNH will pay $50 per loan or demonstration event up to a combined total of $8,000. Only events for which complete data has been submitted may be invoiced against. 7. Other. ATinNH will offer continuing technical assistance to Partner Agency in the further development of its AT services and list Partner Agency as a partner on its website. Budget permitting, ATinNH may provide additional funds for additional loans and demonstration events.

8. Contract Period. The period covered by this agreement is the fiscal year 2011-12 (10/01/2011 – 09/30/2012).

9. Data Reports. Data must be collected and submitted in a conscientious and timely fashion using the ATinNH online data collection system; data reports will be run quarterly (01/15/2012, 4/15/2012, 7/15/2012, 10/15/2012).
