FIELD TRIP: SIX FLAGS Friday, May 14, 2010

I grant permission ______________________ to participate in the field trip. Departure and return times will be approximately: 9:30 a.m. (departure), 7:00 p.m. (return to Harborside Academy). Travel arrangements: School Bus Trip is open to all Harborside Academy students who are in good standing.

Students with a critical referral will not be permitted to attend.

Trip cost: If you have a season pass you pay $5.00 (cost of bus), and if you do not have a season pass you pay $35 (includes cost of ticket plus bus cost). Parent/Guardian ___________________ may be reached at__________________. An alternate responsible adult_____________________ may be reached at _________________ if the above listed parent/guardian is not able to care for the student. I have read the Responsibility Contract for Field Trips (on reverse side) and understand student expectations.

Parents, please review the following expectations with your child: Responsibility Contract for Field Trip.

1. Students are required to remain in the park at all times. 2. Students are required to check in with chaperones at designated times. 3. All school rules are in effect while on field trip. 4. Possession and consumption of alcoholic beverages and tobacco products

are prohibited. Prescribed and over-the-counter medication must be in possession and dispensed by school personnel. All other drugs are strictly prohibited.

5. Students must conduct themselves in such a manner as to bring pride to the student, his or her family, school, and community.

6. Students must comply with any and all instructions directed to the student and/or the group by a chaperone or staff member.

7. Students may only ride in school vehicles or vehicles driven by teacher or chaperone. Any exceptions must be pre-approved by the administrator.

8. Students are to conduct themselves in accordance with the rules and regulations as outlined in the Student Handbook (located in student’s assignment notebook). Any violation of these rules will result in appropriate disciplinary action.

9. Any illegal conduct, for example, vandalism, theft possession of a fire arm, etc., is prohibited.

10. Students are responsible for their personal belongings and agree to permit inspection of personal belongings by school personnel or chaperones.

11. Students may not travel anywhere alone. 12. Abusive language or conduct endangering the safety of trip participants is

prohibited. 13. Whenever a student is in doubt about what to do, he/she is expected to seek

advice from the chaperone or other adult member from the group.
