DearHunterdonCountyCommunity,NewJerseyHealthInitiativesandRobertWoodJohnsonFoundation,

ItiswithmuchexcitementandpridethatwepresenttheNJHI:BuildingaCultureofHealthNJ,Round2GranteeBlueprintforAction.Didyouknowthatsince1995,agroupoforganizationshavebeenworkingbehindthescenestoensureHunterdonCountyisthehealthiestitcanbe?TheHunterdonCountyPartnershipforHealth(PFH)comprisesnearly60communityorganizationsandindividualresidentswhoallhavethecommongoaltoseeathriving,healthyHunterdon.

ThevisionofPFHissimple:EveryindividualofHunterdonCountyhasastateofoptimalphysical,mental,spiritual,andsocialwell-being,whichallowstheindividualtopursuethemostfulfillinglifepossible,andnotmerelyalifeabsentofdiseaseorinfirmity.

PFHincludesrepresentationfromleadersacrossdiversecommunitystakeholdergroupsincludinghealthcare,government,lawenforcement,business,education,publichealth,socialservices,faith-basedorganizationsandmembersofthecommunity.

Ongoingmajorcoalitioninitiativesidentifycommunityhealthneedsandwhatactionscanbetakentoimprovehealthandwellnessacrossthecounty.OvertheyearsthePFHhasaddressedissuesrangingfromsmokingcessationanddrugaddiction,tocommunityaccesstoparksandhealthyfoods,toraisingawarenessoftheimportanceofannualwellnessvisitsforseniors.

ThecurrentfocusofPFHispromotingahealthierweightbyencouraginghealthychoiceswithavarietyofprogramsfocusedonhealthybehaviors,healthyweightandeating,mentalhealthandstressmanagementandaccesstohealth-focusedprograms.

ExecutiveSummary

After conducting a Community Health Needs Assessment in 2016, the Hunterdon CountyPartnershipforHealthreachedacollaborativeconsensustofocusonHealthierWeight.ThreeActionTeamswerecreatedtodevisegoalsandobjectives incorporatinghealthierweightasamechanism to achieve improved community health and wellness. The Action Teams are:AccessandTransportation,HealthyBehaviorsandMentalHealth.Inaddition,thePartnershipformed a new Communications Action Team in recognition that increasing the emphasis onqualitycommunicationsand includingthe incorporationofsocialmediawouldsupportactionteamobjectives. AsarecipientoftheNJHICultureofHealthgrant,actionteamleadershavebeen trained and received coaching and leadership skills from the Center for CreativeLeadership.Thegrantleadershipteamhasalsofocusedonanenhancedgovernancestructurefortheorganizationthatmaximizednewcollaborativeleadershipskills.

TheseskillsareincorporatedintodecisionmakingfortheActionTeams,LeadershipTeamandthe Executive Committee. NJHI funds are allowing us to add capacity and enhance ourcollaborative impact on our community, ultimately maintaining Hunterdon’s status as thehealthiestcountyinNewJerseyasdefinedbytheCountyHealthRankings.

HunterdonCountyPartnershipforHealthHistory&Vision

TheHunterdonCountyPartnership for Health, Hunterdon’s “healthier community” initiative,wasco-foundedin1995byRobertWise,PresidentandCEOofHunterdonHealthcareSystem,John Beckley, former Hunterdon County Health Department Director and Bernadette West,PhD, thenExecutiveDirectorof LocalAdvisoryBoard (LAB) III, a CentralNew Jersey regionalhealth-planningorganization. ThePartnershipforHealthisacountywideinitiativethatinvolvesnearly60communityserviceproviders/agencies sharing a common interest in promoting and improving the health, well-beingandqualityoflifeforHunterdonCountyresidents.ThisisreflectedinthePartnershipforHealth’s vision statement of achieving health as defined by the World Health Organization(WHO)andtherecentlydevelopedmission.Vision: “Every individual of Hunterdon County has a state of optimal physical, mental, spiritual and social well-being

which allows the individual to pursue the most fulfilling life possible, and not merely a life absent of disease or

infirmity.”

Mission: The Hunterdon County Partnership for Health is a community collaboration dedicated to achieving optimal

health for all residents.

Social Determinants of Health: Health, and what influences, it has evolved with therecognitionofthesocialdeterminantsofhealth.Itisnowunderstoodthathealthisnotaresultof just one factor but is shaped, promoted and protected by amultitude of variables in ourenvironment including where we live, work, learn and play. These factors include income,education, housing, access to quality healthcare and to healthy food, employment,

transportation,familyandsocialsupport,amongothers.TheCountyHealthRankingsarebasedonamodelofpopulationhealththatemphasizesmanyfactorsthat,ifimproved,canhelpmakecommunitieshealthierplaces to live,work, learnandplay.These indicatorshavebeenat theforefrontofthePFHworkandwillcontinuetobeamodelforourBlueprintforAction.

CommunityDemographics: What does our community look like? Hunterdon County consists of 26municipalities over 437 squaremileswith apopulationofapproximately126,000people.HunterdonCounty is the third-leastdenselypopulated county in NJ with higher thanstate average median income. However, alarge variation exists across the 26municipalities. Hunterdon is made up of predominantly white, non-Hispanic residents; thesecond largest race/ethnicity is Hispanics. Our elderly population (65 years and above) isincreasing.

Areas identified in our CHNA, survey and other data sources have led us to concentrate our resources on the starred areas. In 2019 we will develop our next CHNA and re-direct our resources if necessary to meet the best health outcomes for our residents; this Blueprint will be a “living” document, changing as Hunterdon’s needs change.

ALICEPopulation:Who needs our help? ALICE is our neighbor who is Asset Limited, Income Constrained, Employed (or retired) andlivingontheedge.ALICEmaybeacollegestudent,arecentgradwhoisworkingandpayingofftuition debt, a young family struggling to pay for child care so they can work, theunderemployed working less than full-time and without benefits, active duty militarypersonnel, a family caregiver, or a senior on a fixed income struggling with health issues.BottomlineisthatALICEindividualsandfamilieslivinginHunterdonCountystruggleeverydaytomakeendsmeet.YetALICEworksinjobsthatareessentialtothecounty’seconomy.Itcostsabout$81,000 fora familyof four tobarelygetby inHunterdonCountybecauseof thehighcost of living and services. ALICE struggles to pay for essentials, has little or no savings, andmustmaketoughchoices.WhatareourCommunityHealthPriorities?Basedon all of the CommunityHealthNeedsAssessment data, future Partnershipworkwasencouraged to consider each of these cross-cutting issues when identifying potentialinterventions and community improvement activities: Healthy Weight, Mental Health,Substance Abuse, Access to Care and Transportation, Chronic Disease, Preventive Care, andHealthyBehaviorsandLifestlye.

AddressingObesity-WhyFocusonHealthierWeight?ReviewandanalysisofalldatahelpedmembersofthePartnershipforHealthidentifyhealthierweight,as thepriorityhealth issue in thecommunity.Action teams,comprisedof individualswith the expertise, passion and knowledge of the identified priority health issue, have beenformed. Action team members are tasked todefine attainable objectives and develop clearstrategies under the overarching goal of“healthierweight,” to address this health issue.All of this will be captured in this Blueprint forAction and HMC’s Community NeedsImplementationPlan(CNIP). Inaddition,actionteammemberswilldiscussandidentifybroadmeasures to track and measure both short-term and long-term progress and success inaddressingobesity,andmovingresidentstowardahealthierweightinHunterdonCounty.

TheHealthyBehaviorsActionTeam is adiverse sub-groupof thePFHwithbackgrounds thatencompass areas related to physical activity, food distribution, healthcare, education,transportation, chronic disease management, business and prevention. This enabled us todevelop objectives that spread across schools, food access (pantries, retail, farm markets,gardens)andtheworkplace;inthiswaywecanreachallagesandsocio-economicgroups.

Schools AccordingtoThe2014USReportCardonPhysicalActivityforChildrenandYouth, routine physical activity is not just about exercising to improve outward

appearance or reducing bodymass index (BMI). Exercise reduces risk of developing chronicdiseases,boostsbonehealth, improvesmentalhealthandwell-beingand improves cognitiveand academic performance. (Alliance, 2016)With this in mind it seems prudent to supportschool-ageactivityprograms.

TeamObjective:Increaseparticipationintheno-costYMCA7thgrademembershipprogramby10%eachyearstartingin2017untiltheprogramisatmaximumcapacity.

AccordingtoTheFoodTrust,anoverwhelmingbodyofevidenceover20years indicates thataccessing affordable, high-quality, and healthy food is a challenge for many families. InHunterdon County, Latinos represent our largest minority population and although we arefrequently referred to as the “Healthiest andWealthiest” county, we know that pockets ofpovertyexist. “Living closer tohealthy food retail is associatedwithbettereatinghabits anddecreasedriskforobesityanddiet-relateddisease.”(Trust,2013)

Strategies:

1.Shortterm:ContactschoolnursestoevaluatewhichschoolshavestudentswhowouldqualifyforthisprogrambyDecember,2017.

2.Intermediateterm:HelpfacilitateeducatingtheschoolnursesonthepaperworkandprocessforgettingstudentsenrolledintheprogramincludingformstranslatedtoSpanishbyOctober,2018.

3.Longterm:Createanongoinglistofbarrierstousingthisprogramforschoolsandparentsandworkwithcommunitypartnerstoeliminatethem.(deliveryandpickupofbackpacks,storage,allergies,languagebarriers,stigmaetc.)by2019.

TeamObjective:ExpandtheuseoftheNORWESCAPBackpackProgrambytwoschoolseveryyearstartingin2017untilallqualifyingstudentsinHunterdonCountyhaveaccesstofoodovertheweekend.

Gardensmay offer physical andmentalhealthbenefitsbyprovidingopportunitiesto:eathealthyfreshfruitsandvegetables,engageinphysical activity, skill building and creating open spaces, and decrease violence in someneighborhoods,andimprovesocialwell-beingthroughstrengtheningsocialconnections.

Over the courseof ninemonthlymeetings in 2016 andearly 2017 theMentalHealthActionTeamreviewedinformationabouttherelationshipbetweenmentalhealthandhealthyweight.Thosediscussions,researchandinputfromcommunityhealthprofessionalsledustofocusonstressasamajorhealthconcern.Itisourconclusionthatindividualsexperiencinghighlevelsofstress often engage in eating behaviors which lead to unhealthy body weight. (Why stresscausespeopletoovereat.,2012)Inaddition,thereisclearevidencethatindividual’sinabilitytodevelop appropriate copingmechanisms is highly correlatedwith substance use andmisuse.(Sinha,2001)The stress response occurs as a result of tensionbetweenone’sreactiontoadifficultsituationwhileunable to manage or resolve the situation. Theabilitytocopewithadverseconditionsisdependentonone’sresourcesandskillsetsinbothrecognizingtheimpactofstressonhealthandutilizingavailableresources. (Terzian, 2010) Indicators of stress,particularly in children, include changes in moodand behavior. Physical signs of stress can includesymptoms such as abdominal pain and sleepproblems. Emotional signsmay include irritability,hostility (acting out behaviors) and feelings ofhopelessness(depression),aswellasthebehavioralchangesofpooreatinghabitsandexcessiveweightgain or loss. Disadvantaged populations such asthose lacking economic resources includingsufficientnutritionorlivinginsocialisolationcanbemorevulnerabletotheimpactofstress.

Healthy coping mechanisms can include access and use ofsocialsupport,behavioraltechniquessuchasmindfulnessandrelaxation exercises, physical activity, and involvement inconstructiveactivities suchashobbies,musicorparticipationincharitableorganizations.The team identified the following factors as relevant toaddressingthisissue.

SafeCommunitiesCoalition

TheSafeCommunitiesCoalition(SCC)basedonextensiveassessmentisworkingtoreduce30day rate use of underage drinking, Rx, marijuana and heroin abuse within thecounty.AccordingtoNJSAMs,HunterdonCountyhasalmostdoublethestateaverageofthoseintreatmentduetoalcohol,andwhileourrateofopioidoverdosesarelowerstatewide,thoseintreatmentforopioidsareatstateaverages.Utilizingenvironmentalstrategies,SCC’stargetisto reduce access and availabilitywhile increasing the perception of harm through educationand awareness around these substances. Additionally, SCC created the S.T.A.R.T. (Steps toActionRecoveryandTreatment)programtoconnectthosemostvulnerableandingreatneedtoservices,whichhasnowgonestate-wide.

LatinoAccessCoalition

The Latino population in Hunterdon County is ourlargestminoritygroup.ManyofourLatinoresidentsface challenges with education, jobs, housing, legalassistance, healthcare, language barriers andimmigration status. The LatinoAccess Coalition’s purpose is two-fold; first to help Latinos inneedandsecondtopromoteculturallycompetentcare.Currentlyourcoalitionisworkingonthree initiative areas: Latino/BusinessManagement/Leadership Training,Mentoring Program,andthelaunchofadigitalresourceplatformforinformationpertinenttoourLatinoresidents.TheMentoring Program introduces English as a Second Language (ESL) students to variouscareer opportunities andhelps assist themwithnavigation into institutes for higher learningpost-highschool.Throughthecreationofawebsiteandcentralizeddigitalhubofinformationthecoalitionwill facilitateaccess tocommunity resourcesandup todate informationon thecoalition’sinitiatives.

Governance

Since its inception, the Hunterdon County Partnership for Health has demonstrated successbuilding a health-focused county coalition and implementing impactfulprogramming. Currently, the PFH has an Executive Board comprised of six (6) members,representing the healthcare system, nonprofit organizations, and county government. Draftbylaws exist, however, they are not finalized and do not include details about approvedbudgetary expenditures, creation of subject-specific white papers, and public statementsrepresentingthePFH.InordertocreateaframeworkthatprovidesforcollegialityamongPFHmembers,butallowforgrowth,thePartnershipwillachievethefollowinggoals:1. The PFH leadership will evaluate the current governance structure and other jurisdictionmodelstodeterminewhetheradjustmentofthePFHisbeneficial.ByJune2017.

OurPosition&Voice

As our governance structure is solidified, it is the goal of our leadership team to achievealignmentofhealthrelatedgoals inpubliclyavailablepositionstatements. Thesestatementswould address pressing health needs in Hunterdon County and will be designed to impactcommunity leaders including elected officials. We will use leadership skills to align thecollective input and shared outcome goals of the Partnership to influence health relatedpolicies in the public and private sectors. We hope to set the example and standard inHunterdon County to use reliable and evidence based sources for information concerninghealth thus positively impacting the implementation of rational and effective health policiesthataddressneedsacrossrace,ethnicity,gender,abilityandincome.Ourinitialgoalistoissueatleasttwostatementsperyear,beginningin2018.

NewJerseyHealthInitiatives,RobertWoodJohnsonFoundationGrantOpportunity:

BuildingaCultureofHealthLeadershipTeam

In 2016, the Hunterdon County Partnership for Healthwas awarded a four-year New JerseyHealthInitiatives(NJHI)grant-partoftheRobertWoodJohnsonFoundation(RWJF).TheNJHIleadershipteamincludesPFHmembers:Dr.CindyBarter,KimBlanda,RN,BSN,ColetteCamisa,Karen DeMarco, MPH, Rose Puelle, PhD and Freeholder, Suzanne Lagay, who serves as theteam’s Coach. The team represents the Partnership from healthcare, government and non-profitsectors.Thegrantprovides leadershiptrainingandaccesstosubject-matterexpertsto“Build a Culture of Health” in Hunterdon County, with a focus on evidence-based andsustainableprogrammingforthegreatestcommunityimpact.Inyearone,theNJHIgrantteamparticipated in training opportunities and enhancing collaborative opportunities for theHunterdonCountyPartnershipforHealth.

ThetrainingprovidedthroughtheCenterforCreativeLeadership(CCL)tothe team is not traditional in terms of healthcare or public healthmaterial, rather much of the tools and time with CCL focused onmembers learning each other’s skills, abilities, and inspirations. In two

overnight, retreat-stylemeetings, teammembersgot toknoweachotheronbothapersonalandprofessionallevel.Takingthetimetounderstandeachother’smotivationsandinterestsattheprogram’s inceptioncreatedasenseofcamaraderieandasafeenvironmentto introduceanddiscussprogrammaticinitiatives.The tools provided through CCL are instrumental in growing the Partnership and keeping apulse on the needs of the membership, as well as the community as a whole. The NJHILeadership Team is committed to sustaining the knowledge and spirit of the CCL tools andmission.Withthisinmindwehavesetthefollowinggoals:1.CCLtoolswillbeintegratedintothePFHbylawscurrentlyunderdevelopment.Begin2017.2.Teamwillcreateatrain-the-trainerprogramregardingCCLtools.Begin2019.3.ActionteamchairswillmodelCCLteachingsineachoftheactionteammeetings.Begin2017withcurrent teamwhile trainingnewchairswhowilleventually takeover the leadonActionTeamsby2020.4. CCL will be operationally implemented in NJHI Leadership Team meetings. Process andlearningwillbeongoingwithagoalofendof2018forfullimplementation.The foundationof theNJHI leadership trainingscenteron respect,anddevelopingameetingscheduleandstructure thatconsiderseachof themembersof thegroupso thateveryone isable to contribute. The NJHI Team developed a number of ground rules for the four-yeargrant.TheTeammeetsataminimumofoncepermonth,andthegrouponlymeets ifeverymembercanbepresent.Theteamagreeduponameetingformatandteamnorms,developingaprocess forcommunicationoutsideofmeetings,decisionmaking,conflictmanagementandroleidentification.TheTeamincludesatleastonetoolfromtheCenterforCreativeLeadershipateachmeetingtoremindthegroupoftheircommitment,direction,andalignmentwiththemission“BuildingaCultureofHealthforHunterdonCounty.”ThegrowthoftheNJHITeaminlessthanayear’stimeistremendous,withanauthenticlevelofcommunicationthatwillspreadtothebroaderPartnershipforHealthmembership.

ImpactofGrantFundingonActionTeams

AsarecipientoftheNJHIleadershipgrant,actionteamleadshavebeentrainedandreceivedcoaching in leadership skills. The grant leadership team has also focused on an enhancedgovernancestructurefortheorganizationthatmaximizednewcollaborativeleadershipskills.

These skills are incorporated into decision making for the Action Teams, CommunicationsCommittee,LeadershipTeamandtheExecutiveCommittee.NJHIfundsareallowingustoaddcapacityandenhanceourcollaborativeimpactonthecommunity.

HealthyBehaviorsActionTeam:ThePFHwantstomakethehealthychoicetheeasychoiceforallofourresidents.ThroughthefinancialsupportofthisNJHIgrantourinitiativeswillincreasehealthy donations to our area food pantries, provide assistance with community gardens,supportworksitewellnessinitiativesandreducebarrierstousingaweekendfeedingprogramforschool-agechildren.

Communications Action Team: NJHI grant funding will allow the PFH to become moreprominent in the community by enhancing our website, media presence, public serviceannouncements,developmentofayouthcoalitionandmentoringprogram.

MentalHealthActionTeam:TheNJHIgrantwillfundastressrecognitionandmanagementoneyear demonstration project in at least one Hunterdon County school district that includes atrain-the-trainercomponent.AccessandTransportationActionTeam: Asanewly formedActionTeam, theNJHI grant isproviding direction and alignment for the team’s objectives. As a result of NJHI toolimplementation, the Action Team is considering splitting into two separate Teams toadequately address the needs of Hunterdon County residents. Potential funded projectsinclude creation of a voucher program for medical appointments, cultural and capabilitytraining for front-line medical workers/medical professionals, program promotion andinformationaccessvideosandwrittenmaterials.
