medicine detox myth is putting people at risk The risks and benefits of many common medicines are widely misunderstood, shows new research by the UK National Pharmacy association (NPa). The findings show that many people think it is right to give your body an occasional ‘detox’ from long term medication. Doing this can allow a disease to become uncontrolled and risks devastating complications.

The NPa survey of over 1000 UK adults found:One in five thinks that it can be beneficial to give your body a medicines ‘detox’, by occasionally stopping taking your regular medicines for a long term medical condition. Doing this can seriously impair health. For someone with diabetes, asthma or depression, the results of doing so can be catastrophic. Nearly a third of people think it is OK to take non-prescription medicines that have been specifically recommended for someone else. in fact, this can be dangerous, even with common cough and cold remedies. Others think that it is OK to give adult medication to a child, so long as the dosage is reduced – an incorrect and potentially dangerous belief. a quarter thinks, incorrectly, that aspirin is just a weaker version of ibuprofen. For some people, making the wrong selection of pain killer risks harm.

paracetamol: repeated ingestion of slightly too much can be fatal repeatedly taking slightly too much paracetamol over time can cause a dangerous overdose that is difficult to spot, but puts the person at danger of dying. Patients may not come to hospital reporting the overdose, but because they feel unwell. This clinical situation needs to be recognized and treated rapidly because these patients are at even greater danger than people who take single overdoses.

These so-called staggered overdoses can occur when people have pain and repeatedly take a little more paracetamol than they should. “They haven’t taken the sort of single-moment, one-off massive overdoses taken by people who try to commit suicide, but over time the damage builds up, and the effect can be fatal,” says Dr Kenneth simpson who publishes the findings of a recent research project in the British Journal of Clinical Pharmacology.

The problem is that doctors normally assess how much danger an overdose patient is in when they arrive at hospital by taking a blood sample and finding out how much paracetamol is present. in the case of a single dose overdose, the blood sample gives valuable information, but people with staggered overdoses may have low levels of paracetamol in their blood even though they are at high risk of liver failure and death.

Working in the University of edinburgh and the scottish Liver Transplantation Unit, scotland, Dr simpson and his team analysed data from 663 patients who had been admitted to the royal infirmary of edinburgh between 1992 and 2008 with paracetamol-induced liver injury. They found that 161 had taken a staggered overdose, usually to relieve a variety of common pains, such as abdominal or muscular pains, headache and toothache.

new resource directory for people with ADhD

Minister Frances Fitzgerald launched the HaDD (Hyperactivity attention Deficit Disorder) family support group’s new guide to aDHD services for people living with aDHD in ireland recently. The guide, entitled ‘The A to Zee of ADHD’, clearly outlines the diagnosis and treatment pathway for people living with aDHD in ireland. The guide has been created by HaDD to help people with aDHD to navigate the healthcare system to receive the best care available in ireland. aDHD affects between 3%-7% of school age children which equates to between 25,000-60,000 children.

However, a new report contributed to by HaDD also revealed that there are major gaps in the services available to people living with the condition. The report entitled ‘ADHD Europe Survey 2011’ includes a review of the diagnosis and treatment of aDHD in europe. The report highlighted discrepancies across europe in terms of the diagnosis, treatment and provision of services for people living with aDHD. This is despite projected increases of people seeking help across europe. in addition, the need for professional training, guidance for teachers and inclusion of children with aDHD in main stream education remains unfulfilled. in ireland, the ratio of psychologist to student is 5,000 to 1, compared with the european average of 3,000 to 1. in terms of diagnosis, in ireland there is only one psychiatrist per 168 children living with aDHD and the waiting time for a public consultation can be over a year in many cases.

‘The A to Zee of ADHD’ covers a wide range of information including assessment, treatment options and support services available. This is the first time that all of this information has been made available in one succinct directory that is available to the general public.

mental health Ireland and ladies GAA launch joint awareness initiative

tooth whitening regulations welcomed

The irish Dental association has welcomed the passing of a new european Council Directive which regulates the use of hydrogen peroxide, the chemical used in tooth whitening products.

The new directive prohibits the use of tooth whitening products containing over 6% of hydrogen peroxide and stipulates that tooth whitening can only be carried out under the supervision of a dentist.

The Council of european Dentists – of which the irish Dental association is a member – had called repeatedly for appropriate regulation of tooth whitening products at eU level, in line with the advice from the european scientific Committee on Consumer safety.

Following the european Council’s decision, tooth whitening products containing up to 0.1% of hydrogen peroxide will continue to be freely available to consumers. For products containing between 0.1% and 6%, a clinical examination and first treatment by a dentist will be required to ensure the absence of risk factors or oral pathologies, after which the patient will be able to continue the treatment by him or herself. The use of these products by persons younger than 18 years will not be allowed.

The iDa had previously warned of the dangers of some products available via the internet which contain very high levels of hydrogen peroxide and which could be lethal in the wrong hands. at the other end of the scale some products contain very low levels of hydrogen peroxide and the iDa says these are ineffective.

The iDa says tooth whitening is a safe procedure if carried out by a dentist but it’s a procedure which shouldn’t be repeated too often. as a rough guide once a year should be sufficient. We don’t recommend it for pregnant women or heavy smokers or drinkers as it can cause particular problems for each of these groups’ Feeney concluded.

Mental Health ireland (MHi) and the Ladies Gaelic Football association (LGFa) are delighted to announce the launch of a new partnership between the two associations with the aim of promoting Mental Health awareness in ireland.

The LGFa and MHi will be increasing awareness of mental health issues and preventative steps through Gaelic4Mothers & Others, an LGFa development initiative, and Building Resilience Together, a MHi awareness initiative.

in 2010, Mental Health ireland launched an initiative entitled Building Resilience Together as a means of informing all age groups of society of the importance of developing individual and community strengths that will help people to cope with and bounce back from the challenges and setbacks that life presents. Gaelic4Mothers & Others began in 2008, with the aim of providing a pathway for all women in ireland to get involved in Ladies Gaelic and to

facilitate a development of community spirit and interaction. Three of the cornerstones of Building Resilience Together are

signing up for and getting involved in activities in your local area; staying connected with family, friends and community; and doing things that make you laugh and feel good. all of these actions will improve an individual’s well-being and their sense of connectedness to those around them.

The ethos of Gaelic4Mothers & Others mirrors the ethos of the Building Resilience Together initiative. Gaelic4Mothers & Others is a recreational form of Ladies Gaelic Football that focuses on getting women who are usually on the sidelines more involved in their local club and community. However it is not only about getting women physically active but more importantly getting them to engage with other women in their area. The social aspect of Gaelic4Mothers & Others and the support network it creates for women is just as important as the physical exercise.

Bereavement support training websiterTe presenter Miriam O’Callaghan launced the first bereavement support training website to be developed in europe which is now hosting its first online training course called Lost for Words.

Developed by the irish Hospice Foundation (iHF) and accessed via its website (www.hospice-foundation.ie, the new on-line training website will enable members of the public to learn about bereavement and offer support to grieving family, friends and colleagues.

Lost for Words, the first online training course accessible on the website, has already received an award from the irish association of Palliative Care and the iHF is encouraging companies to use Lost for Words as part of its induction package or in its e-Training programme for all employees.

it is estimated that 300,000 people are directly affected by bereavement each year in ireland. approximately one in 10 of the working population is directly affected by a death annually while one in 25 workers are caring for a seriously ill relative.

The iHF is the national charity which promotes the hospice philosophy and supports the development of hospice and palliative care. Work in the area of bereavement is a core activity for the iHF.

‘Get gutsy’ raises IBD awareness The ‘Get Gutsy’ public health awareness initiative was launched recently following the findings of new research which shows that there is an alarming lack of understanding of iBD in ireland and a need for more information for those people already diagnosed or suffering symptoms of this extremely debilitating illness.

The Get Gutsy website (www.getgutsy.ie), smartphone application and information literature provides essential information to people who may be suffering from symptoms associated with iBD and to those who may already been diagnosed with iBD such as a useful symptom tracker which will help them manage the illness and track symptoms to discuss with their healthcare provider.

an estimated 15,000 irish people suffer from iBD, with the vast majority diagnosed when they are between the ages of 15 and 30 years.

speaking at the launch Professor Hugh Mulcahy, Consultant Gastroenterologist, st Vincent’s University Hospital commented, “iBD is very prevalent for a disease which strikes people at such a young age and which is so debilitating. However, treatments are constantly improving so if someone is concerned about changes in bowel habits, nausea and unexplained weight loss, they should visit their GP as soon as possible.”

The cause of iBD is unknown and the disease is not preventable however as a result of advances in treatment, many patients will go through long and sustained symptom free periods even though iBD is still a chronic condition. The research published showed that 60% of the population mistakenly believe that iBD is preventable, curable and not that prevalent in ireland.

Bad breath and erection problems – a turn offWomen of all ages want to have more sex, according to the results of a pan-european survey commissioned by Pfizer. Two thirds (67%) of women aged over 45 years of age want to have sex at least once a week; however, only half (54%) of them are actually currently doing so. The Partners survey which examined the sex lives of 2,500 european women who were in a committed sexual relationship with a man, also reveals that bad breath and erectile dysfunction (eD) are the biggest physical sexual turn-offs.

The Partners survey found that 61% of women aged over 18 years are having sex at least once a week with their partner. Contrary to societal stereotypes, the research also revealed that a woman’s desire to have a satisfying sex life does not decrease significantly as she gets older, with three-quarters of women aged 45 and over reporting that a satisfactory sex life is important to them (76%).

“While The Partners survey did not specifically include irish women, anecdotal evidence tells us that the findings of the research broadly reflect irish attitudes and experiences. The research highlights the impact eD can have on a couple’s ability to achieve a satisfying sex life, and existing research shows that almost half of men over the age of 40 may be experiencing this problem” commented Dr stephen Murphy, GP. “What is reassuring to see from the survey is that the majority of women are willing to initiate a conversation about eD with their partner, and that they would encourage them to seek advice from a health professional.”

‘Invisible’ symptoms of parkinson’s campaignCelebrity chef Neven Maguire is lending his distinguished nose to an educational campaign which seeks to highlight the lesser known non-motor symptoms of Parkinson’s disease.

The Understanding Parkinson’s Disease – it Makes sense campaign aims to raise awareness of the non-motor symptoms associated with the disease which include the loss of smell and taste, constipation, and sleep disturbances.

These non-motor or often ‘invisible’ symptoms require long-term management in the community.

There are currently over 8,000 people with Parkinson’s disease in ireland and it is the second most common neurodegenerative disorder in the world, after alzheimer’s disease. indeed, the World Health Organisation (WHO) has estimated that by 2040 dying with Parkinson’s disease will overtake cancer as the second most common cause of death.

At the launch of understanding parkinson’s Disease – It makes sense campaign: pat o’rourke chairman of parkinson’s Association of Ireland. Aimee casey, lundbeck Ireland; neven maguire and Joe lynch, ceo parkinson’s Association of Ireland.

the colourWhen choosing a foundation, you should choose a colour as close to your own skin colour as possible. You can do this by testing the colour along your jawline, as this part of your skin never changes colour. i also recommend testing the colour of the foundation on your chest, as the foundation colour should not only match your face, but your chest and arms too.

Women who want to appear more tanned then their natural colour often opt for a darker foundation. However, foundation often tends to become quite blotchy and patchy throughout the day. One way to improve matters is to use a bronzing powder to give your skin more warmth. Blushers, eye and lip colours will also give more warmth to your skin, which is a much better option then choosing a foundation which is darker then your natural skin colour. However, if you do want to have a tanned appearance, you can use false tan, ensuring that it is suitable for your face and body. You can then choose a darker colour foundation to match the false tan. The false tan will also help hold the foundation longer.

remember the aim is to look like you have beautiful skin, not a beautiful foundation!

for mature skin, as it reflects the light to give your skin a very clear, luminous complexion, disguising the appearance of lines and wrinkles.

Chanel’s Lift Lumière and Bobbi Brown’s Luminous foundations are excellent for any client who wants a light reflecting foundation, to give them a natural glow.

ApplyInG founDAtIonFor best results when applying foundation i recommed using a foundation brush. The other plus with a foundation brush is that it helps to give full coverage without wasting any product. sponges tend to soak up a lot of foundation, therefore wasting product.

apply the foundation to the centre of the face, as this area needs the most coverage, and blend outwards. You do not need to overload the face with foundation, it should just be applied on the centre panel of the face and the remainder of product on the brush can be blended over the rest of the face. The centre panel of face is where oil is produced, and therefore needs the most coverage.

conceAlersWhen you see the coverage afforded by the foundation, you can then apply concealer to the areas of the face where needed. Warm toned, pink/peach concealers will disguise dark circles around the eyes. a light moisturizing concealer is most suited to the eye area, as it won’t show up lines around the eye.

To cover blemishes, a yellow based concealer will neutralize any redness from blemishes or broken capillaries.

a translucent face powder will take away any excessive shine and help hold the foundation longer, without distorting the colour of the foundation.

To achieve a matt finish, press the powder all over the face. For a more natural, satin finish, press the powder on the high

planes of the face – across the forehead, nose and cheekbones and dust the powder gently over the rest of the face.

For a dewy look, lightly press the powder only on the high planes of the face, leaving the rest of the skin powder free.

all product should be applied lightly, and built up where needed.

remember the aim is to look like you have beautiful skin, not a beautiful foundation!

InformAtIonFor more advice on colours and techniques, email: [email protected] or log onto www.

unDertonesFoundations also have different undertones; some have a warm pink/peach undertone, while others have a cool yellow undertone. Warm foundations with pink/peach undertones are suitable for very fair skinned people, as it gives more warmth to their complexion, while cool toned foundations are most suited to sallow skinned women, as it will tone in with their own natural yellow undertones. i also recommend a yellow based foundation for women with high colouring or redness as the yellow undertones will counteract any redness.

the textureThe foundation texture is very important because if you use the incorrect texture it will not blend properly on your skin. For people with very oily skin, i recommend an oil-free matifying foundation. This will help keep your skin shine free. Oil-free foundations generally are non-comedogenic, which is imperative, as this will help prevent pores becoming clogged and so prevent breakouts. My favourite is Lancôme Teint idole Ultra as it gives great coverage and matifys oily skin.

a skin balancing foundation is best suited to people with combination skin: it will help matify the oily areas on the face, while hydrating the dehydrated areas on the skin.

Teint Miracle from Lancôme is a good option; it is 40% water based and so helps to hydrate the drier areas of your skin and it is oil free, therefore matifies the excessive oily areas.

Bobbi Brown skin foundation is also a great foundation for normal and combination skin. it is also water-based, giving a very natural fresh complexion.

a cream oil based foundation is the best option for people with dry skin. This will blend easily on dryer skin to give a flawless finish.

Bobbi Brown has a moisturizing foundation which works very well on dry skin. Lancôme’s Teint rénergie Lift is also a suitable foundation for dry skin, as it is oil based and is a firming foundation for people concerned with loss of muscle tone. a light reflecting foundation is perfect

Dry eyes- causes and cures Dry eye syndrome (Des) is a common disorder affecting a significant percentage of the population, especially those over forty and particularly women.

Dry eye syndrome results when the eye under-produces tears or tears leave the eye too quickly. a normal healthy eye produces tears constantly to form a tear film, which acts as moisturiser and lubricant. For someone with dry eye, the resulting lack of moisture and lubrication can cause a

variety of problems.

cAusesPoor production of tears by the tear glands is often a result of increasing age. Other causes of Des are: the onset of menopause, other hormonal factors or as a side effect of autoimmune diseases, such as lupus, sjogern’s syndrome or rheumatoid arthritis. Our living conditions and working environment can also affect the eye – particularly air conditioned spaces. Des can also result from straining our eyes when we spend too long doing close work or staring for hours at a computer screen.

some medications may decrease tear production; antihistamines sleeping pills, oral contraceptives, antidepressants, and beta-blockers can lead to Des in some people. if blinking is decreased or if the eyelids cannot be closed, the eyes may dry out because of tear evaporation.

symptomsDry eyes can range from mild to severe. The mild version results in occasional irritation but if it is not treated it can grow into a constant irritant. eventually it can result in inflammation of the anterior (front) of the eye.

Persistent dryness, scratching and a burning or gritty feeling in the eyes are symptoms of dry eye. some people also experience a “foreign body sensation,” a feeling like there is something in the eye. Many people suffer from light sensitivity. Other signs are redness of the eyes and blurred vision.

Curiously, contrary to the name, dry eye syndrome can lead to watery eyes. This results when, in response to excessive dryness, the eye produces a large amount of tears but this production brings no relief as the tears tend to flow out of the eyes and down the cheeks and are wasted. soon afterwards the dry gritty feeling returns. Unfortunately, your eye can only handle so many tears at any one time; those tears that pour down the cheeks do not help the eyes and are wasted. a short time later, your eyes will become slightly dry and irritated again, and the whole process repeats itself.

treAtments Mild to moderate cases of dry eye syndrome may be treated by applying artificial tear eye drops as little or as often as necessary. There are a wide range of products available without a prescription. systane a product widely available in ireland has a number of products which provide lubrication to the eye. systane gel drops can be applied during the day and wont interfere with driving. another household name, Optrex Dry eye Drops, are designed to rehydrate tired, dry or irritated eyes.

Blink intensive tears protective eye drops are an advanced eye drop specifically formulated to give the eyes’ natural moisture a helping hand, providing the client with lasting comfort and protection from the feeling of dry, irritated eyes. For more severe cases of dry eye you might choose e Hycosan extra eye Drops.

speaking of severe cases – a doctor may prescribe medications which can sometimes increase tear production for those with chronic dry eye – such medication does this by targeting inflammation in the eye.

preVentIonPatients should avoid situations in which tears evaporate quickly. in very dry environments such as air conditioned offices the use of a humidifier can bring relief. sufferers can also help themselves by wearing wrap-around glasses in windy conditions and by not smoking.

Omega 3 oilsin recent years, studies have reported that a diet rich in omega 3 oils can reduce the risk of developing dry eye and also ease symptoms. Two of the best sources of omega 3s are oily, cold-water fish and flaxseeds. There are also a variety of high quality omega 3 supplements on the market.

GenerAl ADVIce for Des sufferers• Blink more frequently if you work on a computer screen or

read for long periods.• Where possible, avoid excessive exposure to air conditioning

and central heating.• if you wear contact lenses ask your optometrist about high

moisture or silicone hydrogel contact lenses.• eat a balanced diet with an emphasis on omega 3 oils.

a note on contact lensesDes is particularly uncomfortable for wearers of contact lenses; in fact, dry eye is the most common complaint among contact lens wearers.

Many forms of contact lenses come with their own set of problems, however, and one of these problems can be an unpleasant dryness over time. While there is no specific type of contact lenses for dry eyes, recent improvements in the materials used to create contact lenses have made them more comfortable for those with dry eye conditions. Contact lens brands such as Proclear Compatibles and acuvue Oasys, for example, are made from a hydrogel material containing much less water than previous lenses

osteoArthrItIsOsteoarthritis is a condition that affects the joints of the body. it mostly occurs in the joints of the hips, knees and hands, but it can present itself in other joints. it tends to be a disease associated with wear and tear in the joint where the two bones meet. The cartilage between the bones erodes and gradually disappears. it is generally seen in the over 50 age group but can appear at a younger age. The onset of osteoarthritis in younger people can occur as a result of an injury. Women seem to suffer more from arthritis than men. There is no known cure for the disease. Once the joint becomes damaged it tends to get progressively worse and may result in a joint replacement. it is associated with getting old, however not all older people suffer with it.

The symptoms of osteoarthritis vary from person to person. Three key characteristics of osteoarthritis are:• Mild inflammation of the tissues in and around the joints• Pain• Damage to cartilage, the strong, smooth surface that lines

the bones and allows joints to move easily and without friction

• Bony growths that develop around the edge of the joints.

glucosamine, it was thought that it may have had positive effects on the GaGs complex which forms part of the cartilidge in between bones. recent studies have shown that there is no real evidence for it’s use in the treatment of osteoarthritis and it’s use remains to be proven. since 2008, The National Health service in the UK has listed it as ‘do not recommend for osteoarthritis’. Cod liver oil has a long history of use – it contains vitamin D and vitamin a as well as small amounts of omega 3. as cod is not an oily fish, the omega 3 needs to be extracted from it’s liver. researchers have now turned their focus to the use of higher doses of purified omega 3 to treat arthritis. results have shown that

omega 3 can slow down the disease progression. Omega 3, particularly in the ePa form from fish oils, is nature’s anti-inflammatory. studies have shown it can prevent the breakdown of collagen in cartilage and prevents the loss of molecules that give cartilage it’s shock absorbing properties. These findings are very promising, however, the take home message for use of omega 3 fish oils is – it is dose dependent. Meaning that you would need to take 1-2 grams per day to improve and slow down disease progression. also, it is very important that the fish oil you take is high in ePa. ePa is the component in fish oils that reduces inflammation.

rheumAtoID ArthrItIsaccording to Professor Douglas Veale, a rheumatologist at st. Vincent’s Hospital Dublin; ‘rheumatoid arthritis is a chronic progressive inflammatory disease of the joints and surrounding tissues. it can cause intense pain, disability and irreversible joint destruction. Fortunately, it is not as common as osteoarthritis.’ it is often referred to as ra. There is no known cause or cure for it. The disease is entirely different than osteoarthritis. ra is what is called an auto-immune illness. This simply means that the immune system in the body is not working properly and produces proteins which attack the synovial fluid in between the joints. This causes inflammation and pain.

omeGA 3 AnD ArthrItIsGetting the immune system under control is the approach taken by rheumatologists in the treatment of rheumatoid arthritis. it is necessary to reduce pain, inflammation and joint damage. Many drugs used to treat ra have side effects. Getting the correct balance between effective treatment and unpleasant effects from medications is the goal. in an interview in Scope magazine, Dr ronan Kavanagh, president of the rheumatology society of ireland, said the value of fish oils in rheumatoid arthritis…is without doubt. However, most patients do not realise that they have to take very high doses of fish oils…before they will see any benefit. research indicates that patients need to take a high dose daily for a minimum of 3 months to achieve results.

similarly, research carried out in the UK by the National institute of Clinical excellence which is responsible for advising the UK government on best practice in medicine, has also shown that high dose omega 3 fish oils aid in the treatment of the disease. Their report demonstrated that after taking 2.7grams of ePa/DHa daily for a period of 36 weeks, patients were able to reduce their medication by more than 30%. The most important finding in this report was that a minimum of 2.7 grams of ePa/DHa from omega 3 fish oils is needed to improve the condition. Furthermore, to achieve results it is important to note that the ePa content of fish oils needs to be greater than the DHa content. similarly omega 3 helps maintain bone health and joint flexibility in osteoarthritis at high doses.

JuVenIle rheumAtoID ArthIrItIsJuvenile ra simply means childhood arthritis. This illness can occur at any stage during childhood and is not pleasant. The disease mechanism is similar to that of adult ra and originates in the immune system. The goal of treatment is to relieve pain and reduce swelling coming from the inflammation. Due to the considerable side effects associated with medication, novel treatments are constantly being researched. a recent study of omega 3 fish oils in this group prescribed 1 gram of ePa/DHa daily for 3 months made up of 600mg ePa 400mg DHa. at the end of the study period, 88% of the children discontinued taking their anti-inflammatory medication. as this disease can be progressive it is very beneficial to treat it early and prevent joint damage. These results are very promising as they are a safe effective intervention. However it is worth noting that these outcomes can only be achieved at high doses with the correct ePa/DHa balance.

When colic strikes, the incessant crying that accompanies it can leave parents feeling helpless.

That’s when Colief comes to the rescue.

For many babies, Colief Infant Drops can help ease the symptoms of colic

by breaking down the lactose in the baby’s usual milk,

making it easier to digest.

The result is less discomfort and fewer tears.

Simply advise parents to add a few drops of Colief to the milk

each time they breast or bottle feed.

nappy rash – causes, prevention and treatmentMargaret Merrigan-Feenan

Nappy rash is a common inflammation of the bottom, genitals and groin areas of a baby. it can range from mild irritation to a painful infection. a baby’s skin is unique and is different from adults and continues to develop over the first years of life. Their skin is thinner than adults and has a higher pH level, making it vulnerable to irritation and infection such as nappy rash.

the skin’s primary function is to provide a protective barrier that maintains internal hydration and electrolytes while preventing entry of exogenous substances such as bacteria, allergens, particulates and pollution. The outermost layers of the skin, the stratum corneum (sC), are often described as a bricks and mortar structure, i.e. cells and

lipids. scientists and clinicians believe that this structure creates a tortuous path through the mortar that inhibits the flow of water from the skin and contaminants into the skin. The stratum corneum provides strong resistance to water transport across the skin. The skin barrier is renewed through epidermal differentiation when cells migrate to the stratum corneum and are eventually shed, a process which takes approximately 28 days. Wearing nappies significantly raises the pH of the skin, destroying the protective ‘acid mantle’. The physical integrity of the outer layer is compromised by friction from the nappy or from overzealous cleaning.

Because baby’s skin continues to mature during the first years of life, it requires milder, gentler cleansing than adult skin. While cleansing with water alone is insufficient, typical adult cleansers are also inappropriate for baby’s skin, since their surfactant systems are too aggressive. The bacteria that produce nappy rash thrive in an alkaline medium. The stools of bottle-fed babies’ are acid. For this reason, bottle fed babies are more prone to nappy rash.

Nappy rash usually starts with a slight reddening in the nappy area. it tends not to affect the skin folds. The buttocks and upper thighs become red and moist. The skin can become sore, spotty and hot to touch. Patches of dry flaky skin may also develop. in more severe cases, blisters and pus filled spots can occur. The baby will be crying, fretful, sore and uncomfortable.

taking care of a baby’s lovely soft skin is a top priority – which means keeping it protected from anything that can irritate or damage it e.g. products, clothes, detergents etc.

‘‘’’ cAuses

Baby’s skin has to put up with very difficult conditions, spending most of the day and night wrapped tightly in a hot, damp nappy and in regular contact with urine and faeces and the digestive enzymes they contain, so it is not surprising that 30% of babies develop nappy rash in the first year.

Nappy rash is a form of contact dermatitis. a wet or soiled nappy, which has been left touching the skin for prolonged periods of time, is the commonest cause of nappy rash and more especially if the child has diarrhoea. The urine is broken down to ammonia by bacteria from the baby’s stools and as ammonia is an irritant, it can cause anything from a slight reddening of the skin in the nappy area to severe cases which causes the skin to break and even blister. it is possible then for the skin to become infected.

Friction caused by the nappy rubbing or being too tight against baby’s delicate skin or using washing detergents that are too harsh or not rinsing cloth nappies carefully, can also contribute to the development of nappy rash. some babies have more sensitive skin, while others may suffer from eczema and psoriasis, which may be irritated further by wearing a nappy.

Babies are more susceptible to nappy rash:• if they are premature• During teething• after vaccinations• Changing from breast to bottle feeding• starting on solid foods• While on antibiotics, which can upset baby’s digestive system,

causing diarrhoea or by upsetting the delicate balance in the body thereby making thrush infections more likely.

• Drinking cows mild for the first time• Their skin is sensitive• There is a history of eczema• They have a tummy upset.

preVentIon

The aim of prevention is to reinforce and support the skin barrier. The skin of all newborn babies is delicate, sensitive and absorbent, but especially premature babies. Therefore it is commonsense, that changing the nappy frequently is key when it comes to skincare. Newborn babies may need their nappy changed as often as 10 times a day.

Taking care of a baby’s lovely soft skin is a top priority – which means keeping it protected from anything that can irritate or damage it e.g. products, clothes, detergents etc. Use products that have been dermatologically tested and that have been clinically proven to be gentle on the developing skin of a newborn baby. reads labels carefully and look for names you can trust. ideally products should contain no colourants, perfume, antiseptics or soap which can dry the skin. a lot of products on the market are designed to treat infected nappy rash and contain ingredients and antiseptics not suitable/needed for everyday use. a strict code of hygiene must be observed when changing baby’s nappies.

Clean the baby’s skin by gently wiping with cotton wool soaked in warm water. With baby girls, wipe from front to back, and don’t try to clean inside the vagina. Clean and dry around the leg creases.

With boys, hold the testicles out of the way and clean under them, then wipe over them and under the penis. Don’t try to push the foreskin back.

Pat dry thoroughly and coat the baby’s bottom with a thin layer of barrier cream/ointment at every nappy change and

especially at bedtime. This reinforces the skin’s natural barrier and helps stop the irritating chemicals in urine and faeces from penetrating.

Give the skin a chance to breathe and let the baby kick their legs without a nappy as much as possible, whilst lying on a towel or mat in a warm room.

if using cloth nappies make sure to wash in non-biological products and use a boil wash to kill any germs.

treAtment

The treatment of nappy rash depends upon the extent of the rash. The simple redness is first treated with barrier creams/ointments which are designed to prevent urine and faeces from coming in contact with the skin. a common example and widely used preparation is zinc and castor oil, which has been around a long time but still works well in mild rashes. Vaseline is also popular.

if the rash is more severe a mild steroid cream containing hydrocortisone may be needed. This will settle down the swelling but also use a barrier or ointment base.

if thrush is present, caused by an overgrowth of a fungus called candida, then an anti-fungal cream or ointment will be needed. This will often be in combination with a steroid.

if the rash is very severe and the skin broken/blistered, a bacterial infection may be present and the baby may develop a temperature an oral antibiotic will need to be prescribed by the doctor.

‘‘’’

…coat the baby’s bottom with a thin layer of barrier cream/ointment at every nappy change and especially at bedtime.

manflu exists…at least it does for menMen are more likely to say they have the flu when they know it is really just a cold, this is according to a survey of 1000 people which was conducted by ireach on behalf of Uniflu. Young men, in particular, are more likely to knowingly exaggerate their level of sickness.

30% of men, admitted to saying they had the flu when they knew it was just a cold, compared to just 19% of women. it comes as no surprise then that more men than women have been accused of exaggerating their sickness level (38% versus 25%).

The survey also, found that 13% of men believe that having a runny nose and a cough is the flu! Overall, 85% of respondents correctly identified flu symptoms as having a temperature, aching all over and being too weak to carry out household tasks.

Orla O’Connor, Marketing Manager, Uniphar commented on the research saying, “While we may joke about Manflu, actual flu is a serious illness which needs to be managed and treated seriously and Uniflu provides the most comprehensive treatment for colds and flu.”

While 19% of women would prefer to be looked after by their Mammy when feeling sick only 7% of men do, but the majority

of respondents agree that being at home warm and tucked up in bed is the best place to recuperate, not the hospital.

Half of those surveyed reported getting most sympathy and attention from their significant other when sick, with 58% of men and 44% of women indicating this. sadly 11% of older men admitted that they get more sympathy from the household cat or dog.

When it comes to basic hygiene habits women are more likely to wash their hands after sneezing however, younger men are more hygiene aware than older men, with 61% saying that they always wash their hands after sneezing compared to 49% of older men.

When asked if they could have an irish celebrity nurse to tend their every need as they recover, the majority of both men and women said their other half would be their number one choice.

However, for the less diplomatic TV presenter Grainne seoige was the men’s number one choice and for the ladies actor Liam Neeson was the most popular choice.

respondents said their number one port of call when feeling sick is the doctor (37%) followed by the pharmacist (21%) and 11% of men said that “no-one cares when i’m sick”. awh.
