What moves you?Every day at National Church Residences

we have the opportunity to witness acts of

generosity that allow us to further our

mission of serving seniors. What we have

come to learn is that what moves one

individual to give is not necessarily what

motivates another. The great part about

National Church Residences is that,

regardless of what fuels your passion, we

can direct your contribution in a way that

produces your desired impact.

For example: One donor felt deeply about

furthering our ministry to residents so the

family gave a sizeable donation to grow

our chaplaincy program. The chaplaincy

program has expanded from X clergy to X

clergy in just X years, largely due to this

generous gift.

Another donor wanted to show gratitude

for the warmth and love shown to a family

member at the end of their life by making

a donation to our hospice program in the

name of the endearing caregiver.

Yet another donor was drawn to our overall

mission of advancing better living for all

seniors, so the family gave an endowment

gift. It was important to this family that

their gift grow over the years, leaving a

legacy to support future seniors in need of

housing and health care services.

While the reasons may vary, the common

denominator that unites all of our donors

is their deep desire to give back and a

heartfelt passion for our mission.

As a not-for-profit organization, we rely

heavily on the continued support of our

donors. 2019 will not only be a year of

housing expansion, but of senior care

innovation. As we strive to do more for

seniors, we want to thank you for your

steadfast support that makes it all possible.

July 2017• Neshannock Woods begins renovation — originally built

in 1983, an 81-unit building in New Castle, Pennsylvania.

• The $10M renovation of Clark East Tower (Ypsilanti, MI) is

completed. The renovation included an expansion of the

community room, update of building systems, new

cabinets, flooring and fixtures in resident units and the

addition of a medical suite, outdoor walking path,

community garden and dog park.

• Anthony Evans is named Senior Vice President and COO

of Senior Living.

September 2017• Avondale II breaks ground — 100 units in Dublin, Ohio to

complete the phased community project. Serving the

housing needs of seniors in Dublin over the last five years

has been a great expansion of the National Church

Residences mission and this new development

reconfirms our commitment to the Dublin community.

• Memorial Garden dedication ceremony is held at Legacy

Village, Xenia, Ohio. The garden was built to honor

military veterans and their families. This space includes a

gazebo, a plaque in remembrance of the hospital that

once stood on the grounds, the placement of artifacts

and the preservation of century-old trees.

November 2017• Valley Bridge starts construction — a 70-unit

construction of affordable senior housing in Toledo, Ohio

with our partner, Friendship New Vision, Inc.

• National Church Residences welcomes new board

member Barbara J. Smoot.

December 2017• Telfair Arms completes renovation — Mary Telfair

Hospital, built in 1885, in Savannah, Georgia became 51

senior affordable housing apartments in 1985. National

Church Residences purchased the building in 2014 and

completed a $5.8M renovation.

August 2017• Ohio Housing Finance Agency (OHFA) awards 9% Low

Income Housing Tax Credit (LIHTC) to Barnesville Manor

(Belmont County, Ohio) for renovations. This renovation

will address critical capital needs of the building to

improve safety, accessibility, and extend the useful life of

the building.

• OHFA award helps fund Broadway Park, a new National

Church Residences affordable senior housing community

in Youngstown, Ohio.

• National Church Residences establishes a Hurricane

Relief Fund to assist seniors who suffered losses from

Hurricanes Irma, Harvey and Maria.

October 2017• Abbey Church Village Grand Re-Opening — Abbey

Church Village is a family property that completed a

$6.5M renovation. The renovation included a significant

expansion of the club house. Thanks to the Strong

Families Fund, the property received a grant for a full

time service coordinator. This service coordinator and

the expanded club house are crucial to providing

programming for children, adults and seniors within

the community.

February 2018• National Church Residences acquired Cranes Landing, a

senior affordable housing property located in Toledo,

Ohio. This acquisition adds 40 units of senior housing to

our Toledo portfolio, which now totals 436 units across

six Toledo communities.

April 2018• Balcones Haus completes renovation — 38 affordable senior

housing units in New Braunfels, Texas. This was a $3.4M

renovation of common areas and resident units.

• Affordable Housing Finance Top 50 Owners Announced

that National Church Residences was ranked #6 in the

largest nonprofit owner category.

May 2018• Abbey Church Village, Dublin, OH, named a Charles L.

Edson Tax Credit Excellence Award Honorable Mention

Recipient. Presented to the most outstanding Low

Income Housing Tax Credit (LIHTC) properties in seven

categories, the Edson Awards bring together Members of

Congress and industry participants to celebrate

developments at the forefront of creating strong,

healthier communities.

• National Church Residences hosts its inaugural power

breakfast, Rise ‘N Shine: A Breakfast to Champion Older

Adults in Atlanta.

March 2018• National Church Residences entered a New Property

Management Contract with JRC Sr. Housing, a senior

HUD 202 property, located in Canton, OH. The property

was developed in 1986 out of the vision of St. Paul’s

Catholic Church and Father John Robert Coleman whose

focus had always been on kids and seniors within the

community of Canton.

June 2018• National Church Residences receives Prestigious Freedom

Award nomination. The Secretary of Defense Employer

Support Freedom Award is the highest recognition given

by the U.S. Government to employers for their support of

their employees who serve in the Guard and Reserve.

Nominations must come from a Guard or Reserve

member who is employed by the organization they are

nominating, or from a family member. Special thanks to

Paul Girdley, Education Specialist, for his service to our

country and for nominating National Church Residences

for this award. Paul is a member of the Army National

Guard.

• Francie Hughlock, Community Service Coordinator at the

Westerville, OH Firehouse was nominated to receive the

City’s WOW (Westerville Outstanding Worker) Award.

• Kelli Meyung joins National Church Residences as Vice

President of Health Care Operations.

• Inniswood Village, National Church Residences newest

senior living community, celebrates its grand opening.

January 2018• Champion Intergenerational Enrichment and Education

Center in Columbus is for preschool children and seniors

who enjoy each other’s company during part of the day.

The center received The Mike Wurst $25,000 grant to

benefit programming there from the ORIX Foundation.

National Church Residences was the first to receive the

Mike Wurst Grant.

• Lakewood Christian Manor, a 250-unit senior living

community in Atlanta, celebrated the award of 199 Project

Based Rental Assistance (PBRA) vouchers under the

HomeFlex Program from the Atlanta Housing Authority.

This was very important because many residents were

losing their rent subsidies due to the expiration of a HUD

program.

Philanthropy Highlights

In June 2018, in collaboration with River Valley Health Foundation,

National Church Residences Transportation launched a new initiative

to support non-emergency medical transports for seniors seeking

life-sustaining treatments, such as oncological, kidney dialysis and

opioid addiction therapy. The River Valley Health Foundation awarded

a $50,000 grant to the National Church Residences Transportation

Program.

For those in parts of east Ohio and West Virginia facing a health care

crisis, finding reliable transportation for crucial appointments and

treatments can prove to be challenging. Supporting our

transportation program helps to remove the barriers between

low-income individuals and necessary services to maintain

independence while reducing premature nursing home placement.

National Church Residences’ Transportation Reaches Seniors Needing Life-Sustaining Treatments

In January of 2018, First Community Village Foundation launched a

campaign to create an innovative, comprehensive memory care

program for the residents of the First Community Village Roxbury

Cottages located in Upper Arlington, Ohio. With the generous

support of the Mirolo Charitable Foundation as a $50,000 matching

partner, the campaign raised in excess of $136,000.

The Roxbury is best known for its devotion, care and respect for all

individuals living with dementia. By creatively repurposing two

underutilized program spaces, the Roxbury Cottages will truly offer

cutting-edge memory care.

The three unique elements of this

project include: a Tranquility

Room, based on the Snoezelen

concept. This space will offer an

array of multi-sensory therapies

that have been proven to provide

both immediate and enduring

benefits in the treatment of a

dementia patient’s behavior. A

Shalom Suite will provide a

dedicated environment where

Roxbury residents in the later stage of dementia will experience a

calm, loving peace through light touch massage, aromatherapy,

soothing sounds and calming visuals. And, by installing flat screen

technology throughout the Roxbury Cottages, Memory Cueing

images will provide residents with a helping hand in navigating their

day. The unique linking of these three components promises

improved quality of life for Roxbury residents regardless of where

they might be on the dementia continuum.

The eighth annual Golf Classic event was another great success. With

13 event sponsors and 10 foursome sponsors, we were host to 96

golfers and over 200 in attendance for the evening’s program. We

raised a record setting $300,000 this year benefitting permanent

supportive housing and memory care.

Our keynote speaker this year was Kim Campbell. Kim was married to

pop-country music legend Glen Campbell for nearly 35 years. Their

marriage became a testament to devotion and overcoming adversity

when they faced their greatest challenge in 2011, when Glen was

diagnosed with Alzheimer’s disease. Kim supported her husband

through the tumultuous hardships that accompany the disease, and

has become a tireless, influential voice for all people living with

Alzheimer’s. While Kim was in Columbus, she toured our Champion

Intergenerational Enrichment and Education Center where she was

able to experience intergenerational programming with everyone

participating in Tai Chi. Later that same day, Kim toured our Mill Run

community and was impressed with the Tranquility Garden. This

space provides rich sensory experiences for memory care residents,

their families and loved ones.

The Hood to Coast event is a 12-person relay that spans 200 miles

starting at Oregon's Mt. Hood and ending on the beach in Seaside,

Oregon.

For over 20 years, the “Running for Shelter” team has participated in

Hood to Coast and over the years has raised more than $800,000. This

year, they supported our hospice program. The funds raised will aid in

the creation of two in-patient rooms at two of National Church

Residences’ communities in Chillicothe and Columbus, Ohio as well as

the expansion of the Memorable Moments programming. This was

an extremely successful fundraising campaign, raising over $109,000

which exceeded the original goal of $100,000.

The new in-patient rooms are designed to create a home-like setting

for those hospice patients who need to leave their homes because

they require a higher level of care. These rooms will include comforts

like a small refrigerator, TV with a DVD/VHS player, and a private half

bath. Visiting hours are unlimited and family members can stay

overnight.

National Church Residences Memorable Moments program was

created to fulfill a patient’s wish that exceeds the financial

wherewithal of hospice’s normal fundraising activities. Wishes fulfilled

can be as simple as a family dinner at a favorite restaurant,

underwriting a visit by a distant family member, or going to the

Zoolights at the Columbus Zoo.

Habitat for Humanity and a

$36,000 grant from Enterprise

Community Partners, National

Church Residences launched

Home for Life, an innovative

initiative with a goal to allow

vulnerable seniors to age in

place, in Atlanta’s Historic

Westside. The program grew out

of our affordable housing

Service Coordinator program

that has a proven history of

impact in the social/financial

domains for at-risk populations. As the nation’s largest employer of

Service Coordinators, we saw an opportunity to enhance and expand

our model to be able to help seniors access resources that will allow

them to age in place even if they don’t live in a National Church

Residences building. With this in mind, we developed a care

management system that enhances the role of a traditional

affordable housing Service Coordinator. Our current program brings

Home for Life to the 120 seniors of Big Bethel Village, one of our

senior affordable housing communities in Atlanta, Georgia, as well as

to the surrounding neighborhood through outreach. In addition, we are

partnering with Atlanta Habitat for Humanity’s Repair with Kindness

program to reach even more at-risk seniors in the community.

A $100,000 grant from the NEF Preservation Fund enabled the

continuation and enhancement of the program at Big Bethel Village

and with Atlanta Habitat for Humanity.

Because of the generosity of the Lucas County

Veteran Services Commission, the residents at

National Church Residences’ Commons at

Garden Lake in Toledo, Ohio, were able to

receive move-in kits, furniture, bus passes,

personal hygiene items and cleaning supplies,

as well as the provision of critical on-site

supportive services to help these veterans

improve their physical, mental and financial well-being. In addition,

the Lucas County Veteran Services Commission provided support to

launch a Safety Captain Program where resident volunteers receive

special training in health and safety procedures, mental health first

aid, and other practices aimed at ensuring a healthy and safe

environment for themselves and their neighbors. These resident

volunteers are also trained to provide leadership during emergency

drills, and to serve as peer mentors to new tenants as they move in

— helping them get acclimated and encouraging engagement in the

services and programming.

National Church Residences’ Home for Life Reaches out to Seniors in Need

National Church Residences’ comprehensive care for seniors includes

spirituality through the lens of Christian care giving. We offer support

to our seniors as they navigate new challenges, loss and celebrate

life’s many blessings.

As we work to serve the comprehensive needs of our residents,

volunteers will play an integral role in delivering these services. The

Volunteer Chaplaincy Program at National Church Residences equips

volunteers with resources and training necessary to offer spiritual

support to those who live in our communities. The program provides

the opportunity for our residents to meet with a faith based

individual who can listen, provide encouragement and help them to

continue to find meaning.

Volunteer Highlights

Huntington National Bank Columbus, Ohio

Over the years, Huntington National Bank has demonstrated what it means

to be a true and authentic partner as their reach has extended to almost all

aspects of our business. Huntington's involvement plays an integral role to

ensure our residents receive financial education. They have expanded their

volunteer roles from Columbus to Toledo, Akron, and Pittsburgh. In 2012,

Huntington’s Military Business Relations Group got significantly involved

with our veterans-only community, Commons at Livingston. They

established the Veterans Day dinners which have now become an annual

volunteer event for Huntington colleagues. Every year, sixty or more

Huntington volunteers serve dinner to 500 National Church Residences

residents. Our partnership with Huntington runs deep, and we could not be

more proud to have them as our 2018 Corporate Volunteer of the Year!

Leon Brodsky First Community Village, Columbus, Ohio

Leon Brodsky has been volunteering with National Church Residences at First Community Village (Columbus,

OH) since he was in middle school. He's now a high school junior with a challenging academic schedule and an

enduring spirit for helping people. Additionally, Leon is still able to balance a jam-packed extracurricular load of

refereeing soccer, volunteering at the library, volunteering for the Columbus Marathon along with his ongoing

commitment to the residents at First Community Village. Leon sees himself becoming a physician to provide

health and wellness to those who will trust him with their care.

Jerry Waugh Baptist Towers, Atlanta, Georgia

Jerry Waugh has volunteered at Baptist Towers (Atlanta, Georgia) for 10 years. Jerry is an integral part of the

Baptist Towers community. He was an employee for 15 years and he has been a resident for 25 years. Jerry

helps with the maintenance and grounds up keep around the community. Whenever anyone needs help in the

community, Jerry is the first name that is called. From calling BINGO to distributing the newsletter or hanging

flyers, Jerry never complains and is always willing to help.

Dolores Sofield Oak Creek Village, East Brunswick, New Jersey

Delores (Dee) Sofield is a valued asset to the Oak Creek Village team. She has volunteered for the past six years,

dedicating her free time to the community. Dee leaves her mark by planting flowers and decorating the inside

of the buildings for each holiday, making it a warm and inviting place. In addition, Dee hosts residents’ activities

every week, helps distribute and clean up for the weekly Panera Bread donation, and participates in Mission

Day activities. Dee’s mission is to leave this world a better place than she found it. And that’s exactly what she

does at Oak Creek Village.

Hurricane Support for Seniors Hotline

Long Term Disaster Recovery

In late 2017, two Category 4 hurricanes and one Category 5 impacted

37 National Church Residences properties in 26 days. While many of

our sites did not sustain significant damage, the commitment of the

site teams to ensure the health and safety of their residents meant

that seniors residing in National Church Residences buildings fared

better than the seniors living around them. As is often the case,

vulnerable seniors faced more challenges than others in the wake of

these natural disasters. Because of an $85,000 grant from Leading Age,

we were able to revive and expand a successful program from our 2005

Hurricane Katrina response — the Hurricane Support for Seniors

Hotline. Within days, the hotline was open to all seniors in the

hurricane impacted regions of Texas and Florida. Forty-five Service

Coordinators volunteered to staff the hotline, spending 1,950 hours

responding to 711 inquiries for assistance with medical help, housing,

food, financial assistance, and other needs.

Recognizing that beyond immediate disaster relief, hurricane-

impacted seniors in Texas and Florida are also in great need of

resources in terms of long term disaster recovery, National Church

Residences applied for and received two grants to distribute Federal

Disaster Recovery Funding. Through these programs, National Church

Residences is providing critical services and assisting seniors with

long term recovery efforts in Manatee and Sarasota counties in

Florida with a $100,000 grant from Volunteer Florida and a $50,000

grant through the HEART program (Housing and Economical

Assistance to Rebuild Texas) created by the Texas State Affordable

Housing Corporation and Enterprize Community Partners to benefit

the residents in Aransas Pass, Texas — two particularly hard hit areas.

Service Coordinators based out of National Church Residences

properties are assisting seniors with finding housing, accessing

resources to conduct repairs to their homes, and in some cases

providing direct support for those with critical needs.

Volunteer Highlights — Disaster

Employees Delivered Supplies and Assistance to Seniors in Puerto RicoWithin weeks after Hurricanes Irma and Maria devastated the island

of Puerto Rico with major infrastructure damage and massive power

outages, National Church Residences’ employees jumped in to make

a difference. Deliver the Difference provided 300 Relief Buckets for

residents filled with canned goods and supplies. Many National

Church Residences' employees volunteered to go to Puerto Rico to

support their colleagues and the residents. The team who traveled to

Puerto Rico took with them 6 suitcases full of supplies that were

collected by employees. While there, in partnership with Volunteers

of America, they packed 600 bags of supplies (totaling more than 300

pounds) and conducted assessments on more than 300 seniors in five

apartment communities (including three National Church Residences'

properties) in four cities across the island.
