Cover• First and last name• Class (Lifetime Fitness or Team Sports)
• Period• Teacher (Brown or Ross)• Optional – decorate with fitness or
sports related pictures and phrases
Author Page• Very first page (Page 1 on the
right)• Write 2-3 paragraphs about
yourself and your likes, hobbies and interests.
Notebook Guidelines• Page 2 (on the left side)• Create a “notebook” sketch

• Fill in the information on the left side of the notebook you sketched (Output side – information/assignments created by the student)–Processing ideas–Thinking Maps–Sketches–Tables and information logs–Notebook activities
• Fill in the information on the right side of the notebook you sketched (Input side – information from the teacher)–PowerPoint notes–Discussion Notes–Teacher provided materials
• Highlighting:–Vocabulary–Rules–Techniques
• Policies:–Care and upkeep is the student’s
responsibility–Notebooks are brought to class daily–Notebooks will be checked at least once
a month

you would like to strive for throughout the semester.
In your notebook…• On page 9, write “Fitness Testing” across
the top (right side)–Create a matching entry in the Table of
Contents–Go back to page 9, record your
fitness test scores for all seven tests.
