Should we use the FGF of the Anesthesia Machine

as a source of oxygen for Nasal Prongs or loose Mask ?

No The breathing circuit will not be ready

to resuscitate from a respiratory emergency

Add a flowmeter somewhere

Page 5: Physics of Reservoir Bags - Anesthesiologyetherweb.bwh.harvard.edu/education/PHILIP/Reservoir_Bag_Physics_J...Physics of Reservoir Bags James H. Philip, ME(E), MD, CCE Anesthesiologist,

Should we use the Y- piece* of the breathing circuit

as a source of oxygen for Nasal Prongs?

No The Physics does not work

* AKA Wye-piece!

Page 9: Physics of Reservoir Bags - Anesthesiologyetherweb.bwh.harvard.edu/education/PHILIP/Reservoir_Bag_Physics_J...Physics of Reservoir Bags James H. Philip, ME(E), MD, CCE Anesthesiologist,

FGF Set to 8 LPM

Close the Relief Valve*

* AKA Pop Off Valve, APL (Adjustable Pressure Limiting) Valve

But, we see and hear an alarm “Continuing Pressure”

Page 14: Physics of Reservoir Bags - Anesthesiologyetherweb.bwh.harvard.edu/education/PHILIP/Reservoir_Bag_Physics_J...Physics of Reservoir Bags James H. Philip, ME(E), MD, CCE Anesthesiologist,

Continuing P = 38 cmH2O

Page 15: Physics of Reservoir Bags - Anesthesiologyetherweb.bwh.harvard.edu/education/PHILIP/Reservoir_Bag_Physics_J...Physics of Reservoir Bags James H. Philip, ME(E), MD, CCE Anesthesiologist,

The bag grows bigger

Page 16: Physics of Reservoir Bags - Anesthesiologyetherweb.bwh.harvard.edu/education/PHILIP/Reservoir_Bag_Physics_J...Physics of Reservoir Bags James H. Philip, ME(E), MD, CCE Anesthesiologist,

And bigger

Page 17: Physics of Reservoir Bags - Anesthesiologyetherweb.bwh.harvard.edu/education/PHILIP/Reservoir_Bag_Physics_J...Physics of Reservoir Bags James H. Philip, ME(E), MD, CCE Anesthesiologist,

The FGF is still 8 LPM

Page 18: Physics of Reservoir Bags - Anesthesiologyetherweb.bwh.harvard.edu/education/PHILIP/Reservoir_Bag_Physics_J...Physics of Reservoir Bags James H. Philip, ME(E), MD, CCE Anesthesiologist,

So, I open the relief valve

Page 19: Physics of Reservoir Bags - Anesthesiologyetherweb.bwh.harvard.edu/education/PHILIP/Reservoir_Bag_Physics_J...Physics of Reservoir Bags James H. Philip, ME(E), MD, CCE Anesthesiologist,

And, I carefully adjust the relief valve to keep the bag constant and reasonable size, with No Alarm

Page 20: Physics of Reservoir Bags - Anesthesiologyetherweb.bwh.harvard.edu/education/PHILIP/Reservoir_Bag_Physics_J...Physics of Reservoir Bags James H. Philip, ME(E), MD, CCE Anesthesiologist,

Pressure falls to 3 cmH2O I am content

Page 21: Physics of Reservoir Bags - Anesthesiologyetherweb.bwh.harvard.edu/education/PHILIP/Reservoir_Bag_Physics_J...Physics of Reservoir Bags James H. Philip, ME(E), MD, CCE Anesthesiologist,

Experiment 2 How much flow was going to the nasal prongs?

Page 22: Physics of Reservoir Bags - Anesthesiologyetherweb.bwh.harvard.edu/education/PHILIP/Reservoir_Bag_Physics_J...Physics of Reservoir Bags James H. Philip, ME(E), MD, CCE Anesthesiologist,

I close the relief valve I set the flow until pressure is 3 cmH2O, just like before

Page 23: Physics of Reservoir Bags - Anesthesiologyetherweb.bwh.harvard.edu/education/PHILIP/Reservoir_Bag_Physics_J...Physics of Reservoir Bags James H. Philip, ME(E), MD, CCE Anesthesiologist,

The flow is 400 mL/min

Page 24: Physics of Reservoir Bags - Anesthesiologyetherweb.bwh.harvard.edu/education/PHILIP/Reservoir_Bag_Physics_J...Physics of Reservoir Bags James H. Philip, ME(E), MD, CCE Anesthesiologist,

The flow is 400 mL/min That is what was flowing before That is what is flowing now

Page 25: Physics of Reservoir Bags - Anesthesiologyetherweb.bwh.harvard.edu/education/PHILIP/Reservoir_Bag_Physics_J...Physics of Reservoir Bags James H. Philip, ME(E), MD, CCE Anesthesiologist,

The remainder of the 8 L/M Flow went out the pop off valve

Page 26: Physics of Reservoir Bags - Anesthesiologyetherweb.bwh.harvard.edu/education/PHILIP/Reservoir_Bag_Physics_J...Physics of Reservoir Bags James H. Philip, ME(E), MD, CCE Anesthesiologist,

The patient was getting almost no supplemental oxygen

Page 27: Physics of Reservoir Bags - Anesthesiologyetherweb.bwh.harvard.edu/education/PHILIP/Reservoir_Bag_Physics_J...Physics of Reservoir Bags James H. Philip, ME(E), MD, CCE Anesthesiologist,

Flow and Pressure are related through Resistance

If the pressure is what it was before, and the tubing is what it was before, the flow must be what it was before. Pressure was always 3 cmH2O. Flow was always 400 mL/min. Remainder of the the 8 L/min flow went out the pop off valve ! Flow = Pressure / Resistance; F = P / R

Page 28: Physics of Reservoir Bags - Anesthesiologyetherweb.bwh.harvard.edu/education/PHILIP/Reservoir_Bag_Physics_J...Physics of Reservoir Bags James H. Philip, ME(E), MD, CCE Anesthesiologist,

When we close the relief valve

Reservoir bag properties are important Compliance is the important property

Compliance = Change in Volume / Change in Pressure

C = ∆ V / ∆ P Stiffness is 1 / Compliance

S = ∆ P / ∆ V

Page 29: Physics of Reservoir Bags - Anesthesiologyetherweb.bwh.harvard.edu/education/PHILIP/Reservoir_Bag_Physics_J...Physics of Reservoir Bags James H. Philip, ME(E), MD, CCE Anesthesiologist,

Bag Compliance is not constant It varies with bag volume

Page 30: Physics of Reservoir Bags - Anesthesiologyetherweb.bwh.harvard.edu/education/PHILIP/Reservoir_Bag_Physics_J...Physics of Reservoir Bags James H. Philip, ME(E), MD, CCE Anesthesiologist,

Bag Compliance is not constant It varies with bag volume

When the bag is empty and filling P = 0 Compliance is infinite C = 2 L / 0 = ∞

When the bag is full and stretching Compliance has a value C = 3 L / 40 cmH2O = .05

When the bag has stretched further Pressure does not rise Compliance is infinite C = ∆ V / 0 = ∞

C = ∆ V / ∆ P

Page 31: Physics of Reservoir Bags - Anesthesiologyetherweb.bwh.harvard.edu/education/PHILIP/Reservoir_Bag_Physics_J...Physics of Reservoir Bags James H. Philip, ME(E), MD, CCE Anesthesiologist,
