définition●Préeclampsie =
– PA 140/90mmHg à 2 reprises– Et protéinurie > 300 mg/24h
● OU insuffisance d'organe– Après 20 SA
Critères de gravité●Cliniques
– HTA sévère >160/110 mmHg– Céphalées, troubles visuels– Dyspnée, OAP– Barre épigastrique– OMI– ROT vifs
●Biologiques– Créat > 100µmol/L– ASAT > 3N– Hémolyse– Thrombopénie < 100 G/L

●En faveur PE – Céphalées, cytolyse, thrombopénie
●En faveur exacerbation néphropathie– Survenue <20 SA– Anomalies complément, ECBU, ...
Biopsie ?●Pendant la grossesse
– Possible si terme <32SA– Risque complication ≈ 5-7 %– Risque MFIU, hématome– Si IRA / sd néphrotique
●En Post-Partum– Si Protéinurie persistante (jusqu'à 6 mois)
Déclenchement ?●2 questions :
– Terme de la grossesse– Signes de gravité
●2 dates :– > 37 SA– < 34 SA
Contrôler la PA ?●Seulement si PA >160/110 mmHg●Objectifs :
– PAs 130-150 mmHg– PAd 80-100 mmHg– Baisse progressive ++
●Moyens :– Loxen 1 mg/h en titration IV– Trandate 10 mg/h en titration IV
