Philips Respironics $100 CASH BACK promotion claim form The Promotion commences at 09.00am AEDT on 30 November 2020 and ends at 11:59pm AEDT on 28 December 2020 (“Promotional Period”).
HOW TO CLAIM YOUR MONEY BACK In one transaction purchase and pay in full: (i) a new DreamStation CPAP; and (ii) a new mask from Philips DreamWear or DreamWisp ranges* (See table below) and follow the steps: 1. Register, download and complete the claim form from the website at www.philips.com.au/sleepservicespromotions2. Email the completed claim form with a copy of your purchase receipt (please retain original) to: [email protected] must be received no later than 5.00pm AEDT on 25 January 2021. Claims received after this date will not be processed.
Successful claimants will receive their $100 cash back in the form of Electronic Funds Transfer (EFT) to the bank account provided on this claim form within approximately 3-4 weeks from the Promoter’s approval of the claim. The cash back amount cannot be redeemed for cash, transferred, exchanged for any other product, or claimed at point of purchase.
The Promoter is Philips Electronics Australia Limited (ABN 24 008 445 743) of 65 Epping Rd, North Ryde, NSW 2113 (“Promoter”). Claims will be processed by DMC Advertising Group Pty Ltd (ABN – 61 074 595 887).
