Register, download and complete the claim form from the website at www.philips.com.au/sleepservicespromotions 2. Email the completed claim form with a copy of your purchase receipt (please retain original) to: [email protected] Claims must be received no later than 5.00pm AEDT on 25 January 2021. Claims received after this date will not be processed. Successful claimants will receive their $100 cash back in the form of Electronic Funds Transfer (EFT) to the bank account provided on this claim form within approximately 3-4 weeks from the Promoter’s approval of the claim. The cash back amount cannot be redeemed for cash, transferred, exchanged for any other product, or claimed at point of purchase.
