  • Insurance Questions

    What Your Client Needs To Do: When trying to file a claim through insurance or Medicare, there are important documents that PMTD/Flexiciser needs in

    order to thoroughly process your request. Each Insurance Acceptance Packet comes with a list of materials needed from each clients ordering physician as well as forms that need to be filled by the client themselves. The request for medical necessity can be from a physical therapist, an occupational therapist, or a primary care physician, however most success is achieved when the prescription is written by the primary care physician.

    What You Need To Do To Submit A Claim: All insurance claims should be handled directly with PMTD. This is a lengthy and methodical process therefore necessary to

    do this within our company. When submitting a claim for your client to PMTD/Flexiciser, please make sure that your client has all documents (the clients and the physicians responsibility) completed and signed from the checklist. All of these documents are available on your intranet as well as upon request by emailing [email protected]. Please direct all insurance questions to this same address.
