A. Organization Profile

Description of the organization or place where these problem situations are obtaining Include only those that are relevant to the study “Setting the tone” – locating the problem situation or issue within its broader context Past and present policies/actions to address the problem/issue (if there are)

Successes and failures of the past and present policy/action Differing opinions on the problem and the current policy/system? What is wrong with the current policy/system?

B. Situation Analysis

Scope of the study: parameters, range, period covered Discuss the current status and extent of the problem/opportunity/issue Cause and effect analysis

legal, social, economic, political contexts and impacts of the problem effects and impacts on people, methods and resources, etc. causes

Discuss the need/value/importance of solving the issue/problem Convince that the issue in focus requires action

Use in this Section all findings/data gathered from survey, interview, document review, etc. Show tables, graphs, pictures

C. Problem Statement

One sentence/phrase about the problem (must not be different from the head of Fishbone diagram) Simple, clear and unambiguous May be in declarative or interrogative form

Description/discussion of problem (issue or opportunity)

D. Stakeholders Analysis

Identify and explain persons/organizations who: can promote/facilitate the solution can prevent the success of the solution are affected/benefited by the solution

Discuss the role of each stakeholder

E. Goals and Objectives

What has to be achieved to eliminate the causes of the problem Use the identified causes as direction of specific targets to be achieved

F. Methodology

Discuss:

Methods and Analytical Approaches/Tools Used Research procedures Respondents of Survey/Interview Time and place of the Study

Discuss the what, who, when, how the study was done
