Envelope 2:Technical Proposal Requirements
Submit an Original and four (4) copies in the Technical Proposal envelope:
• Do not include any reference to price• Submit on typewritten 8 ½ x 11” plain white paper• Assemble in a three ring binder or staple; No other binding methods are
acceptable• Do not exceed FIFTY (50), double-spaced, single sided pages
- Exhibits and samples of previous work are not included in the fifty page limit
• Address the Evaluation Criteria in the order they are presented in Attachment 1:- Criterion 1: Qualification and Experience - Criterion 2: Project Organization Management- Criterion 3: Technical and Production Capabilities
Envelope 3:Price Proposal
• The Airports Authority will only open the price proposals from Offerors whose technical proposal was rated as PASS
• The Price Proposal must be submitted in accordance with the Price Schedule
• Following its assessment of the Offerors’ responsiveness to the Solicitation requirements and determinations of responsibility, the Airports Authority will provide a Notice of Recommended Award to the apparent successful Offeror, which will be the Offeror that has submitted the lowest Contract Price on the Price Schedule and has been determined to be responsive and responsible - A copy of this Notice will be posted on the Airports Authority’s website

Scope of Services: Base Services
• Provide design and obtain permits for the Dulles Airport Station glazed and louvered windscreens, consisting of structural steel support framing, glazing, architectural louvers, lighting and precast sills
• Furnish and install the windscreens at the Dulles Airport Station
• Design, permit and implement necessary maintenance of traffic measures
• Coordinate with the Airports Authority and Station Design/Build Contractor

requirements required by the Federal, State, local governments, the contract or MWAA’s Construction Safety Manual (CSM), Revision 17.
The following slides only serve as an overview for some of the safety requirements of the CSM. Be sure to read your contract and the CSM.

Safety – Pre-Start Pre‐start Required Subcontractor Information Package Checklist (PRSIPC) MUST be
completed prior to the start of work.
PRSIPC must be signed acknowledging that the contractor has read and understands the Airports Authority’s Construction Safety Manual (CSM). CSM Must be followed by the contractor and their subcontractors.
Emergency Contact names and numbers for senior Company managers (Include President, Project Manager and Safety Director).
Experience Modifier Rate (EMR)‐ If EMR is 1.0 or greater, submit the OSHA 300 logs for the past three (3) years, plus the current calendar year. Additionally, create and submit a Safety Mitigation Plan addressing the safety issues that the contractor has identified from the OSHA logs mentioned above.

OSHA 30 Hour Construction certification required for supervisors. One 30 Hour OSHA trained employee per subcontractor must be on site at all times. All workers are required to have 10 hour OSHA construction training (provide copies of cards for all of the aforementioned).

Contractors duty to prevent jobsite accidents
Contractor and ALL their subcontractors required to enroll in the Owner Controlled Insurance Program (OCIP).
Definable Features of Work‐ List to be submitted along with Job Hazard Analysis (JHA) for all known work activities.
JHAs to be prepared on the Pre‐Task Work Plan (PTWP) Form in Appendix E of CSM.JHAs required prior to any work on‐site. Be very detailed on this plan.
Safety-Pre-Start CPR/First Aid Qualified Personnel, Certification cards must have expiration date (provide copy of
each CPR/First aid trained person).
List of Competent Persons per OSHA definitions. Provide Contact numbers and credentials for each. Provide training certificates/documentation for each competent employee.
List of Equipment to be used on site (update as needed). All equipment must Identify owner company. Provide training records.
Cranes: provide annual inspection, Certified Crane Operators Card(s), Riggers training and certification card. Signalman training and certification card, Lift Plan and Erection Plan (provide copies).
Fork lifts (including rough terrain), Powder Actuated, Boom & Scissor lifts authorization cards/list. Provide training records.

Safety-Preparatory 10 and 30 hours OSHA certification must be obtained within 6 months of Notice to Proceed (NTP)
Contractor must develop a safety plan that is site specific and follows the guideline set forth in Appendix C of the CSM.
Safety Plan must be submitted and approved prior to the start of any work.
Safety Professionals‐ Submit resumes for review and candidate for interview .SM has the final approval/disapproval of candidate.
o Safety engineers‐ Required for the Project/shift. Must have 5 yrs. Of continuous construction experience relevant to the work being performed (dual roles will not be accepted). Perform daily inspections of contractors and subcontractor’s job sites.
o Safety Managers‐ A full time on‐site safety Professional with a minimum 10yrs of experience managing safety programs on large construction projects. Monitors efforts of Safety Engineers and performs administrative duties set forth in the CSM and OCIP manuals.5/30/2018 34
Safety-Preparatory o The contractor shall provide adequate safety coverage if the approved safety engineer or safety
manager is “absent” or “for any other reason”. Therefore, additional safety professionalsmust be pre‐approved to cover safety staffing requirements.
o On‐site work may not be performed until the specified safety professional(s) is/are approved andpresent on‐site.
o Controlling contractor or sub tier contractors who have over 30 working employeesworking shall have a dedicated safety engineer.
o Safety personnel can only work on one project at a time.

Safety- Reporting Metropolitan Washington Airports Authority (Airports Authority) shall require
the contractor and their subcontractors to preplan their work activities on adaily basis. Prior to the actual start of work, the foreman shall conduct a Pre‐Task Work Planning (PTWP) meeting with the work crew performing thework.
Job Hazard Analysis/ Activity Hazard Analysis (JHA/AHA) required for each Definable Feature of Work (DFOW).
Report unsafe conditions immediately to the Project Safety Manager (PSM).
ALL injuries , Incidents, Near misses are to be reported immediately to the PSM.
Written incident reports are to be submitted to the PSM within 24 hours.
Monthly man‐hour report must be submitted by the 10th of each month.

Safety- Cranes
Critical lifts are any lift exceeding 75% of the rated capacity of the crane or Iinvolving two or more cranes.
Lift plans shall be submitted for review to the PSM prior to any crane lift.
Chains are prohibited for lifting.
Crane mats must be a minimum of 9 square feet for cranes of 50 tons or less.
Cranes with a capacity greater than 50 tons will use the following formula for determination of the crane pad size.. Capacity of the crane / 5= area of crane pad.

Prior to excavation a preplanning meeting will take place with the Airports Authority
Prior to excavation ALL utilities must be identified in an area 5 feet beyond the intended area of excavation. Look in manholes and for other clues.
Contractor will use the Excavation checklist (appendix F)
MWAA requires protection of excavation starting at 4 feet in depth.
Benching, sloping, trench boxes all permissible.
Means of egress must not be more than 25 ft. from employee at any point in the excavation.

Competent person shall inspect the excavation /trench prior to anyone entering excavation.
Competent person shall be at the excavation/trench site while work is being performed.
If using a trench box, tabulated data shall be submitted to theAirports Authority COTR and reviewed by the PSM. Trench box data shall be stamped by a PE.
Excavation spoils and equipment operating next to an excavation shall be aminimum of 2 feet away from the leading edge.

Policy Statement
Metropolitan Washington Airports Authority (Airports Authority) Fall ProtectionPolicy requires 100% fall protection when employees are exposed to a fallhazard six (6) feet or greater. There will be a zero tolerance for not being 100% tied off over 6 feet. Contractors working on an Airports Authority projectinstalling and using fall protection systems shall be in compliance with 29 CFR1926, Construction Industry Regulations, Subpart M ‐ Fall Protection inaddition to the following procedures and requirements

All fall protection systems shall not be over 5 years (depending onmanufacturer) of the manufacturers issue date or service date bythe contractor (documentation required).
Employees shall be protected from falling objects by theinstallation of toe boards, barricades, snow fence, mesh or canopystructure.

MOT‐ Prior to performing construction, maintenance, or other work activitiesalong the Dulles Toll Road (DTR), Dulles International Airport AccessHighway (DIAAH) and/or other connecting arterial roadways supportingvehicle conveyance at DCA or IAD a Traffic Control Plan shall be developed.
MOT‐ ANSI Class 3 high‐visibility clothing is required on all AirportsAuthority construction projects and roadway maintenanceoperations
TOOLS‐ No cartridge style nail guns, nor any tool that uses a cartridge orany explosive charge, shall be permitted in public areas, unlessauthorized by Airport Operations / Security.

HOT WORK‐ A Fire watch is required during operations and for 30 minutesafter completion of welding/cutting unless otherwisespecified by the fire marshal or other hot work operations.
HOT WORK‐ (2)A:(20)B:C rated fire extinguisher is required where work isbeing performed. No combustibles within 35 feet of Hot work unless an approved cover is used.

HOUSEKEEPING‐ Housekeeping shall be maintained as an integral part of every work operation. It’s the controlling contractor’s responsibility to conduct routine housekeeping inspections of their project.
PPE‐ Contractors shall establish a “Glove Protection Policy” appropriateto the hazards identified by specific task(s).
PPE‐ Gloves will be required at all times when working on AirportsAuthority construction sites. Gloves worn to be selected on basis of hazard
PPE‐ Employees shall wear full length chaps (e.g. Kevlar) or theequivalent, wire mesh face shield, leather gloves, safety goggles,Steel/composite toed boots and hearing protection when operatinga chain saw.

Risk Management Requirements
Rail OCIP Overview
• Package ‘G’ for the Dulles Corridor Metrorail Phase 2 construction project will be insured under an Owner Controlled Insurance Program (Rail OCIP).
• The Rail OCIP insures contractors and subcontractors for Work performed On-Site.
• Participation by contractors and subcontractors is mandatory.• Enrollment must occur before performing Work On-Site.• Notify your insurer(s) to endorse your coverage to be excess and
contingent over the Rail OCIP coverage for On-Site activities.• Exclude from your bid price and requests for payment the cost of
insurance coverages provided by the Rail OCIP.• Contractors and subcontractors must purchase their own insurance
for off-site activities and exposures not covered by the Rail OCIP.
Summary Description of Rail OCIPCoverage
Airports Authority provides the following coverages through Rail OCIP:– General Liability, including terrorism
• $2,000,000 each occurrence limit– Excess Liability, including terrorism
• $298,000,000 combined single limit– Contractor’s Pollution Liability

Verification of Required Coverages
• Evidence of insurance for all Enrolled and Excluded Contractors shall be provided to Rail OCIP Administrator before performing Work On-Site and upon renewal of required coverage.
• Contractors are responsible for:– Monitoring their Enrolled subcontractors’ and Excluded subcontractor’s
certificates of insurance.– Maintaining copies of their subcontractor’s certificates of insurance.– Contractor and all Subcontractors must upload Certificates of
Insurance through the Rail OCIP web portal.• The Contracting Officer, Contracting Officer’s Technical
Representative, Risk Management Department staff, and RAIL OCIP Safety Consultant have the right to stop work or prevent any non-enrolled Contractor or subcontractor of any tier from entering the Site until the Contractor's certificate has been filed.
Contractor Responsibilities
• Comply with Rail OCIP Manual and cooperate in the administration and operation of the Rail OCIP.
• Comply with the Airports Authority’s Construction Safety Manual and all other contractual safety and loss control requirements.
• Provide each subcontractor with a copy of Rail OCIP Manual and the Construction Safety Manual.
• Enroll in the Rail OCIP and assure all subcontractors enroll in the Rail OCIP prior to the start of any work.
• Include Rail OCIP provisions in all subcontracts.• Provide timely evidence of insurance to Rail OCIP Administrator.• Notify Rail OCIP Administrator of all subcontracts awarded.• Comply with insurance, claim, and safety procedures.• Report incidents and claims promptly and cooperate with all follow-
up requests.
Enrollment
• Use of Rail OCIP web portal is mandatory.• Enrollment Application (Rail OCIP Form – 1GL) required for each
contractor of any tier that performs Work On-Site.• Pre-Enrollment (Rail OCIP Form – 3GL) required for every
subcontract awarded and necessary to obtain user account for web portal access.
• Detailed web portal instructions are located in Section 6 of the Rail OCIP Manual.
• Contractor is not enrolled in the Rail OCIP until the Rail OCIPAdministrator has approved the information submitted via the web portal and confirmed the enrollment is complete.
Completion of Work
• Notice of Work Completion (Rail OCIP Form – 2GL) is required to be submitted to the Rail OCIP Administrator when contractor at any tier has completed its Work at the Site and no longer has On-Site workers.
• Final payment will not be released by the Airports Authority until all necessary forms have been submitted to the Rail OCIPAdministrator.
• The Contractor is responsible for any missing documentation of its subcontractors.
Loss Control Inspections
• Daily loss control observations will be conducted by the Rail OCIPSafety Consultant.
• Monthly loss control inspections will be conducted by the General Liability insurance carrier.
• The Contractors Pollution Liability insurance carrier may also conduct inspections.
• Contractor is expected to remediate any issues found and provide written response to close out any identified recommendations.

Incident Reporting
All incidents, injuries, property damage, pollution, occupational-related illnesses, etc. must be reported to Rail OCIP Safety Consultant within 24 hours.• Take appropriate measures to prevent additional injury or damage,
including contacting police and fire authorities as required by law.• Complete and submit General Liability Loss Report
(Rail OCIP Form – 4GL) within 24 hours of the incident to the Rail OCIP Safety Consultant.
• Cooperate with Airports Authority, Rail OCIP Safety Consultant, or Rail OCIP insurer representatives and assist in the investigation, analysis, and defense of any accident, occurrence, or insured loss.
Rail OCIP Insurance Claims
• Contractor shall NOT self-handle Rail OCIP insurance claims.• The Airports Authority Risk Management Department will negotiate
all Rail OCIP insurance claims.• Contractor will not attempt to adjust or settle any insurance claims
unless Airports Authority Claims Program Manager determines loss is not insured by Rail OCIP.
General Liability Obligation
• Safety On-Site is important.• To encourage adherence to safe practices by all Parties, the
Contractor shall pay to the Airports Authority a sum of up to $5,000 of each occurrence for losses payable under the Rail OCIP General Liability Policy.
Required Forms
