Introduction

This outline method statement describes a proven approach to the successful

management and control of activities associated with the Civil & MEP Services and

Installations. Separately we describe our proposals and procedures of management from receipt of

contract award through;

- Operation & Maintenance Manuals Delivery. - Plant, equipment and subcontract procurement. - Quality Assurance and Quality Control program. - Relevant construction activity method statements. - Testing & commissioning Method. - Safety Regulations.

This methodology demonstrates major project team working experience to ensure

overall project objectives are achieved.

A. PROJECT MANAGEMENT PROCEDURES.

The aim of the project manager is to carry out the work efficiently to achieve

completion within the cost limitation of necessary of the contract sum. Company

policy is to manage our proportion of the work and if necessary other trades, to

succeed. In the absence of realistic recourse to contract law taking a heavily

contractual and negative stand will only cause bad feeling. A more imaginative

approach is favored to achieve results

It is essential to commence work in an efficient and effective manner. It is so often

the case that more time is lost on site in the opining few weeks of a project than at any

other. Time lost is rarely recovered so as far as is possible each week should be treated

as the last.

The Project Manager is the highest authority on site. He has full control over the

works and is empowered to represent the company and make decisions he sees fit. He

is responsible to the Operation Manager and may call upon the support of the Main

Office at his discretion.

B. Testing & Commissioning Method.

Inspection and test plans will be prepared and agreed in advance of any

testing. These will identify what tests or inspections are required, when they have to

be performed and what records have to be kept. The tests will basically follow the test

requirement stipulated in the tender document. The testing and inspection will be

generally performed at 3 stages: inspection at receipt and unpacking of equipment;

testing and inspection of the equipment at completion of assembly; testing and

inspection at completion of connection and prior to commissioning.

All tests and inspections will be coordinated with the Authority in advance for

arrangement of witnesses and certification.

All energized equipment will be controlled via a permit system, which will be

strictly enforced. Permanent locked doors will be required on plant rooms before

energizing any equipment.

C. Quality Assurance and Quality Control program. Mighty Trading & Contracting is committed to seeking continuous improvement

in all areas related to the management system. Quality Management System is a set of

plans & procedures established to ensure compliance with the quality assurance set for

the project.

Quality means understanding our client’s requirements and making every effort to

surpass them, while at the same time complying with quality standards and policies.

Our goal is to satisfy our client and to comply not only with the contractual

requirements but also to provide excellence in engineering practice.

The primary objectives are:

• The establishment of quality standard and criteria.

• To maintain our excellent record in construction & building industry.

• To produce the product that the client wants.

• Quality system for testing, monitoring & reporting.

• To complete all projects on time and within budget.

• To exploit our accumulated knowledge and experience.

The Quality Assurance and Quality Control Plan will be prepared for every new

project to satisfy our client requirements and the job specifications.

D. Pre Construction Method Statement.

Immediately following contract award our project management team will be

established to initiate project liaison with the professional team. Our early involvement

is welcomed on all projects to ensure the best production levels are achieved, during

the site construction period. The liaison with the site team is a fundamental necessity

for communication prior to any working activities. Initial activities will focus on the

development and submission of:

- A Project Specific Quality Plan Project Programmes

- Design and drawing production schedules

- A procurement plan with appropriate submissions

- Details of specialist equipment manufacture and operating performance

- Finalization of construction sequence activities

- A logic linked cost plan and management procedures

- Detailed drawing development

PLANNING METHODOLOGOY

The Preparation and Content of the Project planning Documents

Mighty Trading & Contracting Co W.L.L. planning documents may be strategic,

tactical or logistic in purpose. Presentation takes a number of forms, appropriate to the

type of the project and the stage of its execution. Typical form of documents used in

the company;

- Illustrative sketches

- Network analysis in presence form Bar charts

- Schedules or matrices of data

Construction Programme

Construction programme shall be formulated, based upon the contractual programme

as amended by subsequent instructions from the Project Manager.

The construction programme shall be of the “bar-line” form, supplemented, at the

discretion of the Project manager.

The construction programme shall contain the following information;

- All key access dates

- Availability of scaffolding (if applicable). Permanent statutory services

facilitate commissioning. Release of necessary information

- All hold points, such as completion of testing, completion of static checking,

completion of cleaning and availability of power, and periods/dates between

after and client’s witness.

Working Drawing Programme

A detailed programme for the full schematics and working drawings shall be

formulated, based upon the construction programme. The programme shall contain the

following information;

- Start and finish dates for each group of drawings

- Required information release dates from client

- Required information dates from supplier

- Client approval dates (and periods) for drawings

All these shall be in accordance with the requirements of the construction programme.

Buying Programme

Shortly after an order is received from the client, the Project Manager shall ensure

that a buying programme is produced. It is a prediction of when orders must be placed

in order to ensure delivery on the programme dates.

Procurement

This procedure requires the listing of all future orders to be placed on suppliers

and sub-contractors, together with the following minimum information against each

future order;

- Description of material/sub-contract

- Order can be placed

- Date material required on site taken from the construction programme.

The standard proforma “Material Control Chart” may be used for the Buying

Programme. Alternatively, the Buying Plan may be set up on a PMS Phase providing

the minimum information listed under the above bullets.

Sub-Contract Programme

Once the construction programme has been agreed, programmes will be prepared

and issued for the major sub-contract elements of the works, typically builders work

ductwork, control/instrumentation and other specialist contractors.

The sub-contract programmes shall contain the following information as

applicable;

- Drawing production periods

- Drawing approval periods

- Manufacturing/delivery periods

- Installation/erection periods

- Commissioning period and handover

As with the construction programme all “hold” points will be indicated.

Commissioning Programme

A commissioning programme shall be formulated in conjunction with the Project

Programme.

Key dates shall be given on this programme as follows;

- Completion of static checks

- Date for Statutory services to be made available

- Setting to work of each system (initial scans)

- Completion of dynamic checks

- Periods and dates between offer and client’s witness

- Client training

Manning Levels for Construction

A programme of manpower levels will be produced to indicate the levels of manpower

required at each stage of construction.

Short Term Programmes

At the discretion of the Project manager, a series of short-term programmes may

be formulated; either for the project as a whole or for smaller sections of the work at

Foreman level.

Such short-term programmes shall be formulated on How United Services

proforma or similar type proforma. If, due to the complex nature, the type of proforma

to be used will be to the discretion of the Project Manager.

E. Construction Method Statement.

KEY SUCCESS FACTORS.

On time completion of the mechanical, electrical and public health building services

installation will be achieved if three key issues are addressed:

- Skilled Human Resource is dedicated to the works with good project team

communication

- Material is available just in time to meet required production rates in line with

the construction programme

- Early liaison to develop final design solution, in the weeks programming stage,

to ensure adequate manufacturing and prefabrication periods are available. This

will facilitate a structured delivery and installation sequence to be set-up to

prevent “bottle-neck” in the construction process.

INSTALLATION SEQUENCING.

In accordance with the mutual agreed programme.

CONSTRUCTION.

Effective planning and co-operation are vital to achieving the construction

programme. We will set up a team as necessary but may include:

- A Project Manager

- Project Engineers

- Trade Foreman

- Supervisors

Each Team will be supported by a core group including:

- Design and engineering staff

- CAD Engineers

- Site wide Safety Team

- Planners

- Document Control and Secretarial Staff

- Commissioning Engineers

- Quantity Surveyors

Progress will be proactively monitored against the various programmes to identify

potential problems- and implement solutions- to maintain efficient progress.

The actual sequence of installations will be determined by our team in collaboration

with the Main Contractor and the Professional team, other Trade Contractors and the

master programme.

We are committed to improving Health and Safety and Welfare at work in the

Construction industry.

Our site teams have all been trained in safety on construction sites and the management

of safe working practices.

SITE LOGISTICS.

Our Engineers will organize delivery of plant and materials to site – in consultation

with the Main Contractor – to suit area/access availability as and when they can be

incorporated into the works.

Best possible practices will be employed and a specialist sub- contractor to deliver

large items of plant safely.

Good communication and notification methods will be essential in controlling all

deliveries, liaison with the construction manager will be developed to co-ordinate all

deliveries and off-loading.

PREFABRICATION / STANDARDISATION OF COMPONENT AND SUB

ASSEMBLIES.

We will prefabricate as much a of the services installation as practical in standardised

components to:

- Reduce to a minimum the need for site storage.

- Reduce our site labour force.

- Maximise the quality of finished works.

Prefabrication/Pre-assembly of Plant and Systems would include:

- Pipework for Plant rooms

- Pipework for Risers

- Repetitive pipework systems

- Ductwork pieces

- Common support bracketry systems

- Module construction and system arrangements

- Standardized equipment

All clearly labeled floor-by-floor and/or system-by-system.

F. Safety Regulations

Mighty Trading & Contracting regards the achievement of the highest standards in

matters of health and safety as mutual objectives for the company and its personnel.

In keeping with our general quality policy we have issued a separate health, safety and

environmental policy.

The company requires all of its employees to conduct their activities in such a way

that the health and safety of themselves and of all other that may be affected by their

actions are safeguarded. It is therefore the policy of the company, to take all reasonable

practicable measures to prevent personal injury to and avoid hazards to the health of

its personnel and other persons involved in or affected by those activities.

By these means we aim to maintain our position as a responsible contractor in

matters of the health & safety of our employees and protection of the environment.

Health and Safety program will be prepared for each project separately and is

available for submission on request.

G. Operation & Maintenance Documentation.

Mighty Trading & Contracting Co W.L.L. is fully appreciative of the importance of

providing O&M documentation and record drawings to coincide with the handover of

the project.

Depending on the size and how complicated the project is we would either prepare

them in-house or subcontract to a specialist organization. The process of accumulating

information and wiring diagrams specific to the plant and equipment to be installed on

the project is initiated at the time of order. At the same time the draft manual can be

prepared based on the specification and drawings and amended ongoing during the

project to reflect variations to the installation, omissions and additions.

A parallel exercise is carried out with preparing record drawings, a set of working

drawings are retained in isolation to the drawings used for installation, these drawings

reflect variations to the installation and as areas of the building are completed the

relevant drawings are amended and recorded as installed drawings. The process is continual throughout the life of the project with key dates identified

on the programme identifying latest information to be available from

suppliers/manufacturers, access to completed areas to record serial numbers and

information from plant and equipment, issue of draft manual for comments and/or

approval. The only section that cannot be incorporated until completion of the project

are the commissioning data sheets, we would liase closely with our commissioning of

the project and preparation of the results for incorporation into the manuals.

SAFETY, HEALTH & WELFARE POLICY STATEMENT

It is the policy of the Directors of the Company and its subsidiaries, that all operations

are carried out in such a way as to ensure, so far as it is reasonably practical, the Health

and Safety of all employees and all persons likely to be affected by the operations

including sub-contractors, licensees and the public where applicable.

This policy is the direct concern of the Managing Director, for its overall

implementation. The directors of the Company and its subsidiaries and divisions are

responsible for the application of this policy throughout the Company’s activities.

At every place of work, the person in charge is required to ensure that the Company’s

Safety procedures and statutory safety requirements are observed by employees and

all persons who are authorized to be in the premises under the control of the Company.

Employees shall, within the terms and conditions of their employment, comply with

appropriate safety procedures. Duly appointed Safety representatives will assist in the

application of safety procedures etc. in accordance with requirements of the Health

and Safety at Work Act.

The Management will keep under review the Company safety procedures to ensure

such measures continue to comply with all relevant statutory regulations and Code of

Practice etc. and are in accordance with this Policy.

1) INDIVIDUAL RESPONSIBILITIES

- SENIOR MANAGEMENT DIRECTORS, SENIOR, CONSTRUCTION &

PROJECT MANAGERS

It is the responsibility of Directors to establish systems and methods of working to

achieve the following.

1. The elimination of risk of injury and damage to health of all persons affected by

Company’s operation and prevention of waste and damage to all property and plant.

2. To provide and maintain safe plant, equipment and working conditions.

3. To ensure that all employees and others working on premises under the control of

the Company, observe safe working practices at all times and in accordance with

relevant statutory regulations, Codes of Practice, Guidance Notes or other

instruction to enable them to carry out their duties in a competent and safe manner.

5. To provide facilities for adequate co-operation in safety requirements and activities

between various departments of the Company and with sub-contractors, other

independent contractors, suppliers of materials and services etc, clients, local and

public authorities,

6. To provide in tenders and other preparatory procedures, for adequate safe working

methods and welfare facilities etc.

7. To achieve the observance of safety procedures, by personnel example and to

encourage all employees and other persons who are concerned or affected in any

way by operations of the Company to do the same.

- MIDLE MANAGEMENT (DEPARTMENT AND CONTRACT)

MANAGEMENT STAFF

1. Understand and comply the Safety Policy of the Company generally and in

particular ensure the cooperation between various departments within the Company

and others concerned to achieve the objectives of the Policy.

2. Determine during preliminary procedures and in advance in any work proceeding,

so far as possible, methods of working, access, lighting, known hazards, fire

precautions, allocation of responsibilities including sub-contractors and others.

Ensure that facilities for welfare and sanitation are considered, and provision for

adequate plant and equipment will be available for operations to proceed in a safe

manner,

3. Ensure that working methods and safety procedures are carried out in a competent

and planned manner and that all relevant statutory requirements are adhered to by

the provision of adequate plant, equipment, materials, services, etc. and operations

are supervised by competent personnel. Arrange as may be required by changing

circumstances, for the revision of working methods and safety procedures.

4. Arrange for instruction, training and the provision of information as required to

ensure that employees at all levels adopt safe working procedures.

5. Ensure that all persons authorized to be working on premises under the control of

company, adhere to safe working procedures and comply with all regulations

applicable to their work.

6. Ensure that all persons, including general public, within the proximity of premises

and operations under the control of the Company are not exposed to risks to their

7. Set personal example by the observance of safety procedures and to encourage

employees and other persons who are concerned with or affected by operations of

the Company to do the same.

SAFETY OFFICER

Duties of Safety Officers include the following

1. Advise management personnel and employees of the requirements and

responsibilities of the Company and individual responsibility and contribution

towards the application of Safety Health Welfare Policy of the Company.

2. Be aware of and keep up to date all relevant safety procedures etc., for reference

and to provide advice or make recommendations regarding their application.

3. Arrange for the conduct of workplace inspections to ensure that safe working

methods are maintained and regulations etc. are adhered to, with authority to

introduce alternative arrangements or improvements if considered necessary.

4. Maintain an adequate system of investigation, reporting and keeping of records of

accidents and dangerous occurrences as required by regulations.

5. Liaise with government Inspectors, Insurance Companies and other authorities as

required.

6. To encourage the observance of safety procedures by personnel example.

7. Obtain and maintain any specific items of specialist safety equipment that may be

required.

8. Assist in the production of safe system of work and procedures that may be required

for specific tasks or operations.

9. Issue work permits and checks that they are being used correctly.

10. Provide first aid training.

11. Perform safety seminars

12. Ensure security issues are implemented

- PROJECT MANAGERS, SITE MANAGERS, GENERAL FOREMAN,

MANAGEMENT SUPERVISORS

Under the Company Safety Policy, its Project Managers, Site Managers, General

Foremen and others in position of Supervision have the following responsibilities.

1. Establish and organize operations under their control to ensure that work is carried

out in a safe manner and to acceptable standards with minimum risk to all persons,

property equipment and materials.

2. Be aware of and observe the requirements of Health and Safety at Work Act,

Construction Regulations. Other relevant statutory requirements, Approved Codes

of Practice. Guidance Notes and Safety Procedures appropriate fo0r the operations

under their control.

3. Carry out instructions, at regular interval, of operations under their control with

particular reference to safety procedures and keep records of each inspection.

Ensure that statutory registers and records etc are completed accurately as required

by regulations and other instructions. |Arrange for any remedial or improvement

work to be carried out without delay.

4. To ensure at places of work under their control that employees and other authorized

to be at that place or in connection with it, receive adequate working instructions,

in particular to ensure arrangements for safe working, the prevention of accidents

and unnecessary risk are carried out,

5. Implement and maintain arrangements with Sub-contracts and other Employers to

ensure that they and their employees observe adequate safety procedures and

statutory regulations and to review any confusion about areas of responsibility.

Liaise with clients or other representatives to ensure the safety of any person

affected by the works or for security of the premises.

6. Plan and maintain safe access to and around places of work. Establish and maintain

a system of security to prevent, so far as is reasonably practical, damage, theft and

injury including periods when the work place is unattended.

7. To arrange for fixed and mobile plant and equipment to be positioned safely and to

ensure that all machinery etc. including power and hand tools are maintained in

good condition, is suitable for the work being carried out and operated only by

persons who are competent to do so in a safe manner.

8. To ensure that protective clothing is available where appropriate and that such

equipment is used and cared for in proper manner and as required by statutory

regulations.

9. To ensure that arrangement for first Aid are available and that the location of

equipment is known to employees. To ensure that proper care is taken of casualties

and to establish a procedure to be followed in the event of serious injury including

the means of obtaining medical and ambulance service. Liaise with Fire Brigade on

Fire prevention and rescue procedures, especially when hazardous situations are

foreseen.

10. Report all accidents involving injury to persons or damage to property and other

dangerous occurrences and near misses, to the Safety Advisor as soon as possible

after the occurrence. Establish the cause of all such incidents and thereafter carry

out improvements to prevent recurrence and instruct employees and other

accordingly.

11. Review requirements for training of supervisors and employees, including any

updating that may be required, to enable them to carry out their duties competently

and safely. Arrange for release of employees for appropriate training when

required.

12. Accompany Government Inspectors on site visits and act upon their

recommendations. Report all such visits to Safety Advisor nothing any

observations made, as soon as possible after a visit. In the event of a prohibition or

improvement notice being imposed or any indication that legal proceedings are to

be initiated, the safety Advisor and Senior Management must be advised without

delay.

13. Co-operate the Company and clients Safety Personnel and act upon their

recommendation.

14. To encourage the observance of safety procedures by personal example and

ensure that arrangements for the safety of persons and property are carried out,

including the implementation of disciplinary procedures.

15. A site safety supervisor shall be nominated. The Site Manager shall undertake

this appointment or other qualified and experienced person.

16.

- TRADES FOREMEN. GANGERS ETC.,

1. Be familiar with their obligations and those of employees under their control under

Health and Safety and Safety at Work Act, other Regulations and working

procedures etc, applicable to the work upon which they are engaged and insist that

all such requirements are observed.

2. Incorporate safety instruction in routine orders and see that they are obeyed. Ensure

any specific instructions or warnings regarding the safe use of materials and

equipment is observed.

3. Avoid the taking of unnecessary risks.

4. Ensure that new employees, particularly young persons, are instructed in proper

around them. Unsafe working by anyone is to be reported to the person in charge

of the operations.

6. Ensure that access to, from and around places of work is safe and clear of

unnecessary obstructions.

7. Ensure that working areas, plant, equipment and materials etc. are safe and secure

when left unattended.

8. Check that all materials, plant, tools and equipment are suitable, regularly

maintained and in satisfactory condition for the work upon which they are being

used.

9. Take appropriate action and report defects, foreseeable faults and hazards without

delay as they arise.

10. Report all accidents and dangerous occurrences to the person to whom they are

responsible.

11. Set personal example by observing safe working procedures during the course

of their work.

12. Apply for work permits when required and check they are used correctly.

2) S A F E T Y P L A N

Mighty Trading & Contracting commitment to Safety is total. Management,

Supervisors, and the individuals who comprise the work force of the organization have

a primary responsibility of effectively build each project in accordance with the

highest Safety and Health Standard.

➢ Sub-Contractors

Employees of sub-contractors, suppliers, and other third parties, while on Company

Property and in the performance of their work, shall comply with Safety and Health

Regulations established by local, State and Federal Agencies and all Safety Rules and

policies established by MTC. Any such employee found in violation of Safety Rules

or Policies will be removed from Company Property and barred from working on any

Mighty Trading & Contracting Project.Each Sub contractor, while working on this

project shall appoint safety representative. The safety representative may be a

superintendent or a Foreman whose safety responsibilities shall be as follows:

Instruct workmen under his supervision in Safe Work Practices and Work Methods

at the time workmen are given Work assignments.

Take immediate action to correct Unsafe Practices and or Conditions when

discovered.

Supply and enforce the use of Proper Protective Equipment and Suitable Tools for

his workmen.

Conduct Weekly Tool Box Meetings with his workmen to:

Discuss observed Unsafe Work Practices and Conditions.

Review the accident experience of his crew and discuss correction of the accident

causes.

Immediately notify Mighty Trading & Contracting of any existing hazardous

condition(s) that workman under his supervision may be exposed to that the Sub

contractor.

3) FIRST AID AND MEDICAL TREATMENT

Designate an employee who has a valid certificate in First Aid Training to render

First Aid at the Worksite.

1. To provide a First aid Kit to be maintained at each worksite in a weatherproof

container with individual sealed packages for each type of item. The contents of

First Aid Kit shall be checked at least weekly to ensure that the expended items are

replaced.

2. Major subcontractors shall provide a vehicle for prompt

3. Transportation of an injured employee to a physician or hospital.

4. In the event of an accident resulting injury

a. See that prompt First Aid is administered to injured employee(s)

b. Make a complete investigation of the accident to determine the facts necessary

to take corrective action to prevent recurrence.

c. Forward a copy of the Accident Personal Injury Report to Mighty Trading &

Contracting Project Manager, which shall include all State, Insurance and

Medical Reports.

The subcontractor’s Safety Representative/ Foreman shall review with their

workmen in Project Safety Rules for Employees prior to commencing Work on

the Project and / or at time of hire.

4) Health, Safety & Environment Management Plan

INTRODUCTION

This HSE plan has been prepared specially for the above Projects. This plan

outlines the precautions to protect people, property, and environment during the

construction of the projects. This plan is to be followed in the conjunction with the

requirements and Safety Plan of the Engineer.

PROJECT DESCRIPTION

The works shall be completed within duration stated in the contract and will involve

close coordination with any other contractors engaged to carry out associated works.

Sectional completion will be follow from approved critical path network for design,

procurement & construction.

The contents of the above documents shall implement where applicable for each

and every aspect of the work to be carried out.

The Project Manager shall establish an internal Project Safety meeting. Minutes

of these internal meeting shall be distributed to the following:

All MTC staff including engineers, Supervisors and all subcontractors will

attend these meetings, which shall be held on weekly basis. The Project Manager

or his designated representative shall chair these meetings.

• The Project Manager or his designated representative shall attend all client

safety meetings.

• All operation meetings when have safety, as Safety Manager to monitor the

safety items discussed shall attend an item on the agenda, and give advice where

necessary.

• SM shall perform a Safety Inspection of the work area on daily basis. A written

report shall be produced detailing which requirements need action.

• On site shall ensure follow up, to ensure required actions have been taken.

- Ensuring that no one is put at risk during the course of any operation.

- Ensuring that all incidents are reported immediately to their foreman

5) Accident Investigation and Reporting

All accidents, incidents, damages and near misses. The PM shall be responsible for

reporting any of the following incidents to the client’s Site representative verbally and

in writing, if requested. The report will include:

• All Fatal Accidents

• All Lost Time injuries

• Near Miss reports

Weekly safety reports containing statistics on the above incidents, Safety Manager

giving a brief description of the incident(s) injuries, etc. shall be produced by Safety

6) Emergency Plan

An emergency is defined for the purpose of this plan as an event or condition, which

has the potential of causing bodily injury to employees and/or damage to the project

facility and/or infrastructure.

The Emergency Response Plan of Owner will be adopted by Mighty Trading &

Contracting in the unplanned event of an emergency situation. If required and

suggested by Owner, Mighty Trading & Contracting will develop a separate

Emergency Response Plan in line with Owners Emergency Response Plan and submit

for approval of Owner.

All employees will be trained to the requirements of Emergency response plan like

❖ Access of escape routes and assembly points Process for raising the alarm

❖ Standing instructions for dealing with typical emergencies

❖ Training in use of firefighting equipment

Emergency Response Team

Emergency Response Team of Mighty Trading & Contracting comprises on plot

discipline Engineers and Supervisors, Off Plot Engineers, Supervisors and Safety

Manager. Project Manager will act as team Leader for the Emergency Response Team

and he will coordinate with the Owner in order to issue directions to the employees of

Mighty Trading & Contracting in case of declared emergency by Owner. The HSE

Manager will assist the Project Manager to carryout emergency action as per

Emergency Response Plan.

The names and contact numbers of the Emergency Response team will be clearly

displayed on the workers notice board as well as in the site office notice board.

7) Housekeeping

Housekeeping will be the responsibility of all the working groups and concerned

supervisors will be vested with the responsibility of maintaining the work areas clean. In order to maintain the good housekeeping, the following points will be stressed upon

1. Access to emergency equipment, Fire extinguishers, Safety showers, Eye wash

fountains and exits should never be blocked by anything.

2. All chemical containers must be labeled with the identity of the contents and the

hazards those present to users

3. Keep all work areas clear of clutter

4. Keep all aisles, hallways and stairs clear of clutter

5. All tools and tackles should be placed in their assigned storage areas at the end

of each workday

6. All work surfaces and floors should be cleaned regularly.

7. Good job site housekeeping promotes job-site safety.

Safety Personnel will carry out regular Housekeeping audit and it is the responsibility

of the construction team to comply with the audit report points. The following is a brief list of key Housekeeping points:

❖ All worksite locations shall be kept clean of excessive waste materials in order

to allow safe access and exit at all times. Each Foreman shall be responsible for

his area of work.

❖ At the end of each job and work shift, all debris shall be removed from area to

an area designated as dumping ground. The Supervisor shall be responsible for

ensuring all necessary facilities are available for safe removal.

❖ Sub-contractors who do not comply with good housekeeping rules will be

instructed in writing by Project Manager to suspend operation, until their area

of work is cleared to the satisfaction of Mighty Trading & Contracting.

❖ A copy of Mighty Trading & Contracting Safety instructions shall be displayed

conspicuously in Sub contractor’s shed.

8) Safety Inspection

o Safety Inspection shall be conducted on a daily basis. Any observation that can

be corrected immediately shall be implemented by informing the relevant

Supervisor.

o Written safety inspection shall be conducted once per week on the prescribe

form. The discipline Engineer shall sign for all observations applicable and

comply with recommended remedial action.

o Action rectifying the item identified on the report shall be the responsibility of

Senior Superintendent/ Supervisor.

o PSO shall follow up all action items until close put of inspection reports, which

shall be filled for any future audit

9) Safety Meetings

Monthly Safety Meeting will be held on site with staff/ Supervision. The Project

Manager, or his designated representative will chair the meeting. All sub-contractors

supervision shall attend this meeting.

The PSO will attend weekly progress meetings on site

The PSO will conduct Safety Training with Site Engineers and Supervisors regarding

toolbox talks and shall advise them to Safety problems.

Site Engineers, Supervisors shall ensure that toolbox talks are regularly conducted to

their workforce for every new task and every new personnel joining the running task.

10) Safety Signs

A notice board shall be erected on site as means of communicating Safety awareness

etc; PSO shall be responsible for ensuring that his action is taken.

Safety News Flashes and Bulletins, copies of procedure etc. shall be updated on the

board on a regular basis.

A statistics signboard will be erected on site, clearly showing:

• Man hours achieved

• Lost Time

• Accidents

• Near Misses

• Training

11) Health and Hygiene

All officers shall be regularly cleaned and maintained. The Project Manager shall

ensure that an office cleaning staff available to ensure that offices, site facilities are

kept in an acceptable standard.

Provision of good hygiene standard shall be ensured by PSO.

First Aid Facilities, Safety Kit shall be available at work site at all times

Adequate toilet facilities shall be made available to all employees.

12) Transportation

Suitable transport will be made available for personnel traveling to and from

site.

Transport shall be road worthy and have all necessary documentation required

by Qatar Law.

All drivers shall possess a valid Qatar1 Driving License.

The correct number of passengers shall be transported on the vehicle only.

13) Fire Prevention

Housekeeping includes fire prevention and protection. All areas shall be kept neat and

clear of combustible and flammable materials. These areas shall be regularly inspected

and ensures that they are free from fire hazards.

Access

❑ Exits will be provided as adequate for the number of personnel working in the

area and the size of the project/site. ❑ Exits will be clearly marked with adequate signs, which can be illuminated

after dark. ❑ Work areas and aisles shall be kept clean and free of unnecessary

materials/waste. Work areas, for each piece of equipment, shall be kept clean and orderly at all times.

❑ A minimum of 1 meter (3 feet) of free space shall be maintained around each piece of equipment.

❑ Personnel will not be permitted behind equipment (i.e., power brakes) without the operator's knowledge and until the equipment has been shut down.

❑ A minimum of 1 meter (3 feet) of aisle space shall be maintained in all shop areas.

❑ Ladders will be appropriate, inspected and tagged. Fixed ladders shall comply with applicable codes.

Electrical

❑ Equipment installed in a designated area for an indefinite period of time shall

have its power source provided in approved conduit. Portable cords shall not be used.

❑ Temporary cords, leads, and hoses will not be placed on the floor or in walkways. All temporary cords, leads, and hoses must be secured along the walls.

❑ Equipment will be grounded and all electrical sources tested semiannually at non-construction facilities or monthly at construction facility to comply with Assured Equipment Grounding Conductor (AEGC) requirements.

❑ A disconnect will be provided within the immediate work area for each piece of equipment. This disconnect shall be labeled as the "Emergency Stop" and shall identify which item of equipment it shuts down.

❑ All distribution panels will be labeled showing the item each breaker serves. Material Storage

❑ Stock material (i.e., pipe, all-thread, angle iron, etc.) will be secured from

shifting or falling while stored. Round stock shall be blocked to prevent rolling. ❑ Materials identified as hazardous, including, but not limited to, paints, thinners,

cleaners, coatings, and adhesives shall be stored in approved storage cabinets in compliance with Occupational Safety and Health Administration (OSHA) 1926.152.

❑ Hazardous materials that are used in the workplace will be stored in approved safety cans. Only a one (1)-day supply shall be permitted in the work area outside of an approved storage cabinet.

❑ Compressed gases shall be stored outside in a designated area. ❑ Ceilings of rooms or elevated platforms used for storing of material will be

posted as to the maximum load limit that may be imposed on the area.

Lighting and Ventilation

❑ Lighting for shop areas shall meet the applicable levels. ❑ Overhead lights below ten (10) feet (3 meters) above grade will be protected

from physical damage by guards or other acceptable means. ❑ Ventilation will be adequate to remove vapors, dusts, fumes, etc., from the

workplace. ❑ Dust producing equipment (i.e., plainer, sanders, saws, etc.) will be equipped

with Local Exhaust Ventilation (LEV) to remove the dust. Fire Protection

❑ Appropriate portable fire extinguishers will be provided in accordance with OSHA and Owner requirements.

❑ Combustible materials will not be permitted to accumulate and create a fire hazard.

❑ Temporary heating devices meeting in-country standards will be approved for each specific work area prior to use.

Compressed Gases

Compressed air used for cleaning will not exceed thirty (30) psi or equivalent in-

country measurement gauge. Office Equipment ❑ Will be placed without causing any obstruction in hallways. Will not block

entrances or exits. ❑ Electrical cords will not cause tripping hazards. Electrical cords will not be frayed. ❑ Electrical equipment will be grounded. ❑ Workspaces will be adequate and clear of debris and obstruction. Equipment on

desks and tables will be sturdy and secure. ❑ Equipment will be inspected monthly and records kept.

1. Fire extinguishers must be:

• Where flammable materials are stored, handled or use. • Where temporary oil or gas fired equipment is used. • Where welding and open place cutting is being done. • Any other place considered necessary

No Smoking signs displayed wherever flammables are stored and potential fire

hazard areas.

14) Scaffolding

This procedure provides guidance to construct and safe use of scaffoldings. All

scaffolding works are performed on an approved Elevated Work Permit and Scaffold

Permit signed by the construction manager, scaffold superintendent and safety. Scaffoldings are safe, inspected weekly by a competent person and that the scaffolds

are properly tagged. The company will not allow any person to use a scaffold that is

not inspected and certified to be safe. The company will employ competent scaffold builders to erect scaffolds and

competent scaffold inspectors, a scaffolding supervisor / superintendent to oversee

overall scaffolding work. The scaffolding engineer will be responsible for the design

and approval of all specialty scaffolds and scaffolds above 12 meters in height and the

inspection thereof. Personal Fall Arrest System

A system used to arrest a person's fall. It consists of an anchorage, connectors, or body

harness and may include a lanyard, deceleration device, lifeline, or combination of

these. Open Sides and Ends The edges of a platform that are more than 36 centimeters (14 inches) away

horizontally from a sturdy, continuous, vertical surface (such as a building wall) or a

sturdy, continuous horizontal surface (such as a floor), or a point of access. Exception:

For plastering and lathing operations, the horizontal threshold distance is 46

centimeters (18 inches). Guardrail System A vertical barrier, consisting of, but not limited to, top rails, mid rails, and posts,

erected to prevent employees from falling off a scaffold platform or walkway to lower

levels.

15) Safety Incentives

A comprehensive safety promotion program will be in place in order to instill safety

culture amongst the work forces. The following are the safety promotion schemes

which will be conducted by Mighty Trading & Contracting under the responsibility of

Safety Manager.

A. Safety Training B. Safe Employee of the Month C. Safety Slogan contest D. Safety Quiz contest E. Safety Suggestion contest F. Best Near Miss Reporting

All the contests will carry cash awards and a Merit certificates will be distributed to

the winning employees during the monthly safety meetings.

16) Safety Work Instructions for Employees

All employees have a Safety Responsibility to themselves and fellow workers around

them. These Safety Rules apply to all employees on all jobs. Your Superintendent or

Fireman may establish special additional Rules.

Report unsafe conditions or unsafe acts to your Foreman for correction.

Report all injuries regardless of how slight to your Foreman. The Superintendent

must be notified before any employee sees doctor concerning a Job related injury

except in extreme emergency.

Horseplay in the job site is prohibited

Everyone on the job shall wear Hard Hats at all times except when otherwise

instructed by Superintendent.

Safety shoes are obligatory. Sturdy, Heavy-duty Work shoes required. Canvas and

loafer type are not being worn.

Work gloves shall be worn when handling rough edges or abrasive material when

the work subjects hands to laceration, puncturing or burns. The Superintendents or

Foreman may designate other hand protection.

Safety Goggles shall be worn when sledging, pouring concrete, hammering and

sawing on metal or concrete, chipping, welding, burning, cutting, grinding, working

in dusty places, handling of acids, preening, cleaning walls or other operations

where eye injuries may result. Don’t watch welding without proper eye protection.

Ear protection in form of earmuffs or approved earplugs must be worn on all high-

noise level jobs as directed. Cotton or waste will not be used as earplugs.

Employees working around moving equipment are required to wear safe clothing.

Employees are also cautioned about the danger of loose clothing, rings, bracelets

and jewelry around moving equipment. Shirts are to be worn by all employees at

all times.

Use of Gasoline is prohibited for cleaning equipment or tools or for starting fires.

Small quantities of gasoline may be transported only in approved safety containers.

Gasoline engines must be shut off when refueling.

Unsafe Tools, defective or prayed electrical cords and unguarded machinery should

be reported to your foreman.

No Smoking rules must be observed in posted areas.

Tampering with unauthorized removal of Fire extinguishers from assigned

locations is prohibited.

Riding of any construction equipment except by operator or maintenance personnel

is prohibited.

Getting on or off a vehicle or equipment while in motion is prohibited.

The operation of any Company equipment without proper authorization is

prohibited.

Seat belts shall be worn in all moving vehicles when so equipped.

Cranes, Backhoes or other equipment with booms must be operated with caution

around power lines. Consult your Foreman for safe operation procedures.

No employees shall work under lifted loads. Equipment operators shall avoid

carrying load over employees.

No employee shall operate any machinery, equipment or tool unless he has been

properly instructed in its use and thoroughly familiar with all details of its

operation.

Defective or unsecured ladders should be reported to the foreman.

Do not go up or down a ladder without the free use of both hands. If material or

tools have to be handled, use a rope to lift or lower them. Always face ladder at the

landing. Never use a stepladder as straight ladder. Never stand on the top step of a

stepladder.

All machine guards shall be kept in place while machinery is in operation.

Tampering with Machine guard is prohibited and any removal enquires the poor

approval of a responsible supervisor. All guards are promptly replaced after the

repair work that necessitated the removal has been completed.

. Hand tools shall not be used for any other purpose than intended. All damaged

tools or worn parts should be reported to the Foreman for replacement or repair.

Tools and other objects should not be left on scaffolds, ladders or overhead working

surfaces.

Electric Power Operated tools shall be properly grounded before put into operation

Know the correct way to lift Heavy Objects and get help if needed(Secure footing,

firm grip, back straight, lift with legs)

Acetylene, Oxygen or other Gas Cylinders are to be stored in upright position,

secured by tying or blocking into position.

No employee shall remove a cover or alter a temporary guardrail, handrail or floor

opening cover without specific authority.

Employees are not permitted to use or possess any intoxicants or drugs on Company

property or to be under the influence of any intoxicants or drugs while on the job.

Any employee found intoxicated or under the influence of drugs while on duty shall

be terminated immediately.

Tools, equipment, Machinery and work areas are to be maintained in clean and safe

manner.

Nails are to be immediately removed from disassembled lumber as the lumber is

disassembled.

No employee shall work on scaffolding higher than ten feet without proper

guardrails, toe boards and proper flooring except when proper precautions, such as

lifelines with harnesses, barricades, etc, have otherwise been made. Scaffold

platforms should be at least forty eight-inches (48’’) wide when possible. All

scaffolding is to be cross-braced on both sides at every stage .All scaffold structure

and brace members to be straight and without defects.

Unstable objects such, as barrels, boxes, loose bricks or concrete blocks shall not

be used to support scaffolding or planks.

Explosives and detonators are to be handled by authorized employees. Special

precautions are necessary and only experienced personnel are to be utilized.

Good housekeeping practices are required of all employees.

Common sense, health and sanitation rules must be observed for the welfare and

consideration o of other employees.

Glass bottles are not permitted on the jobsite.

All employees are required to remain on the Site during working hours. No

employees are permitted to leave the Site except to pursue company business.

Any questions regarding Safety or Safety Equipment should be directed to

Supervision

Willful violation of these or other Safety Rules of the Project will be cause for

dismissal.

17) Illegal Drugs, Substances And Probited Items

To help insure a Safe, Healthy, Efficient and Productive Working Environment,

Mighty Trading & Contracting, (hereafter referred to as “Company”) has adopted a

policy to prohibit the use, possession, sale, distribution, concealment and

transportation of any prohibited item and substance on Company Property as defined

in the attached Drug, Alcohol and other prohibited Items Policy. The Policy

enforcement program involves reasonable searchers of all persons, their personal

effects, including vehicles, while working or present on Company Property. These

searches may include the use of scent-trained dogs and taking of urine and or blood

samples for drug testing and analysis.

A copy said Policy is attached and set forth “Company Property” items and substances,

which are prohibited and are covered by said Policy, penalties for violation of said

Policy, involvement of law enforcement authorities, and the possibilities of your

employees while on Company Property.

As Sub-contractors/Supplier Employer, you will be immediately notified of refusal by

any of your employees to consent to a search or a urine and or blood-screening test, or

to cooperate fully with this Policy. Should any of your employees be found in violation

of this Policy, they will be subject to immediate removal from Company Property.

Violation of this Policy by your employees may also cause cancellation of the Contract

between the Companies, and may result in your losing the right to do business with

the Company.

Sub-contractors/Suppliers are to control this item. You should inform your employees

that this Policy is in effect. New employees should be informed of this Policy at time

of Hire.

Policy

I order to assist in maintaining a Safe, Healthy Working Environment for all

employees, to protect Company Property and assets, and to insure efficient operations,

Company has established a Policy regarding illegal drugs, substances and prohibited

items.

Company Property/ Premises

For purposes of this Policy, the terms Company Property or Company Premises are

used in their broadest sense and include all property, facilities, land, and offices, living

quarters, buildings, structures, fixtures, installations, trailers, equipment, boats,

vessels, barges, aircraft, automobiles, trucks, all others vehicles, and parking areas,

whether owned, leased, used or under the control of a customer, or to and from those

locations while in the course and scope of Company Employment.

Prohibited Items

The use of, possession, promotion or sale of the following items or substances is

strictly prohibited item and grounds for dismissal.

1. Illegal Drugs and or Controlled Substances

2. Unauthorized Alcoholic beverages

3. Firearms, Weapons, Explosives and Munitions

4. Unauthorized Prescription Drugs.

Prescription Drugs May be taken under the following Conditions

1. Employees must inform their Supervisor immediately upon arrival at the Project

Site and prior to using Prohibited Drugs on the Job.

2. Each vial shall be in the employee’s Name.

3. Each prescription shall be no older than one (1) year of the date issued.

The Company reserves the right to have its Company reserves the right to have its

company Physician determine if a prescription drug or medication accordingly. This

may also include restricting the Employee’s Work Activity.

Searches and Inspections

Mighty Trading & Contracting reserves the right, at all times, to have Company

Supervisors or authorized Search and Inspection Specialists conduct searches and

inspection of employees, Subcontractor’s employees, their person’ clothing, vehicle,

toolboxes and possessions, for the purpose of determining if such employees or

Subcontractor’s employees are in possession, use or concealment of any of the items

and/or substances prohibited by this Policy.

The Company Supervisor has the right to conduct on the spot search and inspection of

employees and their personal effects as described above if he has a “Reasonable

Suspicion” that all employees are in direct violation of any part of this Policy.

All searches and inspections conducted by outside authorized specialists will be in the

presence of the Company Supervision.

Searches; inspections and testing may be conducted from time to time without prior

announcement. Searches will be performed with concern for personal privacy of each

employee.

No employee search, urine drug screen or inspection will be conducted without written

consent, however, employees who refuse to submit to search, urine drug screen, blood

or plasma sampling, inspection or is found possession, use or transportation of any

illegal substance’ contraband’ Company property; or any of the above mentioned

drugs and unauthorized items, will be considered in violation of this Policy.

Illegal substances, drugs, and unauthorized items discovered through searches and

inspections may result in the proper law enforcement authorities being advised in this

regard.

Testing

Urine drug screen test or blood and plasma tests may be conducted under the

following circumstances:

1. Pre-employment examination

2. Annual employee physical examinations

3. Compliance with contractual Agreements.

4. When employee’s Supervisor has a reasonable suspicion” that an employee is

intoxicated, using or under the influence of drugs and alcohol.

5. Part of an overall search and inspection of an employee’s or other person’s work

area or location of employment.

6. Whenever an employee has caused, contributed to, or has been involved in an

accident while at work.

Penalties for Violating Policy

Any employee found using possessing, selling, distributing, concealing or transporting

any of these items or substances prohibited by this Policy or who refuses to submit to

a search, Urine and/or blood analysis, or other detection procedure will be considered

in violation of this Policy’ will be removed from Company Property and will be

discharges from employment.

Any employee who’ as a result of drug testing and screening; is found to have

detectable levels or identifiable trace quantities of a Prohibited drug or Substance in

his or her system, regardless of when or where the Drug or Substance entered that

person’s system, will be considered in violation of this Policy, and will removed from

Company Property, and will be discharged from employment on the Project.

All Sub contractors and supplier Personnel and other third parties on the Company

Property will be subject to this Policy. Any such individual found in violation of this

Policy’ or who refuses to submit to search, urine and/ or blood analysis; or other

detection procedure will be considered in violation of this Policy, and will be removed

from Company Property. Violation of this Policy by Sub Contractor or Supplier

Employees may result in the Contractor or Supplier losing the right to do business with

the Company.

For the Purposes of this Policy

• “Reasonable Suspicion” is a belief based on objective facts capable of articulation

sufficient to lead a prudent Supervisor to suspect that the employee, or other person,

is using a Prohibited Drug or Alcohol Substance.

• Under the influence” is the presence of an illegal or Prohibited Drug. Alcohol or

Substances found in the body fluids at levels of detection above the lowest cut-off

level as established by analytical methods used by the testing laboratory. Also, it

means that the employee, or other individual’ is affected by a Drug Alcohol or

Prohibited Substance, either singularly or in combination, in any detectable

manner. The symptoms of influence are not confined to those consistent with

misbehavior; or to obvious impairment of physical or mental ability, such as slurred

speech or difficulty in maintaining balance.

18) Orderliness

1. Keep work area clean- remove / stack rubbish.

2. Do not allow soiled clothes, foot scraps accumulate.

3. Keep tools and working materials in proper containers.

4. Store trash, waste and scrap in proper containers.

5. Keep walkways clear.

6. Do not leave Aerosol cans unattended. Store or contains material so that fire has no

place to start

7. Clean up tools and work areas as your job progresses.

8. Keep electrical cords and hoses 7 feet overhead or lay them flat out of walkways.

9. Keep all materials, tools and equipment in stable position (tied, stacked or chocked)

to prevent rolling or falling.

10. Maintain clear of debris.

11. Keep stairways clear of debris.

12. Clean up liquid spills right away.

13. Toilet and washing facilities and drinking area should always be kept clean. Do

19) Access

1. Walkways and stairways must be clear, ladder must not be blocked and emergency

exits are identified and clear.

2. Do not block any emergency equipment or electrical disconnected switch / box.

3. Stack, store or spot material so workers or material handling equipment can reach

that it readily

20) Ladders

1. Ladders must be constructed of sound material and in good condition.

2. Broken or damaged ladders must not be used. Repair them or destroy them

immediately.

3. Only Custom-made ladders are permitted- no site made specials.

4. The base of the ladders must not be on a firm footing and raked at an angle of

approximately 1 to 4.

5. Ladders for access to a floor or scaffold must extend at least 3’6” (1.06 meters)

above the landing.

6. Ladders must be tied, blocked or otherwise secured to prevent them from being

displaced.

7. Do not over-each when working off a ladder.

8. Painted ladders should not be used-paint hide defects.

21) Crane Operators

1. All cranes, hoist etc, must be operated and maintained to comply with established

and mandatory standards.

2. All cranes and lifting gear must have current test certificates.

3. Only authorized and suitably trained persons shall act as banks men and shall use

the hand signals prescribed by British Standards.

4. Know the current color code for the use of slings etc.

5. Only authorized and suitably trained personnel shall operate 2-way radios.

6. Only trained crane operators are allowed to operate the cranes.

7. Hoist, travel and S.L.I. are to be tested at the start of each shift.

8. During lifts the crane operator must not allow anyone to ride the load, hook chains.

9. Watch for correct rigging of loads and know correct weight of load.

10. Loads to be correctly balanced prior to lift.

11. Ensure you are aware of all / any obstacles within lifting radius and check before

everyone movement.

12. Never use the point of the hook for lifting.

13. Banks men to ensure at all times, that areas are clear and workers are aware of

loads being lifted near them.

22) Electrical Safety

1. All electrical tools and equipment must be adequately earthed.

2. Employee must not work on, or proximity of unprotected live circuits unless

adequate safety measure have been taken.(Permit to work)

3. Only authorized employees are allowed to work on or repair electrical equipment.

4. Portable electric tools and extension leads must be lifted with proper plugs and

sockets and must never be connected by inserting bare wires into the socket.

5. Electric leads must be preferable hung in safe positions, not lying on the ground

and especially not in water.

23) Excavation

The sides of all excavations must be adequately supported or battered back to afford

safe working.

Almost any excavated ground will collapse, under certain conditions, unless it is

temporarily supported, or is safely battered back or of stable rock.

If the sides of an excavation collapse, there may be no escape-death by crushing, or

suffocation.

Follow these basic guidelines:

a. Do not enter an excavation unless it has been inspected and is considered safe to do

so by your foreman.

b. Make sure there is safe access into excavation. Adequate ladders must be available

before anyone enters a trench more than 4’0” deep.

c. Always wear your safety helmet.

d. Never interfere with any of the support systems.

e. Do not stand on bracing struts.

f. If dumpers or Lories are used to tip back-filling materials into trenches, make sure

‘stop charges’ are placed and secured.

g. Keep spoil heaps and materials at least 1 meter from excavation edge.

24) Hand Operated Power Tools

1. Completed personnel shall only use power tools.

2. All portable electric tools must be earthed, have guards in place and only be used

by qualified employees.

3. Inspect tools prior to use to ensure they are in good safe working order- if not-

returns to stores / management.

4. Do not remove any guards- they are for your protection.

5. Do not place tools so that they may fall or cause a tripping hazard.

6. Power tools should be hoisted or lowered by hand line never by the cord hose.

25) Power Actuated Tools/Cartridge Tools.

1. Only certified employees are permitted to use this type of foot. A certified person

is someone who has been formerly instructed and trained by the manufacturer or

manufacturer representatives.

2. Cartridge tools must be dismantled and examined for defects once in every seven

days of use.

3. Cartridge tools should not be used unless fitted with a guard or shield suitable for

the work.

4. All unused cartridges must be returned to the stores/ General Foreman.

5. Adequate eye, ear face protection must be used by the operator.

6. Tools must be designed so that it will not fire if the muzzle is not in firm contact

with the surface to be work on.

7. Shield or guard must be used to confine flying particles and prevent escape of

ricocheting studs and pins.

8. Tool must be provided with safety lock designed to Fall Safe, that is if the safety

lock is broken worn or damaged the tool will not fire.

9. Cartridges or shells must be kept in the original containers or in the carrying case

provided with the tool.

10. Each tool should be tested each day before loading to see that all safety devices

are in proper working condition.

11. Tool should be loaded just prior to use. Never leave a loaded tool unattended.

12. Explosive activated tools should be handled like firearms with hands clear of

muzzle and barrier pointed away from all persons.

13. In case of “MISFIRES” continue to hold the muzzle on the tool firmly against

the surface no less than 30 seconds. Care must then be taken to keep the muzzle

away from persons while the barrier is removed or when gun is opened to eject the

cartridge.

14. Never store the tool loaded.

15. Materials that are easily penetrated should be backed by a substance that will

prevent the pin or and stud from passing completely through.

16. asteners must not be driven into brick or concrete closer than 3” from the edge

or corner or into steel surfaces closer than ½” from the edge or corner unless a

specials guard jig or fixture is used to avoid flying particles to escape.

17. Never use a power impact tool unless the retainer ring or pin is in place and the

tool or plunger is against a sound object to prevent it from being thrown out.

26) Burning

1. Do not use matches to light torches. Spark igniters must be used. Torches must not

be used to light smoking materials.

2. You must wear the appropriate personal protective equipment.

3. When a key is required to open / close the acetylene valve the key must be kept in

position on the valve.

4. Store and transport cylinders in the correct approved manner – secure them on a

bottle cradle – secure all cylinders, empty and full in the vertical position with a

valve.

5. Open the cylinder valve only when work is being done. Close the valve when not

in use. Flash back arrestors must be used.

6. Locate gas bottle outside buildings or confined areas.

27) Working In Confined Spaces.

1. Never work alone in any confined space, and always have a watchman standing by

the entrance to the confined space.

2. When working in a closed or confined space, proper and adequate ventilation

MUST provide.

3. Make sure all necessary equipment is available before starting work, which must

be used by all persons entering the confined space.

➢ Circumstances requiring use.

Safety belts, safety harnesses and lanyards are required to be worn and used in the

following circumstances.

1. On all stages, floats and any other type of suspended scaffolding.

2. On all scaffolds with incomplete decking or incomplete guardrail.

3. On sloping ropes

4. Within six feet of the edge of floors or roofs where there are no guardrails or wire

rope railing.

5. In any unprotected elevated location six feet in height or more.

6. In precarious positions at any elevation (i.e. less than six feet high leaning or off

balance)

7. In areas exposed to protruding, unprotected, reinforcing steel at any height.

➢ Manner of Use

In circumstances requiring the use of safety belts, full body harnesses and lanyards

they must be used in the following manner.

1. The lanyard shall be fastened to the safety belt/ full body harness and secured to

substantial objects; if possible these objects should be overhead.

2. The lanyard shall not allow a fall substance of more than five feet and shall be

secured above the user.

3. The employees required to wear a full body harness shall be trained in how to

properly wear the full body harness.

4. Lanyard and safety belts shall not be used for any other purpose.

➢ Inspection

Prior to each use, a visual inspection of the safety belt, harness, lifeline and lanyard

shall be made by the employee who will be wearing and using the equipment.

28) Concrete Operation

A. General Requirements

1. No construction loads will be placed on a concrete structure of portion of a concrete

structure unless based on information received from qualified person that the

structure or portion of the structure is capable of supporting the loads.

2. All protruding reinforcing steel onto and into which employees could fall should

be guarded to eliminate the hazard of impalement.

3. No employee shall be permitted to ride concrete buckets, nor work under concrete

buckets while it is being elevated or lowered into position. Employees will be

required to wear proper clothing, boots, gloves, hardhat and safety glasses to

prevent cement burns. Employees applying cement, sand and water mixtures

through a pneumatic hose will wear face protection in addition to safety glasses.

4. No employee shall be permitted to place or tie reinforcing steel more than 2 meters

above any adjacent working surface unless the employee is protected by the use of

safety belt or equivalent fall protection.

B. Equipment Tools

1. Storage bins, silos and containers must be equipped with conical or tapered bottoms

and have mechanical or pneumatic control to pour the material.

2. Power and rotating type concrete trowel machines that are manually guided shall

be equipped with control switch that will automatically shut off the power

whenever the hands of the operator are removed from the equipment handles.

3. Positive safety latches or similar safety devices shall be installed on all hydraulic

or pneumatic gates of concrete buckets to prevent premature or accidental dumping.

Buckets will be suspended from shackles or approved safety type hooks.

29) Discipline

It is the duty and responsibility of the construction group and field safety team to detect

safety violations and unsafe situations and provide the necessary correction. Whenever

a safety violation or an unsafe situation is detected, the appropriate party will carry out

the required corrective measures upon simple verbal request. In the case of situations implying immediate accident hazard, work will be stopped

until the hazard is eliminated. The Safety Manager shall keep a constant check on the methods used by supervisors

and employees to prevent safety violations. Should Safety Manager / any authority

witness an unsafe work situation, which requires the application of the project's

disciplinary program, it must be brought to the attention of the appropriate project

supervisor. The supervisor is responsible for carrying out the necessary corrective

measures. The Safety Manager may advise or recommend the future training or type of reprimand

or discipline to be applied, except for the serious safety violations requiring immediate

dismissal/termination from the project as stated below. The Safety Violation Notice is to be used by Supervision to identify observed safety

violations, to record the observation and to indicate appropriate correction. The employees of Mighty Trading & Contracting, who are found violating the safety

rules and procedures, may be subjected to the following disciplinary actions.
