I have been authorized by the school listed below to sign for and accept responsibility for all above listed General Admission Adult Pre-Sale Tickets ("Tickets"). I verify that all information is correct. I have inspected and received all 'Tickets', all of which were sequentially numbered and none were missing or damaged. I understand that each and every 'Tickets' shall be sold for Pre-Sale Ticket Price listed above.

I understand that each 'Ticket' that is returned to the above listed Alumni Football USA® representative ("AFUSA Rep") by the deadline listed above will be deemed “Unsold” and each 'Ticket' that is not returned to the 'AFUSA Rep' by the deadline listed above will be deemed “Sold.” I understand that I must provide payment in the amount of for each "Sold" 'Ticket' to the 'AFUSA Rep' no later than the deadline listed above. I understand that acceptable forms of payment are cash or check and there will be an additional fee of $25.00 for each returned check. If Alumni Football USA® has to pursue legal action to acquire payment, the school and/or party listed below will be responsible for any and all attorney’s fees incurred. Jurisdiction for any and all legal action shall take place in Sonoma County, CA.
