Pre-Operative Documentation: Your Checklist
Obtain a “Medical Narrative” from your Primary Care Provider. The narrative should include a summary of your medical history as well as provide a list of current medications and dosages.
If you are under the care of any medical specialists, have them write a letter addressing your
readiness for surgery.
Complete the enclosed Pre-Operative Patient Questionnaire.
Complete the enclosed Health Care Proxy form.
Upload your radiology images Visit nebh.org/imageupload and follow the onscreen instructions.

Sample Itinerary for Hip and Knee Replacement Patients
Who are Flying to Boston
This is a sample schedule to illustrate a typical patient’s experience when traveling from out of state to participate in the program. A
personalized schedule will be developed for each patient, taking into account travel requirements and medical needs.
Day 6 or 7: Departure from
Boston
Call with nurse practitioner or

Before You Leave for Boston: What You Need to Know
Preparing Your Home for After Your Surgery
In the first few weeks after surgery, everything in your daily routine will require adjustment. It will take
concentrated attention and practice to learn how to safely get out of bed or out of a chair. Going up and down
stairs, showering, dressing, and preparing meals will be difficult at first. To prepare for your return home, there
are a few things you can do before you leave for Boston.
Place clean sheets on your bed.
Reduce clutter. Your balance and mobility will be limited for a number of weeks, so make it easier to get
around by getting clutter out of the way.
Remove loose wires and cords. If it can trip you up, it shouldn’t be there.
Prepare and freeze meals. For you and your caretakers, the demands will be significant. Pre-made,
pre-frozen meals can be a big help.
You will need to turn on lights when you get up at night. Walking around in a dark room is too risky for
someone who has just had surgery, so make sure you have lighting that is easily accessible.
Ensure your pets will be cared for. Walking and feeding them may be difficult when you first return
home from surgery.
What to Bring with You to the Hospital
Make sure to bring the following items with you.
If you are under the care of any medical specialists, have them write a letter addressing your readiness

Bring all prescription medications you take, in their original container with prescription label. Bring all
other medications, including vitamins, herbal supplements, and over the counter medications. You will
need these for your pre-admission screening appointment.
Health insurance card(s)
Photo ID (license, passport, or government ID)
Cash, check, or credit card for copayments or deductibles, if applicable
Your Healthcare Proxy form
Personal hygiene products – toothbrush, comb, etc.
Glasses, hearing aids with a case
Reading material
Candy, gum - your mouth can feel dry following surgery
