Protocol for Placement in Transition

Transition Years

Preparing students with disabilities for the transition

from school to the adult world.

By age 16, every student will have an Individualized

Transition Plan (ITP) along with his/her Individualized

Education Plan (IEP).

Instruction for the ITP goals is designed to support the

student’s transition from the public school system to post-

secondary adult life. This instruction can occur in various

environments including classroom, community, or both;

determined with input from the entire IEP team (including student, family, teacher(s) and

program specialist).

Review Transition Brochure

Case Manager completes transition questionnaire

Review of Records/Units

Obtain input from a transition teacher

Parent/Adult student completes transition questionnaire

Contact Program Specialist to schedule a team meeting

Community Transition Class Locations

Eligibility CriteriaFor Program Options B and C

Age 18+ by the start of the new school year

Attendance in Good Standing

At least 2 out of 4 years of some transition instruction in high school

Review of Records/Units

Anticipated Certificate of Completion,

not HS Diploma

Input from IEP team and authorization from Program Specialist is needed for a change of placement outside of the high school setting

See your current high school for information.

Held in a classroom just outside of the main Burbank campus.

A. Staying in the current HS Settinga. Redesign the program to

meet the student’s transition goals.

b. Primary service on enclosed campus

c. Work experience through the WorkAbility program.

B. Community Transition Classa. Majority of instruction

occurs in communityb. Program is likely to include

vocational training, bus/light rail training, independent living skills, social and recreational skills, and safety and abuse awareness lessons.

C. Functional Transition Academic Class

D. Transition Partnership Program (TPP)

a. Based on referral & application

b. Department of Rehabilitation eligible

c. Student is anticipated to receive a HS Diploma
