QHSE-10-574Provision of Medical Staff for Company Clinics at HAS and Qalhat ComplexGUIDELINESResponse to this questionnaire shall be completed in English by a senior member of your company. The requirements of each question shall be addressed comprehensively, with all supporting documentation duly provided, enabling Oman LNG to evaluate thoroughly against the requirements for this Service.

Oman LNG reserves the right to verify any and all information contained in responses to this form. Oman LNG shall be advised immediately should there be any significant change in the information provided in this document. It should be noted that this document is solely for the Pre-qualification evaluation of the company, for the purposes of inclusion on Oman LNG Tenderers Lists for the stipulated requirements. Submission does not guarantee that the contractor will be invited to Tender.1.0Introduction

Oman LNG LLC (hereinafter referred to as the COMPANY) intends shortly to issue a tender for operating and managing Hay Al Shrooq (HAS) Clinic and Qalhat site Clinic. The Company wishes to pre-qualify potential Contractors, Sur based, capable of managing and executing this service, and hereby invites you to submit pre-qualification documentation as required. Further details are given below.1.1Background

Oman LNG owns an equipped clinic at Hay Al Shrooq, Willayat Sur and provides a primary health care service to approximately 400 employees and their dependants.

2.0 Brief Description of Scope

The CONTRACTOR shall:

a)Provide Female General Practitioner and female staff nurse for the medical services required at COMPANY Hay Al Shrooq Clinic and for Male Practitioner and staff nurse for site Clinic on Call- Off basis.

- The working hours for Female Doctor and female staff nurse for HAS clinic will be from 8:00 till 17:00 Saturday to Wednesday and on Thursday between 08:00 to 12: 00. The female doctor will be on call after working hours for HAS clinic.

- For Site clinic the Male Doctor and Female/Male staff nurse will be on call Off basis during working hours from 7:30 till 16:15 from Saturday till Thursday or as and when required by COMPANY. In case of absence of OLNG doctor / staff nurse the coverage should be provided within maximum of 24 hours for a period determined by the COMPANY.

b) The clinic/ hospital should have a minimum of 4 medical practitioner (2 male and 2 female) and 4 staff nurses to ensure continuity of services within the COMPANY.

c) The minimum requirement for the requested medical staff is a qualification from a recognised medical University with minimum of 5 years postgraduate experience of independent practice.

d) The clinic/ hospital must be licensed to practice medicine by the MOH in Sultanate of Oman.3.0Objective of Pre-Qualification

The objective of this exercise is to invite and identify potential capable service providers who can provide this service.Tenderers shall be pre-qualified based on information submitted that demonstrate capability in the performance and management of requested service.

Should you wish to be included (if pre-qualified) on the bidders list, kindly indicate that you have firm intension of bidding by providing the following:

3.1Technical and Commercial Information

3.1.1Registration:

-Submit copies of commercial registration and registration by MOH in Sultanate of Oman.-Give the name of the Owner(s) and their percentage of ownership.

-Give the name(s) & position (s) of your authorized representatives.

-Address and contact numbers

3.1.2Financial:

-Submit a copy of your last 2-years Audited Annual Reports and most recent published Accounts.

Current Structure of Tenderers organization listing:

Numbers of personnel directly employed by your company

Number of personnel and personnel working in the field that are involved in the provision of review and auditing Quality & Environmental Management System (give job description of each person)

3.1.4Key Personnel

Submit CVs of the medical personnel (doctors, staff nurses) who are going to work as per this contract and the CV of clinic/hospital Manager.

3.1.5 No. of Staff Currently Employed

State the number of medical personnel (doctors, staff nurses) currently employed in your organization, and for each if male or female.

3.3Health, Safety & Environment (HSE) Information

Health safety and Environment based on industrial and oil and gas Plant standards and environment.

3.3.1HSE Policy:

-What is your companys Policy on HSE?

-If your Companys HSE policy exists as a formal document please submit a copy.

-Submit copy of your organization chart. Identify who has the HSE role / responsibility and briefly describe their HSE related job / works.

-If no policy exists, explain how you intend to deal with HSE-related issues.

3.3.2HSE Training

Submit list of training programmes and records of HSE training attended by your staff and clearly indicate if these staff(s) will be working on this project.

3.4Quality AssuranceSubmit one copy of the following. If none issued, clearly state so:

-Quality policy, endorsed by your Managing Director
