Edema cerebral en la cirrosis hepática
Pre-Tx Post-Tx
Small vessel disease and encephalopathy
Actitud ante síndrome confusional en cirrosis hepática
1. Reconocimiento de la encefalopatía hepática
• Cirrosis hepática descompensada• Insuficiencia hepática por enfermedad hepática crónica agudizada (“acute on chronic”)• Enfermedad hepática terminal
Enfermedad hepática terminalMala calidad de vida previa
Encefalopatía lentamente progresiva
Ausencia factor precipitante
Lesión parenquima: hepatocarcinoma, cirrosis avanzada con hígado pequeño
Alteración bioquímica hepática
Irreversible
Objetivo: paliar
Insuficiencia hepática por enfermedad hepática crónica agudizadaPreviamente estable
Factor precipitante: causa lesión hepática + inflamación
Ictericia +++
Ejemplo: hepatitis alcohólica aguda, sepsis
Potencialmente reversible
Mejoría lenta con mortalidad elevada
Insuficiencia hepática por enfermedad hepática crónica agudizada
Criterios diagnósticos APASL(2) Jalan & Williams(3)

Encefalopatía hepáticaó
Síndrome hepatorrenal +

2. Por Enfermedad Hepática Crónica
Enfermedad hepática crónica (puede no haberse diagnosticado previamente)
Cirrosis hepática
3. Agudizada Insulto hepático que causa ictericia y coagulopatía
En un periodo inferior a 4 semanas
Deterioro de la función hepática
Entre 2 y 4 semanas
- Mejorar función hepáticaantiinflamatorios?
- Prevenir y tratar infección- Tratar disfunción circulatoria
terlipresina + albumina- Soporte intensivo

Tratamiento de la encefalopatía hepática en cirrosis hepática descompensada

NUTRICION
• Malnutrición: muy frecuente
• Afecta pronóstico
• ¿Cómo alimentar pacientes con encefalopatía?
Restricción de proteínas: empeora pronóstico

• Nutritional intervention alone does not appear to have a major impact onthe outcome of patients with cirrhosis and prior HE.
• Thus, there is no reason to recommend a low-protein diet in these patients.
Amoníaco: probióticos, catárticos, antibióticos..Nutrición-sueroterapiaFactores precipitantes-cofactores: hemorragia digestiva, infección, hiponatremia, BZD…
Tratamiento del paciente con cirrosis + EH
Encefalopatía prolongada (> 4 días)
• Insuficiencia hepática grave
• Factor precipitante “oculto”
• No es EH
Porto-systemic shunts may be occludedCardiovasc Intervent Radiol 2009

Clinical case of prolonged HE
Esophageal and paraesophageal varices
Lack of large portosytemic shunts

Hidden prostatic abscess

Disappearance of HE
CONCLUSION• Encefalopatía hepática:
lesión astrocito + edema cerebral
• Proceso diagnóstico: incluir valoración hepatopatíaterminal, IHEHCA, CH descompensada
• Tratamiento: Terminal: paliaciónIHEHCA: soporte intensivoCH descompensada: disminuir amoniaco+ tratar precipitantes (búsqueda activa)
• Nutrición: normoproteíca
