Total Contact Casting (TCC) is one of the most well recognized and documented successful treatments for the diabetic foot ulcer. At the same time, it is one of the most underutilized treatments. Understanding the different products can assist in increasing utilization and thereby increasing the number of healed DFU’s. Technology has changed dramatically and the utilization of a TCC for proper offloading is easier than ever before. In this HANDS-ON session, you will learn how to apply a TCC in less than 20 minutes, from preparation to the patient walking out of the room, all while achieving proper offloading. Also remove a TCC in approximately one minute without the need or use of a cast saw. We will review the selection process of a TCC candidate as well as the indications and contraindications for using a TCC for proper offloading.

This hand-on class is aimed at the importance of compression. The session addresses the vital role that compression plays in wound healing and prevention of wound recurrence. Participants will understand the indications for various compression levels and different compression types and know the importance of choosing the compression that best stimulates venous return. During this portion of the workshop we will encourage clinicians to trial different compression systems and practice don/doffing so that they can best coach their patients in the life-long need for compression.

To maximize wound healing, additional protein and calories are often required; however, calorie reduction is usually recommended to facilitate weight loss. This session will outline nutrition care issues related to this clinical conundrum and guide the clinician on how to prioritize treatment.

Stiehl MD, MBA, Managing Partner at Stiehl Tech LLCTreatment of chronic infected wounds requires dealing with bacteria and understanding the mechanisms by which these pathogens persist in the wound. Mechanical surgical debridement is the classic treatment for open wounds but must be combined with other important modalities such as antiseptic delivery with wound dressings, pressure relief, negative pressure wound therapy, and selective mechanical debridement with jet lavage irrigators. Important concepts include the application of these modalities in the context of the TIME and treatment step down approach. New technologies must perform in the context of cost, efficiency and efficacy. These will be reviewed.

Angiosomes are an anatomical location supplied by a specific artery. The artery is the only means of healing the wound. It is critical to identify a patient with a non-healing wound of the lower extremity to prevent limb loss.

Decongestive lymphatic therapy may ameliorate this disorder.

9:20 a.m. - 10:15 a.m. 400 Biofilm Management – Myths and RealityDr. Gregory Schultz, PhD Professor Obstetrics and Gynecology Director Institute for Wound Research University of Florida Wound healing is a complex biological process that progresses through a sequence of phases (hemostasis, inflammation, repair and remodeling) that are regulated primarily by cytokines, growth factors, proteases and extracellular matrix components. Chronic wounds do not progress through these phases, and typically become “stuck” in an inflammatory condition that is characterized by elevated levels of proinflammatory cytokines and proteases, which impair healing by degrading extracellular matrix molecules, growth factors and receptors that are essential for wound cell migration, proliferation and scar formation. Recent evidence suggests that bacterial biofilms contribute to the chronic inflammation in a high percentage of chronic wounds. Bacteria in biofilm structures develop substantial tolerance to most antibiotics and antiseptics, which has led to the concept of biofilm-based wound care, which emphasizes starting with aggressive debridement techniques to remove biofilms combined with dressings that prevent reformation of biofilms. When biofilms and inflammation are adequately reduced, treatments can step-down to less aggressive options that simply maintain low inflammation and

bioburden (planktonic and biofilm bacteria) then step-up to advanced therapies (growth factors, cell and tissue products, skin grafts) that will actively accelerate healing.

By the end of the session participants will be able to:• Review the four sequential phases of normal wound healing and

recognize the beneficial effects of controlled inflammation and protease activities

• Establish the link between chronic inflammation caused by planktonic and biofilm bacteria and elevated proteases and reactive oxygen species that destroy proteins that are essential to healing (extracellular matrix, growth factors, receptors) leading to impaired healing

• Understand how biofilm bacteria develop high tolerance to most antibiotics, antiseptics and disinfectants

• Describe key principles of biofilm-based wound care that emphasize debriding biofilms and preventing reformation of biofilms as part of the step-down-step-up approach for effective therapies to treat biofilms and promote healing.

This imbalance leads to tissue destruction of essential extracellular matrix components, which in turn further delays all phases of the healing continuum. Most antimicrobials including silver are less effective at treating biofilms and there is a critical unmet need to develop new advance wound dressings that can destabilize the biofilm and restore healthy tissue. Our lab has been developing the next generation of advanced wound dressings using state-of-the-art approaches in 3D printing of biopolymers. Here we demonstrate the benefit of incorporating novel antimicrobials

into 3D printed fibers to enhance effectiveness at treating bacterial biofilms both in vivo and in vitro. The next generation 3D printed wound dressings will prevent wound infections, reduce odor management, and provide better patient outcomes for healing chronic wounds.

The impact of SWD can be considerable: increased mortality, delayed hospital discharge, readmission, further surgery, delayed adjuvant treatment, suboptimal aesthetic outcome and impaired psychosocial wellbeing. It is imperative to raise awareness of SWD and improve identification, prevention and management. Prevention of SWD comprises excellence in surgical practice, prevention of surgical site infection, reducing risk of healing impairment and use of strategies such as single-use negative pressure wound therapy in appropriate high-risk patients. Management also involves a holistic approach that includes amelioration of impediments to healing, optimising conditions in the wound bed and using appropriate treatment modalities to ultimately close the wound.
