INTRODUCTION
•NP Bilatérale symétrique distale et synchrone
•Fréquence : PN (80% ) /MN (PFP, SCC), PRN
•Symptomatique ou infra-clinique (détection)
•Nécessité de prévention car traitement difficile
•Qualité de vie = handicapante
–Douleur : 15 à 20 % (DT2)
–Pied diabétique:
-Amputation = 0.3– 45%
-mortalité précoce:>50% (Kenya - Ethiopie)

EPIDEMIOLOGIE • Facteurs aggravants: -Etat nutritionnel (Ethiopie), -Mauvaise hygiène du pied, -Retard de diagnostic, -Facteurs socioculturels, -Vasculaires: HTA , dyslipidémie, Facteurs modifiables : prévention éducation - Bon contrôle glycémique, - Bonne hygiène du pied

1- Interrogatoire: algoparesthésies distales ,
spontanées ou provoquées, insomniantes, / DN4 ≥4
2- Examen:
- Sensibilité /toucher et piqûre: Hypo ou anesthésie – algique , en chaussettes
- Sensibilité vibratoire (diapason)
- ROT: abolis ou diminués
DN4 ≥ 4 + EXAMEN ANORMAL

• Normal (neuropathie petites fibres)
• Anormal : PN sensitive et axonale (80%)
TRAITEMENT - SURVEILLANCE
Etiologique: contrôle glycémique
Symptomatique: coanalgésiques
diminuer hyperexcitabilité neuronale:
antiépileptiques/douleurs paroxystiques (décharges)
Stimuler mécanismes d’inhibition: antidépresseurs
douleur continues( brulures)
Surveillance / clinique: sensibilité, réflexes,
examen du pied
