You will receive an Email that looks like this

You must verify yourself with last name, Zip Code of the address

that you gave to Medical Registration and date of birth

***International guests all use 00001 as the Zip Code regardless of

actual country code***

You will be required to give your consent by

checking the “agree and consent “ box.

Your username is assigned by the system as your First name

Dot Last name. The first letter of each name is capitalized.

Choose a password and confirm that password

***Write the username and password down.***

username and password?

Once you have created your account the portal log-on screen

looks like this

If you ever forget your password or username the portal log-

on page has links to recover them

After log-on this page will open and your records

are located under “Health Information”

This is the view of the “Health Information” page

Medical staff sends your records to the portal as those

records become available. It can take up to 14 days for

some of your medical records to be reviewed by your

Pritikin physician and then be sent to the portal
