HOW TOCLAIM YOUR
PRIZECONGRATULATIONS ON BEING
A TATTS NT WINNER!
The information in this guide is current at time of printing in October 2018 and this information is subject to the Rules of Authorised Lotteries available online at thelott.com or at any Tatts NT Lotteries Outlet. Tatts Group Limited (ABN 19 108 686 040) and/or its subsidiaries is the owner of registered trade marks ”Tattersall’s”, “Tatts”, Tatts logo, “Instant Scratch-Its” “TattsLotto”, “Powerball”, “Lucky Lotteries”, Lucky Lotteries logo, and is a joint owner of registered trademarks “The Pools”, “Oz Lotto”, and “Super 66”.
This form is required by all winners claiming a prize that cannot be paid at a Tatts NT Lotteries Outlet.
Prizes less than $1,500 may be claimed at any Tatts NT Lotteries Outlets. At their discretion, some Tatts NT Lotteries Outlets may pay prizes up to $3,999.99.
Tatts NT Lotteries claimants must use a Golden Casket Prize Claim Form for all Instant Scratch-Its Prize Claims. Forward the Golden Casket Prize Claim Form and original Instant Scratch-Its ticket to: Administration, Locked Bag 7, Coorparoo QLD 4151
To claim your lottery prize, complete the following steps:
1. Complete the Prize Claim Form
F Section 1: Fill in your personal details.
F Section 2: Fill in your bank details.
F Section 3: Sign the Claimant Declaration.
F Section 4: Provide details of lottery ticket/s.
F Take a photocopy of the Prize Claim Form for your records.
2. Sign ORIGINAL winning lottery ticket/s
F Write your name, address and signature on the back of the ORIGINAL winning lottery ticket/s.
F Take a photocopy of your winning ticket/s for your records.
3. Submit Prize Claim Form in person or by post
F In person – Visit 5 Bowen Crescent, St Kilda, Melbourne, VIC 3004, during office hours.
F By post – Tatts NT Lotteries recommends using Registered Post for added security.Please refer to Claiming Prizes - Draw Information for information on where to post your Prize Claim Form and original ticket.

Claiming major lottery prizes (first division, Set for Life 1st Prize, Lucky Lotteries jackpot or Lucky Lotteries first prize)Major lottery prizes are available for payment from the first business day two weeks following the draw.
Claiming other division lottery prizesLower division prizes are available the first business day following the draw, with the exception of The Pools where prizes are available the Tuesday after the draw.
Claiming prizes drawn prior to and including 30/07/2012 Prize payments associated with lotteries drawn prior to, and including 30/07/2012 will purge to the Northern Territory Treasury Director of Licensing as unclaimed money. Complete the Prize Claim From and send with original ticket to: Northern Territory Treasury Director of Licensing GPO Box 1154, Darwin NT 0801.
Claiming prizes drawn from 31/07/2012Lotteries drawn from 31/07/2012 must be claimed within 3 years from date of the draw.
Complete the Prize Claim Form and send with original ticket to: Administration, PO Box 888, Melbourne VIC 3004Prior to payment of a Prize, players may be required to provide:
