Mr. David John, VP Process Safety and Assurance

Exploration and Production Public Company Limited

Mr. David John has worked in the oil and gas industry for over 30 years, including 10

years working in support of North Sea operations and safety. For the past 20 years he

has worked in international locations including France, Qatar and Thailand. During this

period he has undertaken numerous serious incident investigations, audits and

technical reviews of assets and projects and has been involved in the development

and application of safety management systems. Since joining PTTEP in 2008 provided

extensive support to PTTEP’s Australian subsidiary following the Montara blowout. He

is currently VP Process Safety and Assurance at PTTEP corporate headquarters in

Bangkok.

PTT Exploration and Production Public Company Limited (PTTEP) is a national

petroleum exploration and production company based in Thailand. It is a subsidiary of

the state-owned PTT Public Company Limited. The company was founded on 20 June

1985. PTTEP’s core business is exploration and production of petroleum in Thailand

and foreign countries. PTTEP has operates assets in Thailand, Myanmar, Malaysia and

Algeria. Current production is about 330,000 barrels of oil per day.

Abstract

The Legacy of Montara

The Montara incident was a 2009 well blowout in the Timor Sea, Australia. There were

no injuries, but it resulted in a 400 barrels/day oil spill which later ignited.

PTTEP is still living with the aftereffects of the incident. This Presentation focusses on

the long-term impacts of Montara highlighting the various ways that business was

affected and that the effects of a Major Accident Event can extend for many years

after the initial incident is controlled.

The key lesson is that Prevention is better than cure. All of these adverse effects could

have been avoided if effective Process Safety Management had been in place.

Ms. Sulo Belawan is currently the General Manager, Health, Safety, Security and

Environment (HSSE), GNE, PETRONAS. Her key role is to lead the development and

implementation of comprehensive HSE strategic plans framework and policies for

PETRONAS. She joined PETRONAS in 1987 as Process Engineer, and since then had

held positions as Process Engineering manager, Project Manager, General Manager

Group HSE Assurance & Performance, General Manager Group HSE Strategy &

Governance, General Manager Process Safety Management, Group HSSE. She is also

currently one of the Advisory Board Members of Center of Chemical Process Safety

(CCPS). She has close collaboration with IChemE and University Technology

PETRONAS (UTP) in the area of capability development.

Mr. En Bashir is the GM of Process Safety Management, Group Health Safety Security

and Environment (GHSSE), PETRONAS. He is responsible to steer the development and

implementation of Process Safety Management for PETRONAS group.

Prior to his current role, he was the GM for Assurance and Risk, where he drove HSE

assurance exercise in PETRONAS and managed major incidents investigations and

reporting to PETRONAS Leadership Team. He served a short stint in Corporate

Strategic Planning, PETRONAS where he was involved in PETRONAS Transformation

Journey (CACTUS).His foundation years were in plant operations. He grew from

Operations Engineer and Process Technologist in ASEAN Bintulu Fertiliser (ABF) a

subsidiary of PETRONAS and later progressed to become manager and then senior

manager at Malaysia LNG (MLNG) another subsidiary of PETRONAS. His stint as Senior

Manager in MLNG tasked him with the responsibility to manage Process Safety

Management, Advance Process Control, Energy & Loss Management and project

feasibilities studies.

Abstract

Achieving Process Safety Excellence by Leveraging on Digitalization

Over the last few decades there have been many improvements in Process Safety as

we go along the path to Process Safety Excellence. The next big step is believed to be

the adoption of digital solutions to Process Safety application.

Its not just a case of relying on technology and automation to improve things;

digitalization is the way forward. Leveraging digitalization to change people’s behaviors

and mindsets and to overcome human limitations.

Abstract

Process Safety Aspects of Green Engineering

The world is getting hotter, weather patterns are becoming more extreme, erratic and

destructive and there is an abundance of evidence that the changes are

anthropogenic (arising from human activity) in nature.

It is equally evident to all but the most ardent climate change deniers that

fundamental change in the way we conduct life on earth is required and a large part

of that will involve using fewer fossil-based energy sources in order to combat

greenhouse gas emissions, together with better utilization of other extracted

resources if we are to support a growing population, limit and eventually reverse

climate change and facilitate sustainable development for all.

However, the world is not going to be weaned off 100 million plus barrels of oil

equivalent per day and finite resource-consuming agriculture, housing, manufacturing

and other pillars of modern society overnight. We therefore need better means of

managing scarce resources and our environment and ‘green engineering’ is just one

innovation that will be required as we transition to a low-carbon, low resource

consumption economy in years to come.

Green engineering, defined by the US EPA as “The design, commercialization, and

use of processes and products that minimize pollution, promote sustainability, and

protect human health without sacrificing economic viability and efficiency” aims to

promote sustainable economic growth whilst minimizing adverse impacts of such

activity on people, the planet and prosperity (3P).

As with all innovations there will be challenges as well as opportunities along the way

and this presentation explores some process safety aspects of green engineering

that must be addressed as part of the equation.

Whilst focusing on green opportunities that are beneficial from a process safety

perspective, the presentation also highlights some unintended side-effects of ‘going

green’ that can occur by reference to process safety incidents that have arisen in

recent years. It is concluded that a holistic approach to design is required in order to

address 3P issues in a coherent manner.

Plant Design and Operations & Maintenance. Ray has comprehensive experience in

understanding the requirements of both Engineering Companies and Owner Operators

Pratap Sathiah is working as a technical safety engineer in Shell India Markets Private

Limited, India. He has 10+ years of extensive experience in technical safety

engineering mostly on Major Hazards management. Last few years he is working in

the area of fire safety and supported various downstream and LNG assets.

Furthermore, he has worked on hydrogen safety for hydrogen refuelling stations e.g.

development of guidance for layout of hydrogen refuelling station, . Moreover, he was

involved in the development of consequence modelling tool for analysis of hydrogen

dispersion, jet fires and explosion. In addition, past several years he is working on

safety of liquid hydrogen for large scale shipping. This involves working with several

universities and industries through EU projects and Joint industry Projects. In the

past, he has been working in the area of hydrogen safety applied to nuclear reactors,

wherein he was involved in developing a CFD codes for dispersion and explosion of

hydrogen-air mixtures. He has presented 10 papers in international conference and

publishing more than 20 articles in a well-known reputed journal.

Virtual Reality for Hazards Awareness

In Oil and Gas industry, it is crucial to be prepared for any kind of hazards. With Goal

Zero being the focus in Shell, we typically conduct practice drills and major hazards

demonstrations to prepare our staff to respond effectively to emergencies.

Assessment impact of hazardous situations are typically done using computer

simulation studies (consequence modelling), using an in-house developed tool called

Fire Release Explosion and Dispersion (FRED). FRED, helps in studying the impact of

a hazard by considering external factors such as wind speed, direction, pressure and

temperature. Currently, what FRED produces are 2D charts and data to represent the

dispersion (of flammable and toxic gas cloud) and jet/pool/flash fire and its impact.

Mock drills and FRED consequence modelling, although specific to the asset, cannot

help visualizing the impact of the hazard to the trainees. These result in a lack of risk

realization at a deeper level and a limited understanding of major hazards and physical

effects in the field.

Our Virtual Reality (VR) based training simulator uses results from FRED studies,

overlays them on a digital twin of an asset and uses VR visualizations to provide an

immersive training experience to emergency responders and field operators. Such

simulation based virtual learning environments help learners gain better risk

realization as they can visualize and experience the extent and impact of hazards.

They can practice in a safe environment and tune responses by building muscle

memory. This will also equip our assets with tools to better prepare their ER

personnel and incident commanders on specific assets under their protection,

complementing other training methodologies. As a result, enhancing preparedness

and adherence to Goal Zero but reducing the number of fatalities.

Effectiveness and benefits of dynamic simulation in safety systems relief verification

Performing Process Safety Relief system verification at key stages in the project

execution lifecycle helps in the prevention of major incidents. We show why late

discovery of safety findings has significant cost impact.

Best practice verification coupled with dynamic analysis, identifies faults, avoids re-

work in design, and reduces capital expenditure on constructed plants or those in

operation.

Through Case Studies, we show how relief valves have inherent overcapacity in relief

scenarios and why it is not always necessary to replace relief valves, or flare systems

when new capacity is added. This is best illustrated with an example from the Refining

Abstract

Opportunities to enhance barrier management through incident analysis

Many hazardous industries use bowties to analyze and assess risks. In the bowtie

model, barriers are identified which aim to prevent, control or mitigate unwanted

scenarios.

Despite this, incidents and near misses may still occur. When they do, investigations

help to come up with recommendations to avoid repeat. Unfortunately, in many cases

investigation reports end up as a collection of unstructured documents. They are

often treated as single events, making it difficult to identify trends over multiple

events.

Scenario-based Incident Registration (SIR) aims to provide a structured process.

Users select a specific scenario from a set of bowties and assesses the effectiveness

of the identified barriers. Questions are asked per barrier about the reasons of

failure. These findings are stored on barrier level. If this process is repeated, the

bowtie will accumulate barrier failure data of multiple incidents in a single diagram.

This information helps to identify the strength of the barriers, which in turn facilitates

risk-based decision-making.

Some incidents require in-depth investigations and a registration only is not enough.

However, this registration can be used as a starting point for the incident analysis.

Doing barrier-based incident analysis allows you to learn from barrier failures and

improve their performances. This incident data together with the SIR data can be

linked back to the bowtie diagram to spot the strengths and weaknesses of your risk

assessment.

SIR is a continuous process and more than a reporting system. The reporter does not

require knowledge about the bowtie methodology, while incident managers receive

valuable insight to their pro-active risk assessment.

Bow-Ties & LOPA: Using Bow-Ties to improve your LOPA

There are many problems with how LOPA studies, especially SIL assignment studies,

are typically performed. In many cases these have become a tick box exercise and

numbers game with widespread inconsistencies in how they are performed and a

failure to capture much team input and realistic data.

The Bow-tie technique presents a clear graphical representation of a risk scenario

which is readily understood by people at all levels and can be used to get better

input and buy-in from the LOPA team when used during a LOPA study.

In order to use the bow-tie effectively it is necessary to structure it in a way that is

consistent with the LOPA calculation process. This presentation shows some of the

issues that must be considered and how the bow-tie can be used together with the

LOPA calculation details , and be updated in real time; thus, overcoming many of the

challenges with conventional LOPA exercises.

Combining the two techniques in this way often helps the team to avoid many

common errors and will overcome many of the LOPA issues mentioned above. This

ensures that any LOPAs are realistic and often adds additional value as all barriers

are easily identified and discussed including those that don’t initially meet IPL

criteria.

The I-EA-T or the Industrial Estate Authority of Thailand, a state enterprise under the

Industry Ministry. Main products of the I-EA-T are industrial estates that it has

operated by itself, industrial ports, and industrial estates that it has operated under

joint-operating contracts. Main services of the I-EA-T are about the establishments of

industrial estates and industrial ports for industrial plants to operate in the same areas

in an orderly manner, the provision of necessary public-utility services for industrial

operations, the management and supervision of industrial estates as well as industrial

ports, and sustainable environmental management. At present, the I-EA-T has

operated 55 industrial estates in 16 provinces across Thailand. Of them, 13 are solely

operated by the I-EA-T. The rest are operated under I-EA-T’s joint-operating contracts

with others. Such developments underline the I-EA-T’s ability to develop products in

response to government policies-driven investments and to provide services that

matter to and support business operators’ operations, expansions, and upgrades in

the Industry 4.0 Era. On the overall, I-EA-T operations have expanded well in

Achieving Goal Zero on Process Safety

Looking at the history of the oil and gas industry, it is easy to recall the dramatic

consequences of major incidents – to people, the environment and to the reputation

of the companies involved. Over the last many years, Process safety performance in

our industry improved through application of leading technology, common standards

and management systems. This continues to be essential and is foundation of our

Goal Zero journey. Achieving the goal of no harm to people, no leaks and protecting

the environment requires a robust approach to Process Safety with the aim of

achieving Goal Zero. This not only requires complying with our policies, procedures

but having a strong process safety culture, visible and felt leadership at all levels,

making it a deep down personal commitment by everyone involved and applying

process safety fundamentals.

Goal Zero is a prerequisite for delivering on our ambitions as a company. Our facilities

must operate with ‘no leaks’ or incidents that can cause serious injuries to our

employees, contractors or neighbours. Process Safety requires the management of

hazards from design through to construction and operation. We need organisational

focus and individual accountability across these phases to maintain the strength of

the ‘barriers’ we put in place to prevent releases and incidents. Everyone in the

organistaion including contracting partners play an important role in identifying

potential hazards, ensuring we have the right controls and verifying that these

controls are working as intended. We must never become complacent about major

accident hazards. We must never assume that we can relax our requirements on one

barrier because another barrier will prevent an incident. We must retain a ‘chronic

unease’ with regard to Process Safety that presses us to look for any emerging

hazards or breakdowns in our controls. This is fundamental to achieving sustained

good process safety performance. We need leadership at all levels to reinforce

Process Safety requirements in our day to day activities and drive business

performance with a focus on Goal Zero – not at the expense of Goal Zero.

Role of leadership in Process Safety Management

Energy, Petrochemicals and Process industries have enjoyed sustained success in

safety performance, yet still faces challenges: We are not yet successful in anticipating

and preventing all major incidents and fatalities. Furthermore, despite process

improvement, increasing regulation and investment in safety, significant incidents

persist. Urgent focus and action are called for.

In this session we will explore some of the strengths and weaknesses of the current

approaches to safety and explore a way forward based on new ways of thinking about

safety. Specifically, we will address the positive role of people in creating Incident and

Injury-Free™ (IIF™) workplaces, using real time approaches and venues to bring

performance, and people, to life! These include coaching, peer-to-peer development

and simulation. World class energy resources and construction companies are

applying these approaches to drive world class performance, building resilience to

match the robustness of their systems. Having added a great deal of systems

improvements in the last decade, these companies are improving performance, and

safety, from the bottom up.

Abstract

Reliable safety instruments in dirty services: Plugged connections eliminated

This presentation talks about a common problem that seriously affects plant safety.

Talk to any instrument technician on an operating plant and they will tell you that one

of their biggest problems is instrument tappings that plug up.

This is expensive, causes the plant to run sub optimally and often out of control.

When the instruments are Safety Instruments or are required as layers of protection,

then this problem has a real adverse safety impact.

Safety can be affected because the instruments give misleading results or simply

don’t work . But fixing the problem also runs the risk of exposing the field technicians

to leaks of hot, flammable or toxic product and there is also the risk of a major

release if something goes wrong during the manual cleaning procedure.

It is well known that Prevention is better than cure. This presentation describes an

automated solution that not only eliminates the problem, but often has tremendous

financial payback too.

Improving Safety through Continuous Integrity Monitoring

Erosion damage in oil and gas wells are inevitable at high velocities. This is especially

true when solid particles or sand are produced. Current method of reduced

production velocity as recommended by API is not satisfactory. The fear of severe

damage due to sand erosion has curtailed production from potentially high-capacity

wells. In this presentation, we will share how Permasense Solution has helped an
