  • INTRODUCTION

    This Contractor Handbook (CH) stipulates the requirements of The Hongkong Electric Co. Ltd. (HK Electric) on health and safety, environmental protection, natural gas precaution, quality assurance and matters relating to housekeeping and security for all activities in the construction sites including but not be limited to the Lamma Island, Hong Kong Island. It serves as the guidelines for all contractors in the course of executing their contractual work for Building, Generation and Transmission Projects managed by HK Electric’s Projects Division (PD). Compliance with this CH shall not relieve the Contractor’s responsibility and obligation in complying with relevant Hong Kong SAR Government Ordinances and Regulations. The Contractor’s site management should develop their work instructions, safe working procedures as well as a safety management system according to the actual construction site environment, operations on their workplaces, formulate precautionary measures, provide information, training and supervision to workers for a full understanding and compliance with the works procedures to avoid accidents.

  • GENERAL SITE SET UP AND LOGISTICS

    1. COMMUNICATION AND WORK FLOW

    1.1 Reporting to Work

    Before mobilisation to Lamma Site, the Contractor shall make initial contact with Site Manager (Lamma), SM(L) who will assign a responsible Site Engineer to coordinate with the Contractor for the execution of the contract. Before mobilisation to HK Island Site, the Contractor shall make initial contact with the Responsible Engineer (RE) who will coordinate with the Contractor for the setting up of the site facilities.

    1.1.1 Setting-up and orientation matters, such as site organisation, communication

    channels, site security, transportation, site office, storage areas allocation, working area allocation, access requirements, methods of work, safety/environmental/ quality requirements and gas safety awareness, etc. can be dealt with at kick-off meetings or by direct consultation with the responsible Site Engineer.

    1.1.2 Latest version of CH will be issued to the Contractor. Should CH be revised, the

    revised portions will be issued to the Contractor on site. SM(L) on Lamma Site or RE on HK Island Site may issue additional site instructions to the Contractor if required.

    1.1.3 As far as the Contractor is concerned, Projects Division (PD) represented by SM(L)

    on Lamma Site or the RE on HK Island Site is the coordinating division for all matters relating to the contractual works. Generation Division (GEN) is responsible for the operation of power generating facilities and is the end user of the Lamma Power Station (LPS) All areas under the control of GEN, including but not limited to all existing buildings, structures, containers, silos, sheds, wharfs, jetty, coal yards, conveyors, fenced-up areas, outdoor or indoor plant, equipment and facilities, accesses, etc, are operational areas. T&D Division (T&D) is responsible for the operation of the power transmission facilities and is the end user in Hong Kong Island and Waters. All areas under the control of T&D, including but not limited to all existing buildings, structures, fenced-up areas, accesses, cable joint bays, trenches, tunnels, etc, are operational areas. PD is also responsible for construction environmental protection matters and contractual plant performance tests.

    1.1.4 Correspondence submission concerning site matters shall follow the table below:

  • 1.1.5 A “List of HK Electric Emergency Contacts” regarding verbal communication and

    emergency contact is attached in the Appendix 1. 1.2 Construction

    1.2.1 The Contractor shall attend Kick-off Meeting, Site Progress Meeting, Working Group Coordination Meeting, Safety Health and Environmental (SHE) Meeting, Commissioning Panel Meeting, Performance Test Coordination Meeting, Technical Meetings and various Ad-hoc Meetings as required.

    1.2.2 For monitoring purpose, the Contractor shall submit timely all the required

    documents including Daily Manpower Report, Site Inspection Reports, safety records / checklists, Site Progress Reports, Payment Reports and SHE Monthly Report (Appendix 2) in a prescribed format and interval as per specified by SML / GMPD. The Contractor is required to submit the “SHE Monthly Report” on or before 5th day of the following month.

    1.2.3 The Contractor if found to have breached HK Electric’s in-house rules or statutory

    requirements on safety, security, traffic, housekeeping, environmental control, etc., will be issued with a “Contractor Non-Compliance Notice” or warning letter/memo. At the discretion of GM(PD) or SM(L), any Contractor’s personnel if found breaching the same Non-Compliance twice, disciplinary action will be taken including ban of access to HK Electric’s sites/premises.

    1.2.4 Initial Survey of Work Site and Handover

    The Contractor shall conduct initial survey of work site before work commencement. When the construction work proceeds to the prescribed check-points, appropriate tests or inspections shall be carried out by the Contractor to the satisfaction of the RE to prove compliance with the construction standards or requirements laid down in the Contract. Prior to the handover of construction work or work areas, a joint inspection shall be carried out to the satisfaction of the RE with respect to work completeness and SHE operational requirements. The Contractor shall submit the initial site survey report and “Site Inspection Record on Handover of Site Area/Works” (Appendix 3) to GM(PD) or SM(L) for record purpose.

    1.3 Quality Control

    1.3.1 HK Electric Quality Policy

    The Contractor shall observe the quality policy as stipulated in HK Electric’s Quality Policy (Appendix 4).

  • 1.3.2 Submission of Quality Plan The Contractor shall prepare and submit a Quality Plan to PD showing the appropriate quality control system for the project at or before the Project Kick-off Meeting. The Quality Plan shall be supplemented with (i) Contractor’s organization chart, (ii) the Inspection and Testing Plant (ITP), (iii) Calibration Plan for all instrument used including calibration requirement, certification, storage and labelling arrangements, and (iv) mill certificates, fabrication records and / or fitness certificates. All of the above supplementary documentation shall be submitted timely to meet the project program.

    1.3.3 Arrangement of Inspections Request for Inspection shall be submitted by the Contractor to the RE at least 24 hours before inspection. The Contractor shall arrange proper tools, approved instrument and/or equipment with valid calibration certificates required for such inspection.

    1.3.4 Non-Conforming Works Reports (E&M Works) (NC Report) During the course of erection/construction, the Contractor shall report if any, non-conformities, such as non-compliance with design drawings, design omissions, pitfalls, substandard fabrication, shipment damages, inconsistency of foundations, interference between equipment, modifications to suit site conditions, etc, by submitting a “Non-Conforming Works Report (E&M Works)” (Appendix 5) to SM(L) or RE. Findings and recommendations for remedial measures/actions shall be detailed in the report.

    1.4 Commissioning and Testing

    1.4.1 Pre-commissioning Checks

    Before performing pre-commissioning check, the Contractor shall submit a Testing and Commissioning (T&C) Procedure to GM(PD) for approval. The pre-commissioning checks stipulated in the approved T&C procedure shall be conducted by the Contractor in the presence of the RE, for safeguarding personal safety and the integrity of their supplied equipment/plant. User Divisions’ representative(s) may attend the checks according to the nature and complexity of work involved. Any faults or defects found shall be rectified by the Contractor before T&C.

    1.4.2 Commissioning

    When the equipment/plant is ready for T&C, the Contractor shall sign the associated Safety Rules Initiation Certificate (SRIC) prepared by the RE. Once the SRIC becomes effective, the T&C activities can proceed and be carried out in the presence of the concerned parties. For any unsatisfactory work or defects found during T&C, the Contractor is required to complete all rectification works and then to resume the T&C of the equipment/plant involved. The Contractor

  • shall record the T&C results and sign the records. If there is a need for GEN to operate the equipment/plant in the absence of the Contractor during non-office hours or due to operational requirements, a Commercial Operation Clearance Certificate (COCC) prepared by the RE will be issued to the Contractor for signature to signify his/her consent.

    1.4.3 Performance test When the equipment/plant is satisfactorily tested and commissioned according to the approved commissioning procedures and suitable for acceptance test, the RE will arrange a meeting to work out the programme and recap the testing requirements in accordance with the approved Performance Test Procedure. The Contractor shall record the performance test results and sign the records. Results of the Performance Test will be analysed by HK Electric according to the prescribed guaranteed requirements. Performance Acceptance Test Coordination Meetings shall be conducted to review the test schedule and results as need be. Any unsatisfactory results found will have to be rectified by the Contractor prior to re-test.

    1.5 Retreat from Site

    1.5.1 Prior to the Contractor’s retreat from site, a final joint inspection shall be carried out. The Contractor shall fulfil where appropriate the following requirements in the final joint inspection to the satisfaction of the RE: a) All guards are secured in place, floor openings are made safe, trench covers

    are in place and handrails are restored to normal; b) All gears, tools, temporary barriers, construction equipment and vehicles

    are withdrawn; c) The plant and work areas are clean and tidy, and all wastes and surplus

    materials including temporary underground facilities are removed; and d) The Contractor's personnel and belongings are all withdrawn from the site.

    1.5.2 The Contractor shall submit duly signed "Site Inspection Record on Handover of Site Area/Works" (Appendix 3) with photos.

    1.5.3 The Contractor shall submit a list of Contractor Pass (if applicable) to be returned to User Divisions’ Security Section before retreat. All Contractor Pass must be returned before departure from site.

    1.5.4 The Contractor shall submit a complete list of surplus materials for handover to HK Electric. Should HK Electric classify those surplus materials as scrap, the Contractor shall arrange for proper disposal according to the contractual requirements.

  • 2. SITE FACILITIES AND UTILITIES

    All sites are provided solely for HK Electric’s project development work. The Contractors and his/her sub-contractors are not allowed to use these facilities for any work/activities not related to his/her contractual scope of work. Any infringement of the aforesaid found will be subject to disciplinary action which including banning from the site/premises.

    2.1 Site Office / Shelter

    2.1.1 The Contractor shall not erect any temporary building or structure to serve as his/her site office/shelter on any part of the Site without GM(PD) or SM(L)’s written approval. Application on details of location, dimensions for site office/shelter and form of structures of buildings shall be submitted to GM(PD) or SM(L) for consideration. Once consent is given, the Contractor shall obtain the approval of any other relevant authorities which are responsible for such buildings (e.g. The Contractor’s shed shall be in compliance with PNAP 273).

    2.1.2 Standard steel containers are allowed to use for storage of general tools & materials.

    All such containers shall be equipped with portable fire extinguishers hanging outside the containers. Each container shall be properly painted and maintained in good condition. The company logo or the name of the Contractor shall be neatly painted on the door of each container for identification.

    2.1.3 The Contractor shall maintain a proper access to/in his/her site office and sheds.

    The access shall be clearly marked by paint and it shall not be obstructed by materials, equipment or debris.

    2.1.4 Wooden container or hut for the purposes of site office, workmen’s resting shelter

    or storage of tools/ materials is strictly prohibited. 2.2 Storage Area and Fabrication Yard

    2.2.1 The Contractor shall submit the “Application for Temporary Storage Area or Container Office” (Appendix 23) to GM(PD) or SM(L) for storage area with details of purpose, location, size, etc. A temporary storage notice must be prominently displayed at the approved storage area.

    2.2.2 Changing the usage of any areas for storage/ laydown/ fabrication, or extending the

    usage period will not be allowed unless prior approval from GM(PD) or SM(L) is obtained.

    2.2.3 The Contractor shall ensure that all tools, materials and equipment are stored within

    the allocated areas and maintained in a safe and tidy manner. No materials or equipment is allowed to lie on adjacent areas or encroach across access roads. Access ways of at least 3 metres in width shall be provided by the Contractor to allow a passage for fire engines for access to every corner within the storage areas. Hydrants or access shall be clear from obstruction by materials, equipment or debris.

  • 2.2.4 In order to minimize the fire hazard arising from the storage of timber, or other

    combustible materials, the following rules shall be followed:-

    a) All timber and formwork shall be stacked neatly and stably. b) Stacks shall not be higher than 4 metres or floodwall at Waterfront Road. c) At least 3 metres shall be maintained between stacks. d) Stacks shall be positioned at least 20 meters away from inflammable

    materials such as oil storage tanks. e) A separate working area shall be set aside for the removal of nails etc. from

    the timber and formwork before stacking. This area shall not be adjacent to any access road.

    f) Sufficient firefighting equipment or portable fire extinguishers shall be placed in the vicinity. In dry season, frequent wetting of the stack shall be performed.

    g) No construction material, plant or equipment shall be placed on armoured rocks without the permission of GM(PD) or SM(L).

    h) No stock-piling of construction materials, plant or equipment shall obstruct the existing fire hydrants, F.S. water inlets or main vehicular access.

    2.2.5 Cable/Wire Drum

    a) During transportation, all drums shall be secured on the vehicles. b) After delivery to site, all drums shall be properly stored and secured to

    prevent movement. Drum shoes shall be used when required. It is not allowed to stockpile cable drums on slope or at close proximity to slope crest without the Engineer’s consent.

    c) All drums shall be labelled with the Contractor’s company name for identification.

    2.3 Electricity Supplies

    2.3.1 Unless agreed otherwise, the Contractor shall normally be responsible for his/her own electricity supply on site by means of diesel generator or connection to HK Electric’s designated supply points. The Contractor shall take precaution on all potential hazards when using diesel generators, especially in locations of inadequate ventilation.

    2.3.2 The Contractor shall arrange at his/her own cost to apply for electricity meters and

    lay power cables from HK Electric’s designated supply point to his/her own switchgear.

    2.3.3 The site supply installation shall comply with all relevant regulations including HK

    Electric Supply Rules, Code of Practice for The Electricity (Wiring) Regulations, IEE Wiring Regulations, etc.

    2.3.4 The Contractor shall submit for the approval of GM(PD) or SM(L) a site layout

    plan with all the necessary details showing locations of distribution panels, switchgear rooms, power transformers, cable routes and supply points before

  • installation. Updated layout plans shall be submitted whenever there is modification.

    2.3.5 Single line diagrams showing supply points, loadings, protective equipment with

    settings and other useful information shall be submitted to GM(PD) or SM(L) for approval. A copy of single line diagram shall be attached to the relevant panel for easy reference.

    2.3.6 The workers for installation and maintenance of all fixed electrical installation shall

    be Registered Electrical Workers (REW) of appropriate grade. The workers carrying out installation inside operational areas shall be Competent Persons who are qualified to receive safety documents from User Division.

    2.3.7 As soon as the installation is completed, the Contractor shall submit a Work

    Completion Certificate – Form WR1 (EMSD/LE/44) together with the checklist to GM(PD) or SM(L) (if applicable). Subsequently, the Contractor shall carry out the routine check on a regular basis and submit the associated Periodic Test Certificate - Form WR2 (EMSD/LE/46) together with checklist to GM(PD) or SM(L) (if applicable). Checklists can be found in the Code of Practice for The Electricity (Wiring) Regulations. All checklists shall be duly endorsed by a REW of appropriate grade.

    2.3.8 Distribution panels shall be numbered. The doors shall be locked and the keys shall

    be kept by a REW for safety reason. A standard label for warning of electricity shall be adhered to or painted on the outside of panel door. The name and contact telephone number/ pager number of the responsible electrical worker shall also be shown on the panel door. Panels with adequate protection against the environment shall be used.

    2.3.9 Adequate earthing shall be provided for the electrical installation. Earth leakage

    protection devices shall be provided for portable equipment. 2.3.10 Cables for electricity supply to temporary facilities shall be laid on or below ground

    level as practicable as possible. They shall be properly protected with shields or conduits to prevent accidental contact by people. In case of overhead cables, they must be installed in the location approved by GM(PD) or SM(L) with a minimum headroom of 6 metres. Exposed live conductive parts or cables shall be properly insulated and protected with shields or fully enclosed to prevent direct contact. All cables, including laid underground, shall be removed by the Contractor after use. As-built drawing showing the cable routing plan shall be submitted.

    2.4 Water Supplies

    Unless agreed otherwise, the Contractor shall normally be responsible for his/her own water supply in Lamma Site. The Contractor shall supply, install and maintain all water pipes and meters for his/her own use and for the purpose of the works. The installation work shall be carried out by licensed plumbers and in compliance with WSD requirements. All underground pipes shall be protected. The water pipe routing shall be submitted to GM(PD) or SM(L) for approval before

  • installation. As-built drawings shall be submitted after installation. The Contractor shall be responsible for any relocation if required, and removal of all such water pipes upon completion of work. The Contractor shall also be responsible to maintain water meter and submit his/her water consumption record to GM(PD) or SM(L) on a monthly basis.

    2.5 Telephone Lines and Facsimile

    The Contractor shall be responsible to supply and install his/her own telephone lines and facsimile on site. He shall be responsible for any relocation if it is required, and removal of all such telephone lines upon completion of work.

    2.6 Eating / Rest Area

    2.6.1 The Contractor shall obtain GM(PD) or SM(L) prior approval before setting up of eating area. Naked flame cooking is strictly prohibited on site.

    2.6.2 The contractor in Lamma shall provide adequate covered rest areas, equipped with

    tables and chair, and adequately illuminated & ventilated, suitable for workers to take their lunch or refreshment on site and to hold toolbox talks.

    2.6.3 The number of table sets (one table with 4 chairs) provided shall be at a ratio of not

    less than 2 sets for every 20 workers.

    2.6.4 The contractor shall ensure that the rest areas cleaned daily and provided with rubbish bins.

    2.6.5 The Contractor shall provide the food waste eliminator/disposer for handling their

    food waste. 2.7 Site Sanitary

    2.7.1 The Contractor shall not use toilets in restricted operational areas without written approval from GM(PD) or SM(L).

    2.7.2 Unless agreed otherwise, the Contractor shall provide site toilets and washing

    facilities for his/her own personnel, and maintain all facilities in good hygienic condition. Detailed plans of such facilities shall be submitted for GM(PD) or SM(L)’s approval.

    2.7.3 The construction of site toilet provided by the Contractor shall comply with all

    statutory and contractual requirements. 2.7.4 The toilet facilities shall have separate male and female compartments and be

    placed at convenient locations close to workers’ workplace.

    2.7.5 Wet type with flushing water supply and the toilet waste properly collected and discharged into a sewerage system, septic tank, soak away system, or in-situ

  • treatment facilities shall be proposed by the contractor and approved by GM(PD) or SM(L).

    2.7.6 Chemical type complete with containers shall be cleaned on daily basis and waste

    removal shall be carried out by a specialist contractor.

    2.7.7 The number of toilet facilities provided on the site shall be at a ratio of not less than one for the first 20 workers and additional one for every additional 20 workers.

    2.7.8 The number of hand-wash facilities provided on the site shall be at a ratio of not

    less than one for the first 20 workers and an additional one for every additional 20 workers.

    2.7.9 The contractor shall ensure that hand-wash facilities are cleaned on daily basis and

    effectiveness of the discharge collection system shall be maintained throughout the duration of the contract.

    2.7.10 When the number of workers working on Lamma site will likely be exceeding 100

    workers in a day, the contractor shall provide showering facilities on site unless otherwise approved by SM(L) that the provision is not appropriate.

    2.7.11 The showering facilities shall be provided indoors or inside containers with

    appropriate drainage connections.

    2.7.12 The contractor shall provide sufficient rubbish bins with covers at strategic locations throughout the site for collection and disposal of non-construction waste generated by the workers.

    2.7.13 The contractor shall maintain the cleanliness of the rubbish bins and arrange

    collection and disposal of the waste on a daily basis.

    2.7.14 The contractor shall provide sheltered & free drinking water facilities for workers working on site throughout the duration of the contract.

    2.7.15 The contractor shall be responsible for maintaining the drinking water facilities in a

    clean and hygienic condition and refilling drinking water to the facilities when empty.

    2.7.16 The contractor in Lamma site shall be required to carry out “Daily Cleaning”

    (30mins) and “Weekly Tidying” (1 hour) of the site.

    2.7.17 The contractor shall assign one or more persons for the overall co-ordination, monitoring and overseeing of the performance on cleanliness and control of mosquito breeding and rodents on site.

    2.7.18 Particular attention shall be given to lunchboxes, cans or cigarette ends in site

  • inspections to maintain the site clean and hygiene.

    2.7.19 Where the contractor needs to apply pesticide for control the breeding of mosquitoes and poisons to control rodents, the pesticide and poisons shall be applied by a specialist contractor or trained person.

    3. TRANSPORTATION AND LOGISTICS

    3.1 Transportation to and within Lamma Site

    3.1.1 The Contractor shall arrange his/her own transportation for goods and personnel to/from and within site. No unlicensed vessels and vehicles shall be used.

    3.2 Jetty Booking for Loading/ Unloading Activities in Lamma / Ap Lei Chau.

    3.2.1 The Contractor shall not berth any vessel alongside the unloading areas without prior approval from SM(L).

    3.2.2 The Contractor shall fill in the “Jetty Booking Application Form” (Appendix 6)

    completed with information such as the quantity, weight and volume of the goods; the required time and date for loading/unloading; the name of Supervisor in charge of loading/unloading activities ; and risk assessment report if lifting operations are involved etc. The Contractor's local forwarder shall book the jetty by submitting the application form to SM(L) at least 3 working days in advance. SM(L) will confirm the booking by returning fax or email.

    3.2.3 Use of jetty facilities shall be restricted from 9:00 a.m. to 5:00 p.m. on weekdays

    and from 9:00 a.m. to 12:00 noon on Saturdays. No loading/unloading activities are allowed beyond the above specified period unless in exceptional cases with the approval of SM(L).

    3.2.4 No noise nuisance shall be caused to the nearby resident such as abuse of

    loudspeakers and generating high-pitched noise during winch operation.

    3.2.5 Effective dust suppression measures (i.e. water spraying/covered by tarpaulin sheet) shall be in place if applicable.

    3.2.6 The contractor shall not unload materials / cargoes / ramps directly onto the jetty

    ground without laying protective mattress, steel plate or timber. 3.2.7 The Contractor shall make arrangement to transport the unloaded materials/cargoes

    from the jetty unloading area to the designated storage area or job site immediately. In order to facilitate all parties on site, storage of material in the unloading area is not allowed without prior approval from SM(L).

    3.2.8 The Contractor shall note that HK Electric has priority to use the Jetty both in

    Lamma and Ap Lei Chau.

  • 3.3 Goods Receiving Requirements

    3.3.1 When the materials/ equipment are delivered to the designated area, the Contractor shall liaise with the RE to carry out a goods receiving inspection as per contractual requirement.

    3.3.2 The RE shall decide whether it is practical for the Contractor to open the packages

    for checking of any damages, short-shipment/ non-conformance. Normally, preliminary inspection shall be carried out within 3 days and detailed inspection shall be carried out within 10 days.

    3.3.3 After the inspection, the Contractor shall submit a “Materials/Goods Receiving

    Report” (Appendix 7) to GM(PD) or SM(L). For any shipment damage found during goods receiving inspection or subsequent detailed inspection, the Contractor shall detail the extent of damage and/or short shipment in the Materials/Goods Receiving Report. Upon requested by the RE, the Contractor shall issue an NC Report to notify the supplier of any damage/loss. For any erection damage subsequent to goods receiving, the Contractor shall, within 72 hours after the incident comes to the attention of the Contractor, send a duly completed and signed Accident Report Form to the Insurance Section (Accounts Department of Group Finance Division) of HK Electric, with a copy to GM(PD) or SM(L), for necessary processing with the Insurers.

    3.3.4 The Contractor shall identify, check, label and fence off the inspected materials/

    equipment at the designated areas. 3.4 Site Vehicle Registration in Lamma site

    3.4.1 All vehicles on site including all forms of mobile equipment fitted with wheels/

    tracks shall be registered. The following types of vehicle are not allowed at LPS:

    a) Petrol powered vehicle b) Saloon or similar type of vehicle

    3.4.2 The Contractor shall ensure that all construction site vehicles including both registered and non-registered ones are in full compliance with vehicle emission standards enforced by the Environmental Protection Department (EPD) of Hong Kong SAR Government. In accordance with the Air Pollution Control (Air Pollutant Emission) (Controlled Vehicles) Regulation, the pre-Euro IV diesel commercial vehicles shall be phased out as per the scheduled from the Regulation.

    3.4.3 The Contractor shall be fully responsible for his/her vehicles and liable for all sequential property damages and injuries to persons.

    3.4.4 The Contractor shall submit “Site Vehicle Registration/Re-registration Form”

    (Appendix 8) completed with photo, approved or exemption labels of Non-road Mobile Machinery (NRMM) if applicable, driving license, LA&LG certificates if

  • applicable, plant operating manual and the latest maintenance record to SM(L) for approval at least 2 weeks before the vehicle is transported to site.

    3.4.5 If the vehicle is not roadworthy, the Contractor shall rectify the defects before

    applying for the registration. If the application is accepted, the approved registration form will be returned to the Contractor.

    3.4.6 For passenger vehicles after registration, the Contractor shall arrange inspection in

    HK Electric’s Lamma Garage at his/her own costs. The current inspection cost is HK$2,100 per vehicle. Exemption will only be granted to site vehicles in possession of a valid permit disc issued by the Transport Department, HKSAR Government, or a satisfactory test report issued by an accredited private vehicle test centre.

    3.4.7 For plant vehicles/ mobile cranes with relevant valid examination certificates issued

    by Registered Professional Engineer (RPE), inspection upon arrival at site can be exempted.

    3.4.8 The vehicle will be issued a non-transferable Permit Disc upon satisfactory

    inspection. The Contractor shall prominently display the Permit Disc on the windscreen of the vehicle at all time.

    3.4.9 The vehicle shall be clearly marked with the abbreviated name of the Contractor

    and an assigned identification number at the two sides of vehicle. The marking must be comprised and the size of which shall not be smaller than 300mm x 300mm. Car plate shall be installed to display the assigned identification number in front and behind, unless valid vehicle registration plate is already in place.

    3.4.10 Monthly vehicle inspection by a competent person is required, and the Contractor

    shall maintain updated inspection checklists at site checking of called for. 3.4.11 The Permit Disc shall be renewed every 6 months. The Contractor shall submit

    the “Site Vehicle Registration/Re-registration Form” (Appendix 8) 2 weeks before the expiry date. All incurred costs shall be paid by the Contractor.

    3.4.12 Any vehicle found either without a disc or displaying an incorrect disc will be

    immediately demobilised or impounded. If the vehicle is not claimed within a week of impounding, the vehicle will be disposed at the discretion of SM(L). No claims by the Contractor or his/her sub-contractors will be allowed.

    3.4.13 The security staff will carry out frequent 'Stop and Check' to both vehicles and

    drivers to prevent unregistered vehicles and unauthorized driving on site. 3.4.14 If the Contractor removes any vehicle from site, the Permit Disc shall be

    surrendered to SM(L). If any vehicle is found abandoned on site, a warning letter will be issued to the Contractor. If there is no response from the Contractor after one week, HK Electric will remove the vehicle with all incurred costs back-charge to the Contractor. Purposely destroying the registered identification in order to deny the responsibility of removal scrap vehicle will potentially result in disciplinary actions including suspension from bidding for future work.

  • 3.4.15 The Contractor’s defective vehicles awaiting repair shall be stored in a designated

    area with proper notice instead of dumped randomly at site.

    3.4.16 All Contractors vehicles shall be enrolled with security guard when travelling in/out LMX construction site wherever security personnel are posted.

    3.5 Driver Discipline

    3.5.1 Drivers at site shall follow HKSAR licensed driving regulations and possess valid Green Card.

    3.5.2 The Contractor shall apply for Lamma Driving Permits for his/her operation if they need to drive at Lamma site. Only the authorized drivers with valid Lamma Driving Permits are allowed to drive in LPS. They have to pass a driving assessment and in possession of a valid driving license.

    3.5.3 Drivers must not drive under the influence of alcohol or drugs.

    3.5.4 The Contractor shall park his/her vehicles only in authorised areas.

    3.5.5 Reverse driving of long vehicles/ heavy construction plant/ crawler cranes shall be

    assisted by a signalman to prevent possible traffic accident.

    3.5.6 The Contractor shall lower the booms/ jibs of his/her mobile cranes/ derrick trucks as far as practicable during travelling in order to prevent potential damage to HK Electric’s overhead utilities.

    3.5.7 The maximum speed allowed on Lamma site is 25 km/h on paved access, 8 km/h

    on unpaved access, or specified by speed-limit road signs. Written warnings will be issued to the over-speeding drivers. Disciplinary actions will be taken against repeated offences, including banning the driver from working at the site.

    3.6 Lamma Site - Bicycle Registration (Repealed)

  • HOUSEKEEPING AND SECURITY

    4. CLEANLINESS, HOUSEKEEPING AND HYGIENE

    4.1 No incineration of rubbish, debris, scrap, etc. is allowed in HK Electric’s Site and Premise.

    4.2 The Contractor shall be responsible for the cleanliness and tidiness of his/her own and his/her sub-contractor’s work areas including site offices, warehouses, eating areas, storage areas, fabrication yards, toilet facilities, etc. to the satisfaction of SM(L) on Lamma Site or RE on HK Island Site.

    4.3 The Contractor shall organize a cleaning team for daily site cleaning purpose and provide sufficient local litterbins with rigid covers for the collection of rubbish. All rubbish and waste materials shall be cleared from the work areas on daily basis and removed off site on a regular basis or as directed by SM(L) on Lamma Site or RE on HK Island Site at the Contractor's own costs. Large scale site tidy-up shall be arranged on week-end (1 hour after afternoon tea break) with manpower not less than 10% of the total workforce. In case of over-storage of scrap/ waste, HK Electric will arrange for removal and will back-charge all incurred costs to the Contractor. The Contractor shall not make use of HK Electric waste disposal areas without prior permission.

    4.4 The Contractor shall be responsible for vehicle cleaning by provision of sufficient wheel washing basins and washing hoses at exits of construction site

    4.5 The Contractor shall take all necessary preventive measures against dengue fever as directed by the Department of Health, HKSAR Government, including, but not limited to, the following:

    a) Ensure no stagnant water accumulation; b) Spray pesticide on a regular basis to prevent breeding of mosquitoes; c) Ensure no blockage of drainage system; and d) Cover up or turn over obsolete drums/ cans/ trays/ bottles, etc. which may

    collect rain or water.

    4.6 The Contractor shall ensure that all naked lights and other ignition sources are extinguished and the concerned electrical appliances are switched off prior to leaving HK Electric’s site and premise.

    4.7 No raising, feeding or keeping of cats, dogs and other pets is allowed. The Contractor is encouraged to report stray pets found in HK Electric’s site and premise.

    4.8 The Contractor shall work at Lamma site with 5S philosophy in order to enhance safety performance, improve quality/ productivity/ image.

    4.9 In case of poor cleanliness/ housekeeping/ hygiene causing nuisances/ potential hazards to the personnel/ plant equipment, a written warning will be issued to the concerned Contractor for immediate action. Disciplinary actions will be taken against repeated offences, including banning the individual offender from working

  • in HK Electric’s site and premise.

    4.10 The Contractor shall take particular care to ensure the scrap/ waste not to promote or become a fire hazard.

    5. SECURITY

    5.1 General

    5.1.1 No unauthorized entry to the operational areas and construction site is allowed. 5.1.2 No person is allowed to access to Lamma site by landing onto armoured rocks of

    LPS seawalls. 5.1.3 The Contractor shall prominently display his/her company logo or company name

    at his/her site office, work areas and containers. 5.1.4 The Contractor shall be responsible for the security of his/her own plant and

    equipment as well as HK Electric plant materials and equipment under his custody. Valuable items shall not be left unattended.

    5.1.5 No materials can be removed from LPS/LMX without a Gate Pass. The material

    gate pass application form (available from PD Lamma Site Office) must be signed by the Contractor’s authorised person and attached with a photo indicating the materials to be removed off Lamma site. The gate pass must be endorsed by the RE and approved by the responsible Section Head or SM(L).

    5.1.6 Any thefts and loss of property within HK Electric’s site and premise shall be

    reported to GM(PD) or SM(L).

    5.1.7 Materials delivered to LPS/LMX or designated area shall refer to 3.3 Goods Receiving Requirement.

    5.2 Security Passes

    5.2.1 All persons shall clearly identify themselves at all times whilst in HK Electric premises by prominently displaying their appropriate identification badge as appropriate.

    5.2.2 Only persons possessing valid security passes will be allowed to get access to the

    construction site areas or premises as designated by HK Electric, namely:

    5.2.3 Visitor Passes/ Day Passes shall be surrendered to Security Posts upon leaving HK

    Electric’s premises. 5.2.4 Contractor Pass shall be renewed biannually, or upon the expiry date. They shall be

    surrendered to HK Electric upon completion of works. 5.2.5 Proper access to Lamma Site by the Contractor shall be through the LPS East Gate,

    East Pier and Passenger Jetty or LMX Passenger Jetty where there are HK Electric Security Posts.

    5.2.6 In case of loss of security passes, the Contractor shall report to GM(PD) or SM(L)

    and bear the replacement cost of HK$200 for each pass lost. 5.2.7 The Contractor shall update his/her employee record list (sub-contractors as well)

    on a monthly basis with corresponding job position, Green Card No., CWR Card No. and HKID No. etc and submit to GM(PD) or SM(L) for record purpose. (The personal data collected will be used for HK Electric’s record purpose and will not be used for any other purpose in compliance with Personal Data (Privacy) Ordinance.)

    5.2.8 The Contractor shall not employ any illegal immigrants to work on the construction

    site. Employment of foreign workmen/ supervisors without local working permit/ visa is illegal and prohibited. Contractors shall ensure that his/her workers having HKID card marked with a symbol “C” must be confirmed “lawfully employable” by referring to the “condition of stay” endorsed by Immigration Department, HKSAR Government on the individuals’ passport/ travel document.

    5.2.9 The Contractor shall strictly monitor his/her sub-contractors' employment record

    and staff identification so as to eliminate any illegal immigrants working on the construction site. As per the context of the Construction Workers Registration Ordinance, all construction workers who personally carrying out construction workers on construction sites shall be registered as Registered Construction Workers of the appropriate categories.

    5.2.10 The HK Electric security staff and REs are authorized to spot-check on contractor

    employees' identity at their discretion if they have any doubt on the specific worker’s identification. Contractor employees without valid Contractor Pass will

  • be sent away by HK Electric Security. 5.2.11 The Contractor shall ensure his/her employees to comply with HK Electric security

    requirements and properly keep their security passes for checking when they are working in HK Electric’s premises. For any misconduct or breach of HK Electric security requirements caused by a worker, a written warning will be issued to the Contractor for immediate follow-up. Disciplinary actions will be taken against repeated offences, including banning the individual offender from working for HK Electric.

    5.2.12 The Contractor is normally not allowed to use HK Electric’s Lamma ferry service.

    For special cases, the Contractor may request SM(L) to arrange for an authorization document. The Contractor shall show it to HK Electric Security when entering or exiting the power station.

    5.3 Overtime Work

    Should overtime or night work be required in LPS, the Contractor shall submit a name list of his/her workers with the request form “Contractor Work Request On Weekdays After 6:00 pm, Sunday and Public Holidays” (Appendix 9) before 4:00 pm on weekdays (Monday to Friday) to SM(L). Should overtime or night work be required especially in T&D substation requiring the standby of T&D attendant, the Contractor shall submit a name list of his/her workers with the formal request at least one week in advance to the RE.

    5.4 Discipline

    The Contractor shall maintain a good discipline and strict control over his/her workers. The following rules, including but not limited to, shall be observed: a) Gambling in any forms is not permitted on HK Electric’s site and premise.

    Any person found gambling will be reported to his/her employer who is required to remove him from site and premise.

    b) Smoking is not allowed within all HK Electric’s site and premises.

    c) No sale, storage, or consumption of alcoholic drink or drugs is allowed on

    HK Electric’s site and premises. Any person found having consumed alcohol or drugs will be banned from entering or sent away from the HK Electric’s site or premises.

    d) Fishing is not allowed within HK Electric premises.

    e) Sleeping in operational areas, roads or open areas is prohibited.

    f) Spitting or littering is forbidden.

  • 5.5 Photograph and Video Taking

    No photograph or video taking at HK Electric premises is allowed without prior approval from GM(PD) or SM(L). For normal case, application shall be made 10 days before photograph or video taking.

    5.6 Reports from Contractor on Security Matters

    The Contractors shall report any security incident to GM(PD) or SM(L) immediately.

  • ENVIRONMENTAL PROTECTION

    6. RELEVANT ORDINANCES, REGULATIONS AND IN-HOUSE DOCUMENTS

    6.1 Relevant Ordinances and Regulations

    The Contractor shall be fully aware of and strictly comply with all Ordinances and Regulations enforced by the EPD of Hong Kong SAR Government related to environmental protection, including but not limited to the following:

    a) Air Pollution Control Ordinance (APCO) - Chapter 311 b) Water Pollution Control Ordinance (WPCO) - Chapter 358 c) Noise Control Ordinance (NCO) - Chapter 400 d) Waste Disposal (Chemical Waste) Ordinance (WDO) - Chapter 354 e) Dumping at Sea Ordinance – Chapter 446 f) Environmental Impact Assessment Ordinance – Chapter 499 g) Ozone Layer Protection Ordinance (Cap.403) h) Code Of Practice and Technical Memorandum issued by EPD i) Construction Waste Disposal Charging Scheme

    6.2 Statutory Permits, Labels and Licences

    6.2.1 The Contractor shall be responsible for obtaining all the required statutory Permits, Labels and Licences as required by the Ordinances and Regulations, and shall submit copies of these documents to GM(PD) or SM(L).

    6.2.2 The permits, labels, licences and exemptions successfully granted by the Authority

    shall be held by the Contractor and properly displayed and filed. 6.2.3 Should non-compliance of legal/contractual requirements be found, the Contractor

    shall immediately report to the RE and take remedial action in order not to interrupt HK Electric’s plant operation, if any.

    6.3 EIA Ordinance and LEMS and TEMS Requirements

    6.3.1 The Contractor is expected to align with HK Electric’s Environmental Policy (Appendix 10) in the course of executing his/her contractual works.

    6.3.2 For projects designated under EIA Ordinance, the Contractor shall comply with the

    requirements stipulated in the EIA Reports, Environmental Permits (EP), EM&A Manual, and all related statutory requirements.

    6.3.3 For designated projects subjected to Environmental Monitoring & Auditing (EM&A)

    programme, requirements stipulated in the EM&A manual such as methodologies on impact monitoring, details on data reporting, compliance assessment and Event & Action Plans, etc. shall be observed and fulfilled by the Contractor throughout the entire course of construction work.

  • 6.3.4 Updated versions of the above documents may be issued to reflect any changes or enhancement of environmental protection measures for the construction works. It is the Contractor’s responsibility to observe and comply with the requirements stipulated in the latest versions of the above documents during the course of the entire contractual period.

    6.3.5 Notwithstanding all the EM&A requirements insofar stipulated in the latest version

    of the EM&A Manual, it is the Contractor’s responsibility to comply with all additional EM&A requirements that may be imposed including any stepping up of EM&A measures to minimise unacceptable impacts.

    6.3.6 It is the Contractor’s responsibility to make available to HK Electric’s electronic

    copies and graphical presentations of all environmental monitoring data of the format acceptable to the EPD in the shortest time and in no event later than one week after the relevant environmental monitoring data are collected or become available. The Contractor shall also be responsible to provide all necessary information to facilitate HK Electric’s compilation of the monthly EM&A Report or any other submissions as required by the EPD.

    6.3.7 HK Electric may separately engage an Independent Environmental Checker (IEC) to

    review, verify and endorse all submissions prepared by the Contractor for EPD submission purpose. It is the Contractor’s responsibility to comply with all requirements and recommendations made by the IEC till the satisfaction and endorsement of the submission document by the IEC.

    6.3.8 The Contractor shall be responsible to prepare all document and reports relating to its

    contractual work wherever required under the EP and EM&A Manual and address to all concerns made by HK Electric/IEC/EPD and make amendments to these document and reports until their final acceptance by the Authority.

    6.3.9 For work within the operational areas of LPS, the Contractor shall execute his/her

    work according to the Environmental Management System for Lamma Power Station (LEMS), which is developed by HK Electric in accordance with the requirement of ISO14001. The Contractor is required to submit an environmental management plan and work to the plan accordingly.

    6.3.10 For work within the operational areas of T&D substations, the Contractor shall

    execute his/her work according to the T&D Environmental Management System (TEMS), which is developed by HK Electric in accordance with the requirement of ISO14001. The Contractor is required to submit an environmental management plan and work to the plan accordingly.

    6.4 Waste Management

    6.4.1 The Contractor shall take preventive measures to minimize wastes generated from his/her work processes/activities. The following are the guidelines for waste management:

    a) Select environmental friendly materials, equipment and processes in

  • carrying out work/activities. b) Make sure that chemical wastes are collected and transferred to the

    appropriate treatment facilities for treatment/disposal by licensed collectors. c) Promote the 4R (i.e. Reduce, Reuse, Recycle and Recover) concept to team

    members to minimize waste production. d) Train team workers the correct methods of waste disposal. e) Establish waste minimization programmes. f) Encourage segregate wastes. g) Prevent contamination to the soil and paved ground at site. h) Submit a Waste Management Plan as required.

    6.4.2 Within 21 days after a construction work contract with value of HK$1,000,000 or

    above is awarded, the Contractor is required to open a billing account with the EPD solely for the contract for disposal of construction waste. For a construction work contract with value less than HK$1,000,000, the Contractor can use a billing account for other contracts each with value less than HK$1,000,000. The Contractor shall submit copies of trip tickets for the disposal of construction waste to GM(PD) or SM(L) for record.

    7. POLLUTION CONTROL

    7.1 Air Pollution Control

    7.1.1 Dusty materials shall be dampened before transportation. All earth-handling vehicles shall have cover extending securely to all sides of the vehicles to prevent dust from being entrained in the wind.

    7.1.2 The Contractor shall arrange to enclose stockpiles of sand, aggregate or any other

    dusty materials greater than 20m3 on three sides with walls higher than the pile and at 1m beyond the front of the pile. If the stockpiles are greater than 50m3, they shall be enclosed or covered and sprayed with water in dry or windy condition.

    7.1.3 The Contractor shall ensure that the working area of any excavation or earth

    moving operation is sprayed with water immediately before, during and immediately after the operation to maintain a wet surface. If breaking of boulders, rocks or concrete is required, the Contractor shall ensure that water spray is undertaken to reduce dust emission.

    7.1.4 Dust arises from paint removal may contain metallic lead. Lead fumes arise from

    hot cutting and dismantling of tanks that may contain leaded petrol or products containing lead. Dust suppression for sand blasting and paint removal work shall be required.

    7.1.5 In the proximity of the identified Air Sensitive Receivers (ASR), a 2.4m-high (or

    higher to achieve effective separation) hoarding shall be placed at the concerned work area boundaries.

    7.1.6 To improve the local air quality by curbing undue exhaust emissions, all

    construction site vehicles/plants powered by fossil fuel must comply with EPD’s

  • requirement. For diesel driven vehicles/plants, Ultra Low Sulphur Diesel (ULSD) must be used. ULSD is defined in Schedule 1 of the Air Pollution Control (Motor Vehicle Fuel) Regulation).

    7.1.7 The Contractor shall maintain a summary record of ULSD delivered to the site,

    including those ordered by his/her sub-contractors, together with the details of consumption of such fuel by individual constructional plant on the site. The record of fuel deliveries should be supported by the original receipts of the delivery notes of the oil companies. The summary record and delivery notes shall be kept in the site office for ready inspection by GM(PD) or SM(L) or his delegate upon request.

    7.2 Noise Pollution Control

    7.2.1 Construction Noise Permit (CNP) from EPD is required for the following works:

    a) General construction work (evening work from 7:00 pm to 11:00 pm and overnight work from 11:00 pm to 7:00 am).

    b) All civil percussive piling (from 7:00 am to 7:00 pm on any day except public holidays).

    7.2.2 The Contractor shall apply for a CNP in accordance with the Noise Control

    (General) Regulations prior to the commencement of works and display the permit as required, and provide a copy to GM(PD) or SM(L).

    7.2.3 For prevention of noise pollution and nuisance, the Contractor shall be responsible

    to install localized sound screens, silencers or mufflers, or to replace with quiet plants as appropriate if considered necessary by SM(L) on Lamma Site or RE on HK Island Site.

    7.2.4 The Contractor shall need to comply with the EPD requirement of "Product Noise

    Emission Labels" (NEL) on all relevant construction equipment. Only those air-compressors and air-breakers which have been tested by EPD authorised test laboratory and fitted with NEL shall be allowed for use on site.

    7.2.5 For monitoring purpose, a patrol team together with sufficient number of hand held

    noise monitoring instruments shall be provided by the Contractor to measure the general construction noise at critical locations on regular intervals.

    7.2.6 The Contractor shall avoid using loud speakers on barges, tug boats, pontoons and

    other marine vessels for material delivery or construction work for communication purpose either at day or night.

    7.2.7 The Contractor shall be aware of HK Electric’s noise monitoring stations in the

    vicinity of the working area prior to work commencement and take preventive measures not to exceed the specific criteria on noise level. In case the noise level is exceeding the specific criteria, the Contractor shall stop his/her work immediately until remedial actions have been taken.

  • 7.3 Water Pollution Control

    7.3.1 The Contractor shall be aware of and fully comply with EPD Practice Note for Professional Person – “Construction Site Drainage”.

    7.3.2 The Contractor shall not discharge directly or indirectly wastewater (such as trade

    effluent, contaminated water, cooling water or hot water, etc.) into any sewers and drains without prior written consent (discharge licence) of the Director of Environmental Protection. The consent granted by the authority and the design of such treatment works shall be submitted to GM(PD) or SM(L) before commencement of works.

    7.3.3 The Contractor shall avoid the release of sediment or other polluted materials into

    the sea. 7.3.4 The Contractor shall avoid the drainage of visible foam, oil, grease, scum, litter or

    other objectionable materials into the storm drains or the sea. 7.3.5 Acidic wastewater generated from acid cleaning, etching, pickling or similar

    activities shall be neutralized and suitably treated before discharging.

    7.4 Chemical Waste Disposal Control

    7.4.1 The Contractor shall comply with all legal requirements in storing, handling, labelling, packaging and disposing of chemical wastes.

    7.4.2 The Contractor, who is likely to generate chemical waste during his/her contract

    period, is required to apply for registration as a "Chemical Waste Producer" with the EPD.

    7.4.3 The Contractor, who is registered as a chemical waste producer on Lamma Site,

    needs to have a proper temporary chemical waste container store in LPS. The storage container shall have an external signboard with the warning wordings approved by EPD. If more than one chemical waste is to be stored inside, container internal partitions are required.

    7.4.4 The Contractor shall submit to SM(L) records of the types and quantities of his/her

    chemical waste either for temporary storage or removal from LPS. 7.4.5 In order to identify the owner of any abandoned empty cans on site, the Contractor

    has to mark up his/her company names or logos on his/her chemical containers. 7.4.6 Contractors shall not reuse any empty chemical tins for other purposes prior to

    proper treatment. 7.4.7 In case of minor chemical spillage, the Contractor shall attempt to stop/confine the

    spillage if it is considered SAFE to do so. The Contractors' workers shall wear appropriate personal protective equipment for handling chemical spillage. The

  • case shall then be reported to the RE without delay. The waste produced from treatment of chemical spillage shall be handled properly.

    7.4.8 In the event of major chemical spillage, the case shall immediately be reported to

    GM(PD) or SM(L). If the Contractor cannot contact GM(PD) or SM(L), he shall report to the RE and/or HK Electric EMERGENCY PAX PHONE 5-333 (or Tel no. 2982 6333) in case of Lamma Site for further assistance.

    7.4.9 When making the report, the Contractor must clearly state the following:

    a) Location where the chemical spillage occurs b) Nature of the spilled chemical if it is known c) Extent of the chemical spillage d) Name of the Contractor e) Name of the responsible person and contact telephone number

    7.5 Marine Dumping

    7.5.1 Under Dumping At Sea Ordinance, the Contractor is required to apply from EPD a "Marine Dumping Licence" for dumping of all his/her dredged materials. The Contractor needs to submit a report to EPD under the "Conditions of the Marine Dumping Licence", with a copy to GM(PD) or SM (L) for record.

    7.5.2 The Contractor needs to ensure that the "Vessel Log Form" is properly maintained

    by the working personnel for EPD inspection. 7.5.3 By Law, all barges listed in Section-10 of the Licence shall be equipped with an

    "Automatic Self-Monitoring Device", in order to prevent any short dumping at the public sea.

    7.5.4 Marine dumping carried out by the Contractor is restricted to the location specified

    in the Licence. 7.6 Other Controls

    7.6.1 The Contractor needs to comply with all EPD related Ordinances and Regulations made by the HKSAR Government from time to time. All relevant records regarding the environmental issue shall be properly retained, filed and available for inspection upon request by HK Electric and the Authority.

    7.6.2 The Contractor shall assign representatives to handle environmental issues related

    to the Contract. 7.6.3 No interference to HK Electric’s environmental facilities is allowed. 7.7 Complaint Handling

  • 7.7.1 In the event of receiving any direct environmental complaint, the Contractor shall politely request for the complainant’s contact information and pledge to follow up promptly. Then the Contractor shall report the matter to GM(PD), SM(L) or the RE immediately. Written report should be submitted to HK Electric within 24 hours in accordance with the procedure outlined in item 24.3.2.

  • CONSTRUCTION SITE SAFETY

    8. HEALTH AND SAFETY REQUIREMENTS

    8.1 The Contractor shall strictly comply with all statutory ordinances, regulations and codes of practice related to health and safety. The general duties of a Contractor regarding health and safety at work are imposed by Clause 6A & 6B of the Factories & Industrial Undertakings (F&IU) Ordinance and the Occupational Safety and Health (OSH) Ordinance

    8.2 The Contractor shall pay particular attention to the F&IU (Safety Management) Regulation that demands the setting up of an effective safety management system with continuous improvements. The Contractor shall provide adequate resources to organise and implement his/her safety policy, and to conduct regular safety audits to review its safety performance.

    8.3 The Contractor shall also comply with Construction Site (Safety) Regulations on general site health & safety requirements. The Contractor shall submit a copy of “Notification of Construction Work” to PD Lamma Office for record. The Contractor is required to inform Labour Department in writing within 7 days of works commencement unless the work can be completed in a period of less than 6 weeks, or no more than 10 workmen are employed at any one time.

    8.4 The Contractor shall meet the same standards and requirements on health and safety as stipulated in HK Electric’s Health and Safety Policy (Appendix 11). The Contractor shall take all necessary measures to safeguard the health and safety of all his/her employees, HK Electric’s employees and the public. The H&S Policy shall be reviewed bi-annually.

    8.5 The Contractor is required to submit a Safety Organization Chart and a detailed Safety Plan within 28 days after contract award, and the monthly accident statistics on or before the 5th day of the following month.

    8.5.1 The Safety Plan is a document setting out the details of the Contractor’s safety

    management system to be implemented on the site to ensure safety and health in the execution works. A comprehensive Safety Plan should cover the Contractor’s management policy and commitment, organisation and detailed arrangement and procedures to achieve the high standard safety performance.

    The Contractor shall review the Safety Plan, hazards & risks, and related control measures including method statements at monthly intervals; and shall revise and update the safety plan if necessary.

    8.5.2 The Contractor shall prepare and submit to GM(PD) or SM(L) the Safety Plan endorsed by a Registered Safety Officer. It shall contain/address details of the following 14 key elements of a safety management system:

    - Safety policy

    - Safety organization

    - Safety rules and regulations

    - Safety committees

    - Safety and health inspections

    - Job hazard analysis

    - Personal protective equipment

    - Accident/incident investigation

    - Emergency preparedness

    - Safety promotion

    - Health assurance programme

    - Evaluation, selection and control of sub-contractors

    - Process control programme

    8.6 If the Contractor fails to comply with either statutory or in-house requirement, appropriate disciplinary action including issue of a SHE non-compliance notice, written site letter/memo, or even stop-of-work shall be taken. In addition, HK Electric reserves the right to exercise the “Delay of Completion and/or Contractor Default” clause(s) as stipulated in the General Conditions of Contract.

    8.7 The Contractor shall comply with the Gas Safety Ordinance on the safety requirements with regard to all Notifiable Gas Installations (NGI) within the premises. The Contractor shall provide adequate training and qualified workforces to carry out their contractual works on all NGIs.

    8.8 The Contractor shall follow HK Electric established procedures on health issue control, such as SARS, Fire Red Ants, Dengue Fever, Avian Influenza (Bird Flu), Middle East Respiratory, Zika Virus Infection and Ebola Virus Disease etc.

    8.9 The Contractor’s site supervisory staff and safety personnel shall attend Site Safety Supervision Training when they newly arrive LPS site.

    8.10 Contractor for LPS project shall monitor and report on daily basis safety supervision arrangement and manpower allocation plan through the Daily Manpower Report to SM(L).

    8.11 The Contractor should compile and submit the SHE Monthly Report in the published format on or before the 5th day of the following month.

  • 9. CONTRACTOR’S SAFETY, HEALTH & ENVIRONMENTAL PERSONNEL

    9.1 Qualified Safety Manager

    9.1.1 For mega-sized contracts with over 400 workers, the Contractor shall appoint a safety manager working full time on site. The appointed safety manager shall be a degree holder or an equivalent qualification, and possess 10 years construction site safety experience appropriate to the types and magnitude of the works at managerial level on health and safety.

    9.2 Qualified Safety Officer / Accredited Safety Supervisor / Environmental Engineer

    9.2.1 After site mobilisation, the Contractor shall submit a Site Safety Organisation Chart to GM(PD) or SM(L). The Contractor shall hire adequate number of Registered Safety Officers and Accredited Safety Supervisors to execute safety matters effectively. The Contractor’s Safety Officer / Supervisor shall wear a safety helmet with Y-type chin strap appropriately marked in Chinese & English to show his/her identity when he/ she is working on site. The Contractor’s safety personnel shall carry out regular site checks and timely complete LD’s prescribed safety inspection reports with copy to GM(PD) or SM(L).

    9.2.2 Notwithstanding the statutory requirement under the F&IU (Safety Officers and

    Safety Supervisors) Regulations, the requirement for the employment of Safety Officers/Safety Supervisors on site shall be as follows:

    9.2.3 If the total number of workers employed for the contract whether directly employed by the Contractor or indirectly by his/her sub-contractor is between 20 and 99, the Contractor shall hire a part-time Safety Officer whose time spent on site shall not be less than 18 hours per week.

    9.2.4 If the contract works bear environmental concern or the total number of workers

    employed for the contract whether directly employed by the Contractor or indirectly by his/her sub-contractor is above 50, the Contractor shall hire a part-time Environmental Engineer whose time spent on site shall not be less than 18 hours per week.

    9.2.5 If the workforce is more than 20, the Contractor shall hire at least a full time

    Accredited Safety Supervisor. If less than 20, the Contractor shall at least appoint a part-time Accredited Safety Supervisor.

    9.2.6 The Contractor shall not commence any construction on site without the

  • appointment of the required number of Safety Officer, unless explicitly permitted by GM(PD) or SM(L) in writing.

    9.2.7 For each proposed nomination of Safety Officer, the Contractor shall submit to

    GM(PD) or SM(L) the particulars of the proposed Safety Officer including the curriculum vitae, a copy of the registration letter issued by the LD and past job reference for approval.

    9.2.8 Safety Officer shall normally be expected to have at least five years’ experience on

    local construction site safety and have completed a safety auditor training course or equivalent.

    9.2.9 Accredited Safety Supervisor shall have at least three years’ experience on

    construction site safety and have completed an appropriate training course provided for safety supervisor run by CICTA (43 hours) or equivalent.

    9.2.10 Environmental Engineer shall have at least five years’ environmental related

    experience and have completed an appropriate training course.

    9.3 Qualified Safety Warden in Lamma Site

    9.3.1 In addition to the safety officer and safety supervisor, the Contractor shall appoint the foreman or ganger of each labour group or team working on the site to act as safety warden. The safety warden shall be responsible for ensuring that the directives from the Contractor, the safety officer and safety supervisor on safety and health matters are duly carried out, safety practices are adopted and protective clothing and equipment are used by the work force at all times on site. At least one safety warden shall be appointed for every 10 workers per job location. The safety warden shall be made aware of his/her responsibilities, number of workers on site and the activities they are engaged in. Every worker working on site shall be aware of the roles of safety warden and from whom he/she may seek advice or receive instructions on safety and health matters.

    9.3.2 Safety warden shall have at least three years’ experience on construction site and have completed a foreman safety training or equivalent.

    9.3.3 The Contractor’s safety warden shall wear a safety helmet appropriately marked in Chinese “安全糾察” & English “Safety Warden” and labelled with a Green tape to show his/her identity when he/she is working on site.

    9.4 Qualified First-Aider

    9.4.1 The Contractor shall comply with the Construction Site (Safety) Regulation which specifies that one qualified first-aider shall be available on site with number of workmen less than 100, and two qualified first-aiders for number of workmen over 100 during working hours (day and night). A standard first-aid box with the statutory required contents shall be available during working hours.

    9.5 Qualified Lifeguard

    9.5.1 The Contractor shall provide a qualified lifeguard when engaging in marine work.

    10. GAS SAFETY

    10.1 General Guidelines

    10.1.1 The Contractor shall observe and comply with the Gas Safety Ordinance Cap 51 and its subsidiary regulations with respects to gas safety at work.
