access to all the health services you need. We cover preventive care, check-ups, and treatment

services. UnitedHealthcare is dedicated to improving the health and well-being of the people we

serve by partnering with our members, health care providers and community leaders.

This book helps you find providers in your area who treat UnitedHealthcare Community Plan

members.

Provider Directory

Always check your provider's status.

This provider directory lists Home and Community Based Service Providers in our network you

can use to get care. This provider directory is current as of the date on the front cover, but listing a

health care provider does not guarantee their continued status as a participating physician or health

care provider.

We have made every effort to ensure a correct listing, but errors can happen with a directory of this

size. Always check to see if your provider accepts UnitedHealthcare before you get care by calling

Member Services, 1-800-587-5187, TTY 711. Member Services is available Monday – Friday from

8:00 a.m. to 6:00 p.m. We will be happy to help you.

Home and Community Based Service Providers listed are independent contractors and do not work

for UnitedHealthcare. UnitedHealthcare pays Home and Community Based Service Providers every

time they see one of our members and provide a covered service. This is known as “Fee For Service.”

We pay hospitals and other types of providers in the UnitedHealthcare network the same way; they

get paid every time you see them, or get a covered service from them. We never hold back payments

to our providers for giving too much service.

Primary Care Physician.

As a UnitedHealthcare Community Plan member, you need to choose a Primary Care Physician

(PCP). If you need help choosing a physician, just call Member Services at 1-800-587-5187, TTY 711.

Member Services is open from Monday – Friday 8:00 a.m. to 6:00 p.m. When you call, always make

sure you have your UnitedHealthcare identification number ready. The number is written on the front

of your identification card.

How to Read Your Directory

This is your UnitedHealthcare Directory of Home and Community Based Service Providers. You will

find information about each one of the healthcare providers in our network.

All Home and Community Based Service Providers are listed in order by name and by the town in

which they are located.

At the back of your Provider Directory, there is an index of all the UnitedHealthcare providers you

can use to get care. They are in order by name. You can use this index to help you find a certain

provider quickly.


If you have any questions about how to read your Provider Directory, or if there is any information you

do not understand, please call Member Services at 1-800-587-5187, TTY 711. Member Services is

available Monday to Friday 8:00 a.m. to 6:00 p.m. We will be happy to answer any of your questions.

Here Is Some Information to Note as You Read

the Directory

Handicap accessible.

Offices that are handicap accessible are footnoted with a wheelchair symbol after the provider's

name. This means the office has special ramps and wide doors for wheelchairs. A provider's office

may be handicap accessible even if it is not marked in this directory. You can always call the provider

to ask if the office is handicap accessible. Our providers tell us if their office is handicap accessible

when they join UnitedHealthcare. The information is also provided to us at least every three years. We

do not check this information to make sure it's right — we take the provider's word that the information

is accurate (true). UnitedHealthcare updates this information if the provider gives us new information

or changes.

Foreign language.

Physicians or health care providers who speak and/or have someone in the office who speaks a

foreign language have a language code under their name. A complete listing of these codes can be

found in the index pages in the back of this directory. Our providers tell us their name, gender, and the

languages they speak when they join UnitedHealthcare. We do not check this information to make

sure it's right — we take the provider's word that the information is accurate (true). We also check this

information at least every three years to make sure it is still true. UnitedHealthcare updates this

information if the provider gives us new information or changes.

You can also have an interpreter go to a medical appointment with you. Call Member Services at

1-800-587-5187, TTY 711, to ask for an interpreter. Please call us 72 hours in advance of your

appointment for a Language Interpreter. For Sign Language Interpreter, please call us 14 days

in advance of your appointment. We will do our best to help coordinate an interpreter for

your appointment.

about the provider's other professional experience (where the provider completed his/her training

or what type of certifications the provider has), go to www.abms.org and click on the section for

“consumers.” Here, you will also find a description of the provider's specialty. For example, a provider
