Sparrow Care Network Offices:

In 2014, Sparrow and physician leaders initiated the creation of Sparrow Care Network (SCN), a Clinically

Integrated Organization. Through this partnership, SCN physicians and Sparrow Health System are

transforming the way we deliver health care to our patients and region. SCN is led by physicians and is

comprised of primary care, medical, surgical and hospital based-specialties. It includes both independent and

employed physicians working together on a collaborative basis to develop and implement a new care delivery

model.

By providing the highest value care, SCN will address the IHI Triple Aim goals of:

1. Improving the patient experience of care (including quality and satisfaction);

2. Improving the health of populations; and

3. Reducing the per capita cost of health care.

Clinical Integration is about physicians, private and employed, working more closely together and more closely

with other health care providers to improve the quality and efficiency of patient care over time and across

healthcare settings. It is a transformation that is led and managed by physicians because only physicians have

the experience and deep knowledge required to bring about true improvements in the way healthcare is

delivered.

Clinical Integration requires greater interdependence and collaboration among physicians in order to achieve

enhanced coordination of patient care. These changes require thought and effort, so physicians typically are

rewarded for the work required to enhance the value of healthcare. In addition to the direct rewards

physicians receive, Clinical Integration improves our patients’ lives, which is the reason most of us went into

medicine in the first place. Patients receive safer, more affordable and convenient care, along with better

clinical outcomes; payers and employers benefit from Clinical Integration as greater efficiency results in

greater affordability. Lastly, employers benefit from a healthier, more productive workforce.

Clinical Integration is the most effective response to the current shift from fee-for-service to value-based

reimbursement. Nationally, the best physicians and health care systems are achieving Clinical Integration.
